われわれは, 数種類の視覚評価の実験に使用可能な新しいファントムを開発した.このファントムは以下の特長を有する.(1)従来のBurger phantomとして, 使用可能である.(2)各信号を任意に配置可能である.(3)無信号のアクリル小片を混入することにより, 偽陽性の評価の可能性がある.(4)特定の信号を抽出することにより, CD法と他の視覚評価法との同一実験環境下における直接比較が可能である.(5)その他, このファントムは観察者の判定基準に5段階のカテゴリーを設定する従来のROC解析に適応できるだけでなく, 信号のコントラストを変化させることにより, 信号を含む試料群の確率分布を操作し, 評価の尺度とすることも可能と思われる.本研究ではこのファントムを用い, ふたつの実験を行った.CD法における信号配置の規則性に関する実験から, Burger phantomの信号配置の規則性は判定結果の変動を抑制することがわかった.またCD法とAFC法の比較に関する実験では, 両者の判定結果は一致せず, AFC法の方が低いThreshold Contrastを示すことがわかった.

研究室的測定法の確立はデータの信頼性などにおいて非常に大事なことである.ところが「より正確な測定法を追究すればするほど実用的な方法からかけ離れて行く危険性がある.」と, 委員会報告自らもその結論で言及している.委員会報告はX線センシトメトリーの方法を細部に渡り規格化し, 確かに研究室的測定法として確立してはいるが, 果して, この測定法が全ての放射線施設に適用可能であるかと言えばそうではなく, 逆に"強い足かせ"になっている感がある.センシトメトリーは基礎的であるが故にどの施設, どのような場所でも測定可能な方法でなければ意味がなく, 簡便に, より実務的で, しかも正確な測定法を確立する必要がある.このような観点から今回考案したNDA法は結果的に距離法とよく一致し非常に有用な方法であることを確信し, 以下にこの方法の特徴を列記し, まとめに代える.1.X線管移動機構など専用の測定装置を必要としない.2.X線撮影装置のあるところならどこでも測定可能.3.試料作製が非常に短時間である.4.露光域が広い.5.露光軸が決まっているので施設間比較が可能.6.センシトメータの代用.

One of the important factors affecting radiographic image quality is the film contrast of the recording system, which may be expressed by the gradient of its characteristic curve. The methods for measuring the gradient have been investigated with regard to their accuracy. It has been published that the most accurate measurement of the sensitometric curves of screen-film systems is an intensityscale X-ray sensitometer employed in the inverse square law of distance. However, in this method, the measuring is rather laborious. In this study, the accuracy of sensitometric curve by the bootstrap method using an aluminium step wedge was examined. The influences on the shape of its curve affected by several factors such as X-ray energy and the scatter factor were examined. We also compared the accuracy of the bootstrap method with the inverse square method by the standard deviation of each density at ten times the sensitometry. The measurements indicated that those factors failed to change the shape the sensitometric curve. At high density of sensitometric curves, the standard deviation at ten times of the bootstrap method were less than the inverse square method using the X-ray tube employed during daily clinical examinations. It is concluded that the bootstrap method is simple and effective to measure the gradient of sensitometric curves except for a laborious type measurement.

a)ρ_eは, ファントムの測定において, 計算値との差が総て1%以内であり, また変動係数も1%以下であった.b)Zeffは, Mayneordの式による計算値と比較した結果, リニアリティーファントムの各ロッドにおいて平均1.5%の差であり, その変動係数は最大2%であった.またZeffが高い範囲にあるCaCO_3ファントムにおいては, 平均4.8%の差を示したが, これはRutherfordの理論式とMayneordの式のエネルギー領域の相違を示唆する.Zeffの変動係数は1%以下であり測定の再現性は良い.c)人体各組織の測定では, 軟部組織における変動係数が1%〜3%と小さく, またそのZeffはNBSなどの組成から求めた計算値とほぼ一致した事から, 従来法と比較して人体における測定精度の向上が認められた.また骨組織では, CT値の変動係数が海面状骨で14%であるのに対し, Zeffでは2%であり, これらを用いた骨組成の解析が期待できるなど, 本法は種々の定量解析への応用が期待できる.

Up to now in Japan, Capacitor discharge generators have mainly been used in mobile X-ray generators, but recently, the stored battery energy inverter X-ray generators are being widely used in this area. This time, the tube voltage, tube current, X-ray output and reproducibility were measured, and were examined using the them with this new type generator. Furthermore, a comparison was made between the inverter generator and the capacitor discharge generator, and as a result, the usefulness of the inverter generator was confirmed.

