    We observed the movement of the lateral pharyngeal wall and soft palate by a flexible fiberscope in 80 cases of cleft palate. The findings of otoscopy, audiometry and tympanometry were recorded at the same time.
    The frequency of normal hearing was significantly high in the group with “good” movement of the lateral pharyngeal wall and soft palate. Also, the rate of type A pattern tympanogram was significantly high in the group with “good” movement of the lateral pharyngeal wall.
    The main muscle involved in the movement of the lateral pharyngeal wall and soft palate appears to the levator veli palatini muscle. Accordingly, “good” movement of the lateral pharyngeal wall and soft palate indicates that the function of the levator veli palatini muscle is good. Therefore, our results showed that good function of the levator veli palatini muscle had a desirable effect on the state of the middle ear.

    Two children with psychogenic vertigo are reported. The first case was a seven year-old boy who complained of a recurrent vertigenous attack since the age of two. Physical examination revealed no abnormality except in Romberg and Mann tests. Consultation with his parents disclosed that his complaint of vertigo was an expression of his dissatisfaction by simply misunderstanding the meaning of “vertigo”. The second case was an eleven year-old boy who complained of disequilibrium, double vision and visual disturbance after he had hit his head on the ground during a Sumo tournament. Physical examination revealed no abnormality except in Romberg and Mann tests and concentric contraction of visual field. Consultation with his family revealed, that the boy felt isolated, believing that his parents loved only his younger brother and sister. Both two children recovered, once the causes of the troubles were pointed out.

    In patients with congenital nystagmus, visual disturbance occurs due to eye osillation and strabisumus appears because the gaze is directed to the object at the neutral point of the nystagmus, with consequent scoliosis.
    We have recently demonstrated the disappearance of such nystagmus along with a transient improvement of the visual acuity as a result of a slow intravenous injection of 3ml 2% lidocaine solution diluted with 20% glucose solution. Eleven of 16 patients with congenital nystagmus treated with intravenous injection of lidocaine, showed remarkable suppression.

    Reoperation was performed on 76 patients with chronic otitis media during a tenyear period. The annual incidence of reoperation in patients undergoing middle ear surgery was between 12 and 17%. In 38% of the cases, no dry ears were detected following the initial/previous surgery. A dry ear, once occurring rarely recurred within a year following the previous surgery. In 9 ears, the same bacteria were found both before the previous operation and at the time of reoperation. Nine patients underwent tubo-tympanoplasty procedure. Retraction cholesteatoma was found in 7 ears without previous cholesteatoma.

  • Five Patients in Two Families

    Five patients in two different families were treated surgically for congenital stapes fixation. The footplate of the stapes was found to be completely fixed to the oval window in each case. Ankylosis was present in the proximal interphalangeal joints of both hands. This type of symphalangism was probably due to a maldevelopment of ossification and a lack of formation of joint apertures through the differentiating regions of cartilage. These processes were presumed to occur at about the eighth week of gestation. In such patients, stapes surgery is indicated and at the same time, all the joints in the body should be examined.

  • -Transmastoid Approach-

    To avoid cochlear and vestibular nerve injury as much as possible, neurovascular decompression for hemifacial spasm via the mastoid was performed. This approach was superior to the classic suboccipital approach with respect to more direct accessibility to the 7th and 8th cranial nerves with less retracting the cerebullum and to minimum chance of subsequent trauma to the 8th cranial nerve. On the other hand, the disadvantages of this approach were less visualization of the cerebello-pontine angle and a more cautious maneuver to prevent CSF leakage. We recommend the transmastoid approach for the cases in which intratemporal tumors can not be ruled out as a cause of the hemifacial spasm.

    A case of leiomyosarcoma of the right maxillary sinus in a 55 year-old female is reported. The patient did not respond to radiotherapy or chemotherapy and subsequently underwent total-left maxillectomy and orbital exenteration.
    Histological examination of the resected tumor revealed interlacing bundles and whorls of spindle like-shaped cells with blunt-ended nuclei. Electron-microscopically, myofilaments, pinocytotic vesicles, dense plaque and dense bodies, which characterized smooth muscles, were observed. Periodic examination of the patient for 10 months has not disclosed local recurrence or any distant metastasis.

