  • - The Role of Midwives in the Prevention and Improvement of Urinary Incontinence -

    The present study aimed to determine the current childbirth conditions of women who have given birth with in the last three years, investigate the relationship between urinary incontinence and factors related to childbirth (obstetric care) and daily life, and identify the roles and challenges midwives face in the prevention and improvement of urinary incontinence. A self-administered questionnaire survey was conducted on mothers who had visited the health center for infant health checkups in city A, G prefecture. A total of 1,000 questionnaires were administered, with a response rate of 33.4%. Data were analyzed using chi-square tests, t-tests, and logistic regression analysis. The results showed a high rate (33.5%) of women with a history of urinary incontinence in the first two months after childbirth. In addition, the relationships between urinary incontinence and factors related to a history of urinary incontinence before, during, and after pregnancy were identified. Specifically, the experience of not being able to take a rest until three weeks after childbirth, vacuum extraction or forceps delivery, and BMI were related to urinary incontinence. The roles and challenges midwives face in the prevention and improvement of urinary incontinence include consideration of childbirth practices that minimize the burden on the pelvic floor.

    The purpose of this study was to clarify the process of construction of relationships between Chinese women married to Japanese men in farming villages and their Japanese families during first pregnancy and delivery. I interviewed eight such Chinese women, collected the data, and subjected them to continuous comparative analysis. The results showed that the process of family relationship construction had one core category: "intention to construct a relationship based on direct relations among Japanese families." I found that this process had three categories. During pregnancy, Chinese women attempted to adapt themselves to their families as a means of "grasping one's own role in construction of a family relationship." The second category was "the realization that their own position within their families was brought about by differences in postpartum customs and role expectation". The third category was "degree of direct relationships among family members, and the qualitative difference of family relationships". Among families where there was good coexistence, Chinese women were given more opportunity to think for themselves. Therefore, I suggest that Chinese women and Japanese families may establish good relationships if they are all involved in nursing care during pregnancy.

    An investigational purpose is to clarify the relationship between "exercise habit" and "health indexes" of people using a health promotion facility. We performed questionnaire investigation to subjects who were older than 20 years old. The number of effective answer of questionnaire was 332. About a subject, a man was almost half and the person with exercise habit was 51.8%.
    We performed 2×3 analysis of variance with "exercise habit" and "age". People who had exercise habits and people that age was high made a high score of HPI (Health Practice Index). SOC (Sense of Coherence) scores of the over-sixties were significantly high. About the over-sixties, as for Physical Functioning and Body Pain (SF-36v2), a group with exercise habit was significantly good. In this way, about the over-sixties, it became clear that exercise habit caused a physical effect. For QOL improvement by physical functional maintenance and relaxation of a pain, exercise habit of a senior citizen in particular is important.

    School children who need special consideration and help to the health care is increasing. The purpose of this research is to look at the present situation of support to school children who are in need of medical care ; how it is being offered and the thoughts about medical care from Yogo teachers (School nurses). A self-questionnaire survey about Yogo teachers in S-Prefecture was carried out and it analyzed 492 answers. The results were as follows: 1) The educational backgrounds of Yogo teachers and thoughts related to medical care varied. 2) Many school children who were in need of medical care were enrolling at the general school. 3) Yogo teachers recognized that cooperation with the home was necessary and often made but that the cooperation with the medical agency was insufficient. 4) The teachers of Yogo working at the schools where the children who need medical care have enrolled thought that a wide range of strategy was necessary compared with the school which isn't enrolling "students at risk". 5) Consideration to privacy for personal information often hindered cooperation with the medical agency. Therefore cooperation should be promoted by introducing specific management guideline and the intervention of the school doctor.

    The purpose of this cross-sectional study was to evaluate relationships among environmental provision and quality of life. Subjects were 83 schizophrenic patients attending day care at mental hospitals. The assessment was carried out using the Environmental Provision Scale, WHOQOL-26 Scale and included personal characteristics. The results of the application of structural equation modeling showed that the model was almost adequate to our date. This means that more greater environmental has more good quality of life. The contribution rate for quality of life was 44%. These results suggested that, in order to raise the quality of life in schizophrenic outpatients, nurses must provide nursing intervention to raise patient's environmental provision.

