    Curative treatments for head and neck carcinomas consists of surgery and/or radiotherapy. However, standard treatment alone is less successful in cases of advanced head and neck carcinomas. With the development of cisplatin (CDDP), the efficacy of chemotherapy, including CDDP therapy, has increased markedly in the treatment of head and neck cancer. Accordingly, neo-adjuvant chemotherapy has been applied in cases of advanced carcinoma. Previous studies have demonstrated that this type of chemotherapy is efficacious in patients with a complete response (CR), especially a pathological CR, in terms of the preservation of primary organs and overall survival following chemotherapy. Therefore, the development of an intensive chemotherapy regimen showing a high CR rate is necessary. Recent developments in chemotherapy, including CDDP and 5-fluorouracil treatment, described in the previous reports were reviewed and our studies were summarized.

    We report the case of a 19-year-old woman with a jugular foramen neurinoma who recovered from severe sensorineural hearing loss after surgical removal of her tumor. She was admitted with a history of progressive hearing loss and tinnitus for ten months.
    Audiometry indicated left sided deafness, but auditory brainstem response recordings revealed a wave I pattern without any waveforms II through lead V. Otoacoustic emmissions were clearly evoked on the left side. Magnetic resonance imaging revealed a large tumor in the left cerebellopontine angle. She underwent a left craniectomy with removal of a neurinoma originating from the glossopharyngeal nerve. Postoperatively, she showed a dramatic recovery of her hearing. We believe that the principal cause of her severe hearing loss was a blockade of conduction in the cochlear nerve by the tumor.

    Generally, an infant with impaired hearing meets many difficulties in the process of growing up. A patient with bilateral atresia of the ear (first and second branchial arch syndrome) was followed over a long period of time. The patiant was fitted with a hearing aid in the right ear initially. External canalplasty was subsequently performed, and we were able to fit her with binaural hearing aids. After the operation, based on the consults of a questionaire, she reported to enjoy having a conversation with her friends. Binaural hearing was a decided improvement on the previous situation. Sound lateralization ability was measured by a self recording apparatus (Rion Co., TD-01). This test demonstrated that both the air (fitting of the binaural hearing aids) and bone sound lateralization abilities were close to those in normal subjects.

    A 43-year-old female with steroid-responsive sensorineural hearing loss (SNHL) was described. She had been treated with steroids for idiopathic thrombocytopenic purpura (ITP) since 1981.
    The patient complained of sudden hearing loss and tinnitus in the left ear on the 9th of June, 1995, at which time, 2.5mg of prednisolone was administered for ITP. The audiogram showed SNHL of low frequency sounds. Immunological studies showed positive rheumatoid factor and antinuclear antibody, high levels of complement, immune complex, IgG and IgA.
    The patient's hearing improved after initial treatment with 40mg of prednisolone. Fluctuations in hearing loss were observed with changes in the dose of prednisolone. The above clinical course suggested steroid-responsive SNHL.
    Steroid administration could be reduced with a supplemental administration of Saireito without any significant deterioration of hearing. Laboratory studies and the effect of steroids suggest a causative role in immune-mediated inner ear disorders.

    An epidemiological survey of the incidence of sudden deafness was performed in the Nara prefecture in Japan during 1992 and 1993, participating many local otolaryngological clinics and hospitals.
    The Nara prefecture has a population of 1, 400, 000. The morbidity rate for sudden deafness was 18.2 per 100, 000 population in 1992, and 21.2 in 1993 (among the individuals with sudden deafness, the number of patients without low-tone hearing loss was 10.3 and 13.0 per 100, 000, and the number of patients with low-tone hearing loss was 7.9 and 8.2 per 100, 000, respectively). However, the morbidity rate was higher in some cities, up to a maximum of 61.2 per 100, 000. The morbidity rate for sudden deafness appeared to be somewhat higher than in previous reports in Japan (13.9-19.6 per 100, 000), perhaps because all of the local otolaryngologists did not participate in our survey.
    The percentage of patients with vertigo was 26% in 1992, and 16% in 1993. These patients preferred to visit hospitals rather than clinics. The patients with low-tone hearing loss, however, preferred to visit clinics rather than hospitals.
    The female/male ratio of the patients was 2: 1 (3: 2 and 3: 1 among the patients without lowtone hearing loss and with low-tone hearing loss, respectively). Female patients tended to visit clinics rather than hospitals and to have low-tone hearing loss rather than other hearing loss types, when compared with males. There were apparently fewer males than females with low-tone hearing loss, or male patients with low-tone hearing loss were less likely to visit clinics and hospitals.

