  • -The Future Treatment of Sensorineural Hearing Disturbance-

    In order to investigate the possibility of the treatment of sensorineural hearing disturbance, two kinds of experiments were performed using animals. First, the ventral cochlear tract in the brain stem to pons was transected in adult rats. Tissue from embryos was transplanted to the lesion site. In 20% of the rats examined, the axons regrew beyond the transected site and regenerated into the denervated side and terminated at the normal targets. The hearing function of animals also recovered. Those findings contradict the widely held view that the adult mammalian central auditory system cannot be restored following damage.
    In addition, adult rat hippocampus-derived neural stem cells (NSC) were grafted into newborn rat cochlea. Within two to four weeks of grafting to the cochlea, some NSC survived in the cochlear cavity and some of them had adopted the morphologies and positions of hair cells. This suggests that NSC can adapt to the environment of the cochlea and gives hope for the treatment of damaged cochlea and sensorineural hearing loss.

    We performed a questionnaire-study of the life-styles of 150 people who were working in the same company (as control) and 21 patients with Ménière's disease. The questionnaire consisted of questions on causative stress, behavioral characteristics (type A, self-restraint, escape), and relaxation. Patients with Ménière's disease showed significantly higher scores on the behavioral characteristics questions than controls. However, the scores on causative stress and relaxation questions were not different between the two groups. Thus, behavioral characteristics may play an important role in diagnosing the onset of Ménière's disease. We conclude that patients with early-stage Ménière's disease should be treated using relaxation therapy as well as by paying attention to their overall life-style.

    A case of fracture of the stapes footplate with ossicular disruption and perilymph fistula caused by earpick-induced trauma is reported. A 51-year-old female was presented with hearing loss and vertigo just after sticking an ear pick into her left ear. Pure tone audiometry showed a mixed hearing loss of 67.5dB. Nystagmus was not observed, and vertigo improved in a week. On exploratory tympanotomy, the fracture of the stapes footplate accompanying a dislocation of the incudo-stapedial joint and perilymphatic leakage from the oval window were found. The perilymph fistula was sealed with temporal fascia, and ossicular chain continuity was reconstructed by type N tympanoplasty. After the operation, the cochlear and vestibular symptoms improved.

    We report a case of neurinoma of the nasal septum. The patient was a 44-year-old male. He visited our hospital because of left nasal bleeding and obstruction. His left nasal cavity was filled with a mass. Because we suspected neurinoma from biopsy, we resected the mass by Denker's operation under general anesthesia. We found that it originated from the nasal septum. The histopathological finding of the tumor was an Antoni A and B type neurinoma.

    We report a case of Kimura's disease (eosinophilic granuloma of soft tissue). A 27-year-old male was admitted to our hospital with swelling due to a left preauricular lesion and cervical lymph adenopathy. Kimura's disease is a relatively rare condition and usually occurs in the soft tissue of the head and neck. It is associated with eosinophilia and an elevation of serum IgE. The main histopathological features are proliferation of lymphfolliculoid structures, granulation tissue with marked infiltration of eosinophil and mast cells, and fibrosis. Our patient was also diagnosed by biopsy of the preauricular lesion. The patient was treated with steroids only, showing remarkable improvement. However, since this disease has been reported to recur often after a long period, further follow-up is necessary.

    We report a case of a 9-year-old boy with pneumoparotitis. This patient complained of recurrent swelling on the left-side of his neck. Crepitation could be heard around the left parotid gland and neck and bubble saliva was dischaged from the Stensen's duct when the left parotid gland was pressed. Radiologic evaluation revealed pneumoparotitis associated with subcutaneous emphysema. A probe was inserted into the Stensen's duct, and although we were not successful in draining all the air out of the parotid gland, the duration of swelling was extremely reduced after the probe was inserted.
    Although we could not discover the cause of this pneumoparotitis, we suspected that the Stensen's duct might act as a check valve, related to sialectasis.

