We menufactured a simple device to estimate the statical compliance of normal dog arteries and various vascular materials. In consequence, Biograft
R(compliance; C=0.107±0.007, mean±SD) is the most compliant vascular material, but dog arteries (thoracic aorta C=0.491±0.079, carotid artery C=0.356±1.01) were several times more compliant.

Intraluminal ringed graft were utilized for surgical treatment of the 4 cases of dissecting aneurysms; typeI, type IIId, typeI with aortic insufficiency grade 4, type IIIa. All were male, and the age range was 35 to 62 years with an average age of 52. Each operation was undergone from immediafely after the onset to four and a half months after the onset. Follow-up has been from 6 to 15 months with a mean of 10 months, and all patients are alive. Aortic cross-clamp time ranged from 50 to 122 minutes with a mean of 72 minutes. The case which underwent concomitant plasty of the left coronary orifice and aortic valve replacement required 122 minutes' aortic cross-clamping. The true lumen diameters were measured by means of angiogram, CT, UCG, but their values were all larger than the actually utilized graft diameters.

We implanted the Gore-Tex reinforced grafts as below-knee bypass for four cases of ASO, and as a popliteal artery replacement for a case of poplitecl pseudo-aneurysm due to Beget's disease. All grafts are now well patent at the time of three to seven months after implantation.
Post-operatire graft angio graphy on knee-flexion showed no fold on the graft, which is usually found on the non—ringed Gore-Tex graft on knee-flexion.
This reinforced graft should be expected to be used as a “beyond-joint” bypass and a thovacic venous prosthesis.

Φ5mm Gore-Tex graft was implanted to the femoral artery of mongrel dog and Prostacyclin (20ng/kg/m) was infused. The platelet aggregation by ADP 30μg was inhibited and the blood flow was increased.
Prostacyclin decreased the adhesion of platelets and the thrombus formation of the graft, therefore prevented the early occlusion. Late graft failure by intimal hyperplasia is also prevented because Prostacyclin reduses the platelet factor that stimulates the smooth muscle cells to miglate and proliferate.

As previously reported, H-PSD polymer shows excellent antithrombogenecity due to the continuous release of heparin from its surface into blood stream for a longer period.
In this study, an application of the H-PSD polymer to bypass tubes was investigated in the animal and clinical experiments. 3 cases (1 dissecting aneurysm, 2 coarctation of the aorta) was operated on using this bypass tubes. It was found that during and after the bypass the hematologic parameters remained in the normal levels and SEM studies showed no thrombus formation. This H-PSD polymer seems a promising material for the temporary bypass tubes in aortic surgery.

For small caliber artery replacement, modified ureter graft was invented from pig ureter, treated with ethyl-ether and glutaraldehyde. 18 new grafts, 3-4mm in diameter, were implanted in the femoral arteries of 9 dogs. in contrast, 10 Gre-tex grafts of 4mm in diameter were inserted in the other 5 dogs. The new graft had a 56% patency rate at 1 year, whereas Gore-tex had 0%. 1 (5.6%) infected in the new grafts and 3 (30%) infected in Gore-tex graf ts. The new grafts had no decrease nor increase in graft diameter except 1. Histologically, the modified ureter graft had not much changes as calcification or cell infiltration. There was no difference between the 1 year patent graffs and 2 year patent grafts. 3 were obstructed from pannus hyperplasia and one of the patent graft showed pannus hyper plasia though not obstructed that this graft may need some immunosupplessants or antiplatelet agents for better results.

Arterial reconstructive surgeries were performed in 101 patients using double velour Dacron prosthetic grafts. Velour Dacron prosthetic grafts have given excellent results as large sized artery replacement and bypass. It appears to be an acceptable vascular substitute for surgical procedures in the aortoiliac resion. On experimental studies of systemic-pulmonary shunts using small caliber grafts (3mm diameter) in dogs, the patency rates of velour Dacron grafts were superior to those of PTFE grafts.