As an introduction of this symposium, four different subjects related to the image evaluation of digital radiography are discussed, which are (1) appearance of different types of DR systems and the survival systems, (2) necessity of the image evaluation of digital radiography, (3) development of the methods of digital image evaluation, and (4) content and speakers of this symposium.

DSAおよびCRシステムでは相反則不軌が生じないため, タイムスケール法が採用できるという利点があるが, タイマ精度を確認する必要があるし, DSAシステムでは, マニュアル撮影ができない装置もあり, 装置に適した方法をとる必要がある.4.1 DSAシステムの特性曲線どの方法をとるにしても, 開口窓寸法を極力小さくして, 散乱線, I.I.のVGの影響を減少させなければならない.アルミニウム法では拡大撮影を行い, グレーデル効果を利用することも有効である.また, アルミニウム法は間接法であり, 多くの測定系を通過するため実験誤差が大きくなる可能性があるので十分な注意が必要である.N.D.フィルタ法ではX線TVカメラの入射光量を変化させているので, I.I.および光学レンズ系の特性が含まれていない.また, 一般にタンデムレンズにはフォトマルの受光部が設置されているため, アイリスサイズの選択に注意する必要がある.堀田氏は乳房専用撮影装置(DFP-50A特, 東芝)の軟X専用I.I.の特性曲線を, N.D.フィルタ法により測定し, 本装置の入出力特性はリニアであると報告している.N.D.フィルタ法は, 多くの装置に適用できる反面, フィルタ交換の度に電源を切る必要があり, 手間がかかる.4.2 CRの特性曲線CRの特性曲線で, ピクセル値の大きいところの傾きの変化は, 極端に強い入力光量を受けた場合の防止のために, 人為的に行っているものと考える.E値が大きいときの足部での非直線性は, E値を設定する時点でフォトマルの印加電圧を設定するため, E値が大きいとフォトマルの印加電圧が高くなり, 暗電流の影響を受け, IPの発光強度とフォトマル電流量の直線性がなくなるものと思われる.CRの特性曲線作成においても, 距離法が適用できた.X線発生装置のタイマを利用したタイムスケール法についても, 距離法と同じ結果が得られ, 測定が簡便である.また, 管電圧を変えても特性曲線の形状に変化はなかった.IPのフェーディングは8時間で25%程度といわれていたがNew TypeのIPでは20分で20%という千田らの報告もあり, 扱いに注意を要する.また, 自然放射線, 残像の影響を考えて, 使用前に十分画像を消去するべきである.

The modulation transfer function (MTF) has been used successfully to characterize the resolution properties of conventional analog X-ray imaging systems such as screen/film systems. However, the interpretation of MTF in digital radiographic imaging systems requires caution because of non-shift invariance and the aliasing effect due to the discrete data sampling with insufficient rate. To overcome these problems, the concept of presampling MTF, which shows inherent resolution property of digital imaging system including the unsharpness of detector and the blurring effect of sampling aperture, was introduced by Giger and Doi. Two different methods for determining the presampling MTF beyond Nyquist frequency were developed and measured for I.I.-TV system and computed radiographic system with photostimulable phosphors by Fujita et al. The purpose of this paper is to show some problems of MTF analysis for digital imaging system and to show the methods for determining the presampling MTF briefly. We also discussed various component MTFs of CR system such as digital MTFs, laser-printer MTFs and overall MTFs.

Multi-format camera or laser printer is important device of digital radiography. We have investigated the characteristics of the resolution for multi-format camera. It is said that the diameter and the luminance distribution of electron beam spot of the cathode ray tube (CRT) are the fluctuating factors which depend upon cathode current. In this study, we have taken a photograph of slit image which is one pixel, and after generating the luminance distribution curve of that photograph. We assessed the characteristics of the spatial resolution by using modulation transfer function (MTF). Results are as follows : 1) The brightness of CRT is controlled with cathode voltage and current. Deterioration of the resolution of CRT was caused by the expansion of the diameter of electron beam spot, which is induced by increment of the cathode current. 2) The contrast is adjusted with amplification of image signals. The maximum value of line spread function (LSF) is variable with ajustment of the contrast, however the diameter of electron beam spot is constant. The MTF was improved with increment of amplification of the image signals. But, this tendencies did not observed in CRT with low resolution. 3) The spatial resolution was inconstant on the locations of the CRT. Especially it was remarkably decreased in the peripheral area of the CRT.

