    The crista transversa was evaluated on tomographic X-rays of the internal auditory canal in 200 subjects (400 petrous bones) in an attempt to ascertain whether they are useful in diagnosis of acoustic neurinomas.
    1) In 137 cases (68.5%) both cristae transversae were visible, in 17 cases (8.5%) one was visible, and in 46 cases (23%) no crista transversa could be seen.
    2) All the cristae transversae were in the upper half of the internal auditory canal fundus.
    3) The average length of the crista transversa was 2.93mm (S. D. 1.40mm).
    4) The average difference in the length of the bilateral cristae transversae was 0.92mm (S. D. 0.93mm), and in 94% it was less than 2.5mm.
    Therefore, we suggest three criteria for suspected acoustic neurinomas.
    1)One crista transversa is not visible.
    2)The crista transversa is in a much lower or higher position than normal.
    3)There is a difference of more than 3mm between the right and left cristae transversae.

    Relapsing polychondritis (RP) is a systemic disorder characterized by recurrent inflammation and degeneration of cartilage. A 49-year-old woman was admitted to our hospital with bilateral hearing loss and tinnitus. Physical examination revealed saddle nose and congestion of bulbar conjunctiva. Both tympanic membranes and external auditory canals were hyperemic. Her sedimentation rate was 30mm/hr and WBC 8370. Anti-nuclear antibody and anti-cartilage antibody were positive. The OKT4 OKT8 ratio in the peripheral blood was 4.17. Audiograms revealed both sensori-neural and conductive impairment. Biopsies of the nasal septum showed degeneration of cartilage, fibrosis and mononuclear cell infiltrates. Treatment with 1000mg Solu-Medrol for three days (every other day) resulted in dramatic improvement in hearing and tinnitus after a few days of treatment, suggesting excellent prospects of pulse therapy of steroid hormone in the treatment of RP.

    Nine cases of misdiagnosed Bell's palsy were analyzed. The patients had been diagnosed and treated as Bell's palsy, because the facial palsy had occurred suddenly and the clinical course resembled that of Bell's palsy. Three of the patients had neurinoma of the facial nerve, one had congenital cholesteatoma, one had temporal bone metastasis of malignant tumor, three had intracranial tumor and one had temporal bone destruction secondary to otitis media with effusion. The facial palsy was progressive in one case, recurrent in three cases and unchanged in five cases. In eight cases, perceptive or combined hearing loss was observed. Vestibular function disorder was seen in five cases and central nervous system disfunction in three cases. We emphasize that a diagnosis of Bell's palsy should not be made without a careful examination of the ears and central nervous system, including the sense of hearing and vestibular function.

    Respiratory activity (gas metabolism) of the middle ear cavity (including the mastoid cells) plays an important role in ventilation by opening and closing the auditory tube in a normobaric environment. Electron microscopy shows that the submucosal capillaries of the human mastoid cells are structures which facilitate the intra- and extravascular transport of substances.
    The measurement of oxygen tension (PO2) in the middle ear cavity by a PO2 sensor inserted via the auditory tube revealed that the oxigen tension remained constant despite the opening and closing of the auditory tube with swallowing or the closure of the tube. However, the tympanic membrane showed varying compliance, suggesting the probable production of another gas (CO2).
    The results indicate that tubal ventilation in a normobaric environment maintains the internal pressure of the middle ear cavity similar to or slightly higher than the external pressure by the production of another gas, by making the gas flow from the pharyngeal side to the middle ear cavity less likely.
    This phenomenon suggests that a low oxygen tension in the middle ear cavity is useful in preventing infection of the middle ear cavity, and that the gas metabolic function of the mastoid cells is, in view of their characteristic submucosal capillary structure and relative volume is larger than the tympanic cavity, very influential in maintaining the ventilatory function of the middle ear cavity.

    Auditory tubal function was evaluated in two age groups: 20-24 years of age and 50-54 years of age.
    Active opening function was determined by the sonometric auditory tubal function test, which measures the duration of patency of the auditory tube during swallowing. Passive opening function was determined by sonotubometry and the Valsalva maneuver, which measures the intranasal pressure when tubal opening is produced by the Valsalva maneuver.
    Significant differences were observed between the two groups.
    1)Valsalva maneuver opened the auditory tube at an intranasal pressure of 534±173mmH2O in the older group and at 418±242mmH2O in the younger group.
    2)In the older group, 28% of the tubes were not opened by pressures up to 1000mmH2O, whereas in the younger group, only 16% were not opened.
    These characteristics of the auditory tubes in the older age groups may be among the causes of adult type serous otitis media, which occurs often in older adults.

