The Auditory Brainstem Response (ABR) was recorded in patients with Multiple Sclerosis (MS), and the relationship between ABR-views and clinical symptom was examined.
The prolongation of I-III IPL, which suggests disorders in the inferior region of the pons, and the prolongation of III-V IPL, which suggests disorders in the superior region of the pons, showed a positive correlation with each clinical symptom. Therefore, ABR appearing to be a good diagnostic aid in determining brainstem foci by giving valuable and objective information as to the affected site and range, especially in patients without brainstem symptoms or signs and no obvious lesions. Further studies are needed on the differences of ABR-views in remissions and exacerbations and on the relationship between ABR-views and the treatment and prognosis of MS.

A case of perilymphatic fistula is reported. The patient was a 58-year-old male, who initially showed moderate conductive deafness in the right ear. However, soon after the furosemide test, his hearing loss worsened and became sensorineural. Electrocochleography revealed the dominant negative SP, and a signs of endlymphatic hydrops.
During exploratory surgery, a fistula was found in the posterior part of the round window membrane, and was closed, with improvement vertigo and hearing loss. As no middle ear disorder was found in otological and radiological examinations, the principal factor causing the conductive deafness was speculated to be an air bubble just beneath the footplate.

Nasal resistance was determined in 14 patients with deviated nasal septum, 13 with complications of deviated nasal septum and chronic hypertrophic rhin.itis, 15 with chronic sinusitis and 16 with nasal allergy before and after intran.asal operation. Adrenaline contraction tests were performed in all patients before the operation. The postoperative nasal resistances was significantly decreased in all patients compared with the preoperative values. The preoperative nasal resistance after adrenaline contraction tests coincided with the postoperative nasal resistance. The result shows that rhinomanometry is useful for evaluations the effectiveness of intranasal operations for deviated nasal septum, chronic hypertrophic rhinitis, chronic sinusitis and nasal allergy. In addition, the preoperative adrenaline contraction test can be applied to the determination of indication of intranasal operation and assessment of the improvement rate of nasal obstruction after the operation.

Headache is a common symptom with various causes. Headache which originate in a frontal osteoma are generally caused by a mucocele or pyocele, which blocks the nasofrontal duct. We report an additional cause of headache due to compression of the anterior ethmoidal nerve.
The headache of a 22-year-old female was completely relieved by removal of a small osteoma compressing the anterior ethmoidal nerve. The nasofrontal duct was patent and there was neither pyocele nor mucocele in the frontal sinus.
On the basis of the operative findings and the relief of the headache, the authors conclude that osteomas accompanied by headache should be removed even when small without a suspected mucocele or pyocele.

Several investigators have reported a considerable number of patients suffering from dryness of the oral cavity or throat along with abnormal sensation in the throat. It is necessary to clarify whether hyposalivation or decreased airway fluid is the cause of abnormal sensation in the throat.
The authors investigated the occurrence or increase of the sensation of a lump in the throat after intramuscular or subcutaneous injection of atropine sulfate (0.5 mg).
The subjects were 105 patients with various kinds of otolaryngeal diseases. Five of them reported the occurrence and one reported the increase of the sensation after the injection. Four of these 6 patients were females and 5 were over 50 years of age.
It is quite certain that hyposalivation or decreased airway fluid can cause the sensation of a lump in the throat.

We report a patient with Shy-Drager syndrome, a disease characterized by progressive degenerative change in autonomic nerves and cerebellar dysfunction accompanied by multiple system atrophy.
The subject was a 62-year-old female who suddenly lost consciousness and underwent emergency tracheostomy. Three months after the operation, she was reffered to our clinic, because of persistent dyspnea. Laryngofiberscopic examination revealed that the bilateral vocal cords were adducted and fixed in the midposition. Shy-Dragersyndrome was diagnosed on the basis of hypotension and cerebellar type bilateral ataxia.
Respiratory impairment was relieved by tracheal fenestration and the patient was discharged. At present, she is responding well to the therapy.

