  • I. Beobachtung infolge der Zerstörung des einseitigen Labyrinths bzw. Kleinhirns beim Kaninchen

    Zusammenfassung
    Nach der abgesonderten Zerstorung der sogen. Vestibularisteile beim Kaninchen wurden hauptsachich die Augapfelhewegung und Korperstellung untersucht.
    Bemerkenswerte Symptome bei Lasionen
    1. des Labyrinths:
    Zuerst deutlich schiefer Spontannystagmus im engeren Sinne zur gesunden Seite in alien Fallen, mit Kopfnystagmus in einigen Fallen. Im Laufe der Zeit veranderte er sich vertikal oder horizontal. Sehr deutliche Ataxie nach der operierten Seite ( im Ablaufe der Frist keine Kompensation). Nach dem Verschwinden des Spontannystagmus im engeren Sinne erschienen ein vertikaler Lage-bzw. Lagerungs-Nystagmus. Wenn der Lagnstagmus erschien, war er immer richtungsbestimmt.
    2. des Kleinhirns : a. Kein Spontannystagmus im engeren Sinne und keine Ataxie b. Lage-bzw. Lagerungs-Nystagmus konnten bei Lasionen von Kleinhirnhemisphare (Pareocerebellum and seinem Ubergangsteil zum Neocerebellum) and Vermis (bis auf Nodulus, Uvula und Pyramis) beobachtet werden. Bei Jenem war häuptachlich ein vertikaler Nystagmus und diesem ein rotierender.
    c. Bei Läsionen von Nodulus, Flocculus, Paraflocculus oder Area temporalis des Grosshirns waren weder Spontan-noch Provokations-Nystagmus zu beobachten, wahrend solche Teile bisher viel ofter betont worden sind, auf die Enthullung des Lagenystagmus auszuiiben.
    3. Es ist ziemlich sicher, dass bei einseitigen Lasionen des Kleinhirns oder Kleinhirnbruckenwinkels ein Nystagmusuberwiegen zur gesunden Seite erscheint.
    4. Im allgemeinen kann man sagen, wenn ein Spontannystagmus im engeren Sinne vorhanden ist, werden der Lage-bzw. Lagerungsnystagmus weitergehend unterdruckt. Ist dagegen kein Spontannystagmus vorhanden, so wird der Lagenystagmus in Ruckenlage und besonders Kopf-Hange-Lage deutlich. Die Nystagmusbefunde beim Kaninchen and Menschen konnen sich miteinander direkt gar nichts vergleichenlassen, denn der Lage der Augäpfele am Kopfe bei beiden ist ziemlich verschieden.

    Various nonspecific phosphomonoesterases in the stria vascluaris and spiral ligament of the normal guinea pig were investigated.
    These included phosphomonoesterase 1 or alkaline phosphatase and phosphomonesterase 11 or acid phosphatase. Activities in the vestibular portion of the membranous labyrinth were also evaluated.
    The tendency of that the activity increases from the basal to the apical turns was seen common to all the phosphomonoesterases invesitgated. In the vestibular portion of the membranous labyrinth, macula utriculi showed considerablly high activities.
    Functional significance of phosphomonoesterases was discussed. The activities of the nonspecific phosphomonoesterases were about one tenth of the total enzymatic hydrolysis of ATP in the stria vascularis and spiral ligament.

    Many believe that the vibration conbined with noise promotes occupational deafness but there are few reports to this effect.
    My research concerns with the change of auditory threshold sensitivity in the human body exposed to vibration with noise and witnout it.
    The experimental condtions in this study are based on the consideration obtained from the results of the previous research.
    These findings were determined on the Békésy type audiometer with the normal hearing subjects in a chair shaking vertically who had familiarized themselves with 4000cpm fixed Békésy audiometry.
    The result showed that there was no significant difference between the heaing of vibration exposure and no exposure.
    The threshold-shifts and its recovery seemed to be worse in vibration with noise than in noise alone at 300cpm and 1000cpm vibrations. On the other hand, there was a tendency for hearing to improve at 1300cpm vibrations with added noise.
    From the above results, according the frequ- ency of the forced vibration it is apparent that the vibration-effects on auditory acuity in noise are quite varied.

    1) The various factors which may have influence upon the effect of radiological therapy for paranasal cancer were reviewed in 233 patients who were radiologically treated during the seven years from 1957 to 1964.
    2) The results of radiological treatment were as follows: Ineffective (Recurrence in three months) : 62 cases (27%) Relatively effective (Recurrence in one year) : 96 cases (41%) Effective (No recurrence in more than one year) : 7 5 cases (32%)
    3) Among the factors which may have influence upon the effect of radiological treatment, the extension degree of the tumor, the direction of the invasion, the histological charactor of the tu- mor, the degree of pearlformation and the dose of radiation were found to be significantly correlated with the result of the treatment. On the contrary, the age, the sex of the patients, the primary lesion of the tumor, the expansive type of the tumor and the number of the mitosis were not significant.
    4) The differences of radiation effects were compared in the cases of similar extension degree since the extension degree of the tumor seemed to be the most important factor. The result was that the degree of pearl-formation had a considerable influence on the radiation effect, where as the direction of the extension of tumor and the dose of radiation, if more than 4000R, seemed to have few significance.