    A 10-year-old female with unilateral psychogenic hearing loss is discribed. The external causes of hearing loss were contusion to the ear region on three occasions. Hearing loss occurred suddenly after the first two occasions of contusion to the ear. Low damage hearing loss on pure-tone audiometry was revealed both times, and hearing was restored to normal completely for a few weeks. These cases of hearing loss were attributed to concussion of the inner ear. After the third contusion, hearing loss occurred suddenly in her right ear as it had the first two times. There was sensory-neural hearing loss on the right side in pure tone audiometry, however the auditory brainstem response and stapedial reflex were normal. The contradictions between subjective and objective data suggested a unilateral psychogenic hearing loss. This case was subsequently diagnosed as unilateral psychogenic hearing loss induced by the experience of contusion to the ear region. Hearing levels subsequently returned to normal without any psychosomatic treatment. These observations suggest that evaluation of the auditory brainstem response is important to diagnose psychogenic hearing loss.

    Between 1987 and 1995, 12 patients with acute mastoiditis were admitted to the Department of Otolaryngology, Hyogo College of Medicine. Four of these patients were cured by conservative treatment methods consists of intravenous antibiotic therapy, myringotomy and puncture of the abcess. Eight patients recovered after surgical therapy, seven of which underwent mastoidectomy. Etiological bacteria cultured from eight patients, revealed the presence of Streptococcus pneumoiae, Pseudomonas aeruginosa and Staph. aureus. The rationale of surgical management, especially mastoidectomy, was discussed.

    Monomorphic adenoma is a rare tumor which account for less than 2% of all salivary gland tumor. In particular, monomorphic adenomas which arise from the nasal cavity and paranasal sinuses are very rare.
    To our knowledge, this is only the second case of a monomorphic adenoma of the nasal septum ever reported. Recently, we treated a 71-year-old male with a monomorphic adenoma of the nasal septum, who presented the sensation of a foreign body in his left nasal cavity. We performed an intranasal operation, and recognized a tumor arising from the nasal septum. There has been no evidence of recurrence since his discharge from the hospital.

    A 37-year-old woman presented with a two-year history of a painless mass in the lower portion of the left side of her face. The pathological profile of the tumor which was diagnosed as a myxoma was described. A magnetic resonance imaging (MRI) scan revealed a mass occupying the masseter muscle. We discuss the characteristic MRI appearance and appropriate management of an intramuscular myxoma of the masseter muscle.

    A case of ethmoid fibrous dysplasia with unilateral chronic sinusitis was reported. The patient was a 21-year-old male who had complained of left nasal obstruction for several years. A plain radiogram of the paranasal sinuses revealed an opaque shadow visible only in the left ethmoid and maxillary sinuses, which induced the unilateral sinusitis. However, amorphous sclerotic changes mainly in portions of the left ethmoid bone were visible in the CT image, which were suggestive of fibrous dysplasia.
    As the patient had no symptoms other than left nasal obstruction, a Caldwell-Luc antrotomy, with partial resection of the left inferior turbinate, was performed. At the same time, a transantral biopsy of the ethmoid bone was carried out, and the subsequent pathological diagnosis was fibrous dysplasia.
    Although fibrous dysplasia of ethmoid bone is rare, physicians should be aware of the possibility of this disease in cases of unilateral sinusitis.

    A case of malignant myoepithelioma originating from the root of the tongue of a 12-year-old female is described. No symptoms except the presence of a neck mass on the left-hand side of the body were recognized. A CT scan and MRI revealed a tumor shadow localized to the root of the tongue and a solitary left cervical lymph node swelling. Biopsy specimens obtained from the left cervical lymph node were studied by light microscopy with hematoxylin and eosin staining and with immunohistochemical staining. The tumor, predominantly composed of highly anaplastic small round cells, showed positive staining with antibodies to S-100 protein, cytokeratin, vimentin and α-smooth muscle actin, on the basis of which we diagnosed malignant myoepithelioma. We performed total extirpation of the tumor, reconstruction of the tongue defect using free radial forearm flap, and left radical neck dissection. The tumor at the root of the tongue showed a histological picture which was the same as that of the cervical lymph node specimens. This tumor was determined to have originated from the minor salivary gland of the root of the tongue. Postoperatively, the tongue and left upper neck were irradiated (50 Gr). Recurrence of the tumor was observed, however, two months after surgery, in the muscular tunics of the tongue. Total extirpation of the tumor was subsequently performed, after which the clinical course has been uneventful.

