    Dermatologists are not so familiar with transplantation immunity, especially graft-versus-host disease (GVHD). In order to understand GVHD, GVHD and other related diseases are discussed. The clinical course of bone marrow transplantation and GVHD are explained. GVHD is classified into congenital, iatrogenic or unknown GVHD with its origin. GVHD is also classified into hyperacute, acute or chronic GVHD with the time of onset. Pseudo GVHD (GVHD-like disease) appears following autologous transplant or syngeneic bone marrow transplantation. Postoperative erythema and post-transfusion GVHD are GVHD-related diseases and are also described.

    A 55-year-old woman had a variety of eruptions which consisted of linear scleroderma on the lower right extremity, both forearms and the left foot, multiple morpheas on the abdomen, skin sclerosis with a wax-like luster on the dorsums of both hands, and small depigmented patches on the lower abdomen. She also had anticentromere antibodies without the clinical symptoms of CREST syndrome. This case seems to be the fifth report of localized scleroderma where the patient had these antibodies. Further accumulation and analysis of similar cases are necessary to clarify their significance and relationship to the types of exanthemas. Serum prolyl hydroxylase was frequently high in these cases, indicating that it might serve as a marker of skin fibrosis.

    We report three patients who developed psoriasiform and/or lichen planus-like eruptions. Past histories revealed that they had received β-blockers (metoprolol tartrate, labetalol HCl) for long periods because of suffering from hypertension. Skin biopsies obtained from all patients showed microscopic pictures similar to the features of psoriasis and/or lichen planus. β-Blockers were stopped in all cases and treatment with topical corticosteroid resulted in a complete cure of the rash with in 1 to 3 months. In old individuals developing psoriasiform or lichen planus-like lesions, the past history of treatment for hypertension may be an important clue for diagnosis.

    A 16-year-old female with contact dermatitis from a ring is reported. Erythema, swelling and red papules appeared around the ring and finally it became stuck to her finger. At the same time, id-like eruptions appeared symmetrically on the lmibs. Patch test results were positive for nickel sulphate, copper sulphate, silver nitrate, and cobalt chloride. Using an X-ray microanalyzer, we found that the major content of the ring was nickel.

    Two patients, who were siblings, showed palmoplantar pustulosis with arthro-osteitis. The younger brother had suffered from pain of the clavicles and palmoplantar pustulosis for three years. Bone scintigraphy revealed an abnormal uptake at the sight of the left sterno-costo-clavicular lesion. The older sister had suffered from pustulosis and pain of the sacroiliac lesion for three months. HLA typing was performed for both patients. Although strong correlation has not yet been found between palmoplanter pustulosis and HLA antigen, it seems interesting to note that this disease occurred twice in the same family.

    We report two cases of neutrophilic erythema. These eruptions were annular or bow-shaped, slightly elevated and clearly bordered. Histologically, many neutrophils were mingled with the lymphocytes and infiltrated throughout the dermis.

    A 55-year-old woman had been treated for cardiac shock with intraaortic balloon pumping (IABP), transfusion through the saphenous vein of the lower left leg and intravenous drips of Dopamin through the vena cava. During treatment, extravasation of the fluids through the saphenous vein in the lower left leg occurred and necessitated re-injection. When re-punctured with the needle, hemorrhagic bullae immediatedly broke out over the left anterior tibial region 10×20 cm in diameter and developed into a deep, wide ulcer which needed a skin graft. We speculated that the cause of the ulcer was associated with venous thrombus, arterial free clots (resulting from IABP?) and the vasoconstrictive function of dopamin.

    The case of a 67-year-old male with pyoderma gangrenosum who had been suffering from diabetes mellitus for 18 years is reported. Painful swelling and an ulcer suddenly developed on his upper left arm a few days after intramuscular injections. Several cultures of the tissue from the ulcer proved negative for micro-organisms. He had no complications except for diabetes mellitus. He was treated with 20 mg intravenous predonisolone daily plus antibiotics and gammaglobulin. The clinical findings of 68 cases of pyoderma gangrenosum reported in Japan during the past five years are reviewed. Their age distribution was between 5 and 85 years, with an average of 39.4 years. The sex ratio of male to female was 26: 42, with a ratio of 12: 26 between the ages of 20 and 49. The lower extremities were most frequently affected (70.0%). Involvement of the face, head and neck regions was 26.7%. Seventy-eight percent of the patients had associated systemic diseases. Ulcerative colitis, which was seen in 19 patients, was the most common complication. Two patients (3%) had diabetes mellitus. Fifteen patients (22.1%) had a history of prodromal factors such as microtrauma or surgical invasions at the site where pyoderma gangrenosum subsequently developed. The incidence of trauma as a prodrome of pyoderma gangrenosum was low in the group with either ulcerative colitis or aortitis syndrome.

