    The ability to locate broad band noise in the horizontal plane was measured in an anechoic room with monaural and binaural hearing aids in 13 children 8 to 16 years of age.
    Children with a single aid showed very poor results, and it was not possible to demonstrate any convincing improvement in directional hearing by the use of binaural aids.
    In experiments with 3 adults with normal hearing, some advantages in angular localization with binaural aids over monaural aid were observed. There was no significant difference between the results with binaural bodyworn aids and behind-the-ear aids and results with directional and conventional microphones.

    Statographic examination showed that the incidence of pathological postural sway in vertiginous patients after head and neck injury is more frequent than in nontraumatic vertiginous patients.
    However, the neural mechanism underlying the pathological postural sway observed in post-traumatic patients has not been fully clarified.
    We found that the pathological postural sway can be clearly analysed, if the patients standing on a platform loaded with a mattress and receiving vibratory stimulation to the neck muscle or bilateral calf muscles are examined statographically.
    Our studies have shown that such sway is largely due to disorders in the supraspinal control of the proprioceptive system.

  • An Electroneuronographic Study

    Using electroneuronography (NG), the prognosis of 60 patients with peripheral facial palsy including Bell's palsy (42 cases), Hunt's syndrome (15 cases) and traumatic facial palsy (3 cases) were investigated.
    In most cases in this series, the first measurement of NG% (amplitude of neuronograph on paralysed side/amplitude of neuronograph in unimpaired side) was done within four weeks after onset of the palsy. NG% deteriorating below 19% within four weeks after onset was observed in 19.4% with Bell's palsy and in 44.5% with Hunt's syndrome.
    During the observation period there was no improvement of NG% in Bell's palsy in which NG% deteriorated below 19% in early stage of the palsy. But in contrast to Bell's palsy, there was some improvement of NG% parallel to the clinical recovery of the palsy those with Hunt's syndrome in which NG% deteriorated below 19% in early stage of the palsy.
    In two with Hunt's syndrome decompression surgery was done and there was some improvement in NG% in the fifth or sixth month, post-operatively.
    In the early stage of the facial palsy, we use routinely the Nerve Excitability Test (NET) and NG for estimating the prognosis. In cases in which both NET and NG showed good responses in the early stage, the palsy recovered within three months. In cases in which both NET and NG showed poor responses, a longer period (more than three months) was required for recovery. The prognosis of patients in which responses between NET and NG showed a discrepancy in early stages of the palsy was almost the same as that indicated with NG. Therefore, NG is more accurate than NET for estimating the prognosis. But at present, the combination of NET and NG is apparently the best way for estimating the prognosis. After the clinical recovery appeared to come to the end in the patients with incomplete recovery, NET showed findings parallel to the clinical recovery compared with the other electrical tests.
    Through the combination with NET, NG is at present a most useful test for estimating the prognosis, but with these two tests, the indication for decompression surgery cannot be determined in most patients with peripheral facial palsy.

    Children with adenoid symptoms reveal high levels of serum immunoglobulin D.
    Several cases such were always accompanied by recurrent rhinosinusitis, relapsing otitis media and cervical lymphatic swelling.
    We selected two such cases and described their clinical picture and course, and further demonstrated that the value of serum immunoglobulin D was remarkably decreased postoperatively by adenoidectomy.
    There was a quasi-coincidence of time between the decrease of value of immunoglobulin D and the mechanism of healing of various complicated factors in these two cases. Furthermore, the serum immunoglobulin D remained low unless there was a relapse of the adenoid-symptoms or recurrent infections.
    From these observations it is suggested that there is a close relation between the recurrent or relapsing infection (upper respiratory tract), adenoid tissue (Waldyer's ring) and serum immunoglobulin D.

