    Phase spectral analysis of the auditory brainstem responses (ABRs) to click stimuli of different intensies and without stimuli was performed in 20 normal adults. At each stimulus level, a collection 10 groups of ABRs, each consisting of 200 sweeps, was used for calculating the phase variance by fast Fourier transform.
    The phase variance for selected frequency components making up the ABRs was compared to the ABR morphology, and the sensitivity of this phase spectral analysis for the automatic detection of response threshold is discussed.
    It was found that the mean phase variance calculated from 97.7 to 293.0Hz frequency components was a good predictor for detecting ABR threshold.
    The results show that this procedure is accurate enough for automatic ABR threshold detection.

    High resolution computed tomography (HRCT), General Electric CT/T 9800 with high resolution program, was performed on 9 ears of 6 patients with so-called Mondini type inner ear anomalies.
    Quite sharp and fine images of the temporal bone structures were obtained by HRCT.
    Seven fine elements of inner ear structures on HRCT figures enabled us to classify so-called Mondini type inner ear anomalies in more detail.
    It is obvious that HRCT will replace conventional polytomography in the diagnosis of inner ear anomalies.

    The postoperative course was evaluated in 49 ears with chronic suppurative otitis media with special reference to the bacteriological findings.
    1) Before operation, 67 strains were isolated from 49 specimens. The most frequently isolated species was Pseudomonas aeruginosa (26.9%), followed by Staphylococcus aureus (23.9%).
    2) Thirty to fifty per cent of the strains of Pseudomonas aerusinosa were resistant to CBPC, CPZ, TOB, DKB and AMK, and most of them showed low sensitivity to MIND, PPA and FOM.
    3) Although the aural discharge was controlled in 72.2% of all the cases of Peudomonas infection by preadministration of antibiotics, the operative findings revealed that the infection of the middle ear cavity was not eradicated in 88.9% of them.
    4) The postoperative course was uneventful, and the results were satisfactory in all ears, regardless of the presence or absence of infection.
    5) It was concluded that the surgical eradication of infected foci is extremely important to control middle ear infection.

    To clarify the actual condition of middle ear diseases in cleft palates and the relationship between middle ear disease and Eustachian tube function, otological examinations and tests for Eustachian tube function were carried out in 100 patients with repaired cleft palates. The following findings were obtained: 1) cleft palates showed the abnormal ear drums in 87.9%; 2) hearing loss was found in 37.0% of the ears and abnormal tympanograms were found in 50.8% of the ears, both hearing loss and abnormal tympanograms tended to decrease until the age of 7-8 years; 3) Eustachian tube function was frequently abnormal in younger children, but it tended to improve at 7-8 years of age; 4) hearing loss and abnormal tympanograms were common in patients with abnormal Eustachian tube function.
    Thus, it was revealed that cleft palates had high incidence of middle ear diseases, and that Eustachian tube dysfunction was thought to be one of main causes of middle ear diseases in cleft palates. As severe middle ear diseases were found in cleft palates, though the frequency was not high, intensive otological care seemed to be necessary for cleft palate patients.

    A 43-year-old female had bilateral otosclerosis with a persistent stapedial artery only in the left ear. This artery measured approximately 0.4mm in diameter. It was seen on the floor of the tympanic cavity, ran between the crura of the stapes and entered the facial canal. At the time of left ear surgery we could perform stapedectomy after cutting this artery and stopping bleeding. A satisfactory hearing gain was obtained in both ears after stapedectomy.

    Eight cases of carcinoma of the external auditory canal were classified into three groups according to the extent of the malignancy:
    Group I. Tumor limited to the external auditory canal.
    Subgroup A: no invasion of bone is suggested radiologically; tumor is located at some distance from the tympanic membrane (three cases).
    Subgroup B: no invasion of bone is suggested radiologically; tumor is close to or in contact with the tympanic membrane (two cases).
    Group II. The tumor invades anteriorly the parotid gland and the temporomandibular joint, although the middle ear and mastoid air cells are not involved (one case). Group III, The malignancy has already spread to the middle ear and/or mastoid air cells (three cases).
    Both Groups I and II were treated surgically to eradicate the disease, while a combination of surgery and irradiation was performed in Group III.
    In Group I, tumor recurred in only one patient (subgroup A). This case was reclassified in Group II at the time of recurrence, and the tumor was removed surgically. One year later there was no evidence of recurrence. Two patients in Group III died within a year and the third remains well 14 months after discharge. In conclusion, successful management of carcinoma of the external auditory canal depends on early diagnosis, correct evaluation of the extent of the malignancy and adequate surgery based on correct evaluation.