In digital radiography, we showed the cause of noise product and the factor that influences it, and indicated that what process are noise of computed radiographic (CR) system caused by. In addition, we measured the overall Wiener spectrum (WS) varying some factors concerned with noise product in CR image. With the increase in X-ray exposure to the imaging plate (IP), the noise was reduced, whereas at high exposure level the noise amounted to constant value. Futhemore the greater is pixel size, the noise was reduced. Compared the CR image with the screen/film system (BMIII/Cronex-10, BHIII/Cronex-10), the noise of CR image was smaller than that of screen/film system.

It is not yet clear that how the Wiener spectrum of Fuji computed radiography (FCR) varies with image processing. Overall Wiener spectra were measured changing 3 parameters of spatial frequency processing, frequency rank : RN, frequency type : RT, degree of enhancement : RE. Influence of rotation amount : GA, that is one of 4 parameters of gradation processing, was also studied. We investigated processing conditons for mammography using FCR, from a viewpoint of granularity. RN, RE, GA contributed strongly to Wiener spectra, but the contribution of RT was small. When we process clinical images, we have to choose adequate parameters, with aims of diagnosis and parts of body.

The image quality in a computed radiography system (CR) was evaluated with observer performance tests using receiver operating characteristic (ROC) analysis. The detectability of low-contrast object (2mmφ beads) was compared CR with screen-film system. Effects of incident exposure to the imaging plate (IP) and image processing (contrast enhancement, unsharp masking) on the detectability of low-contrast object were also studied. The results indicated that the detectability of low-contrast object in a CR system was comparable to that of the medium-speed screen-film system. However, the detectability of low-contrast object in a CR system depended on IP size and decreased with incident exposure decreasing and increased by using contrast enhancement or unsharp masking.

(1)種々のサイズのIPにおけるプリサンプリングMTFは低空間周波数領域では同様であった.高空間周波数領域では, サイズが小さいもの程, プリサンプリングMTFは向上した.(2)CR画像のプリサンプリングMTFはスクリーン/フィルム系のMTFに比べて, 低空間周波数領域で大きかったが高空間周波数領域では低い値を示した.(3)二画像同時撮影法で得られたCR画像とそうでないCR画像のプリサンプリングMTFはほぼ同様であった.(4)CR画像のウィナースペクトルはスクリーン/フィルム系のそれに比べて, 約2倍, 二画像同時撮影法を用いたときで約3倍程度大きな値を示した.(5)CR画像の出力画像が縮小されると, そのウィナースペクトルはスクリーン/フィルム系のそれと, 見かけ上だけは, ほぼ同等またはそれ以下の値を示した.しかし, CR画像は, 本質的にスクリーン/フィルム系よりも粒状の多いシステムである.(6)各種試験彼写体を撮影したCR画像ではノイズがその画質を低下させているのが明かに認識できた.(7)患者胸部のCR画像の画質はスクリーン/フィルム系のそれと大体において等価と考えられた.(8)二画像同時撮影法はCR画像の診断能をスクリーン/フィルム系のそれと比較検討するために有用な手法である.今後さらに, 二画像同時撮影法を用いて症例を増やして検討を続け, 十分な数の症例を用いてオブザーバーテストを行う予定である.

消化管X線検査に関係する諸事項を述べてきたが, この検査ほど他の検査に比較して, 術者の技術, 技量が検査結果の内容に影響を与える検査はないように思う.癌を早期に見つけることの難しさをいつも感じつつ, もっと容易にできる検査はないものかという疑問と, 日常の業務に対する反省をしながら, 悪性新生物の正体がつかめる日の早やからんことを願いつつ, 次のような結語で締め括りたい.(1)消化管癌の発育, 進行過程(自然史)は大変複雑であり, 1回の検査で早期発見が困難な場合もあり, したがって次回の検査で発見したときは, 手遅れの場合がある.これを未然に防ぐには, その時々の検査を慎重に実施することが必要である.(2)結核で果たしたと同様, 放射線技師は, 消化管癌患者の拾い上げに積極的に参加し, また参加する準備をしなければならない.(3)消化管癌の実態を知り, 放射線検査を行う上の法律や諸規則を理解し, よき指導者を得て謙虚に研究や話し合いをもち, ことに当たる努力をしたい.(4)よい検査のために, 創意, 工夫, 勉強が必要であるが, 個人の技術を磨くと共に, その周辺技術に目を向け, 医師および医療関係者, メーカのスタッフと協力していきたい.
