    Vestibular neuronitis, a vestibular disorder that causes dizziness, was reviewed epidemiological using a survey sponsored by the Ministry of Welfare and a preliminary survey by Yamaguchi University Hospital. Follow-up data of the clinical symptoms and examinations at Yamaguchi University Hospital are reported, especially on the computed galvanic body sway test, serum viral antibody titers, CSP changes (biochemical and virological), vertical writing test (Square drawing test) and the prognoses. In addition one case is presented and a newly developed combined galvanic test is described.

    Neuro-otologic examination of a 38-year-old woman with headache and dizziness revealed left hearing loss, disorders of auditory brain stem responses (ABR), lack of a stapedial reflex and central equilibrium disorders. The ETT was saccadicataxic pattern, the OKP showed a poor response bilaterally and the Caloric test revealed slowing of the mean maximal slow phase. A skull base tumor was found on a CT scan. Tissue removed surgically was diagnosed histologically as chordoma. Neurosurgeons of our hospital planned to perform a second operation, but she died from bleeding inside the tumor about a month after her first operation.
    This paper scribes etiology, clinical findings, neuro-otologic examination and prognosis reviews reference the literature on this rare tumor.

    From 1975 to 1986, 1568 patients with otitis media were treated surgically at the Department of Otorhinolaryngology of Kitano Hospital. During surgery, we removed granulation tissue from 648 ears, and with light microscopy we found 122 cases of cholesterol granulation. In these 122 patients the following items were recorded: age, sex, culture of the otorrhea before surgery, treatment of the ossicular chain, hearing before and after surgery, lesions in the Fallopian canal, and complications of the cholesteatoma and the labyrinthine fistula.
    Cholesterol granuloma was found in various parts of the ear, but most frequently in the mastoid and attic. In 122 cases, cholesteatoma was present in 78 and deafness in 15. Moreover, vertigo, acute worsening of hearing and difficulty in the healing of wounds occurred frequently.
    Therefore, in treating otitis media, one must always been in mind these possible complications.

    A 58-year-old male suddenly developed left peripheral facial palsy. When he was 54 years old, he had experienced an episode of right facial palsy, which was healed by conservative therapy. This time his blood sugar level was 229 mg/dl, and urine sugar was high. He was treated with diet therapy for diabetes mellitus and conservative treatment for facial palsy, including steroid. His blood sugar level decreased, and his facial palsy gradually improved.

    Lid function in facial palsy can be restored by the implantation of auricular cartilage. As shown in the diagram, fashioned cartilage is placed and sutured between the tarsal plate and periorbital connective tissue under the eyebrow. Cartilage is kinked by the function of the palpebral levator muscle by opening the eye, while its elasticity works to close the eye when the levator muscle is relaxed. Two patients treated by this method are described: one with traumatic facial palsy who had been treated by facial nerve rerouting and edge-to-edge anastomosis, and one with a facial nerve schwannoma who had been treated with a nerve graft. Our method is better for such cases than classic tarsorrhaphy from the cosmetic point of view, and the surgical procedure and postoperative management are easier than wire spring implants.

    External auditory canal cholesteatoma was diagnosed in seven ears of six patients, all male, aged 65-83 years. Four patients were treated with surgery, and no recurrence of cholesteatoma has been noted. High resolution target CT imaging, especially coronal section, was useful in demonstrating the spatial distribution. The lesions were classified into three patterns according to the clinical findings. The pathologic features and surgical findings suggested that the variations could be attributed to the degree of bony destruction in the early stage.

    It is well known that the bony stenosis of the external ear canal is common in professional divers. The same lesions are not rare in surfers and have been named“Surfer's ear”.
    In 1986 we examined 51 professional surfers (46% of the Japanese Professional Surfers) and 191 amateur surfers for surfer's ear and have already reported the data on the professionals. This paper is concerned mainly with amateurs, 52.7% of whom had some degree of stenosis of the external canal; 88.1 % of these had early stage Surfer's ear. Surfer's ear tends to develop after 4 years of surfing and progresses through the 5th year. The bony stenosis of surfer's ear usually arises from the tympano-squamous suture (which is said to be the most likely site for osteoma) and the anterior and posterior wall of the meatus and progresses until the three parts fuse. We perform plastic surgery on the external meatus of 5 ears in 4 patients. Pathology and other points will be reported in future papers.

    Acute maxillary osteomyelitis was diagnosed in 4 neonates. Three of them were born in the same hospital: a 10-day-old female, a 12-day-old male and a 17-day-old female. Staphlococcus aureus was cultured from all 4, so it was suspected that the infections were hospital-acquired. It is important to inform the hospital where in-fants were born when such infections are diagnosed. Each infant of the patients was treated surgically and with antibiotics. Three responded well, but one developed a brain abscess. CT scanning was very useful in evaluating complications at an early stage. In a patient treated surgically 10 years ago, well-developed sinuses were observed on X-ray examination. Aggressive treatment with both surgery and antibiotics should be pursued for patients with this disease.

    One hundred and forty seven patients with facial bone fractures were treated during the past 6 years and 3 months. Thirteen of them had malar bore fractures. All of these were reduced effectively by the trans-antral approach. For fixation, in 12 cases Foley catheters were placed in the maxillary sinuses. In 3 cases, additional interosseous wiring was done at the zygomatico-frontal sutures. In all 13 cases, good results were obtained by this method.

    We examined nasal mucociliary transport by the saccharin method and nasal ciliary beating frequency by the photoelectric method in cleft palate patients and normal subjects. The saccharin time was longer in cleft palate patients than that in normal subjects. Cleft palate patients were devided into two groups: group 1 with sinusitis and group 2 without sinusitis. The saccharin time was longer in group 1 than in group 2 and longer in group 2 than in normal subjects. Ciliary beating frequency of the nasal septal mucosa was lower in cleft palate patients than in normal subjects. Cleft palate patients appeared to have impaired nasal mucociliary function.

