    In the 20-years from 1991 to 2010, we pathologically diagnosed 24 subjects with parapharyngeal space tumors by biopsying and/or resecting of 24 tumors. Of these, 18 were benign and 6 malignant. Pleomorphic adenoma and schwannoma predominated, accounting for 7 cases each.
    Enhanced CT and MRI were very useful in diagnosis, judging involvement and localization, and assessing radiological and pathological tumor diagnosis. Important radiological factors are determining whether tumor origin is pre-or post-styloid, and how the tumor is enhanced in CT or MRI.
    We treated 15 subjects surgically, telling 9 the diagnosis, and used only biopsy or scans. It is vital in follow-up to determine long-term treatment response.

    We report a case of malignant epiglottic natural killer (NK)/T cell lymphoma. A 33-year-old man seen 1-month period for throat pain was found in endoscopic larynx examination to have inflammation with plaques and redness epiglottic. The 4 month period, right epiglottic inflammation showed progressive necrosis. The diagnosis of malignant lymphoma was confirmed by 3 biopsies. Laryngomicrosurgery specimens histologicalily showed moderate leukocytic infiltration mainly of atypical lymphocytes. Neoplastic cells were UHCL1+, CD3+, L26-, CD79a-, and EBER-ISH+. Despite 4 units of DeVIC chemotherapy and regional irradiation, the man died of metastatis 1 year and 9 months after initial treatment. Among malignant laryngeal tumors, malignant epiglottic NK/T cell lymphoma is extremely rare, with only one case reported in the literature.

    Dacryocystorhinostomy (DCR) is done commonly in subjects with chronic dacryocystitis and nasolacrimal duct stenosis, and endoscopic DCR is now an established alternative to external approaches. Mucosal preservation is important in preventing granulation tissue and synechia from forming around the ostium. Our modified endoscopic DCR using a mucosal flap involves creating a large bony opening to form an anastomosis for the lacrimal sac and nasal mucosa. Instead of silicon tubing, we use a silicon sheet to avoid manipulating the canaliculi. Our modifications enable an easy, safe, effective form of DCR easily implemented by otolaryngologists familiar with endoscopic sinus surgery.