Seventy-three patients with previously untreated carcinoma of the maxillary sinus, treated during an 11-year period were investigated retrospectively
The overall three-year and five year survival rates were 51.5% and 42.7% respectively. The five-year survival rate for the group treated with combined therapy consisting of local chemotherapy, radiation and necrotomy was 12.9%, it was 54.8% in the group treated with combined therapy followed by surgery, such as partial or total resection of the maxilla.
The most successful treatment was intraarterial infusion of adriamycin (bolus) and 5-FU (continuous), combined with radiotherapy and necrotomy, followed by surgery. The five-year survival rate in this group was 61.7%.

One hundred and one cases of neck masses seen over a ten-year period at our clinic in Jichi Medical School are presented and analysed.
The incidence of tumors, especially malignant ones, increases in patients over 40 years of age. There are no difference in the mean duration of symptoms among infections, developmental anomalies and neoplasms. In seven cases the clinical diagnosis was different from the histopathological findings.
It is stressed that neck masses require a thorough thoughtful evaluation and that early injudicious open biopsy is inadvisable.

We conducted a psychosomatic study on patients with cancer of the head and neck. The Yatabe-Guilford personality inventory was conducted repeatedly before and after treatment. The results revealed psychological instability in particularly in the patients with early cancer. In addition, the relax image method was employed in patients having psychological problems. This method was found to be useful in promoting psychological stabilization. The psychosomatic approach to the treatment of cancer will enable physicians to proceed with the treatment of cancer more positively.

Thirty-five children with moderately severe and severe hearing impairment wersurveyed to assess their adaptability to kindertens. The subjects had previously received the speech training at our department. Speech training increased their ability to communicate with other children and gave them a positiveattitude. Three points were found to be necessary to achieve good results in preschool integrated education: 1)sufficient recognition by teachers of childrens' handicaps, 2) close communication among teachers, parents, speech therapists and doctors, 3)childrens' positive attitude towards adaptation as well as the achievements of the language ability. However, difficulties were found in integrated education; these involved problems in communicating and associating with normal children.
These difficalties were mainly affected by the subjects' age and hearing level.

Glycoconjugates in the nasal and maxillary mucosa of normal controls and of patients with inverted papilloma, epithelial dysplasia and carcinoma were studied by the lectins-HRP staining method. Four kinds of lectin were used: WGA, PNA, UEA-1, and RCA.
    1. Control nasal epithelium reacted to WGA and UEA-1. Some sugar residues stained with WGA, PNA, UEA-1 and RCA were detected in various amounts in the goblet cells and mucous cells of the subepithelial glands. Serous cells in the glands were stained by UEA-1.
    2. There was a similarity of WGA and UEA-1 staining in inverted papilloma, epithelial dysplasia, and control epithelium.
    3. Adenoid cystic carcinoma was not stained by any of the lectins. According to cellular differentiation, Squamous cell carcinoma showed various staining patterns depending on the degree of differentiation.
    4. Lectin staining may be a useful marker in the early diagnosis and cell differentiation of nasal and maxillary carcinomas.

For the treatment of tumors in the head and neck region, surgery, chemotherapy and radiotherapy can be used. To obtain better chemotherapeutic results, it is important to know the antineoplastic mechanism and/or how to combine the drugs that are available at present. The effect of an anticancer drug, cisplatin, on cell dynamics was display two-dimensionally by flow cytometry with FITC-label bromodioxyuridine and propidium iodide, converted to a three-dimensional color display by computer processing. This makes it possible to demonstrate the cell dynamics changes better visually. Simultaneously, using fluorescein diacetate, the viability was measured to study the cytocidal effect. Three dimensional histogram by administration of cisplatin in concentration of 0.5μg/ml showed the cell accumulation in S phase on 1st day and then represented the block of G2M phase on 5th day.
Newly deviced our methods could be applied to obtain the cell kinetics in vitro and also to solve the mechanism of other antineoplastic agents.

Large doses of metoclopramide as an antiemetics were administered to 9 patients recieving cisplatin (CDDP) (50mg/m2) for head and neck cancers. Metoclopramide, 2mg/kg was administered four times intravenously on the first day of CDDP therapy and once a day on days 2-7. Neither emesis nor nausea occurred in 8 of the 9 patients (89%). Side effbcts were minimal: mild sleepiness(66.7%)and diarrhea(22.2%). It is concluded that the antiemetic effect of intravenous metoclopramide in large amounts can prevent cisplatin-induced emesis.