    Recently, a consensus that nasal continuous positive airway pressure (n-CPAP) treatment is the first choice therapy for severe Obstructive Sleep Apnea Hypopnea Syndrome (OSAHS). However, it is also recognized that n-CPAP is an imperfect therapy and many patients find this treatment intolerable. In this study, four severe n-CPAP intolerant OSAHS patients, with a shallow oropharyngeal space, surrounded by large uvula and enlarged tonsils recieved Uvulopalatopharyngoplasty (UPPP) to decrease the pressure, and then remove n-CPAP. We also encouraged the patients to lose weight. After the operation, sleep apnea hypopnea improved in all the patients, two of whom could remove n-CPAP completely and two of whom continued to use n-CPAP at a lower pressure level. UPPP is an effective treatment for patients with a shallow oropharyngeal space who are unable to tolerate n-CPAP because of its side effects.

    Neurilemmoma (schwannoma) is frequently located in the head and neck, but is relatively rare at the base of tongue.
    We treated a 6-year-old boy with schwannoma at the base of tongue. Extirpation of the tumor was performed by the oral approach. A histopathological study revealed diagnosis neurilemmoma including both Antoni type A and B regions. He had no lingual paralysis or taste loss after the surgery. The origin of the nerve could not be identified. A review of the literature disclosed 30 cases of this tumor. We conclude that it is important to be aware of this tumor when we are diagnosing oral tumors in children. The clinical features of this tumor were discussed.

    Localized laryngeal amyloidosis is rare. Only six cases have been reported in young men. We present the case of a 19-year-old male with localized laryngeal amyloidosis. The patient presented with a 5 year history of progressive hoarseness. Flexible fiberscopy revealed a smooth and submucosal mass of the right false vocal cord. Biopsy findings were consistent with amyloidosis. The mass was excised and transpired with a CO2 laser. An extensive workup for systemic amyloidosis was negative.

    Cricohyoidoepiglotto-pexy (CHEP) and Cricohyoido-pexy (CHP) are indicated for early stage and selected advanced stage laryngeal cancers. In this subtotal laryngeal surgery, thyroid cartilage is removed along with its intrinsic tissues leaving both arytenoids. The laryngeal defect is closed by approximating and fixing the cricoid cartilage to the hyoid bone.
    Between 1997 and 2000, four patients received CHEP and one patient received CHP at our institute. The average admission dates were 110 days. Decanulation and initiation of swallowing rehabilitation were achieved at a mean of 22 and 40 post-operative days respectively. All patients acquired breathy but satisfactory vocal function shortly after surgery. Three patients obtained excellent swallowing function after 1 month of intense rehabilitation. Two patients still have some difficulty with liquid foods. The clinical and technical problems, which might affect the postoperative course, are discussed in detail.

    We treated an ectopic parathyroid tumor which had not been detected by several tests, but was detected by MIBI scintigraphy. The patient was a 34-year-old woman. In December 1987, she was diagnosed as having left renal calculus and primary hyperparathyroidism. Exploration of her cervical region was performed at our urological section. Post-operative pathosis for the former section was normal, but progress after operation had kept showing higher value on Ca. In March 1996, MIBI scintigraphy was performed. Intussusception was found in the right submaxillary region, and she was therefore readmitted for enucleation of an ectopic parathyroid tumor on January 20, 1997. The ectopic parathyroid tumor was present between the right common carotid artery and the right internal carotid artery, and the histological examination revealed parathyroid hyperplasia.

    Between June 1988 and December 1996, 57 patients with non-Hodgkin's lymphomas of the head and neck were treated at our department. The primary lesion was located in Waldeyer's ring in 31 patients and cervical lymph nodes in 26 patients. These patients, who were diagnosed as Stage I or II, were treated with chemotherapy and radiation. The chemotherapy regimen consisted of pirarubicin (45mg/m2; day 1), cyclophosphamide (750mg/m2; dayl), vindesine (1.5mg/m2; day 1) and predonisolone (100mg/body; days 1-5). The complete and partial response rates were 79% and 82% respectively. The five-year estimated overall survival rate calculated by Kaplan-Meier method was 75%. We concluded that the combination of chemotherapy regimen (THP-CVP) and radiation were useful and safe for treating non-Hodgkin's lymphomas of the head and neck.

