    During the past ten years, the authors treated a total of 15 patients with early squamous lung cancer in the hilar region. Of these 15 patients, five were detected by mass surveys for lung cancer and 10 presented with symptoms related to lung cancer such as fever, cough, sputum or bloody sputum. We report the results of clinicopathological studies of these 15 patients.
    Cytologic examination of sputum was performed in five cases detected by mass surveys for lung cancer. Cytologically, 3 patients were diagnosed as suspicious and 2 as positive. Four of 5 patients were roentgenographically diagnosed as “occult” lung cancer. Macroscopic findings of five patients revealed superficial infiltration in 3 cases, nodular tumors in 2 and polypoid tumors in none.
    In the 10 symptomatic cases, cytologic examination of sputum was performed in 8 cases. Cytologically, two patients were diagnosed as suspicious and 4 as positive. Only three of 10 patients were roentgenographically diagnosed as “occult” lung cancer. Macroscopic findings of 10 patients revealed superficial infiltration in 2 cases, nodular tumors in 5 and polypoid tumors in 3. None of 3 cases with polypoid tumors were diagnosed as suspicious or positive by cytologic examination of sputum.
    We conclude that it is occasionally difficult for cases of polypoid type early lung cancer in the hilar region to be detected by mass survey for lung cancer.

    From August, 1968 to December, 1989, 58 patients with thymoma, and 3 with thymic carcinoma were treated by radiotherapy using cobalt-60 gamma ray. Eleven cases were treated by radiotherapy alone, 1 by preoperative radiotherapy, 45 by postoperative radiotherapy, and 4 in combination with intraoperative radiotherapy. In thymoma, postoperative and intraoperative radiotherapy were effective, while concerning postoperative radiotherapy, operability was the major factor influencing survival and local control, and Stage I and II tumors resected totally or subtotally as well as Stage III tumors resected totally were good indications for such therapy. Cases of thymoma complicated by myasthenia gravis had a longer survival time and better local control rate than those without it. In the treatment of thymic carcinoma, it was suggested that the tumors can be controlled using complete resection and sufficient postoperative radiotherapy.

    The prognosis of 537 lung cancer patients who were 70 years old or over, was compared to that of 1319 patients whose age was younger than 70. In resected cases, the 5-year survival rates in patients who were younger than 70, 70-79 and 80 or older, was 50.5, 48.8 and 42.9%, respectively. There was no statistical difference in survival among the above age groups.
    In unresected cases, the 5-year survival rates in elderly patients with stage I disease was only 13.5%. There was no statistical difference in survivals between those treated classified by supportive care, chemotherapy, radiotherapy or combined therapy.

    The usefulness of adjuvant chemotherapy in patients with pulmonary carcinoma undergoing curative resection was jointly studied by 30- institutions in Wester Japan. The results of this trial after a 5-year follow-up are reported here.
    The subjects were patients with pulmonary adenocarcinoma who underwent absolutely or relatively curative resection. They were treated with 20+10mg MMC, plus 600 mg tegafur orally for one year (Group A) or with 20+10mg MMC, plus 400-600mg UFT orally for one year (Group B). Between November 1982 and November 1985, 224 patients received this therapy (113 in Group A, and 111 in group B). Excluding 43 cases which were judged inappropriate for evaluation, 88 patients in Group A and 93 in Group B were evaluated. There was no significant difference in any background variable between the two groups.
    The overall 5-year survival rate did not significantly differ between Group A (64.3%) and Group B (55.6%). The overall 5 year disease-free rate also did not significantly differ between the two groups. In stratified analysis, N2 cases in group B had higher 5 -year survival and recurrence-free rates than those in Group A (p=0.029 and 0.048, respectively).

    In some lung cancer cases, it is known that there is significant granulocytosis due to the production of colony-stimulating factor by the tumor itself. Recently, the authors encountered two cases of primary lung cancer with significant thrombocytosis (>100×104/μl) and mild granulocytosis. In the first case, adenocarcinoma, we analyzed thrombopoietic factors in the conditioned medium of the tumor cell line from the malignant pleural effusion of this patient, and showed that the conditioned medium contained activity of GM-CSF and IL- 6. In the second case, anaplastic large cell carcinoma, we analyzed the patient's urine and found activity of M-CSF, GM-CSF and IL- 3. Measurement of IL- 6 activity was not done in this case. These data may suggest that the combination of some cytokines, including M-CSF, GM-CSF, IL- 3 and IL- 6, is important in the regulation of the production of the megakaryocyte/platelet lineage.

    The cytotoxic effects of peripheral blood mononuclear cells (PBMC) and regional lymph node mononuclear cells (RLNMC) in 31 primary lung cancer patients were studied following administration of OK-432 and/or recombinant interleukin-2 (rIL-2).
    When PBMC or RLNMC were cultured with OK-432 or rIL-2, the cytotoxic activity of the induced cells increased, but the cytotoxic activity of the cells after simultaneous stimulation of both OK-432 and rIL-2 was lower than that with rIL-2 only.
    When mononuclear cells (MNC) were pretreated with indomethacin before the addition of both OK-432 and rIL-2 together, the cytotoxic activity of the induced cells increased compared to cells that did not receive indomethacin pretreatment. This means that MNC cultured with OK-432 and rIL-2 together produce prostaglandin E2 (PGE2), which suppresses the effects of rIL-2 on MNC.
    In contrast, when PBMC were first cultured with rIL-2 and then with OK-432, or when RLNMC were first cultured with OK-432 and then with rIL-2, the cytotoxic activity of these cells was equal or greater in comparison with cells cultured with rIL-2 only.
    These results suggest that a certain combination of OK-432 with rIL-2 might be an effective method for generation of killer cells for clinical use.

