    [Purpose] Purpose of this study is to measure the changes in various physiological markers and performance criteria for women basketball players over the course of a travel heavy season. [Participants and Methods] Fifty one Division-II female basketball players and a control group of 54 females joined this study. Measurements began at the beginning of the competitive season and concluded with final measurements at the end of the competitive season. [Results] The female basketball players showed noticeable increases in resting salivary cortisol, visceral trunk fat, resting heart rate, and resting blood pressure. These athletes also showed diminishment in isokinetic force of leg muscles, particularly in knee flexion strength. Vertical jump measurements also indicated a slight diminishment. In contrast, the control group experienced none of the same changes. [Conclusion] Over the course of a grueling flight schedule in combination with a full-length basketball season, the female athletes in this study showed significant declinations in many indicators of overall health. It is concluded that resulting prolonged intermittent stress of a travel-heavy season can lead to significant changes in certain physiological markers with notable decreases in isokinetic force of leg muscle.

    [Purpose] The beneficial neuromuscular effects of whole-body vibration are explained by the tonic vibration or bone myoregulation reflex. Depending on factors that remain undefined, whole-body vibration may activate the tonic vibration or bone myoregulation reflex. We aimed to examine whether voluntary contraction facilitates activation of the tonic vibration reflex during whole-body vibration. [Participants and Methods] Eleven volunteers were included in this study. Local and whole-body vibrations were applied in a quiet standing (without voluntary contraction) and a semi-squatting (isometric soleus contraction) position. Local vibration was applied to the Achilles tendon. Surface electromyography was obtained from the soleus muscle. The cumulative average method was used to determine soleus reflex latency. [Results] In the quiet standing position, the bone myoregulation reflex latency was 39.9 ± 4.1 milliseconds and the tonic vibration reflex latency was 35.4 ± 3.6 milliseconds. Whole-body vibration application in the semi-squatting position activated the tonic vibration reflex in four participants and the bone myoregulation reflex in seven participants. Local vibration activated the tonic vibration reflex in both positions for all participants. [Conclusion] Simultaneous whole-body vibration application and voluntary contraction may activate the tonic vibration reflex. Determining the spinal mechanisms underlying the whole-body vibration exercises will enable their effective and efficient use in rehabilitation and sports.

    [Purpose] This study aimed to evaluate the changes in lower-extremity muscle strength, balance performance, and body composition. [Participants and Methods] In this study, 21 healthy university students who underwent short-term whole-body vibration training without previous whole-body vibration training participated. The study design was randomized between-groups design. Participants were randomly assigned to one of the three groups; control, training, and whole-body vibration training. All participants completed a six-week training protocol comprising a first two-week training period, two-week rest period following the first training period, and second two-week training period. Over four periods, the participants’ lower-extremity muscle strength, balance performance, and body composition were evaluated. Separate three-by-four repeated-measure analyses of variance, with three exercise mode groups and four periods, were initially used to analyze the primary outcome variables; lower-extremity muscle strength, balance performance, and body composition. [Results] In the three groups, lower-extremity muscle strength, static and dynamic balance performances, and body composition showed no changes during all periods. [Conclusion] This study provides a better insight on the responsiveness of short-term whole-body vibration training and will help determine whole-body vibration programs in revalidation and training.

    [Purpose] This study aimed to clarify the relationship between one-eye visual deprivation; thus, interfering with stereoscopic perception, and movement and obtain insights on the influence of visual perception on movement to step over obstacles. [Participants and Methods] Participants were 25 healthy individuals. There were two conditions of visual perception (stationary and approaching conditions) and two additional conditions of binocular and monocular visions. Under the four conditions, participants were asked to step over an obstacle immediately after a 90° turn while walking. Distance between the foot and obstacle, foot pressure distribution, and stance phase time were measured. [Results] Toe clearance was lower in the approaching condition than that in the binocular stationary condition. The trajectory length ratio was greater in the approaching condition than that in the stationary condition, and heel-ground contact, metatarsal-ground contact, and stance times were all shorter in the binocular condition. Additionally, heel contact, midfoot contact, metatarsal contact, and stance times were shorter in the approaching condition than that in the stationary condition. [Conclusion] In walking with a 90° turn, the binocular approaching condition provided more visual information and positively affected motor control of movements to step over an obstacle.

    [Purpose] The purpose of this study was to examine the relationships between physical, cognitive, and social frailty and locomotive and non-locomotive physical activity of moderate to vigorous intensity in community-dwelling older adults and to explore effective intervention methods for preventing frailty. [Participants and Methods] Participants were 82 community-dwelling Japanese older males and females. Measurement items included basic information (age, gender, height, weight, body mass index, and the number of underlying diseases), physical activity, and assessment of physical, cognitive, and social frailty. Associations of physical, cognitive, and social frailty with physical activity were analyzed by group comparisons and multivariate analyses. [Results] The comparisons of physical activity indices for each frailty type revealed that physical frailty was associated with the number of steps and locomotive physical activity of moderate to vigorous intensity, whereas cognitive frailty and social frailty were not. Two overlapping types of frailty were associated with locomotive physical activity. When adjusted for age and gender, step counts and locomotive physical activity were each associated with physical frailty. [Conclusion] Future interventions to increase step counts and locomotive physical activity of moderate to vigorous intensity may be effective for preventing physical frailty; however, interventions other than simple physical activity need to be considered for the prevention of cognitive and social frailty.

