    A patient with the main complaint of pharyngeal pain who visited our otorhinolaryngology department as a result of many tests was diagnosed as having AIDS. The patient was a 42-year-old male who worked as a dancing instructor and was a homosexual. Tonsillar candidiasis, swelling of lymph glands in the cervical region, and dermal erythema in the neck were found. Blood tests showed a reduction in WBC, increase in the erythrocyte sedimentation rate (ESR), increased levels of immuno globulins IgA and IgG, reduction in T-cells, and a OKT4/OKT8 ratio of 0.2. The tuberculin response was negative and the HIV antibody titer was 320x. Pneumo cystis carinii and Kaposis salcoma were not present.

    Meningioma is a benign tumor with no symptoms in the initial stage. It is not unusual for a meningioma to show no objective signs even in the advanced stage. Neurootological examinations are often required.
    We examined a patient with a gigantic meningioma which displaced upward the tentorium on the affected side. In this case objective signs were minimal, and neurootological examinations were very important. The tumor was gigantic, but hearing, optokinetic nystagmus, smooth pursuit and ABR (auditory brainstem response) were restored after the operation. We report this unusual case and describe the neurootological examinations.

    In this investigation of possible changes in hearing after glycerol intake, puretone thresholds were measured before and 1, 2 and 3 hours after glycerol intake in 23 normal subjects and 27 patients with Meniere's disease. Pure-tone thresholds were also measured before and after saline intake in 12 normal subjects as controls.
    There was a change in hearing after glycerol intake in 19% of normal subjects and in 58% of patients with Meniere's disease. Only 4% of the control group had a positive reaction after saline intake. Improvement of hearing was observed in the middle frequencies after glycerol intake more frequently in patients with Meniere's disease, while in normal subjects improvement was noted more often in the low frequencies. The hearing threshold for high frequencies was raised by glycerol intake; this phenomenon was more prominent in normal subjects than in patients with Meniere's disease. This study reveals that hearing changes after glycerol intake can be demonstrated in normal subjects. The effect of glycerol on inner ear function seems to be more complicated than postulated previously.

    A 69-year-old female complained of bilateral tinnitus and vertigo. At admission for tinnitus therapy, blood analysis revealed severe microcytic hypochromic anemia and liver dysfunction. Further examinations led to a diagnosis of advanced stomach cancer and multiple liver metastases. The patient had noticed a slight loss of appetite, but had attributed it to sleep disturbance caused by her tinnitus. In this case, cancer symptoms had been masked by the patient's tinnitus and had not been reported to us. It is believed that the severity of her tinnitus was partly influenced by her general condition. This case is an illustration of the importance of general as well as local examination of patients with tinnitus.

    Secretory otitis media occurs most frequently in infancy. Since the Eustachian tube functions differently in children and adults, it is considered unlikely that secretory otitis media in the two age groups is due to the same cause. Since the infantile Eustachian tube is a kind of pressure open type, we preffered high-pressure therapy in the treatment of secretory otitis media in children.
    We used high-pressure therapy for 111 children aged 3 to 12 years 188 ears) with secretory otitis media. As a result, 129 ears(68.6%)showed improved average hearing levels within 10 dB in the low, medium, and high tone ranges. No patient experienced earache or other trouble when exposed to high-pressures. In the highpressure therapy, air escapes through the Eustachian tube when the middle ear pressure is regulated. In this case, the fluid can be excreted together with the air. Since the partial pressure of oxgen rises with the pressure, it is estimated that the highpressure therapy favorably affects ciliary movement and restores the mucous menbranes in the tympanic cavity and the Eustachian tube.

    In a clinical study of 74 cases of thyroid tumor operated on in our clinic from 1981 to 1985, the following results were obtained:
    1) Thyroid tumor was more common in females.
    2) Males had a higher incidence of malignancy.
    3) The incidence of thyroid cancer rose higher in the third and fouth decades.
    4) Soft tissue radiography was easy and fairly useful as a diagnostic tool.
    5) Combination of clinical findings and laboratory data was valuable in determining the characteristics of thyroid tumor.

  • -Six Cases Controlled without the Use of Antithyroid Drugs-

    Six cases of Graves' disease were prepared for surgery without the use of antithyroid drugs because of their side effects. Lugol, β-blocker, and steroids were used instead of antithyroid drugs. Three of these six cases were operated on with the T3 values still much higher than normal. The appropriate period of lugol administration is said to be from two to three weeks. In cases 1 and 3, the period of lugol application was too long bringing about an “escape” phenomenon., whereas in case 2 it was too short to get enough antithyroid effect.If antithyroid drugs cannot be used, it is important to make a treatment plan carefully before starting preoperative control.

    In cleft palate cases, disharmony of the maxille appears at 18 years of age in males and 16 years of age in females, when the growth of facial bones stops. In order to repair this disharmony, we have performed orthognathic surgery transorally. In planning the orthognathic operation, we use dental casts, computer-aided analysis of cephalography, postoperative prediction of the profile applying computer simulation, and three-dimensional cephalography. The problems of operating procedures, anesthesia, velo-pharyngeal function and postoperative relapse are discussed.