Various methods of shunt operation are available for gaining blood access in hemodialysis, among, which internal shunt is in most favorite use nowadays. We make it a practice to perform a side-to-side anastomosis of the radial artery and radial cutaneous vein somewhat proximal to the wrist on the opposite side of the whip arm as the procedure of first choice. Unfortunatery, however, there are cases in which the vein is too small to ensure sufficient blood flow to accomplish hemodislysis or in which, after the surgical creation of a shunt, the formation of a varix, or even occlusion, results from frequent puncture. In such an instance we have used a vascular prosthesis. The purpose of this paper is to descride our experience with newly developed 4-6 tapered Gore tex graft in the creation of A-V fistula. The procedure consists of anastmosing a prosthesis of 4mm diameter with the artery and another of 6mm diameter with the vein. The prostheses provided a satisfactory run off with an inproved patency rate and thus proved to be effective in the creation of A-V fistula under unfavorable conditions.

Braided tubes were constructed evenly and processed by means of alkaline reduction method to produce porosity. 4mm I. D. and 12-18cm long grafts were implanted to bypass from left subclavian artery to coeliac trunk by end to end fashion in the chest cavity of dogs. Porosity of the stretched graft is minimal enough to prevent bleeding through meshes at the time of implantation and biological porosity will be produced as the graft shortening develops in a long time. 40% of patency was obtained in the group of 65%-reduction as well as in the control 5-7 months after implantation. Improved patency rate will be obtained by approximation of graft compliance to that of the host artery.

Ascending aorta replacement under extracorporeal circulation is often associated with uncontrollable bleeding through the prosthetic wall in spite of the preclotting with fresh blood. Instead of the preclotting with blood, vascular prostheses were pretreated with fibrin glue and clinically utilized. Fibrin glue consists of 2 components; highly concentrated fibrinogen as the substrate and thrombin, calcium and aprotinin as the thromboplastic substances. The high concentrated fibrinogen solution was firstly spreaded, then, the solution of thromboplastic agents was applied to the outer surface of prostheses. After a few minutes a thin fibrin film developed. The pressure tolerance test was performed and leakage through the pretreated prostheses was not seen under the pressure of 300mmHg.
In three patients undergoing ascending aorta replacement under extracorporeal circulation the pretreated Dacron grafts were implanted and there was no blood loss across the prostheses. It is concluded that the fibrin glue is available for the vascular surgery.

Among seventy two arterial reconstruction using Biograft
R 3 late failures developed from 30 months to 5 years postoperatively. Reoperation of all three showed severe pannus hyperplasia at the anastomotic sites. In microscopic examination of the Biograft specimens, the layers were compressed, but the basic architecture remained intact.

The extraanatomic bypass procedures from the left ventricle to the descending thoracic aorta using the valved conduits graft were performed in the experimental animal with supravalvular and subvalvular aortic stenosis. The effect of Apico-Aortic Bypass procedure on the left ventricular fuction, coronary blood flow and oxygen consumption was compared between in the suprava lvular stenotic group and subvalvular stenotic group. These Procedures provided significant decrease of the left ventricular loading in the both supravalvular, and subvalvular stenotic groups. However, Apico-Aortic Bypass significantly increased the coronary blood flow in the subvalvular stenotic group, and on the contrary, decreased the coronary blood flow in the supravalvular stenotic group.

Sutureless vascular prosthesis with quick connector was utilized for outflow side of total artificial heart surgery in order to saving time and decrease surgical bleeding. Four calves were operated with this method. Cardiopulmonary bypass time was shortened significantly and there was less postoperative bleeding compared with suture method. Immediate postoperative course was satisfactory in all calves and one survived 34 days which was the longest in our laboratory. There was no discouraging findings macroscopically and histopathologically in aorta and Pulmonary artery.

Elevation of CVP in a total artificial heart (TAH) goat was studied from the view point of venous conditions. Increase of venous tension was observed in a TAH goat in comparison with a normal control.
Although other vital conditions were quite well, the result indicated the difficulty of controlling the venous tone in a TAH goat by pump output alone.