    Two cases of tuberculous lymphadenitis in the parotid gland are reported. The patients were women, 18 and 53 years old. One case was suspected to be a malignant tumor or tuberculosis of the parotid gland, and the other case was thought to be pleomorphic adenoma. These diagnoses were based on the findings of physical examination, sialography, sialo-CT, and echography. Later, both cases were confirmed as tuberculosis by postoperative histological examinations. Drug therapy with Isoniazid, Rifampicin, Ethambutol and Streptomycin was performed for about nine months, and no recurrence has been noted in the two cases. Thus, drug therapy should be given even in cases when surgical resection is performed.

    69-year-old male had noticed a mass on the right side of his neck but no other symptoms 5 months before visiting our clinic. A careful examination failed to reveal any lesions except for the neck mass which was adherent to the surrounding tissues. The clinical diagnosis was branchiogenic carcinoma and the mass was removed surgically. Cervical lymph nodes were normal. The histopathological diagnosis was squamous cell carcinoma of the inner lining of a cystic lesion. Postoperative radiation therapy (40Gy) was performed. There has been no recurrence.

    Malignant tumors of the sublingual gland are rare neoplasms. An extensive review of the literature reveals a total of 63 cases from 1930 to 1985. We present here a case of mucoepidermoid tumor of the sublingual gland in a 58-year-old female. The tumor was completely removed by left upper neck dissection in continuity with the sublingual gland and the floor of the mouth, and reconstructed by using a tongue flap. The differential diagnosis and surgical procedure for this neoplasm are discussed.

    The voices of 20 males with a mean age of 78 years and 24 females with a mean age of 75 years were examined with a micro-computer and a phonation analyzer. In this study we examined the voice quality and phonatory function of aged persons.
    The voice of aged male is characterized by a decrease of harmonics and that of aged female is characterized by a decrease of fundamental frequency. These changes may be caused by an atrophy of vocal fold in aged males and by an edematous change of vocal fold in aged females. In both aged males and females, amplitude perturbation becomes large and especially in aged males frequency perturbation also becomes large. These changes are thought to be caused by several factors related to aging, such as atrophy or edematous change of vocal fold mucosa, clumsiness of fine regula-tion of laryngeal muscles or a decrease of lubrication.
    As a phonatory function test, both aged males and females showed a decrease of maximal phonation time. And aged males showed an increase of air flow rate. These changes may be caused by a decrease of lung function, and especially in aged males, these changes are thought to be related to the glottal gap.

    Thresholds of auditory brain stem responses were determined in 51 normal guinea pigs using needle electrodes inserted onto the parietal bone along the median line. The mean threshold was 11±7.2 dBSPL (M±SD). Variance of the threshold was small, and did not increase even after five consecutive measurements once a week.

    Morphological and immunohistochemical examinations were performed on human olfactory epithelium from two patients with olfactory disturbance caused by common cold. The specimens were obtained using Nakano's forceps type 3. One patient was a 54-year-old male who visited our clinic 20 days after the loss of smell. The other was a 55-year-old female who was seen two years after lose of smell.
    In the first patient, a few olfactory receptor cells were observed and neuronspecific enolase (NSE) immunoreactivity was found in the dendrite and perikaryon. In the second patient, olfactory epithelium appeared atrophic and thin. NSE immunoreactivity was not found in the epithelium. Cytokeratin (CK) immunoreactivity was found in one layer of cells on the basement membrane, and were assumed to be basal cells. Other cells, which did not react to anti-CK serum, were suspected to be sup-porting cells.
    Biopsy of human olfactory mucosa using Nakano's forceps and immunohistochemical method were useful to characterize the olfactory disturbance.

    The effect of pre-seasonal injections of histamine-gammaglobulin conjugate (Histaglobin®, HG) for cedar pollinosis was investigated in 6 hospitals. There was no sigificant difference in the symptoms between the HG pre-treated group (HG group) and the non HG-treated group (no-HG group) . However, the onset of symptoms was later in HG group, and these patients needed fewer medications during the pollen season. Moreover, the patients who recieved 5 or more injections of HG had significantly fewer symptoms than those who recieved less than 5 injections. Preseasonal treatment with HG was therefore concluded to be effective for cedar pollinosis.

    After administering 2 g of CTT intravenously, we studied how the drug was absorbed by the maxillary sinus mucosa and correlated the drug's concentration with the type of tissue.
    1) The serum levels 30, 60, 90, 120 and 150 minutes after injection averaged 191.3±73.7 μg/ml, 149.0±7.9 μg/ml, 110.9±44.7 μg/ml, 84.1±3 1.7 μg/ml and 71.8±19.9 μg/ml, respectively, almost the same as in healthy adults.
    2) The drug was well absorbed by the maxillary sinus mucosa and was long-acting. The relevant values were 91.9±43.0, 70.9±27.2, 39.6±10.6, 63.7±7.5 and 28.1±13.4 μg/g, respectively, in the frontal wall mucosa, and 43.9±18.0, 59.1, 42.4±10.7, 31.3±19.2 and 19.1 μg/g, respectively, in oral mucosa.
    3) More CTT was absorbed by the frontal wall mucosa than by the oral mucosa.
    4) There was no definite correlation between tissue types and absorption rates, but the rate of absorption was somewhat lower in growing tissues than in fibrotic tissues.
    5) CTT is considered to be a useful drug for chronic sinusitis.