    [Purpose] The authors’ institute offers a comprehensive inpatient approach to rehabilitation after anterior cruciate ligament reconstruction surgery, providing nutritional and psychological support in addition to exercise and physical therapy. This study aimed to determine the outcomes of athletes undergoing this comprehensive rehabilitation program and to compare the outcomes of bone-patellar tendon-bone and semitendinosus/gracilis autograft recipients. [Participants and Methods] Elite athletes who underwent comprehensive inpatient rehabilitation at the authors’ institute for at least two weeks after anterior cruciate ligament reconstruction were mailed a questionnaire. Their recovery levels, which were measured against preinjury performance, and secondary anterior cruciate ligament injury rates, were evaluated and compared according to graft type. [Results] Valid responses from 45 athletes were analyzed (bone-patellar tendon-bone [n=12]; semitendinosus/gracilis [n=33]). The frequency of return to preinjury activity levels and secondary anterior cruciate ligament injury were comparable between bone-patellar tendon-bone and semitendinosus/gracilis graft recipients. A greater proportion of athletes returned to preinjury activity levels, while a lower percentage experienced re-rupture compared to previous studies. [Conclusion] A comprehensive rehabilitative approach after anterior cruciate ligament reconstruction may contribute to improved postoperative performance irrespective of graft type. Objective evaluations are needed in the future to clarify the benefits of specific rehabilitative approaches.

    [Purpose] To identify factors that affect the return to solitary living of patients with stroke who had lived alone prior to stroke onset. [Participants and Methods] From January 2017 to March 2020, we enrolled a total of 103 patients with stroke who had lived alone prior to stroke onset and retrospectively analyzed their age, gender, length of hospital stay, outcome (return to living alone or not), functional independence measure at discharge, and social score at discharge. We also analyzed the relationship between the above factors and the outcome. [Results] Functional independence measure and social score at discharge were significantly associated with the outcome. The cutoff value of the functional independence measure at discharge was 91 (area under the curve: 0.91; sensitivity: 0.96; specificity: 0.72), while the rate of return to living alone was 23.5% when the social score was ≥3. The sensitivity and specificity for return to living alone were 0.91 and 0.88, respectively, when cutoff values of the functional independence measure and social score at discharge were 91 and 3, respectively. [Conclusion] Social factors and ability to perform activities of daily living are important for return to solitary living for patients with stroke who lived alone prior to stroke onset.

    [Purpose] The purpose of this study was to determine the factors associated with decreased physical activity levels among community residents over a long-term observation period during the coronavirus disease 2019 (COVID-19) pandemic. [Participants and Methods] We conducted a cross-sectional study using a self-administered questionnaire and daily steps as an indicator of physical activity levels. The study population consisted of 704 community-dwelling residents aged 40 years and older who participated in the health program from 2019 to 2020. We compared the daily steps from March–December 2019 to March–December 2020 and performed multivariate analysis to identify the factors associated with decreased daily steps. [Results] Of all participants, 447 (63.5%) returned the questionnaire and 309 (43.9%) were included in the analysis. During the COVID-19 pandemic, 133 (43.0%) respondents had decreased physical activity levels. The multivariate analysis showed that working (odds ratio, 2.08; 95% confidence interval, 1.10–3.94) was significantly associated with decreased daily steps during the COVID-19 pandemic. [Conclusion] There was a significant association between decreased physical activity levels and working during the COVID-19 pandemic. When restrictive measures such as teleworking are implemented, it may be necessary to take measures to prevent a decline in physical activity levels.

    [Purpose] Lateral balance and the righting reaction are related to trunk function and many activities of daily living. However, there are no studies evaluating the assessment of the righting reaction while ensuring safety. This study aimed to clarify the reliability of assessments for the righting reaction from a tilted sitting position in healthy participants. [Participants and Methods] The participants of this study included 28 healthy young adults. Before the start of the study, markers were attached to the participants for data collection. Participants were asked to sit on a vertical board, tilted 10 degrees, and were asked to perform the righting reaction. This test was repeated four times on each side. The actions were recorded with a video camera, and two analysts used the Image J software to calculate the angles of the righting reaction of the neck, trunk, and left and right lower legs. [Results] High reliability was obtained regardless of the tilt direction and site of righting reaction. In addition, no adverse events occurred. [Conclusion] It was possible to evaluate the righting reaction while ensuring safety, and reproducibility was confirmed.