When the blood is drawn from the cardiovascular tract into pulsatile pump, sufficient flow rate cannot often be attained, because the collapsible venous wall clings to the cannula entrance and the contraction of the flow path occurs. To avoid this contraction, we have designed a new cannula entrance which has the cage (made of stainless steel wire) with larger amount of area of side apertures. The difference between pulsatile and non-pulsatile circulation on the three awake goats, whose hearts were substituted by the artificial heart, was studied. The goats had two sac pumps (SAC P.) initially and the only left sac pump was switched to a centrifugal pump (C. F. P.) 1-2 weeks later. If the non-pulsatile circulation could not be continued the centrifugal pump was replaced with a sac pump again. One goat survived for 51 hours, increasing right pump flow by 20mL/Kg/min to 110mL/Kg/min in non-pusatile circulation. The renal function was within normal limit but total systemic resistance was increasing during non-pulsatile circulation.

Nonpulsatile biventricular bypass during chronic ventricular fibrillation. Chronic nonpulsatile blood flow was established in 4 animals, ranging from 34 to 93 days. Two centrifugal type nonpulsatile pumps were used to bypass both ventricles which were kept fibrillated during the entire course of experiments. After acute responses during the first 7 to 10 days postoperatively, hemodynamics returned to normal and afterwards all organ function was maintained within normal ranges, provided adequate flow and pressure were maintained. Therefore, nonpulsatile blood flow is not a limitation for animal's survival.

During LVB, left ventricular support was sufficient from the viewpoint of bypass flow, decrease in myocardial oxygen consumption, TTI reduction and decrease in LV max. dp/dt. When LVB was applied on failing heart, increase in cardiac output was observed. BVB was effective in terms of the left and right ventricular decompression. BVB was also effective on failing heart in terms of decompressing the right ventricle. During ventricular fibrillation, bypass flows were not sufficient to maintain systemic, pulmonary and coronary circulation.

The performance of an intra-aortic balloon during IABP was successfully monitored by the newly designed device, “IABP MONITOR MODEL-2”, which computed and displayed the change in the actual volume of the balloon, with input of the change in aortic pressure and transthoracic electrical impedance, etc. Additionally, compliance of the aorta was obtainable using this device.

The most adequate way of the mechanical assist for cardioplumonary failure was studied in endotoxin induced shocked dogs by V-A bypass with oxygenator combined with or without IABP. Hemodynamicaly, IABP was as useful as V-A bypass. But V-A bypass, perfused from carotid artery, with IABP was the most effective. As far as blood gas analysis was concerned, IABP alone was no effect in comparison to V-A bypass. Porous glass beads have been evidenced to show specific adsorbability to gamma globulin in spite of very weak adsorbability to albumin in aextracorporeal circulation system. The adsorbability of 150g of the glass beads to 2000ml of the separated plasma was known to be saturated approximately 60 minutes after a start of the perfusion, but regain after being rinsed with physiological saline. An apparatus in which two glass beads adsorbent columns are alternately perfused with blood and rinsed with saline in order to un interruptedly conduct the adsorption has been produced and investigated for the performance. The experimental results show that the columns satisfactority maintain the adsorption and can be unitized in clinical practice in terms of the adsorbability.

How polymyxin-B binds to and inactivates endotoxin was studied. Antibacterial property of 30μg of PX was inactivated by 500μg of ET (extracted from S. min R. 595), while the biological activity of 4μg of ET was inactivated completely by 40μg of PX. We developed the new system with our idea that is to controll the inlet pressures of double tube pump to drain and return blood by automatic pressure controller, improving our “Handy type Plasma Exchange Hemofiltration System in 1979”. It is 250mm Width, 350mm Hight, 366mm Depth and about 15Kg Weight. It contains new type of blood pump, heparin infuser, T. M. P. and Venous pressure monitors with upper and lower limit alarms, bubble detector. Results of in vitro and in vivo examinations are satisfied with safty use and various applications. Deviation between substitution and filtrate volume is -2.93% averaged 3 times in vivo examinations (filtrate: 2000ml).

We tried plasmapheresis on different cases of SLE. Case 1 was a CIC positive case with an acute lupus nephritis. Plasmapheresis combined with hemodialysis caused the urinary output to increase again and the serum creatinine coefficient to drop. And attempt was made to drop hemodialysis. Case 2 had an acute case of renal failure. But it was CIC negative. No improvement was observed with regard to urinary output and serum creatinine coefficient. Case 3 showed Raynaud's phenomenon and marked skin abnormalities. This case was CIC positive. With plasmapheresis, CIC turned negative and skin abnormalities disappeared. These results indicated that, as reported in a number of studies, plasmapheresis is effective for CIC positive cases in SLE. And the combination therapy with steroid is necessary.

