    The audiometric findings in five patients with syphilis of the labyrinth were classified as progressive type or fluctuating type. However, they were not peculiar to this disease.
    Disturbances of equilibrium detected by Mann's test and a marked reduction of caloric response were common in all cases. A significant reduction of VOR gain observed in the computerized rotation test was useful in the diagnosis of this disease.

    Ten patients (10 ears) with cochlear tinnitus were treated by infusion of dexamethason and mecobalamin solution through the Eustachian tube.
    This treatment was effective in cases. The rate of effectiveness was similar to that of transtympanic infusion procedur.
    No severe side effect was observed. This method of treatment seems to be useful since the ear drum is not punctured.

    In recent years some youngsters have been indulging in what is called thinner inhalation, posing a serious social problem.
    Organic solvents have also been widely used industrially as adhesives or degreasing and rinsing agents, generating a kind of occupational disease which has become a medical problem.
    Furthermore, some schoolchildren who refuse to go to school complaining of headache, head heaviness, blurred vision, diplopia, dizziness, or equilibrium disturbances, may actually have toluene toxicosis caused by the adhesive they use in constructing plastic models.
    The authors examined 35 such patients neurotologically and found some impairment in the cerebellar cortex, cerebellar nuclei, or efferent pathways. This report is presented to invite comments from other researchers.

    This is a case report of paralabyrinthitis (paralabyrinthäre Eiterungen), a type of pyramidal inflammation. A 47-year-old male with left acute otitis media was treated with antibiotics, but recovery was slow. In the 7th week he developed left facial paralysis and pain in the left temporal region; subsequently hearing impairment in the left ear increased and episodes of dizziness persisted. Four months later, the patient was admitted to our department. Treatment with antibiotics was not effective. Mastoidectomy was performed one month after admission. He developed paralysis of the left cranial nerves V, IX, X and XI, so left radical mastoidectomy with curettage of the perilabyrinthine cells was performed 2 months after admission, but still there was no improvement. However, the nervous symptoms were improved by the concurrent use of steroids and antibiotics. The patient was discharged after about 6 months of hospitalization, but partial paralysis of cranial nerves VII, VIII, IX, X and XI remained.

    We examined the effects of a new type of calcium entry blocker, flunarizine, which acts specifically on intracranial blood vessels, upon patients with idiopathic sudden deafness. Eighteen patients with sudden deafness who exhibited flat pure tone audiogram were selected for evaluation. Eleven patients were administered one tablet (10mg) of flunarizine a day, with or without other modalities unless their pharmacological effects were similar to this substance. Seven patients not receiving flunarizine served as controls. The regimens of their therapy were mostly stellate ganglion block or intravenous steroids injection.
    Patients who were cured or showed marked recovery accounted for 64% in the flunarizine group, compared with 43% in the control group. The difference of the hearing level between before- and after- treatment was 34.3dB in the flunarizine group and 24.9dB in the control group. The findings suggest that flunarizine has several pharmacological advantages and seems to be a good candidate for the drug used in combination with other established regimens for sudden deafness.

    A 33-year-old female came to our hospital because of bilateral facial weakness. She had had rubella 7 days previously. On admission, she also had weakness and numbness of both lower extremities. She was treated with corticosteroid, γ-globulin, and antibiotics. Her knee jerk and ankle jerk were exaggerated bilaterally, but no Babinski response was noted. The symptoms gradually improved, and after about 40 days she was discharged, apparently fully recovered except for slight right facial weakness. We diagnosed her illness as myelopathy and bilateral peripheral facial paralysis due to rubella, because of exaggerated tendon reflexes, proximal muscular weakness of both lower extremities, and elevated rubella antibody in the serum and cerebrospinal fluid.

    A 52-year-old man with Kallmann syndrome (olfacto-genital dysplasia) had abnormally low serum gonadotropic hormone and testosterone levels. The LH-RH test revealed little response.
    Small pieces of mucosa were experimentally resected from the olfactory area of both sides of the nasal cavity and examined morphologically with scanning and transmission electron microscopes. Ciliated cells, mucus secreting (goblet) cells, and basal cells, which are characteristic epithelial cells of the respiratory system, were densely distributed, but no olfactory epithelial cells or their neural processes were seen. In this case, these observations indicate a complete olfactory deficiency caused by agenesis of at least the peripheral portion of the neural olfactory system. Further investigations are necessary to determine the relationship between the pathology and the degree of olfactory disturbance in Kallmann syndrome.

    We encountered 40 cases of blow-out fracture in the past seven years. Twenty cases were fracture of the medial orbital wall. In one of six cases of involving the medial orbital wall, the patient who complained of diplopia and had ophthalmological disorders was operated on by the endonasal approach with satisfactory results.

    Oropharyngeal carcinoma, which did not respond to radiotherapy, recurred twice after surgery.
    The patient was a 58-year-old woman with squamous cell carcinoma of the anterior faucial pillar (T1N0M0). The cancer did not respond to irradiation with 60Co 55Gy, and recurred twice, 4 years and 6 years after partial pharyngectomy. The pantomogram revealed bony destruction of the mandible at the time of the second recurrence. Intraarterial infusion of adriamycin 100mg and 5-Fluorouracil 5000mg in addition to irradiation with 60Co 22Gy resulted in marked shrinking of the tumor. We performed segmental mandiblectomy and transplantation of an iliac bone. Although the transplanted bone was twice infected, she has had no recurrence of the tumor and satisfacting function of the jaw joint.