    A 25-year-old man with masseteric hypertrophy is reported. He was operated on by the external approach according to modified Gurney's method.
    CT scan was very useful for diagnosing this case. Pathologic examination of the hypertrophied masseter showed normal striated muscle tissue.
    Surgical correction of the appearance should be performed if the patient desires esthetic repair. The extraoral approach is recommended because it is easier than the intraoral approach. The cause of masseteric hypertrophy still remains obscure, postoperative care is necessary for a long time.

    We operated on 86 parotid masses of different etiology during the past nine years and three months. Based on our surgical experiences, we established principles for managing the facial nerve in parotid surgery. In case of malignant tumors, a wide excision of the tumor with the surrounding soft tissue is of prime importance for controling the tumor. To achieve this purpose, the facial nerve was inevitably sacrificed with the tumor. When a tumor, even if it is benign, adheres to the facial nerve, the involved branches of the nerve should be resected to prevent recurrence of the tumor. When the facial nerve was sacrificed, nerve repair or muscle transfer technique was employed according to the extent of the removal of the nerve. Nerve grafting, total or partial, was indicated when the facial nerve was sacrificed in its extratemporal portion. On the other hand, muscle transfer technique (the temporalis, masseter and digastricus) with multiple fascial suspension was employed when the nerve was removed in its intra-and extra-temporal portions.

    Three cases of extramedullary plasmacytoma originating in the head and neck region are reported.
    Case 1 was a 50-year-old female with λ light chain-producing plasmacytoma in the right nasal cavity. Case 2 was a 70-year-old male with an IgA-λ producing plasmacytoma which recurred twice bilaterally in the nasal cavity. Case 3 was a 26-year-old female with an IgG-κ Plasmacytoma developing in the left tonsil.
    In all three cases, tumor-producing immunoglobulin was immunohistochemically demonstrated in tissue by the PAP method, but not detected in the serum.
    All three patients underwent surgery and irradiation. We used a contact Nd-YAG laser technique to surgically remove the tumor in case 2 and 3.
    Contact Nd-YAG laser was very useful to excise tumors of the head and neck region. These three patients had no signs of local recurrence or multiple myeloma after a follow-up period of 12 to 18 months.

    From 1967 through 1982, one hundred and seventy six patients with squamous cell carcinoma and anaplastic carcinoma of oropharynx were treated at the Departments of Otolaryngology and Radiology, Osaka University Hospital.
    All cases were initially treated by external radiation therapy. Recurrent cases were treated by various kinds of treatment, such as surgery, interstitial iradiation or external radiation. Surgical treatment was most effective in the management of recurrent cases.
    Local lesions were controlled in 77 cases by the initial radiation alone. Out of 99 recurrent cases, 25 cases were treated by surgical procedures, of which 14 cases were successfully controlled. Local control rate by radiation therapy was found to decrease as the size of the tumor increases. Local control rates according to T stage were T1: 60%, T2: 51%, T3: 32%, T4: 0%.
    Radical radiation therapy can be curative in the early stages while preserving function, whereas, surgery alone, or in combination with radiation therapy should be performed in advanced stages.

    A 70-year-old man with dysphagia was reffered to our clinic for diagnosis and treatment. Indirect laryngoscopy revealed a large mass in the hypopharynx. Direct laryngoscopy demonstrated that the tumor arose in the right piriform sinus but the larynx was not involved.
    Under light microscopy, the tumor was composed of clumps and cords of atypical large cells with clear nuclei. Many mitoses were seen. Electron microscopy showed small desmosomes between the tumor cells. This tumor was diagnosed as undifferentiated carcinoma from these histological findings.
    Although his condition was well controlled by irradiation and chemotherapy for 2 years, he died of lung metastasis 28 months after the initial treatment. Autopsy revealed no evidence of hypopharyngeal carcinoma.

    In 82 patients with stage II or III malignant tumors of the head and neck, the cumulative survival rate was compared between 55 treated with and 27 not treated with OK-432. The annual survival rate was consistently higher in those treated than in those not treated with the agent; a significant difference was observed after 3 years and a slight different after 5 years.
    Intracutaneous administration of OK-432 had no effects on the cumulative survival rate, presumably because activation of pyrimidine fluoride compounds was unaffected.
    Of 57 parameters tested, we describe 9 immunological and biochemical parameters used in our hospital that indicate the onset and prognosis of malignant tumors of the head and neck. These parameters were:aspirin esterase, fibrinogen, a2-globulin, and cholinesterase levels; lymphocyte count, Su-Ps skin test, and PPD skin test; lymphocytes blastgenesis to PHA; and iron level.