    The records of hospitalized patients in the Department of Otolaryngology, Tohoku University School of Medicine during 1978 and 1985 were analyzed, and a comparison was made between the present statistics and those of two earlier periods (1940-1962 and 1972-1978).
    The following trends were seen:
    1. The number of hospitalized patients per year decreased and the average length of hospitalization increased, mainly because of an increased number of malignant cases requiring longer hospitalization.
    2. The incidence of tumors, trauma, foreign bodies, cysts of the paranasal sinuses and nasal bleeding increased, tumors most of all. In contrast, the incidence of inflammatory diseases declined.
    3. Recently our department handles not only ear, nose and throat problems, but also those of other areas of the head and neck.

    The possibility of ototoxicity caused by gentamicin (GM) aerosol into the nasal cavity was evaluated by measuring its serum concentration. The maximum permissible concentration of a commercially available GM injection solution for nasal nebulizer therapy was 4%as determined by counting ciliary activity measured by a photoelectric method. Using this tolerable concentration, clinical evaluation of GM aerosol therapy for chronic sinusitis was also performed.
    According to the serum concentration of GM, ototoxicity via nasal mucosal absorption of nebulized 4%GM injection solution was not expected becaused of influx into tympanic cavity via the eustachian tube.
    From the data obtained by human paranasal mucosa cultured in RPMI-1640 medium, a relative low concentration, from 0.5 to 1 %, of a GM injection solution was recommended because of its solvent toxicity. On the other hand, a 4%preparation of GM powder dissolved in physiological saline solution was permissible.
    GM nebulizer therapy was clinically evaluated using 1 %GM injection solution. Chronic sinusitis without nasal polyp improved by 25%in terms of subjective and objective symptoms, and by 12.5%in terms of total judgement, X-ray examination showed 14.3%improvement. Sinusitis wity nasal polyp showed no improvement.

    The role of laryngeal adjustments for allophonic variations of voicing in Japanese — vowel devoicing and intervocalic /h/ voicing — was investigated using laryngeal electromyography and fiberscopic endoscopy. The results indicated that the vowel devoicing was accompanied by EMG activity patterns of the posterior cricoarytenoid and interarytenoid, which were different from those for fully voiced vowels, causing the glottis to be wide open. This suggested that vowel devoicing mainly reflects a kind of free variation at a higher level than the EMG signals. In contrast, the voicing of intervocalic /h/ occurred while the glottis remained as wide as it did for voiceless /h/ or /s/ with comparable EMG patterns of those muscles, despite the presence of vocal fold vibrations. Therefore, it may be that this latter phenomenon is chiefly dependent on some other condition at the level of the glottis.

    From April 1984 to August 1985 dilazep hydrochloride was administered to 37 patients (15 males and 22 females), 22 with peripheral vertigo, 9 with central vertigo, and 6 with vertigo, for 8 weeks.
    1)Slight improvement or better was noted in 86.5% and moderate or better improvement 59.5%. The final assessment of object findings, equilibrium function and auditory test findings, showed slight or better improvement in 58.8% and moderate or better improvement in 32.4%.
    2) The global improvement ratings which combined subjective symptoms and objective findings, showed slight or better improvement in 86.5% and moderate or better improvement in 59.5%.
    3)The global utility ratings of over all effectiveness were “ slightly satisfactory ” or better in 86.5% and “ satisfactory ” in 62.2%.
    4) Administration for 8 weeks caused the improvement rates to be significantly higher than at 4 weeks.
    5)Improvement rates were higher in patients with acute central or peripheral vertigo.
    6)Improvement rates were higher when the symptoms had been present for less than 3 months.
    7) Only one patient (2.7%) had an adverse reaction: slight dryness of the mouth.

    The effectiveness of Dilazep hydrochloride (Comelian Kowa) was tested in 84 patients with vertigo (14 with central vertigo, 50 with peripheral vertigo, and 20 with so-called vertigo).
    1. Improvement occurred in 85.2% ofthose with central vertigo, in 92% of those with peripheral vertigo, and in 78.9% of those with so-called vertigo.
    2. Subjective symptoms began to be relieved and objective findings improved as global improvement ratings and global utility ratings rose 2 weeks after the start of treatment. The ratings were significantly improved at 4 weeks and at 8 weeks, but no difference was observed between the 4 weeks and 8 weeks evaluations.
    3. Antivertigo drugs were avoided, in general, but tranquilizers were administered in a few cases. There was no significant difference between the group given combined theraphy and that given Dilazep only; improvement rates were 90.9% and 86.1%, respectively.
    4. The improvement rate in patients below 50 years of age was 80.5% and that in patients over 50 years of age was 95.2%.
    5. Adverse reactions occurred in 4 patients(4.8%), but they were mild.