    The patient was a woman, aged 41, who had complained of abnormal sensation in the pharynx for about 2 weeks. CT scan disclosed a low-density mass in the epipharynx with extention to the parapharyngeal space and the prevertebral space. Surgical removal was performed via the oral route. A thyroid cancer with cervical node metastasis was detected during tracheostomy. The tumor of the epipharynx was chondrosarcoma histologically.
    Cartilaginous tumors of the epipharynx are rare. Moreover, chondrosarcomas of the head and neck region associated with a second primary neoplasm are extremely rare and only one case has been described before the present report.

    Laryngeal carcinoma was studied by exfoliative cytology of materials collected during indirect (16 cases), direct (7 cases), flexible fiber laryngoscopy (16 cases) or of the sputum (1 case).
    The results and conclusions are as follows:
    1. The overall accuracy of cytological diagnosis was 77.8%. If 3 cases in which satisfactory material could not be obtained are excluded, it was 84.2%.
    2. The diagnosis was correct in 70.6%, 85.7% and 81.3% of patients examined by indirect, direct, flexible fiber optic laryngoscopy respectively.
    3. Laryngeal cytology with direct laryngoscopy was superior to the other methods.
    4. However the flexible fiberoptic laryngoscopy should be painless, frequently under local anesthesia, so laryngeal cytology with this method is recommended.
    5. The accuracy of laryngeal cytology should improve with the use of the fiber optic laryngoscopy, the accumlation of laryngeal cytological findings and greater experience with cytological diagnosis.

    A 6-year-old boy had long term subglottic stenosis due to prolonged tracheal intubation. The tracheal lumen was 2mm in diameter at the stenotic site, and was surrounded by mature scar. The stenotic scar was resected by TUR (Transurethra)-resectoscope, and a silicone T-tube was inserted into the site of the resection. The patient was allowed to stay at home with the T-tube in place. Eight months after surgery, the T-tube was removed. The patient has remained free of respiratory distress and has achieved good phonation.

    A case of drug-induced esophageal ulcer caused by Bacampicillin is reported. The patient, a 27-year-old female, took a Bacampicillin capsule without water just before lying in bed. Two hours later, she felt substernal burning pain and dysphagia. As the pain continued, she visited us six days after taking the drug. Esophageal endoscopy revealed fresh ulcers at the broncho-aortic constriction of the esophagus. Ulcers were shallow and irregular in both shape and size. She was treated with an antacid drug and her symptoms disappeared 10 days after the onset. We speculate that her clinical features were caused by the release of acid content from the dissolving capsule that was lodged in the throat.

    The relevant literature on the treatments for sudden hearing loss except for surgical treatments were reviewed. Five types of drug and stellate ganglion block were selected and their expected effects were summarized. It was shown that each treatment is directed toward at least one of the possible etiologies of sudden hearing loss. It was pointed out that it is still not clear which treatment is truly effective. It was emphasized that it is necessary to consider many factors influencing the prognosis, such as the initial hearing level, time from onset, and existence of vertigo, as well as the spontanous recovery, in order to properly evaluate the treatment for this disease.

    Cold stimulation is known to induce nasal obstruction and accelerates the degree of nasal symptoms in patients with nasal allergy. To elucidate the effects of different caloric stimulation to the legs on the nose, nasal airway resistance (NAR) was measured by a rhinomanometer and the changes in the degree of nasal response to allergen-containing paper discs during caloric stimulation was also examined. For caloric stimulation, water of different temperatures, i. e., 47, 43, 37 and 20°C, was applied for five minutes and at 47 and 0°C for one minute.
    Warm stimulations (47 and 73°C for five minutes) induced a decrease of the NAR. In contrast, cold stimulation (0°C for one minute) induced an increase of the NAR.
    Warm stimulations (47 and 43°C for five minutes) tended to decrease the NAR in the nasal provocation by allergen, and stimulations of 47°C for five minutes decreased the degree of the nasal provocation reaction. On the other hand, cold stimulations (20°C for five minutes and 0°C for one minute) increased the NAR in the nasal provocation, and accelerated the nasal provocation reaction.
    These results suggested that different caloric stimulations affect not only swelling of the nasal mucosa (nasal airway resistance), but also reflex-mediated nasal response (sneezing and nasal secretion) to allergen in different manners.

    Radiation damage of the salivary gland was quantitatively investigated by determining the activity of muscarinic cholinergic receptor, which regulates the parasympathetic nerves in the parotid glands of rats. The receptor was measured by the receptor binding assay method using 3H-QNB two weeks after radiation of 2, 000 and 4, 000 rad in rats intraperitoneally injected with a radiation protectant (YM-08310, 200mg/kg) 30 minutes before radiation, and untreated controls. The number of receptors decreased in proportion to radiation doses, which was mild in the parotid glands of the rats treated with YM-08310. Therefore, YM-08310 was considered to have a protective effect on the parotid gland.

    To evaluate the clinical efficacy Tsumura-Saireito (No. 114), an open trial study was performed on 16 children and 2 adults with otitis media with effusion. Of the 29 ears, remarkable effects were observed in 8 ears (27.6%) and moderate effects in 13 (44.8%), while no effects were seen in 8 ears (27.6%). On the pure tone audiogram, 52.0% of the ears showed improvement, and 54.2% on the tympanogram. Clinical findings of the ear drums were also improved in about 50% of the ears examined. No side effects were noted in any patients.

    We administered an oral Kampo (traditional Chinese) medicine, Saibokuto, to 20 male and 30 female patients with abnormal sensation in the throat. The patients' subjective signs and symptoms showed the following results.
    1) The drug was effective in 56% of the patients in 2 weeks, in 68% in 4 weeks, and in 72% in 6 weeks (two weeks after the cessation of treatment).
    2) This drug was more effective in patients who never drank alcoholic beverages than in those who drank every day (p<0.01).
    3) No side effects were noted.
