    Mediastinoscopy is a method by which observation, palpation and biopsy of the upper anterior mediastinum can be done through a metal mediastinoscop. This mediastinoscop is inserted in the wound tube made in the upper anterior mediastinum, along the median line and from a small skin incision made in the jugular fossa.
    This method was first introduced in 1959 by Carlens as a method of examining the upper anterior mediastinum of lung cancer patients for the presence of metastasis in lymph nodes and the condition of infiltration of tumor, but at present it is also applied to other pulmonary and mediastinal diseases besides lung cancer.
    This article has been written for the purpose of introducing mediastinoskopy to otorhinolaryngologists.
    The author have applied this method to 34 cases of various chest diseases since the end of 1962, and have found as result that it is significant as an auxiliary method of diagnosis or differential diagnosis of pulmonary and mediastinal diseases.

    The intensity and frequency of sound in varying workshops of a rubber plant were measured in order to study the influence of noises on the workers' hearing acuity. In addition, the effect of TATD (Neuvita), a conjugate of vitamin B1 and thiotic acid, was tested on the workers with hardness of hearing. The results obtained will be summarized as follows:
    1) Among the workshops making a noise exceeding 85 db., a processing workshop which showed a metalic and impinging sound (90-104 db.) with its peak at 2.5-3KC was evidenced to cause a typical and severe deafness.
    2) Ninety-five percent of 1, 076 workers in those workshops consisted of females; 78 percent of these being below 25 years of age and only 5 percent, over 31 years of age. Therefore, comparative studies could not be made on deafness by sex and age distinction. The ratio of male to female was also 5:95 on all workers of the plant.
    3) The deviation in hearing acuity at 4 KC of both ears was 0-10 db. in 254 of the 289 workers (88 percent) in the processing workshop, and this deviation was noted in 83 percent of 128 workers in other workshops with the loudness of sound exceeding 85 db. According to the expression of 0.5+(1×2)+4KC/4, this deviation in hearing acuity was found in as high as 97 percent of all workers. In any case, however, the degree of deviation in those occupational deafness was small. Besides, the hardness of hearing at 4 KC may have a great significance.
    4) TATD given to the processing workers with hardness of hearing, in daily doses of 10mg for 3 months, did not show any preventive or therapeutic effect. While, in those receiving TATD in daily doses of 150mg, however, the hearing acuity came to be improve after 1 month of treatment. After 3 months of such a treatment, the improvement was marked in those working for less than 5 years and was better than our expectation in those working for over 5 years.
    The development of occupational deafness can possibly be minimized, by taking adequate means such as the use of earplugs, reduction of working hours, transfer to a secure workshop, and in addition the use of drugs of this kind.

    A case of cystadenoma originating in the nasal cavity and accessory sinuses which was diagnosed in view of X-ray, pathological finding and operation was described.
    The patient was 34 year-old male with chief complaint of nasal obstruction. And this tumor increased slowly in size of 20 years duration.
    The adenofibroma extending through the nasal cavity, maxillary sinus and the ethmoidal sinus is rare on the place of origin and had not been reported in Japan to say the least of it on the incunabula.
    Judging from the literature available on this subject, then histological and, embryological and symptomatical findings of this tumor were discussed.

    The use of NUTRASE in 2 cases of sudden deafness is summariszed as follows: favarable results are obtained by use of this agent, there was no side-effect in its use.

    Therapeutic effect of TERRA-CORTRIL aural drops composed of oytetracycline and hydrocortisone was tested by using it in the treatment of 16 ears affected with acute suppurative otitis media, 16 ears with chronic suppurative otitis media.
    Once a day the ear under treatment was filled with 0. 2-0. 5 ml of TERRA-CORTRIL aural drops.
    In acute suppurative otitis media the agent was effective in establishing a cure in 100% of total number ; in chronic otitis media, 88%.

    A case of the squamous cell carcinoma of soft palate was reported. The patient, a 54 year-old farmer, was treated with 100mg of Endoxan daily (total dosis 4000mg) by local and intravenous injection and the satisfactory result was obtained.
    No marked side effects were noted.

    Endolaryngeal Application of the corticoid has been widely accepted as a most reliable treatment for the various inflammatory laryngeal diseases. Intralaryngeal injection of dexamethasone providing higher concentration of the corticoid in the vocal cord, showed rapid and exact controll ability of the lesions due to chronic inflammation. Two mgs. of the water soluble dexamethasone was injected into submucous layer of the diseased vocal cord with interval of three days. A group of 49 patients containing 16 cases of chronic laryngitis, 19 cases of the vocal nodule, and 14 cases of the sessible polyp of vocal cord were treated by this injection therapy. Notable effect was seen in the 79% of the total patients. There were no systemic or local reaction caused by the intracordal injection of dexamethasone. The objective sign of the improvement was observed after two or three times of the injections in the cases responded to this therapy. On the basis of our experienes, it can be said that the satisfactory result is attained within seven times of the successive injections. The best results were noted in the cases of chronic laryngitis with reddness and edema of the cords. The granulation tissue appeared after resection of polypoid tissue was also well controlled. The long standing fibrous vocal nodule showed the resistance to this therapy. It is considered that the intracordal injection of various materials can be used in the treatment of many common laryngeal disorders, when the injected material has no local irritability. In conjunction with other forms of laryngeal therapy, this form of treatment gives a more effective result of longer duration.
