The purpose of this paper is to examine panoramic radiographic features of post-Caldwell-Luc maxillary sinus. A characteristic feature of the post-Caldwell-Luc maxillary sinus on panoramic radiographs is a right-angled triangular shape, due to the absence of a floor of the maxillary sinus, radiopacity of the zygomatic bone, thickening of panoramic innominate line, and flattening of the posterior wall. This triangular shape is comparable to the contracted sinus with thickening posterior walls on CT scans. The CT scans also show contents consisting of low or high density tissue in the contracted sinus. Whereas, in MRI contents show a high signal intensity area in a T1-weighted image and a low signal intensity area in a T2-weighted image. This paper also discusses the history of Caldwell-Luc operation technique. Caldwell-Luc was developed concurrently by George Caldwell in the US and Henri Luc in France in the 1980s. The procedure affords a means of access to infected maxillary sinuses and allows for the inspection, diagnosis, and treatment of maxillary sinus diseases. In a Caldwell-Luc operation, first, the maxillary sinus is opened anteriorly through a gingivobuccal incision positioned in the canine fossa. Next, the primary intrasinus disease may then be treated by evacuation of secretions or by excision of pathologic lesions or mucous membrane. Finally, a nasoantral window is formed.

光重合型コンポジットレジンは,最も使用されている修復材料で,近年では,軟化象牙質の石灰化機能を有する光重合型フッ素徐放性コンポジットレジンが注目されている.本研究では,光重合型フッ素徐放性コンポジットレジ「Unifil^[○!R] F」ンと光重合型ボンディング材「UniFil^[○!R] Bond」を乳歯の修復に用い,3年6か月間臨床的観察を行った.乳前歯では3年6か月経過後に修復物が脱落した症例が1例,ギャップおよびステップを認めた症例は経年的に増加し6例であった.乳臼歯では,辺縁破折のみられる症例は1例であったが,探針による触診でステップを認めた症例が経年的に増加し4例であった.辺縁部に着色のみられた症例は,乳前歯で4例,乳臼歯では3例であった.乳前歯修復では,修復時を比較して形態的変化はみられず,良好な経過を示したが,乳臼歯では,修復直後の形態を比較し肉眼的に一部変化を認める症例が4例みられた.修復物の変色および着色,歯髄反応,二次齲蝕の発症はみられなかった.走査型電子顕微鏡による観察では,乳前歯修復物の表面性状は比較的滑らかであったが,辺縁部では歯質との不適合部分が認められた.乳臼歯修復では,経年的に表面が粗造になり,辺縁部ではわずかに破折している像が観察される症例も認められた.以上の結果,臨床的診査及び走査型電子顕微鏡的観察において概ね良好な経過を示しているが,経年的に歯質と修復材料の接合に不適合な部分の増加が認められた.

o examine the cooperation between EMG activities of the masticatory muscles and mandibular movement, the mandibular movements and the EMG activity of the masseter and temporalis muscles were simultaneously recorded by Gnathohexagraph during chewing a gum in 40 adult subjects. They consisted of 20 Class I and 20 Class III subjects. The timings for the onset, peak, and offset of EMG activity during chewing a gum were estimated relative to the maximum opening and occlusal terminal points in the traces of incisor point. The timings of EMG activity and the maximum opening and occlusal terminal positions were compared between two groups. The following results were obtained : (1) The maximum mouth opening position shifted to the working side in Class III subjects as compared with Class I subjects. (2) The occlusal terminal position shifted to the balancing side in Class III subjects. (3) The ratio of closing phase to a chewing cycle was significantly greater in Class III subjects than in Class I subjects. (4) Duration of EMG activity was longer in Class III subjects than in Class I subjects due to the extended offset time of EMG activity. These results suggest that the size and position of the mandible and the inter-occlusal relationship could influence the masticatory movement.

A case having abscess formation in the pterygomandibular space and a case having malignant tumor (adenocarcinoma) in the same area, which had been difficult to be distinguished from temporomadibular disorders (TMD), are reported. These cases initially showed the symptoms similar to those of TMD, and few clinical signs of those diseases. No apparent abnormality in the routine radiographic examinations was shown in both cases. In the case of inflammation caused by marginal periodontitis of the left maxillary second molar, the symptoms of the inflammation was made unclear by prescription of non-steroidal anti-inflammatory drug for the initial diagnosis of TMD. Computerized scanning tomography (CT) was useful to establish the diagnosis. In the case of adenocarcinoma, nevertheless an advanced tumor involving the pterygomandibular space to the parapharyngeal space existed, the patient just complained pain and joint sound in the temporomandibular joint (TMJ) during jaw movement. The tumor was coincidentally detected by magnetic resonance imaging (MRI) for the TMJ. In both cases, the lesions mainly occupied the pterygomandibular space which is difficult to image by screening radiographic examination for TMD. CT and/or MRI were useful equipment for confirmation of the diagnosis for this kind of unusual cases.