Continuous cryofiltration (CCF) was performed a total of 821 sessions on 26 rheumatoid arthritis patients. The clinical effectiveness of CCF was defined according to the criteria described by Wallece et al. By these criteria, patients were evaluated as follows: Excellent 9 patients (34.6%), Good 4 patients (15.7%), Fair 7 patients (30.0%), Poor 5 patients (19.2%), Undefined 1 patients (3.8%). 20 out of 26 patients showed clinical improvement. A study of clinical response in relation to the number of treatment of CCF performed showed that favorable responses tended to be observed more than 25 sessions. In the examination of immunological data, no significant differences were observed in the course of CCF treatmentt for all patients. In cases of A-V fistula occlusion, the superficialization of brachial artery is recomended for blood access. We have not a fatal complication in the 821 sessions of CCF therapy. CCF is a safe, inexpensive and effective for the treatment of the rheumatoid arthritis therapy.

Plasma exchange (PEx) was performed in 20 patients with collagen disease. In SLE, various symptoms were improved immediately following PEx except for nephrotic syndrome secondary to lupus nephritis. As circulating immune complexes (IC) were frequently detected in patients with clinical effect, it seems to be that the positive IC is one of the indication for PEx. In PSS and MCTD, severe Raynaud's symptom was improved but there was no change on the other clinical features. As to PEx for Raynaud's symptom, plasma viscosity may be useful parameter of this procedure. When the results of PEx with double filtration plasmapheresis and centrifugaton method were compared, the advantage of the former was able to reduce the amount of albumin for substitution significantly.

The plasmapheresis with plasma exchange was performed for 71 times upon 24 patients using Haemonetics model 30. Thirteen of them suffered from postoperative hyperbilirubinemia and were included in the study. The plasmapheresis was applied upon these hyperbilirubinemic patients in order to reduce serum bilirubin level. The plasmapheresis seemed to be effective for hyperbilirubinemia following hepatectomy but not to be effective for hyperbilirubinemia among multiple organ failure patients. Therefore the indication of the plasmapheresis should be considered cautiously since it needs huge amounts of fresh frozen plasma.

It is now gradually accepted that the sera of cancer patients contain the immune inhibitory factors which suppress a number of lymphocyte functions in vitro, increasing with the progress of the disease, Plasma exchange seems to be useful and adequate procedures to dilute or eliminate these inhibitory factors. In this paper, plasma exchange was combined with immunochemotherapy of the patients with advanced cancer and its effect was discussed. Nine patients underwent plasma excharge. Five of them had an partial response documented one or two weeks after the first plasma exchange, and various immunological parameters were remarkably improved. Combined treatment of plasma exchange and immunechemotherapy might be beneficial on cancer patients.

In the cryofiltration the macromolecule filters having the pore size 0.2 need the cooling at 4°C. It is able to treat with cryofiltration using macromolecule filter at 15-25°C which have the pore size between 0.1 and 0.08. But the removal ratio has been influenced by the filtrate temperature. It is good for macromolecule filter having pore size 0.2 to treat with below 4°C in the two way method, also it is the best for macromolecule filter having 0.1-0.08 to treat with between 15-25°C in recirculation method.

A new immunoadsorbent (I-02), adsorbing rheumatoid factor and immune complex, has been developed. Batchwise adsorption studies revealed that I-02 is more efficient than Protein A- Sepharose CL4B. During ex vivo I-02 plasma perfusion in the dogs, coagulation and fibrinolysis factors were almost constant. Blood biochemical parameters were within normal limits. Platelets aggregation value is almost constant. I-02 plasma perfusion was carried out in the patient with chronic rheumatic arthritis with satisfactory results.