    Culturette Brand 10-minute Group A Strept ID was developed as a means of rapidly diagnosing Group A streptococcal infections. Its clinical usefulness was compared with that of conventional bacterial cultures in 300 cases of tonsillitis and pharyngitis.
    1) The detection of Group A streptococci by culture and Strept ID agreed in 91% of all the cases.
    2) Culture revealed Group A streptococci in 50 cases, Strept ID detected the same microbials in 37 cases. Thus, the sensitivity of Strept ID was 74%. Culture for Group A streptococci was negative in 250 cases, and Strept ID was negative in 236 cases, 94.4% specificity.
    3) Strept ID can diagnose Group A streptococcal infections in only 10 minutes with high accuracy. Since the technique is also simple, Strept ID is very useful in the diagnosis and treatment of outpatients.

    Magnetic resonance imaging (MRI) was used in the diagnosis of head and neck tumors. The advantages and the problems of MRI are discussed and compared with those of X-ray computed tomography.
    MRI provides more detailed information about the nature and the depth of in-filtration of tumors, not only because of the excellent soft-tissue contrast, but also because multiple directions can be used. An additional benefit is the much easier identification of cervical vascular structures by MRI.

    A 29-year-old male had a liposarcoma in the right maxillary region, a rare tumor. Multidisciplinary treatment continued for 6 years and 10 months. Treatment included two radical operations, chemotherapy, radiotherapy, cryosurgery and laser surgery.
    For this localized liposarcoma, tumor extirpation was considered to be the most effective therapy. Chemotherapy and radiotherapy were also effective for recurrences. Cryosurgery was easily applied without any side-effects and reduced tumor volume. However, the tumor relapsed repeatedly 5 months before death, in spite of extensive resection of the right facial bones and the right eye.
    Autopsy confirmed the recurrent tumor and revealed micrometastasis in the regional lymphnodes. Although intracranial invasion by the tumor had been suspected clinically, neither direct invasion nor hematogenic metastasis present in the brain. Inflammatory changes and a brain abscess were found.

    A 43-year-old male had eosinophilic granuloma of the soft tissue in the parotid region. The treatment of this disease has not been established. The possibilities are surgery, radiation, steroid therapy, antiallergic therapy, antiinflammatory therapy, etc. Single therapy is not very effective, so a combination of these therapies should be used. Surgery, low dose radiation, antiallergic therapy, antiinflammatory drugs and steroids were effective. During outpatient follow-up care, the drugs should be changed when the tumor increases in size.

    This is a case report of occult thyroid carcinoma with metastasis to a cervical lymphnode.
    A 45-year-old female complained of a painless cervical tumor which had grown slowly for ten months. Metastatic carcinoma of a cervical lymphnode was suspected. General physical examination and several tests, including RI-scintigraphy, ultrasonography and CT-scan of the neck revealed no primary lesion in the head or neck.
    Histological examination of the biopsied node showed metastatic carcinoma of the thyroid. The original tumor in the thyroid gland was only 2×2mm in size so could not be found prior to surgery. This is called “occult thyroid carcinoma”.

    Operations requiring a direct laryngoscope or a rigid bronchoscope under general anesthesia are difficult for both surgeon and anesthesiologist because the airway overlaps the field of operation.
    Recently, jet ventilation (J. V.) or high frequency jet ventilation (H. F. J. V.) has been used in general anesthesia for bronchoscopy or direct laryngoscopy instead of conventional ventilation bronchoscopy or tracheal intubation.
    We have used J. V. or H. F. J. V. in general anesthesia for bronchoscopy in 2 cases and for direct laryngoscopy in 11 cases. For bronchoscopy, since both patients had tracheotomies, a metal cannula (about 16G) was inserted into the trachea through the tracheostomy opening and J. V. or H. F. J. V. was performed through the cannula. For surgery using direct laryngoscopy, an S-shaped flexible cannula (16G) was inserted through the crico-thyroid membrane, and J. V. or H. F. J. V. was performed through the cannula.
    In each operation under general anesthesia with complete muscle relaxation, ventilation was almost adequate and the field of operation was not obstructed.

  • -Influence of Anatomical Differences in the External Ear Canal-

    The frequency characteristics of the external ear canal in patients wearing hearing aids were measured by a probe tube microphone under several pathological conditions: stenosis of the external ear canal, central ear drum perforation, destruction of the external attic wall, open mastoid cavity after tympanoplasty. The sound pressure level was higher in small external ear cavities than in normal ears. In central perforation of the drum, a somewhat lower sound pressure level was found in some cases. In ears with destruction of the external attic wall or with an open mastoid cavity, relatively deep dips were found in the frequency range between 1KHz and 5KHz, and the peak frequency was different in each case. The necessity to take these facts into account in fitting hearing aids is emphasized.

