    We evaluated 66 patients (72 ears) with auditory ossicle malformations, but with normal findings in the tympanic membrane, who underwent ossicular reconstruction and stapes surgery at our department between 1984 and 1997. There were 27 men (30 ears) and 39 women (42 ears). Classification of pathologic condition based on surgical findings showed separation of the incur-stapes joint in 43%, fixation of the malleus or incus in 16%, fixation of the stapes footplate in 26%, in incidence of fixation of the auditory ossicles that is higher than that previously reported. In 33% of the patients, conductive hearing loss or mixed hearing loss similar to that obseved on the affected side was also present on the contralateral side. Abnormalities in the arrangement of the facial nerves were noted in 4%, inner ear malformation in 1%, and congenital cholesteatoma in 3%.
    Auditory ossicle reconstruction was performed by the modified type III method in 57% and by the modified type IV method in 37% of the cases. During stapes surgery, 86% underwent small-fenestra stapedectomy. Improvement in hearing after the operation was observed in 90% of the cases and the improvement was relatively high compared with tympanoplasty for chronic otitis media.

    We analyzed statistically the prognosis of hearing impairment in patients with idiopathic sudden sensory neural hearing loss. 119 patients (HBO group) were treated with a combination of hyperbaric oxygenation (HBO) and stellate ganglion block (SGB), as well as oral administration of vasodilator and vitamins. Another 107 patients (control group) were treated with various other therapies. Collectively, the therapeutic outcome in the HBO group was better than in the control group. In addition, the recovery rate of hearing in the HBO group was superior to that in the control group for those cases which had severe hearing loss at the first medical examination, had been seen more than two weeks after onset, and had resisted other treatments. As a result of this study, we conclude that HBO is an effective therapeutic tool.

    We report clinical features in patients with delayed endolymphatic hydrops (DEH) with juvenile unilateral deafness. Among 23 patients with DEH, 15 cases were diagnosed as ipsilateral DEH and 8 cases as contralateral DEH. The distribution of onset age showed two peaks at ages of <30 years and> 40 years. In 80% of the ipsilateral DEH cases. the onset of episodic vertigo was at younger ages. On the other hand, in 75% of the contralateral DEH cases. the onset of fluctuation hearing loss of the contralateral ear was at older ages. Ispilateral DEH and Meniere's disease may show different pathophysiologies. The incidence of domint negative summating Potential in the better-hearing ear was 20% in the ispilateral DEH cases and 60% in the contralateral DEH cases. It is suggested that endolymphatic hydrops is in the better-hearing ear of contralateral DEH.

    We investigated one hundred and fourteen ears of 60 children (8 males, 52 females, aged from 6 to 13 years) with diagnoses of functional hearing loss (FHL), and were not aware of their own hearing loss. Forty nine (81.7%) of 60 cases examined were detected by school screening tests, 6 (10.0%) were referred to our hospital because their families noticed poor hearing responses, and 5 (8.3%) were enrolled because they complained of otalgia or discomfort in the ear. Forty (66.7%) showed only pure-tone threshold loss without complications, and the remaining 20 associated nonorganic disorders. In addition, our investigation tound 11 cases with nonorganic otalgia, 5 (8.3%) with functional visual disturbance, 1 (1.7%) with enuresis nocturna who refused to attend school, 1 with tinnitus, 1 with vertigo, and 1 with tic. Moreover, 11 (18.3%) of the 60 cases were suspected of being in conflict with school and/or home. The Type V Bekesy pattern, which is frequently observed in FHL and it has clinical utility to distinguish FHL from other types of organic hearing loss, was detected in 44 ears (38.6%). Fifty two (45.6%) of 114 ears showed normal pure-tone thresholds during the clinical course. Sixteen (14.0%) ears needed more than 1 year for thresholds to normalize. These findings suggest that some FHL cases without awareness of their hearing loss resemble psychogenic hearing loss. In such cases, otolaryngologists should carefully check the patient's individual circumstances, and when appropriate, refer patients for psychiatric consultation.

    Four psychological tests, namely the Cornell Medical Index (CMI), the Manifest Anxiety Scale (MAS), the self-rating depression scale (SDS), and the self-rating anxiety scale (SAS) were performed to investigate the role of psychological factors in vertigo, in a study of 111 vertiginous patients and 81 controls. Vertiginous patients comprised the following three groups : 56 vertebrobasilar insufficiency (VBI group), 29 autonomic nervous dysfunction (AND group), and 26 Meniere's disease (MD group). The incidence of abnormal scores by CMI and SAS in the vertiginous patients was significantly higher than that in the controls (p<0.01). No significant difference between the vertiginous patients and the controls was observed with respect to MAS and SDS scores. The abnormal rates by CMI in the VBI and AND groups were significantly higher than in controls (p<0.01). Also, the abnormal rates by SAS in the VBI and MD groups were significantly higher than in controls (VBI group: p<0.01, MD group: p<0.05). However, no significant differences between the three groups of vertiginous patients and controls were found with respect to MAS and SDS scores. The average score from other psychological tests in the CMI-abnormal group was significantly higher than that in the CMI-normal group (p<0.01). There were significant correlations between the MAS and SDS, MAS and SAS, and SDS and SAS scores (r=0.682-0.688, p<0.01). The matched data rate between the psychological tests was about 70 80%. Considering the results of MAS and SAS, vertigo induces anxiety fowards vertigo, and thereby initiates a vicious cycle. In vertiginous patients, CMI and SAS are most usuful for determing whether or not psychological factors play a role in the onset of and persistence of vertigo. But CMI consist of 195 factors, and therefore, is time concerning. On the other hand, SAS has 20 factors and takes only 5 minutes to complete. thus, SAS is more suitable for screening psychological factors.

