    Partial hypopharyngectomy was employed for 20 selected cases of hypopharyngeal cancer, preserving the larynx. Survival has been achieved in 15 (75%) cases being free of tumor. Defect in the hypopharyngeal wall was reconstructed by a staged operation using a deltopectoral skin flap or primarily by one-stage procedure using a skin flap with de-epithelized pedicle. Postoperative function of the laryngopharynx is good enough to live a comfortable life. Criteria for this conservative surgery is discussed with some comments on surgical technique. It is stressed that a partial resection of the hypopharynx can be the treatment of choice for some selected cases of this “supermalignancy”.

    The present studies were carried out to assess the metabolic differences between saturated and unsaturated long chain fatty acids in the process of lipid absorption and transportation to lymphatics. Male Wistar rats provided with intestinal lymph fistula were used for this experiment. Mixed micellar solutions of linoleic acid (18-carbon, unsaturated) or palmitic acid (16-carbon, saturated) were administered intraduodenally with labelled fatty acids ([1-14C] linoleate or [1-14C] palmitate) and absorbed fats were determined as the values for radioactivity of these two fatty acids found in intestinal lymph.
    The absorption and transportation of linoleic acid into intestinal lymph exceeded those of palmitic acid. Puromycin, an inhibitor of protein synthesis resulting in the block of lipoprotein formation of intestinal absorptive cells, decreased the lymphatic absorption of linoleic acid, whereas not significantly decreased that of palmitic acid. It is conceivable that puromycin will mainly inhibit the formation of chylomicron rich lipoprotein from linoleic acid and that the phase of lipoprotein formation will be atttributable to the difference of absorption of these two long chain fatty acids. Colchicine, an inhibitor of microtubular function of intestinal cells which may block the release of synthe-sized lipoprotein out of cells, caused a delayed and decreased lymphatic ab-sorption and transportation of palmitic acid as well as linoleic acid. Therefore we considered that there was no significant difference between two fatty acids in the phase of lipoprotein secretion.

    Recently, numerous studies were strongly suggestive of the presence of humoral factors in the thymus. To see if the thymuses from myasthenia gravis (MG) produce humoral factors, thymocytes from C57BL/6 mice were incubated with the supernatant from thymic epithelial cultures (STEC) of MG patients. The activity of thymic factor was determined by enhancement of cyclic nucleotides levels in the thymocytes of mice after the addition of STEC. STEC produced marked increase of cAMP levels in thymocytes but not cGMP, whereas the addition of the control media resulted in only modest increase in cAMP. Fractionation of STEC by using Amicon membrane re-vealed that maximal increase in cAMP levels were obtained with a fraction having MW between 10000 and 50000. Thus, these data suggest that the thymuses extirpated from MG patients produce a factor which increases intra-cellular cAMP levels of thymocytes.

    The task of auxiliary personnel in eye care differs considerably from country to country. To illustrate an over all picture of eye care in Japan may help understanding the inter-relationship of different auxiliary personnels and useful for international comparison as a pertient reference. Auxiliary per-sonnel assisting in ophthalmic practice includes orthoptist, ophthalmic medical assistant, ophthalmic nurse and optician or optometrist.
    Orthoptist and ophthalmic medical assistant have recently been introduced to the ophthalmic practice in Japan, and for this reason described in some detail.
    Optician and optometrist in relation to ophthalmologist have become an important and popular topic among the people concerned. Some of the prob-lems involved were briefly discussed.
    Health care of the eye and screening of eye diseases in different com-munities were described. The job of personnel engaged in rehabilitation of the visually handicapped, and auxiliary personnel working in emergency situa-tion were also stated.
