    Long-term results of endolymphatic sac operation were evaluated in 15 patients with Meniere's disease. At the time of surgery, the endolymphatic sac could be identified in all cases. It was situated on Donaldson's line in 8 cases, and in a posterocaudal position in 7 cases. The clinical results were evaluated according to the AAoo classification as A class in 4 cases, B class in 8 cases, C class in 2 cases and D class in 1 case. The D class patient, who had had Arenberg's inner ear valve implant surgery was operated on again in an attempt to control recurrent attacks of vertigo. The other 14 patients, who were treated with Kitahara's endolymphatic sac operation, had no recurrence of vertigo. The significant electrocochleographic findings after endolymphatic sac surgery were a decrease in the SP/AP ratio and a reduction in the SP area in 5 cases and a decrease in the positive SP in 4 cases.

  • -3rd Report: The Fistula Sign Caused by Sound Stimuli-The Tullio Phenomenon-

    We have treated 6 patients with the Tullio phenomenon and the fistula sign at the same time, and were able to make ENG records. The findings of the tympanic membranes were almost normal and serologic testings for syphilis were negative in all cases.
    The frequencies of 250Hz, 500Hz and 1kHz were the most effective in producing the Tullio phenomenon.
    In one case, nystagmus toward the stimulated ear during the sound stimulus and transiently toward the opposite ear after the stimulus, was observed. Both the ves tibuloocular reflex and the vestibulospinal reflex were recorded in this case.
    Our investigations in these 6 cases indicated that either a hydrostatic route, from the footplate via the membranous labyrinth to the cupula, or hyperexcitablity of the vestibular labyrinth contributed to the production of the Tullio phenomenon and the fistula sign.

    Eustachian tube function was examined in 30 patients with middle ear cholesteatoma who underwent middle ear surgery. In 90% of patients with retraction type cholesteatoma, the forced opening pressures of the tube were either lower than or within the normal range of forced opening pressures. 69% of patients with retraction type cholesteatoma could equilibrate the applied negative middle ear pressure by swallowing. Clearance function examined by a dye technique was not impaired in 81% of patients with retraction type cholesteatoma. These results show that eustachian tube function in retraction type cholesteatoma is not significantly impaired.

    Myringotomy was performed in 36 ears of 21 children with otitis media with effusion, and their middle ear effusions (MEE) were examined. S-carboxymethylcysteine (SCMC) was administered in a dose of 30mg/day orally for four weeks following the myringotomy. Seventeen of the 36 ears (47.2%) showed complete or partial recovery on clinical examination. Myringotomy was repeated in the other 19 ears, and the MEE were collected again.
    Both storage modulus (G')and dynamic viscosity (η')of the paired MEEs were determined by an oscillating sphere magnetic rheometer at a frequency of 1 Hz under a constant temperature of 25°C. No significant difference was observed in G' or η' between the MEEs before and after treatment. These results indicate that the combination of myringotomy and SCMC did not change the viscoelastic properties of MEE within 4 weeds.

    Otological studies of 220 patients with cleft palate are described. Eustachian tube function was evaluated by inflation-deflation and clearance tests. Our results were as follows:
    1. Intact ears of the cleft palate patients showed a comparatively low opening pressure, and more than half of them could not equilibrated applied negative middle ear pressure, suggesting a potentially unfavorable anatomical condition of their eustachian tubes.
    2. The opening pressures in the ears which needed repeated tube insertion were significantly lower than in the ears which healed easily, suggesting that opening pressure plays a very important role in OME in the cleft palate population.
    3. Contamination from the oral cavity to the pharyngeal orifice of the tube showed no correlation with OME in cleft palate patients.

    The factors involved in determining the prognosis of Bell's palsy were evaluated by multivariate analysis according to the theory of quantification II. The factors chosen for analysis are the sex and the age of the patients, the seasons of the onset and period between onset and diagnosis.
    The season of onset was the most closely related to prognosis and sex was the least. The sample score of each patient was calculated, and on the basis of this score it was possible to divide the patients into two groups : good or poor prognosis.
    These results suggest that multivariate analysis is useful in evaluating the prognosis of Bell's palsy. Further analysis should be done on a larger scale.