    Thirty patients with tracheal invasion of differentiated thyroid cancer underwent partial resection of the tracheal wall between 1978 and 1996 at National Cancer Center Hospital. In most cases, the defect in the tracheal wall was reconstructed secondarily using a local flap. This method was easy in comparison with end-to-end anastomosis of the trachea following circumferential resection of the wall. Partial resection of the tracheal wall proved to be a effective treatment for thyroid cancer invasion of the tracheal, because of low incidence of local reccurence. When the defect of the tracheal wall was too large to be reconstructed using a local flap, hydroxylapatite was employed and good results were obtained.

    I investegated several factors that can predict the effect of tonsillectomy for the prevention of renal failure in IgA nephropathy patients. Samples were obtained from 71 patients (35 male and 36 famale) who were diagnosed as having IgA nephropathy by renal biopsy and the patients were followed-up for more than 5 years. Mean age at the time of renal biopsy was 28.3 years (8-58 years), and the mean follow-up period was 9 years 9 months (5-19 years). The overall remission rate of IgA nephropathy after tonsillectomy was 28.2%, and the retention rate of renal function was 90.1%, showing a considerably better prognosis than alternative therapies previously reported.
    Statistical analysis of multiple prognostic factors were examined including the following: sex, age-at-biopsy, renal pathological findings, renal function, serum IgA level, hypertension, past history of tonsillitis, preoperative results of tonsillar provocation test, and the period from diagnosis to tonsillectomy. A statistically significant poorer prognosis after tonsillectomy was observed in the following conditions: ≤20-years-old when diagnosed, serum creatinine level≥1.3mg/dl, urinary protein≥1.0g/day, serum IgA level≥350mg/dl. No other factors affected the prognosis of renal function statistically, including past history of tonsillitis or positive result of tonsillar provocation test.
    The present study indicates that there is a beneficial effect of tonsillectomy for mild to moderate IgA nephropathy and that this operation may be indicated for IgA nephropathy as long as the pathological grading is not advanced. On the other hand, tonsillectomy was not effective in cases with poor renal function. Although a past history of tonsillitis and positive results in tonsillar provocation tests have been widely considered as good indicators of tonsillectomy for IgA nephropathy, the present study showed no benefit to consideration of these factors preoperatively.

    We studied the espression and function of CD27 and CD70, which are reported to play an important role in the T-B cell interaction in tonsillar lymphocytes of patients with chronic tonsillitis. In comparison of CD27 and CD70 in peripheral lymphocytes of each patients, no significant difference was found, Furthermore, CD27 and CD70 in TB cell interaction and antibody production in the tonsil was studied. Our findings was confirmed that CD27 and CD7O are involved in interactions between T/T. T/B and B/B cells.
    A comparative study of CD27 CD4+ cells and CD70 CD4 cells, so-called memory cells in the T cell subset, was made in tonsillar lymphocytes and peripheral lymphocytes. No significant difference was found in the presence of CD27- CD4+ cells, but the CD27 CD4 cells increased in tonsllar lymphocytes. This suggests recurrent antigenic stimulation and the presence of memory cells tonsillar lymphocytes. Locahzation of CD27 and CD70 in tonsillar tissue was also investigate.

    The deletion of region 36 in the short arm of chromosome 1 (lp36) was investigated in 18 patients with oral squamous cell carcinoma by means of the double-target fluorescence in situ hybridization (FISH) method. lp36 has been suggested to harbor a putative tumor suppressor gene, and thought to carry several genes such as p58, FRAP and TNF-R II. The deletion of lp36 may play a part in cell regulation, apotosis, promotion and progression of carcinoma cells.
    The double-target FISH was completed by the us of a biotin-labeled chromosome 1 centromeric repetitive probe and a digoxigenin-labeled lp36 probe in biopsied specimens obtained from these patients before the treat-ment. The ploidy pattern of the nuclear DNA of these specimens was also examined by flow cytometry.
    The deletion of lp36 was observed in 3 patients with poor prognosis showing aneuploidy in their nuclear DNA. These results may indicate that the deletion of lp36 can be a reliable factor in the poor prognosis of patients with oral squamous cell carcinomas.

    Lingual carcinomas were found in the early stages. However, even weth early detection carcinomas may show lymph node metastasis in the neck, and this may play an important role in their outcome. For this reason, neck dissection is the treatment of choice for T2 lingual carcinomas. A surgical technique of submandibular dissection preserving the mandibular branch of the facial nerve has been done in our department for the past 3 years. The effectiveness of this technique was studied by means of histopathological investigations of superficial nodes that were included in fatty tissues together with the mandibular branch. The conclusions are as follows;
    1. No lymph node metastasis was observed histopathologically in superficial fatty tissues that contained the mandibular branch in any of the 26 cases of T2 lingual carcinomas.
    2. In 8 cases of those 26, lymph node metastasis was found histopathologically in deep submandibular tissues resected during neck dissection Even in these 8 cases, however, no metastasis was ditected in the superficial lymph nodes.
    3. CH40 (activated carbon particle) was injected into the lateral surface of the mobile tongue and its uptake into the submandibular lymph nodes were investigated in 10 cases of N(-) head and neck carcinomas. CH40 was detected in none of the superficial lymph node.
    4. These results indicate that early lingual carcinomas localizing in the lateral surface of the tongue do not metastasize into the superficial lymph nodes of the submandibular region, and that our surgical technique of submandibular dissection is useful for T2 lingual carcinomas.
