  • 超音波パルスドプラ法による血流パラメータと血流パターンおよびスコア解析

    Arteriosclerosis is regarded as the main cause of cerebrovascular and ischemic heart diseaes. Recently, it has became possible to evaluate the severity of arteriosclerosis without interventional methods. We measured blood flow in radial artery with the US pulse doppler method in the region of wrist and elbow joints, and this procedure is considered to be significant and useful for the diagnosis of arteriosclerosis. This study included 15 arteriosclerotic cases and 78 healthy cases. The arteriosclerotic group indicated a significant stenosis on the coronary angiogram, on the other hand there was no stenosis in healthy cases. Sixteen cases were selected from the healthy group having similar aging distribution and were named as the control group. We used a US pulse doppler method for measuring blood flow parameters comprised of peak systolic velocity, mean systolic velocity, mean velocity, dicrotic notch, peak diastolic velocity, mean diastolic velocity, end diastolic velocity,pulsatility index and resistance index, and detected blood flow patterns. Blood flow patterns were classified into type I to III by the shape of a S2 wave. Besides, we calculated scores from the parameters and patterns indicating a good correlation with aging and a significant difference between arterioscrelotic and control groups both of wrist and elbow joints. In this study, we have evaluated the relationship with aging in 78 healthy cases and the differences between the arteriosclerotic group and control group, by analyzing blood flow parameters, blood flow patterns, and the scores resulting from. Results: 1) Both in the region of wrist and elbow joints, dicrotic notch, peak diastolic velocity and mean diastolic velocity at the diastolic period were well correlated with aging, and showed significant difference between the control and arteriosclerotic groups. 2) Our results indicated that blood flow pattern had the tendency to change from type I to type III with aging, and that type III occupied the highest percentage in the arteriosclerotic group. 3) The score showed better correlation with aging and more signficant difference between the two groups than within the parameters themselves. In conclusion, evaluating the score by the Pulsed Doppler Ultra-sonography is useful in the diagnosis of arteriosclerosis.