    A patient with Hunt's syndrome had not only facial and vestibular nerve paralysis, but also glossopharyngeal, vagal and hypoglossal nerve paralysis. The patient was a 74-year-old fbmale who complained of right auricular pain, vertigo, hoarseness and right facial palsy. Physical examination revealed right auricular herpetic vesicles, peripheral facial palsy, paralysis of the soft palate, right laryngeal paralysis and deviation of the tongue. Hearing was also impaired. Vestibular examination showed right CP and left DP. A varicella-zoster virus(VZV)skin test reflecting cellmediated immunity was negative; a PPD skin test was positive. T and NK cells were decreased. VZV-antibody titers was remarkably high. These laboratory examination showed that humoral immunity was relatively normal but cellmediated immunity was depressed in Hunt's syndrome.

    We treated three patients with neurinoma of the oral cavity. One was on the hard palate of a 50-year-old man, one was on the floor of the mouth of a 19-year-old woman, the other was on the floor of the mouth of a 21-year-oldman. Histologically, two were Antoni type A and one was a mixed type. These cases are dlscussed and the literature is reviewed.

    A 46-year-old woman wasfound to have tuberculosis of the parotid gland with tuberculous cervical lymphadenitis. The most provable route of infection was lymphogenous from the oral cavity.
    A review of the literature indicates that the parotid gland is more frequently involved by tuberculosis than any of the other salivary glands, and that tuberculosis of the sublingual gland is rare. Two distinct types are found:a subacute type, which often runs its course with pain and/or fistula formation etc., and a chronic type which sometimes produces no symptoms.
    We must consider tuberculosis in cases of parotid gland tumor with acceluated E. S. R. value, positive tuberculin reaction or a family and/or past history of tuberculosis.

    Anterior rhinoscopy, radiographic examination of the nose and anterior rhinomanometry were performed in 150 patients with cleft palate. Normal values of nasal resistance were determined in 826 normal human subjects aged 3-85.
    1) Total nasal resistance was significantly higher in cleft palate patients than that in normal human suhjects.
    2) The cleft palate population has a high incidence of deviation of the nasal septum, hypertrophy of the inferior turbinate and sinusitis. It was thought that these pathological conditions raised total nasal resistance in cleft palate patients.
    3) About 50% of cleft palate patients had a sensation of nasal obstruction.
    4) The degree of sensation of nasal obstruction correlated well with the value of total nasal resjstance.
    5) The patients with high total nasal resistance had associated middle ear diseases more frequently than those with normal total nasal resistance.

    Magnetic resonance imaging (MRI) of the nasopharynx, the eustachian tube and the middle ear was performed in nine patients with nasopharyngeal carcinoma.
    MRI revealed the extent of the tumor more clearly than CT (computed tomography) when the tumor was situated in the parapharyngeal space. But when the tumor extended superficially in the nasopharyngeal mucosa, its margin could not be identified clearly by either MRI or CT because of hypervascularity and long Tl and T2 of the nasopharyngeal mucosa. Seven of the nine patients had unilateral otitis media with effusion. Their eustachian tube ventilation function was evaluated by an inflation-deflation technique. Failure of active equalization of negative pressure applied to the middle ear was found to be a characteristic disorder of their eustachian tube ventilation function. This dysfunction seemed to be correlated with the lateral dislocation of the eustachian tube cartilage caused by the tumor.

    A 28-year-old male had hemangioma in the submandibular region. Phlebolithiasis was present in the hemangioma, which originated independently from the submandibular gland, although the gland was involved by the hemangioma. The histology showed cavernous hemangioma. Its characteristics were similar to those of the 9 previously reported cases in Japan.
    Palpation, ultrasonography and computed tomography were useful in the diagnosis.
    The tumor was removed surgically. Early ligature of the main supplying arteries, which had been demonstrated by angiography, was useful.

    Carotid body tumor is a rare neoplasm; only about 500 cases have been reported in the world literature. This paper describes a recent case of malignant carotid body tumor and reviews 17 cases reported in the Japanese literature (20% of all carotid body tumors). A 38-year-old female first visited Tokyo Metropolitan Fuchu Hospital in November, 1984, with the chief complaint of a non-tender mass on the left side of her neck. A CT scan revealed a high density area with a clear margin at the level of the left carotid bifurcation. An angiogram demonstrated a vascular-rich mass with slight widening of the carotid bifurcation. A diagnosis of left carotid body tumor was made preoperatively. The tumor(3.5×2.5×2.5cm)and two lymphnodes were resected by the subadventitial technique under general anesthesia.
    The diagnosis was malignant carotid body tumor with metastasis to the regional lymphnodes. Because of the discrepancy between the histological findings and the clinical course, malignancy was proved by the evidence of regional lymphnode metastasis, distant metastasis or invasion of the adjacent organs. We discuss the clinical difference between benign and malignant tumors and the possibility of malignant change induced by biopsy or partial resection. Lastly we stress the usefulness of the angiogram in the preoperative diagnosis and the importance of long term postoperative follow-up.

