    623 cases of esophagoscopy have been experienced in our clinic for the past one and half years. Many illnesses were found, such as cancer, non-specific ulceration, varices, diverticula, esophagitis, cicatrical stricture, etc. In about half of these cases, esophago-fiberscope (Olympus EFS) was used. No complications have been experienced yet.
    Some studies of the esophago-fiberscope will be reported from the above experienced cases. The insertion by the fiberscope is much easier and the patients experience less pain.
    Upon direet examination, the findings of the mucosa could be enlarged by manipulating the lens. Especially, the diagnosis of the lower part of the esophagus could be easily done. The fiberscope which could be flexed at an angle at the distal end, makes possible the ability to see the whole portion of the esophago-cardio-junction. Because of the flexibility of the fiberscope, the patients with marked kyphosis and patients with heart failure and varices, it could be inserted safely.
    On the contrary, the dysadvantages of the esophago-fiberscop are that it can not remove the foreign bodies and dilate the narrowed portion of the esophagus. At present, the photograph of the esophgo-fiberscope is less clear than that of the rigid esophagoscope.
    We hope the advantages of the esophago-fiberscope is to spread the esophagoscopic examination which will result in the early diagnosis of cancer of the esophagus.

    21 cases of carcinoma of esophagus, which were treated by us for the past 7 years, were reviewed.
    15 cases of these were treated with CO60 irradiation, 7 of which were treated further with surgical operation after 3-4 weeks.
    Repeated cytological examinations were done during the irradiation, and it was recognized that the effect of the irradiation became apparent when 1, 000r. or more was given. Also recognized was that the preoperative irradiation was useful.

    The present study of electro-intraluminal pressure measurement of the esophagus conducted in the author's department was reported, and the resting pressures in normal individuals or in patients with achalasia, cancer of the esophagus or cardia of the stomach, regurgitative esophagitis or heatus hernia were obtained by using the drawing method and their characteristics were discussed.
    The drawing method is beneficial on researching the mechanism of disturbance of deglutition and has a great significance as a diagnostic tool.

    The authors have already reported the excellent result obtained by glottic reconstruction with Conley's method procedure in partial laryngectomy.
    The special feature of this operation is to reconstruct the larynx temporarily by covering the defected area of the extracted larynx with frontal neck skin flap previously prepared.
    Five patients of laryngeal carcinoma treated with this operation, one died of another disease but the remaining four are presently in good health.
    The reccurrence of carcinoma cannot be recagnized in two of them after five years.

    During the period of full five years from June, 1961 through May, 1966, we extracted the patients with chief-complain that seems to relate with the gullet among the outpatiens who visited the National Nagoya Hospital and examined them comparing the otorhinolaryngology, surgery and internal treatment.
    1. The rate of patients of esophagenal disease against the total number of outpatients is 0.7per cent in the otorhinolaryngology, and less than 0.1per cent in the internal treatment and surgery. The phenomenon of increasing the nervous disease were observed though small number, and the cases of extraneous showed the phenomenon of decreasing year after year.
    2. In regard to the chief complain, the defficulty of swallow is decreasing in numder but the increase of stick-feeling of food in the throat may have related with the diagnosis nervous disease. Besides, such nervous disease is occupied mainly by their twenties and the morbid fear is increasing among the young generation.
    3. Among chief compalin regarding the gullet, there were patients other than the gullet, on the contrary, those patients whose chief complain is not the gullet were the esophagenal disease. For diagnosis and treatment, we recongnized the fluidity of internal treatment and surgery, especially the meaning of trachea esophagus.