    The incidence of multiple primary cancers in the head and neck has increased a long with the progress made in the diagnosis of these cancers.
    In 537 patients with head and neck cancer treated in Kagawa Medical University from October, 1983 to March, 1998, 51 cases (9.5%) had multiple primary cancers. The mean age of 51 patients was 66.2 years and the male to female ratio was 4:1. The highest incidence of multiple primary cancers was found in hypopharyngeal cancers (31.0%). Fourteen of 51 cases (27.5%) exhibited as second primary cancers synchronously within 6 months, and 37 cases (72.5%) metachronously. About 70% of the additional cancers were in the “multicentric zone”, the stomach, lung, esophagus, head, and neck.
    The 5-year survival rate for all patients was 35.6%. There was a significant difference in the survival rate between ‘synchronous’ multiple primary cancers (15.6%) and `metachronous' ones (41.6%).
    In treating synchronous multiple primary cancers, the patient's prognosis, the selection of the optimum treatment, quality of life and the location and extent of cancers are important factors to consider. To improve the prognosis in patients with metachronous multiple primary cancers, more sensitive and convenient examinations should be performed for the early detection of the second cancer.

    A case of an abscess of the neck caused by Mycobacterium tuberculosis is reported. A 23-yearold female visited Ako City Hospital complaining of a high fever and swelling of the left side of her neck. A CT scan revealed multiple lymph node swelling and an abscess formation on her neck. Mycobacterium tuberculosis was diagnosed by the polymerase chain reaction (PCR) method of the abscess obtained by fine-needle aspiration (FNA). After the diagnosis, anti-mycobacterial therapy was started, and FNA was performed three times to prevent rupture. The indication of surgical therapy is discussed.

  • -Changes in Response of Peripheral Blood Mononuclear Cells to Cry j 1-

    This study was designed to investigate the effect of a single year of immunotherapy on cytokine synthesis by allergen-stimulated peripheral blood mononuclear cells (PBMCs) of patients with seasonal allergic rhinitis. This study included 12 nonatopic subjects (nonatopic group) and 28 patients with seasonal allergic rhinitis due to Japanese cedar pollen. Immediately after the pollen season 1998, 14 patients started receiving immunotherapy (immunotherapy group), and the remaining were treated with antihistamines during the pollen season 1999 (medication group). PBMCs (1.0×106 cells/mL) were obtained from each subject before and during the pollen season 1999, and cultured for 96 hours in the absence of and in the presence of 10μg/mL of Cry j 1. The levels of IL-4, IL-5 and IFN-γ in the supernatants were determined using enzyme-linked immunosorbent assay (ELISA). Seasonal increase in Cry j 1-specific IL-4 and IL-5 synthesis was significant in the medication group and in the immunotherapy group, but not in the nonatopic group. Although the levels of Cry j 1-dependent IL-4 and IL-5 before the pollen season did not differ significantly between the untreated group and the immunotherapy group, the levels of Cry j 1-dependent IL-4 and IL-5 during the pollen season were significantly lower in the immunotherapy group than in the untreated group. The levels of Cry j 1-dependent IL-4 before as well as during the pollen season did not differed significantly between the good and poor responders to immunotherapy, whereas the levels of Cry j 1-dependent IL-5 during the pollen season were significantly lower in the good responders than in the poor responders to immunotherapy. Therefore, we concluded that the suppression of Cry j 1-dependent IL-5-producing Th2 cells during the pollen season is a major working mechanism of a single year of immunotherapy for seasonal allergic rhinitis due to Japanese cedar pollen.