The purpose of plasmapheresis is to remove antigens, antibodies, immune complexes, complement and coagulation factors in the treatment of renal transplant rejection. However, unnecessary removal of immune regulatory factors may simultaneously occur and stimulate immune reaction to result in rebound after plasmapheresis.
Suppressor and growth soluble factors (kinds of lymphokins), well known as regulators in immune responses. In vitro study demonstrates adsorbability of 65 materials to suppressor and growth soluble factors. Of these, Amberlite XAD-2 shows large capacity of adsorbability to growth soluble factors, but not plasma proteins. Adsorbability of Amberlite XAD-2 to growth soluble factors which is contained in suppressor soluble factors is confirmed by the experimental result that suppressive effect is sharply incresased from 19.2 to 59.0% after 10 minutes incubation and to 73.0% after 30 minutes incubation. Irradiation to lymphocytes by Ultraviolet (UV) ray has strong capacity of immune suppression, and we studied suppressive effect of UV with irradiation time, wave length and its intensity.

Relation between blood compatibility of sputter deposited metals and their surface charge, wettability and roughness was investigated. Materials tested were: (1) titanium (Ti), nickel, copper and aluminum and (2) composite metal of Ti and the above metal. Blood compatibility of the materials was evaluated by the in vitro “kinetic test”. Findings were: (1) Blood compatibility of the metal, which is low in the electromotive series was better than that higher in the series. (2) Antithrombogenecity of the composite metal was better than that of Ti. (3) No good relation between blood compatibility and wettability or surface roughness was obtained.

Glass coverslips and plastic sheets for tissue culture were coated with dilutions of the solution of three different polymers. These films varied between 4 and 6000nm in thickness. The coated specimens were measured for contact angle, and cultured cells and platelets were incubated on the specimens for observation of their morphological change. Coating thickness of more than 400nm was necessary for the coated polymer to be independent of the base substrates.

Collagen is a major component of the extracellular matrix in living tissues and can influence the adhesion, morphology and migration of cells and in some cases, differentiation. On the other hand, since mucopolysaccharides (glycosaminoglycans) possess multiple anionic charges and have strong affinity for water, the resulting water layer may inhibit adhesion of platelets to the surfaces. For this reason, a hybrid matrix consisting of collagen and mucopolysaccharide may be a blood- and tissue-compatible material. Our results indicate that collagen/hyaluronic acid matrix markedly inhibits platelet adhesion, furthermore, the proliferation of fibroblasts on collagen/hyaluronic acid matrix as well as on collagen matrix is well.

To develop a new biomedical polymer, it was suggested that the most important point was how naturally to induce a function and ability of the living body by the polymer. The living body has a nature of adaptability and restoration ability. Many of the artificial organs have been accepted to the living body by the nature. For example, against several types of vascular prostheses and grafts, the living body accepted them, however, the living body had made efforts to accept themin its own way.
So that. if the living body falls into a poor condition, the limit of the adaptability of it becomes narrow, and the artificial organs resulted to be functional disordered So we have develop a new materials which do not forced the living body to accept it.

We have already experienced the many complications such as sepsis, abscess, infection etc, when the postoperative hemorrhagic effusion failed to be eliminated from thoracic or abdominal cavity through the drainage tube. the main cause of its failure is based on the occlusion of drainage tube, due to the thrombus formation. This report presented the possibility of clinical application of new polyvinyl chloride drain, on which urokinase was immobilized via Gantrez. The urokinase-treated drain had the high anithrombogenicity and followed the superior clinical results.

Poly (2-hydroxyethyl methacrylate), polyurethane, (polystyrene)-(ethylene/butene-1) block copolymer, and polysulfone were implanted subcutaneously in rats, hamsters, and guinea pigs. The tissue response and tumorigenesis caused by the polymers were compared among the three species of the animals. Thick fibrous capsule formation and high tumor production were noted only in rats, while no tumorigenesis and thin fibrous capsule formation were observed in hamsters and guinea pigs.

In ECMO, it will be indispensable to control the gas exchanging rate in artificial lung automatically to keep the outlet blood gas at appropriate level independent of the inlet condition. However, a simple feedback control is not applicable for this purpose because it tends to lead the system unstable. To realize the stable control with good transient characteristics, the new control method where a feedback controller is combined with a feedforward controller was designed. The computer simulation and animal experiment revealed the feasibility of this newly developed control method. A device which supplies the desired Po2 and Pco2 in the gas was useful in this study.