    The purpose of this study was to assess the effect of the inside hospital training by applying Andragogy principle for one year. Performed by 27nurses of 3 ~ 5 years with clinical experience in A hospital. Factorial structures were confirmed by using Leader Nurse's Assessment Scale (LNAS). Factor analysis was performed promax rotation with coefficient α. Tukey-Kramer after analysis of variance (ANOVA) was assessed 3 times : ① after the training, ② after 6 months, ③ after 1 year. The results revealed as the two main factors, the contribution of which 76.1%, internal consistency: 'α= .9260 ~ .9796'. As SL item remain that the 4 items: ① " faith · original idea · diligence", ② " planning", ③ "technical skilling", and ④ "thoroughly mindedness", with significant progress 1 year after the training (p < .05). We considered that their consciousness should be enhightened through the steadily approaching. Through this hospital training the increasing consideration an richment. Continuation to learn, organic typical approach hold problem to communicate. As conclusion we discover the SL an activity with control as a leader and L as an assess.

    Psychological Distance is an unclear concept in the nurse-patient relationship. In this research, we sought to clarify what the Psychological Distance is in the nurse-patient relationship.
    We applied four questionnaires in 281 psychiatric nurses: 1) Nurse Attitude Scale (NAS), 2) Feeling Checklist Japanese version (FCJ), 3) Attitude scale toward patient with mental illness, and 4) Psychological Distance Scale. The NAS measures nurses' Expressed Emotion (EE). The FCJ is a self-report questionnaire for the assessment of nurses' countertransference toward patients. The Attitude scale toward patient with mental illness measures the social distance between people and patients with mental illness. In the Psychological Distance Scale, the scale we devised originally for this investigation, we ask questions in ten phases on a scale from 1 to 10 in various situations assuming that the most friendly psychological distance is 1.
    Multiple regression analysis showed significant associations between Psychological Distance and the core model variables. A positive association between psychological distance and FCJ-Distance was observed. Conversely, a negative association between Psychological Distance and NAS-Positive Remarks, FCJ-Closeness, and years of psychiatric nursing experiences was seen. Moreover, data showed that nurses working on open wards tended to affect close Psychological Distance with patients.

    In the present study, 13 nursing university students (10 women, 3 men) were interviewed in order to elucidate the process by which they decided to pursue a career in nursing. Semi-structured interviews were conducted using a qualitative, descriptive approach, and the results were analyzed using content analysis. Twelve of the subjects desired to become nurses, while one desired to become a midwife. Interview contents were recorded on Mini Disk with the permission of subjects, and interview contents were later transcribed verbatim. A total of 1121 codes were identified, and the following 10 categories were established following analysis: "imagery / no imagery of nurses in the social environment", "active and passive selection of nursing", "experiences that led to devotion to nursing", "experiences that led to loss of confidence in one's aptitude for nursing", "relationships with teachers, instructors, and clinical nurses", "self-reflection", "integration of nursing profession and personal values", "progress in specialization", "doubt and confusion", and "selection of a career in clinical nursing". Based on these results, a model for the process of professional socialization in nursing students was established, and its characteristics were investigated. we discover the SL an activity with control as a leader and L as an assess.

    Small-plastic-bottled beverages (SPBB) are becoming increasingly popular among the inpatients and most of them directly drinking from a bottle (DFB), bacterial contamination is inevitable and seems to be important especially for the immunocompromised hosts. Answers on the questionnaire from 40 inpatient was shown that 26.7% of the inpatients take SPBB, especially green tea, sports drinks and mineral water are drunk as the most popular, and SPBB are stored at room temperature or in the refrigerator almost at the equal rates. Based on these data, DFB-mediated bacterial contamination of these three SPBB was examined by the storage manners. The bacterial contamination in sports drinks was almost negligible they are stored either. The number of living cells in green tea and mineral water gradually increased with the lapse of time at room temperature. However, if they stored in the refiigerator, these numbers in green tea retarded at the level of 46 colony forming units/ml until 24hours, bacterial growth in the mineral water was observed until 10hours and then decreased under detectable levels. These data suggest that inpatients should pay hygienic consideration to DFB-mediated bacterial contamination in SPBB and should be recommended to store them at least in the refrigerator.

    Accuracy of pulse counting by radial palpation was examined in 25 nursing students and 22 registered nurses. Measurement intervals for pulse palpation were 10, 15, 30 and 60sec. Pulse rates in each interval were compared with heart rate by electrocardiograms which were simultaneously recorded for overlapped 60sec. Pulse rates counted by the students were less than actual heart rate, although this bias was not observed in registered nurses. Mean absolute error of 10, 15, 30 and 60sec measurements were 4.26, 2.46, 1.36 and 1.42bpm for the students, 2.86, 2.44, 0.97 and 0.82bpm for the nurses respectively. In both groups, no significant differences were observed between 30sec and 60sec measurements, suggesting effeciency of 30sec measurement. The pulse rates counted by registered nurses were more accurate than the rates by students, although the difference was not statistically significant.
