Purpose: Intestinal malrotation is diagnosed and treated mostly in infancy but rare in adults. In this study, 10 cases were diagnosed with adult intestinal malrotation. Among them, 2 cases indicated that ultrasonographic examination was useful for detecting adult intestinal malrotation. This study reviews the availability of utrasonographic examination from these diagnosed cases with including some additional points of bibliographic consideration.
Methods: 10 cases were diagnosed with adult intestinal malrotation from March 2002 to June 2010. They were reviewed according to the following categorization data: gender, age, diagnostic clues to intestinal malrotation, type of initial examination, and intestinal malrotation based on Wang Classification.
Results: In the 10 cases, the gender was divided into 60% for male and 40% for female and the average age was 40. Diagnostic clues were based on 20% for the detection of midgut volvulus accompanied by intestinal malrotation and the rest of 80% were detected during screening test or further examinations. Initial examination included 70% for ultrasonography, 20% for computed tomography (CT), and 10% for contrast barium enema examination of inferior digestive tract. Type of intestinal malrotation was 70% for non-rotation and 30% for mal-rotation type.
Discussion: According to Wang Classification, intestinal malrotation is classified into 4 types: non-rotation, mal-rotation, reversed-rotation and paraduodenal. It is reported that 66.7% for adults resulted in non-rotation type according to Chiba and 70% for those by Matsumoto. These results corresponded with the outcome we obtained. In the diagnostic clues, according to Seki, 34% of patients were accidentally found during examining or treating other disorders. However, our result showed much higher, 80%. The reason for the higher result is because there were many findings from the sign obtained by ultrasonographic examination.
Conclusion: In order to detect adult intestinal malrotation, it is important to understand embryologic comprehension of the normal intestine, classification of intestinal malrotation and its anatomical characteristics. Moreover, intestinal malrotation is rare in adults, however, it is significant to always keep in mind this disease.