    Vestibular functions were investigated in mice with inner ear damage induced by Mycobacterium fortuitum injected into the caudal vein.
    After the injection of M. fortuitum, the behavior of the mice was observed and recorded:
    1. behavior in the open field, 2. auditory startle reflex, 3. transversing a narrow path, 4. descending a rope, 5. negative geotaxis, 6. grasping a rod, 7. swimming.
    Seven days after the injection, some mice showed abnormal behavior, such as inability to standing, swim, transverse a narrow path, graspe rod, or descend a rope or negative geotaxis. Such abnormal behavior was observed in 60-70% of the mice 14 days after the injection of M. fortuitum.

  • -Injection of Steroid Solution into the Pharyngeal Orifice of the Eustachian Tube in Cases of Occluded Tube and Otitis Media with Effusion-

    Eighty ears of patients with occluded Eustachian tubes and 39 ears of those with otitis media with effusion (OME) were treated by injection of steroid solution into the pharyngeal orifices of the Eustachian tube.
    Eustachian tube function was examined by several methods: 1) inspection of ear drums, 2) audiometry, 3) tympanometry, 4) tubo-tympanoaerodynometry, 5) endoscopy of the epipharynx.
    Edematous swelling of the pharyngeal orifice of the Eustachian tube was observed in all cases and this condition was considered to be the chief cause of tubal disturbance. In all cases, injection of steroid solution was performed through the nasal cavity under direct observation of the tubal orifice with a nasopharyngoscope under local anesthesia in the sitting position.
    Sixty-five ears (81.3%) with occluded tubes and 26 ears (66.7%) with OME responded well to this injection method.
    Therefore, this seems to be a reliable treatment for these diseases.
    In a five year follow up study no severe side effects were noted.

    It is said that Coenzyme Q10 augments the inhibition of DNA synthesis of cultured mouse sarcoma cells (Meth A cell) produced by Adriamycin in a dose dependent manner.
    To elucidate this enhancing mechanism, we analyzed the intracellular concentrations of Adriamycin in Meth A cells and Swiss albino 3T6 immortalized cells by flowcytometry techniques. In Meth A cells, a large part of Adriamycin's action is due to an active transport system, especially in influx. The intracellular concentration of Adriamycin rises with increasing doses of Coenzyme Q10. Thus, the augmenting antitumor effect of Coenzyme Q10 depends on its rapid increase and on the maintenance of a high intracellular concentration of Adriamycin. On the contrary in 3T6 cells, the level of Adriamycin decreases with increasing doses of Coenzyme Q10, because active efflux is more enhanced by the presence of Coenzyme Q10 than active influx, while passive influx and active efflux are the main actions of Adriamycin in the usual state without Coenzyme Q10.
    It is concluded that Coenzyme Q10 plays an opposite role in tumor cells and normal cells by the different way and it affects the transport mechanism of Adriamycin. These findings suggest that combination chemotherapy with Coenzyme Q10 and Adriamycin is more effective, not only because it prevents adverse reactions such as cardiopathy, but also because it augments the anticancer effect of Adriamycin.

    Myonal® (a muscle relaxant) was used alone or in combination with other drugs in the treatment of 33 males (53.1±13.6 y. o.) and 26 females (54.8±12.5 y. o.) with tinnitus. The treatment was effective in 42.4% of the patients. A better effect was obtained in the following patients:
    1. Those in the 5th and 6th decades.
    2. When Myonal® was used in combination with other drugs.
    3. When treatment was continued for 8 weeks or longer.
    Side effects were observed in 4 patients (6.8%).

    This study evaluates the effect of serratiopeptidase (SER), a proteolytic enzyme, on the viscoelasticity of nasal mucus in adult patients with chronic sinusitis. SER was administered in a dose of 30mg/day orally for four weeks. Sixteen specimens of mucus were collected from each side of the nasal cavity of the 11 patients before (week 0) and four weeks after the start of the administration (week 4).
    The storage modulus (G') and dynamic viscosity (η') of each specimen of nasal mucus were determined by an oscillating sphere magnetic rheometer at frequencies of 0.5, 1, 5, 10 and 20Hz at a constant temperature of 25°C.
    The dynamic viscosity (η') of the mucus at week 4 was significantly lower than at week 0 (at frequencies of 5, 10 and 20Hz). No significant difference was observed in the storage modulus (G') between the mucus at week 0 and week 4. Thus SER reduces viscosity but not in elasticity and is considered to be a useful drug in the conservative treatment of chronic sinusitis.

    The effect of Intal 2% Nasal Solution was evaluated in 37 patients suffering from Otitis Media with Effusion (O. M. E.).
    The subjects were divided into two groups;
    group A: O. M. E. with atopic allergy (15 cases).
    group B: O. M. E. without any allergic factor (22 cases).
    1) The effect of Intal was significantly different in the two groups.
    The overall rating was excellent or good in 66.7% of group A and in 13.6% of group B.
    2) In group A, the effect of Intal on O. M. E. was especially related to its effect on nasal obstruction.
    3) The results suggest a relationship between O. M. E. and atopic allergy, for example nasal allergy.
    4) No side effects were observed in this study.