    Authors investigated cholesteatoma histologically in the human ear and experimentally prduced cholesteatoma in the rabbit ear, and the following results were obtained:
    The squamous epithelium of the external auditory meatus and the eardrum came to proliferate with the invasion into the inflammatory granulation tissue in the tympanic cavity, and with its augmentation, the invaded epithelium fell into central necrosis and involved an accumulation of the desquamated epithelium, resulting in microcholesteatoma. This microcholesteatoma became developed towards the neighbouring tissue. From the matrix of the cholesteatoma, new chlesteatomas were produced with the same process. Being accompanied by the augmentation of each cholesteatoma, cholesteatomas became confluent to one sack. The immigration theory could be supported by the above results. However, perforation of the tympanic membrane is not always necessary for production of cholesteatoma. Perforation of the tympanic membrane may occur by rupture of cholesteatoma to the external auditory meatus after cholesteatoma grows up in some degree. The existence of the loose granulation tissue in contact with the squamous epithelium is one of the most important conditions for development of the cholesteatoma.
    The squamous epithelium of the external auditory meatus is very likely to invade the loose granulation tissue but hardly the compact connective tissue. The Shrapnell's membrane has no lamina propria, so if the granulation tissue is produced behind the Shrapnell's membrane, squamous epithelium will touch the loose granluation tissue directly. Absence of lamina propria in the pars flaccida is one of the most important reasons why cholesteatoma is mostly found in the epitympanum.
    As squamous epithelium hardly invades the compact connective tissue, flapcholesteatoma is not produced when the fascia, the venous valve or periosteum are transplanted in tympanoplasty.
    The epithleium of the pars flaccida not only invades the granulation tissue with the plug shape, but also, as the entire, comes to grow within the granulation tissue falling into keratinization and desquamation from the surface, so the retruction of the pars flaccida occurs in the cholesteatomtous otitis media.

    Ossification of the human ear in fetal stages was studied by using a soft X-ray apparatus (Softex Type CMB).
    Materials consisted of eight fetuses, from the 4 to 8th fetal month, with normal uterine life in normal pregnant women.
    X-ray photographs of the skull in all fetuses were taken from various directions to find the conditions of ossification of the temporal region.
    Besides, histological specimens of 3 fetuses with staining, were made to compare with the Xray findings.
    Conclusions were as follows:
    1. In Japanese fetuses, ossification of the temporal region first appeared at the tympanic ring in the 3rd fetal month and at the region of the lateral semicircular canal in the otic capsule during the 5th month.
    2. The otic capsule was ossified from the circumference of the lateral and superior semicircular canal ampullae. The posterior canal was most delayed in ossification in the otic capsule.
    3. Ossification of the auditory ossicles became evident from the 5th fetal month. The first ossification appeared at the head of malleus, body of the incus and the obturator surface of the stapes footplate.
    4. At the end of the 6th fetal month, ossification of the otic capsule and the auditory ossicles was mostly completed.
    5. Application of the soft X-ray apparatus to embryological studies was most valuable. Analysis of the details of soft X-ray photographs and its comparison to the histological findings will establish the process of ossification in the human ear during fetal life.

    Since there is a tendency for pseudomonal infection to increase recently in every field of medical sciences, the author attempted to clarify clinical problems of this infection in the ORL field. The results were as follows: Pseudornonas aeruginosa was detected in average 32.6% of the patients with chronic otorrhea in the Department of ORL, Japan Red Cross Central Hospital during the last three years (35.6% in 1963, 37.1% 1964, 25.7% 1965), second to the most frequent staphylococcal infection. Mixed infections were present in 37.6% of the patients with pseudomonal infection. Alterations in bacterial flora from Pseudomonas to Staphylococcus were more frequent than those from Staphylococcus to Pseudomonas.
    Although the color of the area infected with pseudomonas is an important clinical finding, only the gross observation may sometimes make to misdiagnose. One out of 35 strains studied was colorless on King's media. Also some of them showed no coloration depending on the condition of the environment. For example, pyocyanin fraction extracted from No.11 strain was blue in alkali and almost coloreless in pH 5.0-5.1.
    In the comparison of MIC (Minimum Inhibitory Concentration) in in vitro dilution method between Colistin sulfate (Ch-S) and Sodium colistinmethanesulfonate (CL-M), the former was more effective than the latter. CL-M was kept higher concentration in the blood and it showed more rapid absorption and excretion than CL-S. The combination of CL-M with Tetracycline, Chloramphenicol or Streptomycin produced a little additional effect.
    For a conservative therapy, bacteriological detection was planned to perform every week.
    The combination of local administration of CL-S and intramuscular injection of CL-M was more effective than single local administration.
    Cresol and mercurochrome were selected as effective disinfectants for Pseudomonas aeruginosa, but boric acid solution was almost without effect.
    In order to know the origin of infections, the pseudomonal strains from chronic otitis media were tested by sero-typing and phage-typing.
    But no definite conclusion suggesting the cross-in-fection with particular strains was obtained from present study.
