    The comparisons of the vital signs (pulse rate, breathing, blood pressure) on the occasion of the washing hair in the forward bending style were performed between the 16 anemia patients (8 males with Hb-value of less than 13g/dl, and 8 famales with Hb-value of less than 12g/dl according to the WHO criteria) and 17 normal controls without anemia (8 males and 9 females).
    The patients had their hair washed in the warm water of 38℃ and 40℃ supplied from a washing car of the ward. They sat on a chair in the forward bending style, for eight minutes in each case.
    Ther were no significant differences between the two groups in pulse rate and breath (by t-test). The changing of the water temperature condition did not produce any significant difference in those signs.
    The comparison of the blood pressures between the groups with 38℃ and 40℃ of water did not produce and significant differences.
    However, by 8, 10, 18 minutes after the beginning of washing with 4046 warm water the systolic blood pressure of the patient-group increased significantly. And also by 8, 10, 18 minutes after the beginning of washing with 38℃ warm water the diastolic blood pressure of both groups increased significantly. Therefore, the washing hair to the anaemia patient should be selected the laying style on the bed than the forward bending style.

  • -Formation of Intraabdominal Pressure and lifting up the upper half-body-

    The purpose of study is to obtain the relation of the bed-lying positions with intraabdominal pressure which is concerned to the respiratory and circulate function. The knowledge is important to select the bed-lying position for the effective nursing care to patient with the probrem of respration and / or circulation.
    The method : For the chair-sitting posistion, standing position, and bed-lying positions holding by 15°, 30°, 45°, 60°, 75°, 90°, up the upper half-body to the horizon by the gadget bed, the intra-abdominal pressure, the amplified respiration curves on chest wall and abdominal wall, the electromyogram with electrod on the surface of M. obliquus ext. abd. and M. rectus abd., and the rate of pulseusing E.C.G., were examined under the states of the quiet respiration, the forced respiration and the strains during inspiration and exspiration.
    The equipment to measure the intraabdominal pressure was consisted of the pressuresensitive radio transducer and the gastric-tube type receiver which was deviced by us. After the top of receiver was settled certainly on 45cm distance from the incisor, we measured the intragastric pressure which was approximately as the intra-abdominal pressure (P.R. DAVIS.).
    The volunteer subjectswere 10 healthy nurses with the age of 19~39 years.
    The results were as follows:
    1) The highest intraabdominal pressure was observed by the inspiraton with strain, and the less higher pressure was seen by the exspiraton with strain, the forced respiration and the quiet respiration.
    2) Under each type of respiration and strain form, the intraabdominal pressure increased according to the degree of holding up the upper half-body, and the highest was obtained 28.3mmHg by the inspiration with strain by 90°holding up position.
    3) The difference of pressure between of the inspiratory and the exspiratoly strain by the every bed-lying position went down almost on the same way. The difference of pressures between the quiet respiration and those with strain, was tended to increase acording rais-ing the upper half-body.
    4) The type of respiration was ofserved as the chest type when the upper half-body was laid horizontally, but it came gradually into the abdominal type during the rpper half-body was raised.
    5) On the electromyogram, the action potential of the abdominal wall muscles tended to decrease acording to lift up the upper half-body by each form of the respiration and the strain.
    Holding the upper half-body with some angle to the horizon seems to be significantly effective for the nursing plan to control the intra-abdominal pressure such as by chance of cough, expectoration, defecation and labor.

  • -Survey of the Patients' Feelings in Multi-bed Ward-

    This present research is the sequel of the report <Suggestion for a Conceptual Framework> which appeared in this journal 12 (1) 1989. The purpose of this report is to make it clear how patients feel about their territory and privacy in their rooms, especially who are sharing the room with other patients, and is to obtain the few fundamental knowledge which is important subject to give support to patients' daily life for nursing care.
    In this report we defined “territory” and “privacy” as follows:
    Territory ; personalised space or extent of which patients feel their own space among others
    Privacy ; the state in which a patient employs psychological barriers to control unwanted intrusion. (Westin. 1977)
    The results are shown below:
    1. How patients perceive their territory was affected by a location of beds.
    2. There was a common tendency of preference about a location of beds. Compared with the relations between the patients' feelings about their territory and the preference about a location of beds, it has been considered that patients hesitatingly recognised their territory each other in their room.
    3. Degree of privacy perceived by patients correlated with some factors such as patients' charactor, structure of their rooms, and states of patients. From the fact, it can be considered that privacy is one of the most important to patients' life during stay in the hospital.
    4. Quality of privacy varied according to the situation of patients' daily life from time to time.

  • -A tentative Plan to Carry out the Assign Patients System-

    We analized nursing services for a week, day works, at one university hospital. In addition, we investigated a consciousness to the patients from the viewpoint of nursing services and impression of nurses, and also, to the nurses on the patients care and the existing care delivery system and others.
    From these results, we offered a tentative plan that nurses have to be made as to how they should carry out the assign patients system.
    1. As the patients do not know the assign patients system enough, nurses must orient it to the inpatients well-known.
    2. As nurses tend to like the exisiting night-work system and they hesitate about having to do with assign patients and taking their nursing developmental responsi-bility, we consider that we must change the goal and objects of their traing and it is necessary to provide a fixed teem-leader.
    3. In consideration of the nursing services, hospitalization and needs of care, we should like to select either a surgical or pediatric ward to carry out the assign in-patient system positively and to examine it by a modified night work system; night, night, evening, evening work and day off.