    Biopsy of a sublingual tumor in a 70-year-old female showed adenoid cystic carcinoma. The tumor was resected, and she is alive 10 months after treatment without any evidence of recurrence. Primary malignant tumor of the sublingual gland is extremely rare, with only 67 cases reported in the literature from 1930 to 1984. The histological types, treatment and prognosis of malignant tumors of the sublingual gland are discussed.

    Nine cases of epiglottal cyst were treated surgically with direct laryngoscopy or by an external approach during the past ten years. Five cases were approached by means of direct laryngoscopy and four through an external incision. Eight patients had never received any surgical treatment. One patient who had been treated with the help of indirect laryngeal mirror had a recurrent cyst, which was removed completely by an external approach. No recurrence has been found in the cases treated with direct laryngoscopy or by external incision. In the former group, Kenl Storz's direct laryngoscope was used in four cases. This laryngoscope can widen the view of the surgical field and make resection of the cyst easier. It is concluded that the best approach to epiglottal cyst is by direct laryngoscopy, and Kerl Storz's Laryngoscope is recommended.

    A clinicopathological study was done on 73 patients with parotid tumors treated during the past nine years in the Department of Otolaryngology, Shikoku Cancer Center Hospital. In 28 cases the tumor was classified histologically as benign parotid tumor, in 21 cases as malignant, and in 24 cases as tumor of non-parotid gland origin. The pathology, diagnosis, therapy and prognosis in these cases were evaluated and discussed.

    We compared the usefulness of magnetic resonance imaging(MRI), with that of X-ray computed tomography in the preoperative diagnosis of parotid tumors. We performed in 13 patients with parotid tumors and 10 of them were operated. The MRI equipment had a magnetic fild of 0.15 Tesla. We used the spine echo acquisition technique and a repetition time of 600, 1000 and 2000 milli-seconds, and echo time of 40 and 80 mi lli-seconds. We found that the T1, weighted image well visualized the duct of the parotid gland, the T2 weighted image provided fine pictures of the parotid tumor. The facial nerve of normal parotid glands could not be visualized by MRI.

    A clinicopathological study was done on 21 cases of nasopharyngeal malignancy seen from 1977 to 1985 at the Department of Head and Neck Diseases, Shikoku Cancer Center Hospital. The male to female ratio was 12 to 9. The age range was 29 to 85 years, with a median age of 70 years. Pathohistological classification showed 14 carcinomas, 6 malignant lymphomas, and 1 rhabdomyosarcoma. The patients were treated with radiation therapy and chemotherapy:the prognosis of malignant lymphoma found to be better than that of carcinoma.

    Many malignant tumors of various origin metastasize to the cervical lymph nodes. We examined a patient with metastatic cervical lymph node cancer secondary to a Meibomian gland adenocarcinoma. The patient was a 43-year-old male treated with right orbital exenteration after a diagnosis of Meibomian gland adenocarcinoma. After 8 months, the patient noted swelling of the right preauricular lymph node and parotid area. Biopsy showed lymph node recurrence of the same Meibomian gland adenocarcinoma. Treatment included right radical neck dissection and subtotal parotidectomy. This case shows that diseases not only of the ear, nose and throat area but also of the eye area can be associated with metastatic cervical lymph node cancer.

    The intrinsic laryngeal muscles contract in a slightly different fashion with each movement during respiration, deglutition and phonation. The present study evaluates the oxygen metabolism of each intrinsic laryngeal muscle, with two different experimental procedures:(1) Immunohistochemical distribution of myoglobin in the intrinsic laryngeal muscles, and (2) tissue oxygen pressure in the intrinsic laryngeal muscles during electrical stimulation of the laryngeal nerves.
    1. In the first series of experiments, the ratio of myoglobin-rich fibers accounted for 67.6% of the cricothyroid muscles, 47.4% of the posterior cricoarytenoid muscles, 33.9% of the lateral cricoarytenoid muscles, 31.6% of the thyroarytenoid muscles and 31.5% of the interarytenoid muscles.
    2. No definite pattern was observed in the arrangement of myoglobin-rich fibers in any of the intrinsic laryngeal muscles.
    3. In the posterior cricoarytenoid muscle, myoglobin-rich fibers were thicker than myoglobin-poor fibers. On the contrary, myoglobin-poor fibers were thicker in the thyroarytenoid, lateral cricoarytenoid and interarytenoid muscles. However, in the cricothyroid muscles there was no relationship between the amount of myoglobin and the diameter of the muscle fibers.
    4. In the second series of experiments, during stimulation of the laryngeal nerves in the cricothyroid and thyroarytenoid muscles, intramuscular tissue oxygen pressures decreased with the onset of muscle contraction. The tissue oxygen pressure dropped further at 30Hz than at 10Hz and showed an even greater decrease at 60Hz than at 30Hz.
    5. Tissue oxygen pressure decreased more in thyroarytenoid than in cricothyroid muscle.
    6. The results of the present studies indicate that for laryngeal function oxygen supply is more necessary in slow muscles, such as the cricothyroid and posterior cricoarytenoid muscles than in fast muscles, such as the thyroarytenoid, lateral cricoarytenoid and interarytenoid muscles.