    This report describes the application of technique for miniaturized screwed osteosynthesis plates inmaxillo-facial surgery. This plate is easily shaped and moulded using modeling pliers or byhand, so it can be adapted to almost all facial bone surfaces. During the past 3 years, 28 patients for 49 maxillo-facial fractures have been treated by this technique. This technique can be used for many types of mandibular fractures, including fractures of condyle, zygomatic complex fractures, and upper or lower orbital rim fractures. This technique has the following advantages: (1) the surgical approach is simple with small incisions, (2) inter-maxillary fixation is hardly necessary, (3) the technique is apPlicable to almost all facial bone fractures with small bone fragments, and (4) the necessity for removing the plates is greatly reduced. We have not encountered any complications or side effects.

  • -A Study of Chemical Neurotransmitter-

    One of the chemical neurotransmitters, norepinephrine (NE), was measured quantitatively in the human palatine tonsil by high-performance liquid chromatography. The NE content in patients with chronic tonsillitis increased in proportion to the frequency of recurrent acute infection. In patients with focal infectious tonsillitis or peritonsillar abscess, the tonsillar content of NE was higher than in patients with simple hypertrophic tonsils. It is concluded that various forms of tonsillits may be closely related to the local sympathetic nerves.

    The mammalian larynx reacts promptly to external stimuli for protective and phonatory purposes. In particular, human speech has been thought to have an elaborate laryngeal control mechanism through the auditory system, although this has not yet been proved by empirical data. In the present study, the mechanism of laryngeal control through the auditory system was studied in terms of change in activity of the intrinsic laryngeal muscles.
    Healthy adults maintained phonation at a constant voice pitch while receiving repeated click-sound stimuli in both ears. The voice signal was transduced with a throat microphone and bandpass filtered to isolate the fundamental frequency (FO). Average values for about 40 recordings of this FO voltage at various click-sound stimuli were determind. The muscle activity of the larynx was recorded with bipolar wire electrodes inserted into the crico-thyroid muscles and/or the lateral crico-arytenoid muscles. The induced EMG (electromyographic)potentials were rectified, the average of about 80 values was calculated and the activity change in the intrinsic laryngeal muscles caused by the auditory stimulus was determined.
    1) A short-latency change in FO (about 50 msec) was noted in response to auditory stimulation.
    2) In most subjects who received sound stimulation of more than 90 dB during high pitch phonation, muscle activity increased with a steady latency, as shown by the electromyogram.
    3) The average time from the stimulus to the maximum amplitude of muscle activity was about 26 msec for the crico-thyroid muscle on both sides, about 45 msec for the right lateral crico-arytenoid muscle and about 47 msec for the left lateral cricoarytenoid muscle.

    A series of 28 women who complained of an abnormal sensation in the throat were treated with Methermon ®-F and the clinical effects were analyzed. The overall effectiveness was excellent in 8 cases, good in 7, fair in 6 and poor in 1. Thus, the drug was clinically effective in 77.8%. The response was better in the climacteric age group than in other age groups. Kupperman's index was decreased in patients with excellent or good results. The increased in plasma concentration of luteinizing hormone (LH) and follicle stimulating hormone(FSH)correlated well with the clinical effect of this drug. As side effects, stomach discomfort was noted by two patients, one whom could not continue this drug.

    (2''R)-4'-0-tetrahydropyranyladriamycin (THP) is a new anthracycline anticancer agent. THP has a tumoricidal effect stronger than or similar to Adriamycin, but weaker cardiotoxic and skin-damaging effects.
    THP was administered to 20 patients with head and neck squamous cell carcinoma. Sixteen patients (group A) received a dose of 20 mg for 3-5 consecutive days, and 4 patients (group B) received a dose of 30-40 mg every other day. Two patients showed complete response, and 7 patients showed partial response, the efficacy rate being 45%. There was no difference in the efficacy rate between groups A and B, but the 2 patients who showed complete response were in group B.
    THP was effective against head and neck squamous cell carcinoma.
