    In the multi-disciplinary treatment of head and neck tumors, anti-cancer chemotherapy was evaluated by the survival rate, response rate and preservation of function and shape. Chemotherapy significantly improved the survival rates in cervical esophageal, hypopharyngeal and maxillary cancer and malignant lymphoma in stage II. A new regimen of Pepleomycin, Vincristine, Endoxan, Mitomycin-C, Predonine, 5-Fu and Adriacin showed a better than partial response in 42% of the patients. Functional preservation with combined chemotherapy was observed only in maxillary cancer.
    Chemosensitivities for head and neck malignant tumors were measured in order to select useful drugs. Although no concrete merit of these tests has yet been achieved, inoperable recurrent carcinoma or maxillary cancer will be treated by more selective anti-cancer drugs. Furthermore, photoradiation therapy with hematoporphyrin derivatives will become a useful modality for head and neck tumors in the near future, because this technique does not destroy normal tissue.

  • Part. 3 Follow up Study on the Long-term Use of Band-noise Masker

    A follow up study was carried out of 12 patients using band-noise masker for 6 months to 3 years.
    Various auditory tests and measurements of tinnitus were performed before and after the long-term use of band-noise tinnitus masker: loudness balance, masking and pitch match tests.
    None of the 12 patients showed any remarkable changes in hearing at the 7 frequencies tested, from 125 to 8000Hz. Tinnitus frequency obtained by the pitch match method demonstrated no changes after long-term listening to band-noise. The contralateral loudness balance method indicated changes of tinnitus loudness; however, both the ipsilateral loudness balance method and the masking method showed no remarkable changes of tinnitus loudness after the long-term use of a tinnitus masker.
    We conclude that the long-term use of a band-noise masker has no effect on the pure tone threshold as long as close follow-up examinations are performed.

    The usefulness of high resolution CT scanning in pre- and postoperative examinations was evaluated in 135 ears treated with tympanoplasty. The CT findings were correlated with operative findings and postoperative results. Preoperative CT scanning can demonstrate the presence and extent of pathological changes in the middle ear space, the status of the ossicular chain (especially the incudo-stapedial joint), the presence of labyrinthine fistulae, etc. The condition of the reconstructed tympanic membrane, the ossicular chain and the aerated space in the tympanic cavity can be clearly observed on CT scanning during the postoperative period.

    We performed attico-antro-mastoidectomy and primary reconstruction with a conchal cartilage in patients with cholesteatoma (36 untreated ears and 7 ears treated by the open method). The use of a large cartilage plate realized the open-and-closed method rather than the open or closed method.
    1. Transmeatal opening of the attic and tympanum and selective antro-mastoidectomy made possible a wide operative view and efficient cleaning.
    2. A cartilage plate from the pinna proved to be an excellent material to reconstruct the opened cavities, since it could be sectioned as a large piece without any deformity, survived well and replaced the fragile ear drum as well as the bony annulus.
    3. Primary closure of the opened cavities was useful in shortening hospitalization and avoiding the postoperative complications of open mastoid cavities.
    4. In a few ears in which the technique and adaptation had not been completed there was residual cholesteatoma and persistent discharge. For success in the open-and-closed method, it seemed important to reconstruct the opened mastoid as a small cavity, to secure ventilation between the attic and the tympanum, and to avoid primary reconstruction in cases with marked inflammation.

    Four cases of peripheral facial palsy developing during late pregnancy or the early puerperium, are reported. In one the diagnosis was polyneuritis and the other three, Bell's palsy. All patients recovered well with only conservative treatment. The relevant literature is reviewed for data concerning the incidence, time of onset, complications, pathogenesis, prognosis and treatment of peripheral facial palsy occurring during pregnancy or the puerperium.

    Five males and nine females aged 14 to 58 years were treated for hemangioma of the nasal cavity and paranasal sinuses from 1979 through 1984. There were eight cases of capillary, four of cavernous and two of non-specific hemangioma. The most frequent symptoms were nasal bleeding, noted in 11 patients (78%), and nasal obstruction, in five cases (36%). The lesions originated most often in the nasal septum, then the inferior turbinates. Eight patients were treated by local excision, three by cryosurgical procedures, two by cryosurgery plus excision and one with cryosurgery followed by an Nd-YAG-LASER procedure. The histological classification of these lesions and the clinical features are discussed.

    IgA nephropathy is a glomerulonephritis with mesangial IgA deposits. It is characterized by the clinical features of mild proteinuria, microhematuria and satisfactory convalescence.
    We report here three cases of IgA nephropathy with chronic tonsillitis treated by tonsillectomy. After the surgery, proteinuria, hematuria and serum IgA levels were reduced in all cases, and renal function was also improved.
    These cases along with 10 others reported in the literature suggest that tonsillectomy is an effective therapy for IgA nephropathy with chronic tonsillitis. The indications for and proper time of operation are also discussed.