    [Purpose] This study was conducted to show the directional specificity of a reaching motion and to verify the three-joint coordination of the sternoclavicular joint, shoulder joint, and elbow joint. [Participants and Methods] The participants in this study were 10 healthy adult males. A reaching task for the upper limbs in a horizontal plane was assessed and measured. The trajectory, joint angle, and joint range of motion were calculated using a three-dimensional motion analyzer. [Results] The joint angle changes of the three joints were related in a circular radiation pattern. The sternoclavicular joint and shoulder joint were related in a fan-shaped radiation pattern. On the other hand, the sternoclavicular joint and elbow joint were related in a linear functional relation except for T3 and T8. [Conclusion] The results suggest that the coordinated motion of the shoulder joint and elbow joint controls the direction and extent of the motion track, while the coordinated motion of the sternoclavicular joint and elbow joint finely regulates the track. There also is a degree of difficulty in motion depending on the direction, which is apparently defined by a combination of joint motions.

    [Purpose] This study aimed to assess the correlation between lateral thrust and clinical symptoms after high tibial osteotomy and determine lower limb alignments that may decrease lateral thrust. [Participants and Methods] We included 54 patients (73 knees) who underwent high tibial osteotomy. Clinical symptoms, including the Japanese Orthopaedic Association score and the hip–knee–ankle angle measured via radiography, were assessed 12 months postoperatively. Lateral thrust was measured using three-dimensional motion analyses. Logistic regression was used to calculate the cut-off values with good Japanese Orthopaedic Association score and lateral thrust as dependent variables and both lateral thrust and hip–knee–ankle angle as independent variables. [Results] The lateral thrust cut-off was 3.1° (sensitivity: 0.83; specificity: 0.74; area under the curve: 0.76), while that of the hip–knee–ankle angle was 1.9° of valgus (sensitivity: 0.71; specificity: 0.81; area under the curve: 0.72). [Conclusion] Good clinical outcomes after high tibial osteotomy can be expected with a lateral thrust of ≤3.0°, indicating that the target hip–knee–ankle angle should be 2.0° valgus. In cases where valgus alignment is insufficient, lateral thrust may develop, which should be assessed using gait analysis.

    [Purpose] This study aimed to extract knowledge for the development of a training program for creating a social model of disability for physical therapists, focusing on the experiential learning of those physical therapists who did not use acceptance of disability according to their subjective judgment. [Participants and Methods] The study included 11 physical therapists who were interviewed about their use of acceptance of disability and the circumstances leading to its non-use. [Results] The study identified the past and current use of acceptance of disability, as well as cases and reasons for its discontinuation, along with changes in clinical content. [Conclusion] The study extracted knowledge for the development of training programs in line with the components of the experiential learning model.

    [Purpose] The purpose of this study was to examine the relationship between postural control and respiratory movements after identifying the characteristics of respiratory movements in different postural holding tasks. [Participants and Methods] The total trajectory length was measured using a stabilometer (G-620; Anima Company, Tokyo, Japan), while respiratory movements were measured using an expiratory gas analyzer (PowerMetz; Anima Company, Tokyo, Japan) during closed-leg and one-leg standing. Respiratory movements were characterized as tidal volume (TV), respiratory rate (RR), minute volume (MV), and metabolic equivalents (METs). [Results] The total trajectory length, METs, RR, and MV were higher in one-leg standing than in closed-leg standing. The rate of change in the total trajectory length during one-leg standing was positively correlated with the rate of change in TV and negatively correlated with RR. [Conclusion] The results suggest that postural control of one-leg stance holding is associated with changes in respiratory motion.

    [Purpose] Behavioral restrictions during the corona virus disease 2019 (COVID-19) pandemic may have affected the physical activity levels of college students. We aimed to characterize the body composition and physical activity of college students during these behavioral restrictions. [Participants and Methods] The body composition (height, weight, body mass index, body fat mass, body fat percentage, total body muscle mass, free-fat muscle index [FFMI], and fat mass index [FMI]), physical activity, amount the of walking, amount of daily activity, and the number of steps were measured in 52 university students. [Results] For both male and females, the number of steps taken was lower than the average steps reported by the Ministry of Health, Labour and Welfare. In males, FFMI had a strong positive correlation with physical activity, amount of walking, and the number of steps taken. In females, FFMI had a strong positive correlation with physical activity and the amount of walking, as well as a moderate positive correlation with the amount of daily activity. [Conclusion] Since physical activity and walking of university students during COVID-19 affect FFMI, it is necessary to develop an exercise program that considers behavioral patterns.

    [Purpose] To assess the effects of continuous rehabilitation on intensive care unit-acquired weakness in a patient who underwent open-heart surgery. [Participant and Methods] This case report describes a 78-year-old female patient who developed intensive care unit-acquired weakness after open-heart surgery. She underwent cardiac surgery, via a thoracotomy, for valvular disease and angina pectoris. Her Medical Research Council sum score at the time of rehabilitative intervention was 28 points, indicating intensive care unit-acquired weakness. Rehabilitation was continued, and the patient’s physical function was evaluated over time. [Results] Continuous rehabilitation improved the patient’s grip strength, knee extension muscle strength, 6-minute walk test performance, and Short Physical Performance Battery score. [Conclusion] Although few reports have been published on rehabilitation for patients with intensive care unit-acquired weakness, our case suggests that appropriate rehabilitative intervention according to the severity of the condition and continuous rehabilitation may improve the patients’ activities of daily living and physical function.