    A rare case of extracranjal chondroma originating from the skull base is reported. The patient was a 66-year-old woman who complained of hearing loss and a foreignbody sensation in the throat.
    Computed tomography and operative findings revealed a tumor occupying the skull base, parapharyngeal space and nasopharynx. The tumor was removed via the external cervical approach.
    A survey of the literature revealed only three previous case reports.

    A 54-year-old man with Pustulosis palmaris et plantaris (PPP) complained of lower abdominal pain. During the clinical course, the sterno-costo-clavicular joint swelled, and PPP relapsed. Bilateral tonsillectomy was performed. Almost immediately, all symptoms disappeared.
    Both PPP and sterno-costo-clavicular hyperostosis seem to be related to the focal infection, and removal of the focus appeared to be effective treatment.

    A surgical procedure for correcting postoperative cleft lip nasal deformity and its indications are described.
    The method employed is basically Isshiki's technique, which features a nostril rim incision on the cleft side to make the nostrils symmetrical, and traction sutures passing through the non-cleft side nostril skin. This technique was useful for achieving a symmetrical nostril and adequate alar tip, producing satisfactory results. No recoils were observed. The key issues for secondary repair of cleft lip nasal deformity are recon struction of nasal function and structure.

    Eustachian tube function was tested by sonotubometry in 25 adult, otologically healthy males in different body positions(sitting, supine, prone, rt. lateral, lt. lateral, and-90° head down).
    To evaluate the ability of the Eustachian tube to open actively, we recorded the duration of tube patency and the sound pressure change with swallowing.
    To evaluate passive opening function of the Eustachian tube, we recorded the increased pressure level in the nasal cavity by Valsalva's maneuvre forcing the Eustachian tube to open passively.
    Eustachian tube patency was less in the horizontal and head down positions than in the sitting position.
    Significant differences between sitting and horizontal positions, and between horizontal and head down positions were found in the duration of tube patency with swallowing and in the opening pressure level with Valsalva's maneuvre. No significant change was found in the sound pressure change with swallowing.
    It seems that one of the factors causing the postural effect on Eustachian tube function might be increased venous pressure.

    The complement system in the nasal discharge and pathological mucosa of patients with chronic sinusitis was studied by immuno-biochemical and immuno-histochemical methods in this study of the pathogenesis of chronic sinusitis.
    C3 and C4 components in the nasal discharge of patients with chronic sinusitis were about one tenth, and C5 component one fifth the level in normal serum. The mean value of CH50 in the nasal secretion of patients with chronic sinusitis as determined by a new method was one fiftieth that in normal serum.
    C3a, C4a and C5a in nasal and paranasal secretions were assayed by radio immuno-assay (RIA). Conversion of C3 in nasal secretions was observed by crossed immunoelectrophoresis. Anti-complement activity was observed in the nasal secretion of patients with chronic sinusitis.
    The results of measuring C3/C4, a new marker in the complement system, suggest that the classical pathway may be activated in chronic sinusitis.

    Niceritrol, a derivative of nicotinic acid, has been used in the treatment of atherosclerotic and obstructive vascular diseases. It was given to patients with facial palsy for 8 weeks or morc in combination with massive doses of ATP and/or steroids used during the initial 2 or 3 weeks.
    1. Assessment by our Paralysis and Secondary Defect Scorc showed that the results could be classified as'good'or better in 37.2% of the patients in 2 weeks, 54.2% in 4 weeks, 60.0% in 6 weeks and 68.5%in 8 weeks.
    2. It was striking that 3 of 4 patients with Hunt's syndrome recovered completely within 4 wceks and one patient with severe paralysis for 20 years due to surgical trauma showed a fair recovery.
    These results indicate that the combination of niceritrol and ATP and/or steroids is effective in the therapy of facial palsy.

    The adherence to and the absorption from the nasal mucosal epithelium of Broncasma Berna was studied in guinea pigs. Broncasma Berna was administered to the left and right nostril in a 0.1 ml volume of solution. The outer membrane vesicles (OMV) of Haemophilus influenzae were used in the absorption study.
    1) Electron microscopic studies showed that bacterial cells of Broncasma Berna attached to both ciliated and non-ciliated cells of the nasal mucosa.
    2) Histochemical studies showed that OMV of H. influenzae was absorbed through the nasal mucosa, mainly via ciliated cells.
    3) The penetration of OMV through the mucosa of immunized animals was less than through that of non-immunized ones.
