    The raison d'etre of a clinical department is to provide effective care., i. e. medicine is based on social needs. Otorhinolaryngology is in no way an exception. Medical care as well as medical sciences are making continual and rapid progress, particulary during the last several decades. One problem with such rapid progress in medicine is an efficient application of the results. More effective and specific strategies have to be devised, probably in a fashion similar to those used in modern technology. In otorhinolaryngology, at least three important areas have shown outstanding progress—diagnostic electronics, microsurgical techniques, and speech hearing rehabilitation. These three provide typical examples of how advances in medicine can fall short of being optimally applied. Socioeconomic, political as well as educational considerations are indispensable for surmounting these difficulties.

    Tomographic study coupled with information obtained from otoscopic observation of the ear drum, middle ear and audiometry provide otologists with a complete picture of cholesteatomatous involvement of the temporal bone. Tomographic studies were performed in our clinic preoperatively using a Hitachi Polytome. The ear was sectioned at consecutive levels 2.5mm apart with 4 cuts for lateral projection and 5 for frontal. Diagnosis of cholesteatoma was made when there were tomographic findings of widening over 3mm of spur interval (distance between the lateral aspect of the head of malleus and the lateral attic wall), destruction of the anterior tympanic spine and an excessively blurred attic.
    In the retrospective review of tomograms of the surgically treated patients, we found changes compatible with cholesteatoma in 89% of 46 patients with surgically confirmed cholesteatoma. Tomographically we were able to determine the size and extent of cholesteatoma as well as the involvement of the ossicles, dural plate, facial canal, lateral sinus and lateral semicircular canal. Forty-six cases of cholesteatoma were classified according to the site and the extent of cholesteatoma into three types: the attic type (13 ears), the tympanic type (15) and the advanced type (18).
    Thus the usefullness and accuracy of preoperative tomographic examinations in cholesteatoma cases have been well demonstrated.

    The nasal provocation test was performed for 7 patients with nasal allergy positive to skin test of H. D. and mite by measuring the threshold level with H. D. in the right nasal cavity and with mite in the left nasal cavity three times —10a.m., 3p.m. and 10p.m.— on the same day.
    The provocation test was performed by the disk method using a Disk manufactured by Torii Co. Ltd. which had a concentration (n. p.) of:
    H. D.: 5ug 25μg 125μg 600μg
    Mite: 0.5ug 2.5ug 12.5ug 60.0ug
    Thirteen (92.9%) of the total number of 14 patients with both H. D. and mite experienced a provocation with the lowest concentration in the morning, and (one case in the afternoon), while two such occurrences were noted in the evening with the same concentration as was given in the morning.
    In conclusion, in no patient was there a lower provocation threshold level in the afternoon and evening than in the morning and in most cases, provocation occurred with the lowest concentration in the morning.

    Malignant melanoma of the mucous membranes is rare and the prognosis is poor. The treatment of this disease has been a challenging problem for oncologists and more recently for the immunologist. We have undertaken chemotherapeutic treatment of malignant melanoma while prescribing DTIC, BCNU, and Vincristine for two patients.
    Case I
    The patient was a 47 year old male with complaints of right nasal hemorrhage and headache. On inspection, a painless, soft, red mass occupying the right nasal septum and the right submand ibular lymphnodes was palpable. A biopsy of the submandibular lymphnode established the diagnosis of malignant melanoma. Treatment with BLM and 5-Fu was initiated on the day of admission May, 21, 1976.
    On June, 7, 1976, the treatment of combined chemotherapy with DTIC, BCNU, and Vincristine was started. The tumor regressed steadily for a short period, but gradually an enlargement of tumor on the right side of the nose was evident.
    In August, 1967, 60CO irradiation, a daily treatment of 200rads was given over a 25 day period, however the tumor progressed steadily.
    On September, 24, 1976, the patient was treated with bacillus Calmette-Guérin (BCG) by scarification. However the response to treatment was poor and the patient died on December, 16, 1976.
    Case II
    The patient was a 60 year old female with complaints of a black mass, pain, and swelling in the right gingiva.
    On inspection, a soft, black, irregular mass occupied the right gingiva. A biopsy established the diagnosis of primary malignant melanoma. The bilateral submandibular lymphnodes were not palpable.
    On September, 29, 1977, resection of the right maxilla was done, and the treatment with combined chemotherapy with DTIC, CCNU, and Vincristine was started post-operatively.
    The patient was discharged on December, 6, 1977 and has remained well.

    A 19 year old female complained of a painless mass on the right side of the submaxillary area, with no particular symptoms and signs. This mass was a hen-egg sized tumor with elastic hardness. Clinical diagnosis was a mixed tumor of the submaxillary area. Under general anesthesia, an encapsulated tumor was excised.
    Histological examination revealed malignant mesenchymoma (Stout) differentiating to fibrosarcoma, liposarcoma, hemangioendthelioma and leioma.
    Malignant mesenchymoma is a rare occurrence and since the first report by Ishikawa, only 27 cases have been reported in Japan.

    We administered NK-631 to 40 patients with head and neck tumor by intra-arterial infusion in 2, by intravenous administration in 38. The tumor was squamous cell carcinoma in 33 cases and malignant lymphoma in 7. Anatomical site of the tumor in 40 was as follows: larynx and mesopharynx 10 respectively, oral cavity 9, nose and paranasal sinuses 5, hypopharynx 4, ear and nasopharynx 1, respectively. Excellent response was obtained in 7 cases and good response in 11 cases of squamous cell carcinoma. The response rate was 58% or 18/31. Excellent response was obtained in 4 and good response in 2 with malignant lymphoma. The response rate was 86% or 6/7. Effect of NK-631 appears to be almost the same as is seen with Bleomycin treatment. NK-631 was most effective in malignant lymphoma as well as in well differentiated squamous cell carcinoma.
    Eruption and thickening of the skin were the most common toxic effects and such were evident in about 30% of all patients. NK-631 was less toxic for the lung as compared with Bleomycin.

    Combipenix, a combination of Ampicillin (AB-PC) and Dicloxacillin (MDI-PC), was administered to 72 patients with acute infections, such as 38 cases of acute tonsillitis, 9 of peritonsillitis, 2 of peritonsillar abscess, 4 of acute pharyngitis, 11 of acute tympanitis, 3 of acute paranasal sinusitis, 3 of chronic paranasal sinusitis (acute aggravation) and one each of dental maxillary sinusitis and periserotinitis. The results were excellent in 25 cases (34.7%), good in 28 cases (38.9%), fair in 18 cases (25%), and poor in 1 case (1.4%). As this compound was prescribed in cases of staphylococcic infections with an AB-PC tolerance, Combipenix was effective in all patients examined. Side effects were observed in 2, but in neither case were the symptoms serious. Combipenix demonstrated an extremely high therapeutic efficacy against various acute infections in the field of otorhinolaryngology.