    A case of amelanotic malignant melanoma occurring on the palatal gingiva side of the upper right molar, in a 43-year-old male is presented.
    The patient was first seen on September 14, 1995, with the chief complaint of rapid growth and alteration in the color tone of the tumor to ash albedo or redishness.
    Initially, we suspected cancer or sarcoma on the basis of oral observations of the ulcerous, nodular mass.
    Examination of a biopsy specimen showed the tumor to be composed of a solid proliferation of round and spindle shaped cells characterized by atypia, mitosis and many nuclei. Many tumor cells showed positive immunoreactivity for 5-100 protein, vimentin and HMB-45 mono clonal antibody.
    Recognition of premelanosome with electron microscopic obsarvation confirmed the crigiral diagnosis.
    Partial resection of the maxilla was performed under general anesthesia on October 3, 1995. The patient was observed periodically and no evidence of recurrent or metastatic lesion was found one year after the operation.

    A case of metastatic tumor of root of the tongue from the liver which occurred in a 59-year-old man is reported in this paper in conjunction with a relevant literature review. The patient's primary complaint on admission was hemosputum. Total removal of the tumor occupying the root of the tongue was carried out under general anesthesia. The histopathological findings revealed that the tumor was a metastasis of hepatocellular carcinoma. A review of the literature revealed very few reports of pharyngeal metastases derived from hepatocellular carcinoma.

    Primary malignant fibrous histiocytoma (MFH) of the larynx is extremely rare. We describe a case of a 61-year-old man with a malignant fibrous histiocytoma of the larynx who complained of hoarseness. The tumor was a red smooth mass on the right true vocal cord, which was approximately 0.5cm in diameter. Histologic examination revealed MFH. A fronto-lateral vertical partial laryngectomy was performed, and chemotherapy was given post-operatively. There has been no recurrence during the 3 years and 6 months since surgery.

    Papillary carcinoma of the thyroid gland rarely invades the larynx because of its low malignancy and slow progression. When it does invade the larynx, however, laryngeal function should be preserved if possible. We describe a case of a 74-year-old man with advanced papillary carcinoma of the thyroid gland. The papillary carcinoma of the thyroid gland severely invaded his larynx, trachea and esophagus. Accordingly, we performed a total thyroidectomy, left neck dissection, partial laryngectomy, and partial excision of the trachea. However, laryngeal function was successfully preserved by reconstruction of the larynx and trachea using a composite auricular cartilage and skin graft.

    Seven patients from the Nara Medical University Hospital with malignant lymphomas of the head and neck were given oral etoposide at a low dose of 25mg every other day, or at 25mg every day for 2 weeks with a 2-week non-intervention interval, for over one year. Two of the 7 patients received prophylactic etoposide against recurrences, and the other five patients received it as part of a maintenance chemotherapy program. The two patients with recurrences were still alive at 49 and 36 months, respectively; one of these patients had a recurrence in the abdominal lymph nodes. None of the five patients on the maintenance therapy showed any signs of recurrence. In the maintenance group, the median duration of remission after the administration of etoposide was 38 months, and ranged from 22 to 54 months. None of the patients had leukopenia less than 2000 cells/mm2 or any other severe side effects.
    The long-term oral administration of low-dose etoposide was considered to be safe and useful for the treatment of recurrent tumor lesions and for maintenance chemotherapy.

    The YAMIK sinus catheter method is a novel modification of the Proetz displacement method. The efficacy, safety, and usefulness of the YAMIK sinus catheter were evaluated in an open clinical study without the use of any medications. Sixty patients with paranasal sinusitis were treated with a YAMIK catheter once weekly for 3 consecutive weeks and the following results were obtained:
    1. The clinical effectiveness rate was judged by the committee to be 53%. The YAMIK method was much more effective in cases with acute sinusitis than in cases with chronic sinusitis.
    2. The effectiveness rates for treatment of rhinorrhea, post nasal drip, nasal blockage and sinus pressure were 64%, 65%, 61% and 95%, respectively.
    3. The improvement based on X-ray findings, and covering the improved and the markedly improved cases, was 42%.
    4. Side effects observed included 5 cases with transient nasal itching, sneezing and/or nasal discharge during the first few days after treatment, and 3 cases where the balloon slipped into the mesopharynx occurred. The latter blocks up the airway and the balloon occupying the mesopharynx was ruptured through the mouth.