    It has been demonstrated that the antifungal lipophylic polyene, amphotericin B(AMB)inhibits eukaryotic cell growth by binding to sterols and making ‘holes’ in cell membranes. Used at subinhibitory concentrations, it enhances penetration of other chemicals into the cells. We report here on the sensitivity to AMB of an established nasopharyngeal normal cell line:Ad cells, and of a hybrid nasopharyngeal carcinoma cell line: A2L/AH cells, produced by a cell fusion technique. A2L/AH cells were more sensitive to AMB in respect to cell growth and colony forming activity than were Ad cells. Tumor cells and normal cells were treated with AMB and the anti-cancer drug 5-FU, and their cytotoxic effect was compared. The synergistic effect on A2L/AH cells suggested that AMB enhances the effectiveness of chemotherapy in the treatment of nasopharyngeal carcinoma.

    Perdipine tablets containing 20mg of nicardipine HCl were administered to 54 patients complaining chiefly of dizziness, three times a day for 4 weeks, and the clinical usefulness of the drug was studied.
    The rate of efficacy against dizziness (patients belonging to this category include those in whom the drug had “good” and “excellent” effects) was 73.9%. The rate was 79.4% in patients in whom the drug was administered alone (group A) and 58.3% in patients in whom the drug was used in combination with other drugs (group B). The general rate of improvement for accompanying systems ranging from slight to marked was 63.4%. The rate was 60.0% in group A and 72.7% in group B. The rate was relatively high for nausea, vomiting, headache, tinnitus, and stiff shoulders. After equilibrium tests, the general rate of improvement was 71.4%:the rate was 78.6% in group A and 57.2% in group B. The rate was high for spontaneous nystagmus, positional nystagmus, and head shaking after nystagmus. The rate of usefulness (ranging from possibly useful to clearly useful) was about 75% in both groups. Ten of 12 patients who showed positive reactions to the Schellong test before treatment showed negative reactions after administration of the drug. In patients with hypertension, the drug showed marked hypotensive action, but in patients with normal blood pressure, the action was mild. The incidence of side effects was 13.0%.

    After an I. V. infusion of 2g of Ceftizoxime (CZX), a third generation cephem, its concentration was determined in 23 samples of mucous membranes of patients with chronic sinusitis and in 44 palatine tonsils of those with chronic tonsillitis.
    1)The mean concentration was higher in the mucous membrane of the maxillary sinus than in the palatine tonsil.
    2)The rate of transfer from serum to tissue was also higher in the maxillary sinus than in the palatine tonsil.
    3)The rate of transfer did not vary with the time of resection and was almost constant in the mucous membrane of the maxillary sinus and in the palatine tonsil.
    4)The tissue concentration was adequate for prophylaxis in almost all cases.

    AA-673 nasal spray (2.7 mg in six doses a day) was administered to 17 patients with nasal allergy in all seasons for 4 consecutive weeks. The clinical efficacy was rated as “moderate to marked improvement” in 47% and as “slight to marked improvement” in 82%. The clinical safety was rated as “no problem” in 94%. The general usefulness was considered “moderate to marked usefulness” in 71%. Subjective improvement: in 31 % of the patients sneezing attacks, in 29% nasal discharge and in 29% nasal obstruction were reduced. Objective improvement : in 55% of the patients eosinophiles in nasal discharge were reduced. The conditions of the nasal cavity were not always improved. Intranasal dry feeling was noted in one case. The authors consider that AA-673 nasal spray can be recommended for the treatment of nasal allergy.