    Two cases of severe tracheal stenosis due to advanced nonresectable adenoid cystic carcinoma of the trachea were reported.
    Case 1 was a 38-year-old male who underwent emergency 60Co irradiation following endoscopic YAG laser irradiation. This approach was quite effective. Proton irradiation was effective against tumor recurrence.
    Case 2 was a 28-year-old female in whom emergency treatment for tracheal obstruction due to edema after laser therapy necessitated transient tracheal intubation. The tracheal stenosis due to recurrent tumor after 60Co irradiation was improved by insertion of an intraluminal permanent stent.

    Two cases of primary mediastinal seminoma were presented. Both chest radiograms revealed huge anterior mediastinal masses involving mediastinal great vessels. The patients responded to intermittent combination chemotherapy including cisplatin and etoposide. They had received adjuvant irradiation of the residual tumors, but no striking reduction in size of the remaining tumors was noted.
    Both patients underwent surgery because of the residual masses. Tumor extirpations were carried out and the great vessels were reconstructed with prosthetic grafts. Almost all the resected specimens of both cases revealed fibrotic changes surrounding tumor necrosis. In the lesion of case 1, a few viable tumor cells existed in the lymphatic ducts and capillaries, but in case 2, no viable tumor cells were detected.
    It was considered that intermittent combination chemotherapy including cisplatin and etoposide is acceptable as the initial therapy for advanced primary mediastinal seminoma, and radical excision of the residual mass after chemotherapy is necessary.

    A 27-year-old male was admitted to our hospital with recurrence of left intrathoracic tumor. Emergency left thoracotomy and tumor extirpation was performed, and histological examination revealed malignant hemangiopericytoma. Repeated extirpation of tumors, left pneumonectomy accompanied by partial resection of the pericardium and diaphragm, and also resection of the left lateral chest wall were performed successively for the recurrent lesion. Neither chemotherapy using a combination of Cisplatin, Doxorubicin hydrochloride and Cyclophosphamide nor Linac irradiation yielded effective control of the tumor growth. He died of disseminated metastases in the right lung and disseminated intravascular coagulation syndrome nine months after the first operation.
    Concluding from the operative findings that the mass was continuous with the mediastinal pleura and since no intrapulmonary lesion was present, the tumor seemed to be of mediastinal origin.

    A case of X -ray negative peripheral lung adenocarcinoma diagnosed after two-years of follow up after sputum cytology indicated malignancy is reported.
    A 69 year-old man with slight lung fibrosis was diagnosed as adenocarcinoma of the lung in right S10. Right lower lobectomy was performed. Histologically, a diagnosis of bronchiolo-alveolar cell carcinoma (BAC) was established.
    Since most cases of BAC do not show any typical tumorous shadow, preoperative diagnosis is difficult. BAC is considered to be a type of peripheral lung adenocarcinoma which can be detected by careful screening of sputum cytology during the phase of minimal change on chest X -ray film.

    A 67-year-old man showing a mass shadow with cavity formation on chest- X ray was admitted to our hospital. Although a preoperative diagnosis could not be obtained, left upper lobectomy was performed. Microscopically the cavity wall of the tumor consisted of moderately differentiated squamous cell carcinoma and the solid part of the tumor showed mucoepidermoid carcinoma. Both histologic types coexisted and both tumor cells were mixed in some areas. We diagnosed this tumor as adenosquamous carcinoma clinicopathologically.

    Elevation of serum CEA levels was recognized in a 74-year-old male case of resected stage I adenocarcinoma of the lung in whom liver metastasis was detected postoperatively. Following 5 cycles of chemotherapy (MVP: MMC + VDS + CDDP) in addition to recombinant human granulocyte colony-stimulating factor (rhG-CSF KRN8601) partial response was recognized on abdominal CT but CEA levels returned to normal. These results suggest that dose-intensive chemotherapy combined with rhG-CSF can yield control of liver metastasis of lung cancer.

    A case of mediastinal malignant fibrous histiocytoma is reported. A 60-year-old man developed cough and dyspnea one month before and was admitted as an emergency case with a diagnosis of left heart failure. A chest X-ray film showed a large mediastinal mass, which expanded at extraordinarily high speed. We could not diagnose this case before death, but active chemotherapy (CPA, VDS and ADM) was administered. However, he died, three days later of multiple organic failure.
    Autopsy examination revealed a large L-shaped posterior-mediastinal tumor invading into the right lung, and histologically the lesion was diagnosed as malignant fibrous histiocytoma.

    A 51-year-old man was admitted to our hospital complaining of anterior chest pain. Plain chest X-ray showed multiple nodules in bilateral lung fields, and thoracic CT showed an anterior mediastinal mass. Whole body bone scintigram revealed abnormal accumulation in the cervical and lumbar vertebra, sternum and right iliac bone, suggesting multiple bone metastases. Exploratory thoracotomy yielded a histological diagnosis of thymoma with pulmonary metastases. He was treated with repeated combination chemotherapy and radiotherapy. Because thymomas with distant metastases at the time of diagnosis are rare, we reported the case and reviewed the literature.