    Adenomatoid odontogenic tumors are exceedingly rare in otorhinolaryngologic practice.
    The patient was an 11-year-old girl who complained of oppression and excessive lacrimation of the left eye and left sided nasal obstruction. X-ray examination revealed a tumor in the maxillary sinus, in which there was an impacted canine tooth and numerous microcalcifications. The clinical findings suggested adenomatoid odontogenic tumor. The histological findings were also characteristic of this neoplasm. Surgical removal of the tumor was the only treatment. Now, 1 year and 5 months after surgery, there is no evidence of recurrence. Reports in the literature also indicate little tendency of the tumor to recur.

    The technique of tracheo-esophageal shunt which Amatsu introduced in 1977 is simple. The voice produced is usually fluent and intelligible, but because of the continuity with the esophagus, aspiration seems to be unavoidable. To eliminate aspiration, we folded the membranous portion of the trachea above the anastomosis and created a valve-like slit. We have used this technique in 15 patients.
    In 13 of the 15 patients, aspiration is almost negligible. Two patients had mild aspiration, but no clinical problem. The speech is as good as in the original technique except in one case.

    An auditory feedback system in phonatory control has been identified in animals, but very little is known about the system in humans. This paper reviews the literature on auditory evoked responses with special reference to auditory evoked laryngeal responses.

    With a servo-nulling pressure system the perilymphatic and endolymphatic pressures were measured continuously through the round window and basilar membranes 1-2.5 months after obliteration of the endolymphatic sac in 7 guinea pigs.
    1) There was no significant statistical difference between the perilymphatic and endolymphatic pressures of the control ears.
    2) The endolymphatic pressure was significantly higher (+0.78±0.32mm/Hg) than the perilymphatic pressure in the operated ears.

    The morphology of macrosections of the normal adult temporal bone were correlated with high resolution computed tomographic findings of 6 axial, coronal and sagittal sections.
    1) The micro-structure of the temporal bone was almost perfectly demonstrated by axial, coronal and sagittal sections. Axial sections provided more information on the structure of the temporal bone than did coronal sections. Sagittal sections gave little information.
    2) Axial sections effectively demonstrated the facial nerve canal (labyrinthine segment-geniculate ganglion-tympanic segment), majorr petrous nerve canal, short process of the incus, stapes (especially foot plate), and relationship of the malleus head to the incus body.
    3) Coronal sections were useful in demonstrating the cochlear aqueduct, facial nerve canal (mastoid segment), crista transverse, tensor tympani and ossicle (incus body, incus long process, stapes head, stapes crus).
    4) Sagittal sections were useful in demonstrating, the facial nerve canal (tympanic segment, mastoid segment), vestibular aqueduct, and the relationship of the malleus head to the incus long process.
    5) To understand the complicated structure of the temporal bone it was necessary to perform CT in many dimensions. The most useful section was the axial. Coronal and sagittal sections were sometimes useful.

    Ceftizoxime, a cephem antibiotic, was given by short-term intensive treatment to 8 patients with otitis media and 12 with otitis media accompanied by irritation of the labyrinth. Concentrations of the drug in the middle ear were determined in some patients.
    1) Ear discharge disappeared or decreased in 80% of the patients given ceftizoxime 1g twice a day by intravenous drip infusion for 6 days.
    2) Dizziness, headache and tinnitis were markedly improved by this intensive treatment. This, however, does not mean that surgery is not necessary. We think that intensive treatment with ceftizoxime is also needed in patients requiring operation.
    3) No noticeable side effects or abnormal laboratory findings were seen.
    4) The ratio of ceftizoxime concentrations in the middle ear to serum concentrations was 97.0% one hour after i. v. injection of 1g and 69.7% at 8 hours. The transfer rates of the drug were very high.
    5) Ceftizoxime, 1g twice a day, is expected to be sufficiently active against S. aureus, S. epidermidis and Proteus sp., considering that the drug concentrations in the middle ear are higher than the MICs for the main causative organisms and are maintained for a sufficient time.
    Ceftizoxime is surely effective against Pseudomonas aeruginosa infections when the frequency of injections and the dose are increased appropriately.

  • II Treatment of Patients with Type I Allergic Rhinitis due to House Dust

    The new allergy prevention drug N-(3, 4-dimethoxycinnamoyl) anthranilic acid (N-5') was effective in 70 percent of 20 patients with type I allergic rhinitis due to house dust antigen.
    The clinical efficacy of this drug was evident in 2 weeks and reached a plateau in 6 weeks. Laboratory results were not affected by this drug. There were mild subjective side effects in two patients.