    Laryngoscopic and vocal changes in aged persons were examined in 20 males and 20 females whose ages ranged from 69 to 85 years, with the mean of 75 years. From their recorded voices, assessment of voice quality, measurements of both the fundamental frequency, and the pitch perturbation were carried out.
    The characteristic findings obtained were as follows: (a) the aged males tended to show a marked vocal cord atrophy and/or edema with a higher fundamental frequency of voice (mean of 162Hz) than that in adult males, (b) the aged females tended to have a vocal cord edema and slight hoarseness with a relatively lower fundamental frequency (mean of 165Hz) than that of adult females.
    Vocal changes in senescence are characterized by slight hoarseness or a noticeable change in fundamental frequency of voice.
    The change of vocal cords in terms of mass, such as atrophy or edema is considered to be the most responsible factor in the above vocal changes during senescence.

    Kallikrein, first extracted from human urine by Frey et al. in 1925, is now widely prescribed as a hypotensive agent. It is a kind of mucoprotein originated from animal tissue, and certain allergic diseases may be caused by inhalation or contact with Kallikrein.
    The clinical course and immunological tests carried out on three patients with an occupationally related allergy induced by inhalation of Kallikrein are reported. All were employed in the same chemical factory which was producing Kallikrein.Case 1- a 42 year-old housewife with nasal allergy.
    This patient had often suffered from congelation urticaria, and visited our clinic complaining of nasal allergy. Three months later the first attack of bronchial asthma occurred following dense exposure to Kallikrein for three months. She retired and is in good health without any complaints.
    Case 2-a 28 year-old unmarried woman with bronchial asthma.
    This patient had often had contact dermatitis after using certain soaps and cosmetics. The first attack of bronchial asthma occurred 9 months after she started working with Kallikrein capsules. Although her position was changed from the capsule making division to the tablet making division, slight symptoms are still evident.
    Case 3- a 57 year-old man with bronchial asthma.
    After dense exposure to Kallikrein for 6 months, serious attacks of bronchial asthma frequently occurred. After retiring he is in good health with no complaints.
    Both in cases 1 and 2, the laboratory examination revealed a remarkable eosinophilia in the peripheral blood and in the nasal smear. Levels of serum IgE. were normal. The intracutaneous test, the nasal provocative test, and Prausnitz-Küstner's passive transfer test showed respectively positive reactions in all cases. Case histories and laboratory data indicate that these three cases are those of an occupational allergy induced by inhalation of Kallikrein.

  • Treatment with Biscoclaurin Alkaloid

    The effectiveness of biscoclaurin alkaloid for nasal allergy was evaluated.
    Biscoclaurin alkaloid was orally administered three times a day after each meal (30mg/day) for two to 22 weeks to 50 patients who complained of symptoms of nasal allergy (hydrorrhoea, sneezing and nasal obstruction).
    In the clinical test of function in nasal allergy, it was found that the compound was effective in 34 (68%).
    No side effects were noted when the above stated dosage was given.
    Pharmacological research revealed that this drug blocks of release of chemical mediators such as histamine, serotonin etc. from mast cells and stimulates the production of antibodies.
    This drug when given in combination with the specific or non-specific desensitization treatments appears to be effective for nasal allergy.

  • Underdeveloped Small Sinuses -Report 1-

    The vertical position (depth) of anterior concave plane (ACP) of the orbital floor was evaluated by plain films of Waters' view. A vertical line was drawn and passed through the center of the infraorbital canal. The point where the vertical line crosses the superior orbital margin was chosen for the start of measurement. The distance (A) from the point to the infraorbital canal is one of the indicators for determining the depth of ACP. Another distance (B), from the point to where the vertical line crosses the inferior orbital margin, was also included. The analysis included; 478 adults with bilateral standard development of the sinuses, 20 adults with unilateral small sinuses and 48 adults with bilateral small sinuses as well as 108 infants with standard sinus development for the respective ages.
    The distance (A) was larger and such was of statistical significance (5% risk) in the small sinus groups, both unilateral and bilateral, in comparison to the standard group, the average difference being 3.2mm. No significant difference was seen in distance (B). The orbital floor was investigated from the standpoint of phylogenetic, developmental and histological aspects. The orbital floor was shown to be both adaptable and unstable under various postnatal conditions. The significance of the maxillary sinus itself was also discussed according to the functional matrix concept of Moss and others.