    We report 9 patients suffering from acne conglobata. Among 164 patients with acne vulgaris who consulted our hospital, 9 patients complained of acne conglobata. The 9 patients were all males who were from 16 to 22 years of age. They were treated mainly with tetracyclines and dexamethasone was used before going to bed in a few cases. Two of the 9 patients had not shown any improvement and have severe scarring on their faces and chests. These cases need to be treated with additional drugs, either 13-cis retinoic acid or dexamethasone. DHEA, DHEA-S, testostrone, 5α-DHT and 17α-OHP were measured in 6 patients. DHEA-S was increased in all the patients; on the other hand, DHEA was increased in only two patients.

    A 46-year-old man with a subcutaneous asymptomatic tumor at the right inguinal area visited our hospital. Clinically, atheroma or lymph-node swelling was considered, but during surgery, we found a plerocercoid. The adult worm and eggs obtained by feeding the plerocercoid in a dog were identified as Diphyllobothrium erinacei.

    Two cases of syringocystadenoma papilliferum are reported. The first case was a 20-year-old woman with a tumor on the lower part of her left leg and the other was a 59-year-old woman with a tumor on the lower left side of her back. Both lesions clinically showed pedunculated tumors with a verrucous surface. Histologically, the epidermis showed hyperkeratosis and acanthosis. The cystic invagination extended downward from the epidermis. A remarkable change was the proliferation of ductal epithelial cells filling the cystic space. These cases differ from most cases reported in the Japanese publications in both clinical and histological appearances.

    A 55-year-old woman was seen because of a pea-sized and slightly reddish nodular lesion of apocrine cystadenoma on the right hand. Histopathologically, the dermis contained one cystic space into which papillary projections extended. The space was lined by a double layer of cells. The inner surface was apocrine-type secretory epithelium with high columnar cells and peripheral elongated myoepithelial cells. 63 cases of apocrine cystadenoma have been reported in Japan and the usual location is the face, especially around the eyes. Our case is the first in Japan where apocrine cystadenoma occurred on the hand.

    Three patients with juvenile dermatomyositis (JD) were investigated by direct immunofluorescence, and in all three cases, deposits of immunoglobulins (Ig) and complements (C) were observed in the skeletal-muscle blood vessel walls. At the same time, 6 cases with adult dermatomyositis (AD) were studied, but deposition of Ig and C was not detected in the skeletal-muscle blood vessel walls. This result seems to suggest the possibility that the pathogeneses of JD and AD are different.

    The effects of 1,25-dihydroxyvitamin D3 (1,25(OH)2D3) on the growth and differentiation of cultured human hair follicle cells (HFC) were studied. 1,25(OH)2D3 markedly affected the growth of HFC. It caused a dose-dependent decrease in both plating efficiency and colony growth, and inhibited DNA synthesis of HFC. 1,25(OH)2D3 also caused an increase in cell size, stimulating cornified envelope formation, and inducing morphological changes in HFC which showed terminal differentiation in vitro. HFC was confirmed to consist of target cells. 1,25(OH)2D3 seems to be a potent inhibitor of proliferation as well as a potent stimulator of terminal differentiation of human HFC. Our findings may give helpful clues towards the mechanism of alopecia seen in patients with vitamin D-dependent rickets type II.

    Minoxidil (MX), a potent antihypertensive agent, has been shown to stimulate the growth of hair in vivo. We previously reported the direct effect of MX on preconfluent human hair follicle cells (HFC). Their growth and differentiation were not affected by MX at concentrations of 1, 10 and 100 μg/ml. In order to know the action of MX on scalp hairs more precisely, effect of MX on postconfluent HFC has been studied by a new culture method using floating mixed collagen membrane as the substrate. MX, at 10 μg/ml, did not prolong the life span of HFC or the time that HFC could be passed after confluence. Differentiation of HFC was not affected by MX, either. Terminal differentiation such as the appearance of denucleated cells, increase in cell size and cornified envelope formation was seen at a MX level of 10 μg/ml as in the control. The senescence of HFC was not slowed by MX, which suggests that vasodilation of hair follicle vessels by MX may be responsible for the clinical effect.

    One hundred cases of secondary lymphedema have been summarized. The incidence in the female was 5 times greater than in the male. The age of the patients was mostly between 40 and 80. In 82 of the 100, the lower extremity was involved. Half of them were unilateral. The primary diseases were uterus cancer (39 cases) and breast cancer (10 patients). 39 cases received radical surgery with radiation therapy, 24 only surgery and 6 patients only radiation. The latent phase was up to 13 years. Histology showed lymphangiectasis in all the examined cases. Aminopeptidase was positive in the elephantiasis dermis. Medical history MRI, tissue clearance and dermal lipoperoxide content were all useful for diagnosing lymphedema. Most cases were complicated with bacterial cellulitis and mycosis. The first choice of therapy was microwave treatment. The air compression massage therapy was uneffective in all cases that received it.

    Sixty-five women suffering from hand eczema were studied concerning the relationship between atopic diathesis or delivery and this disorder. 54 patients (54/65, 83%) had atopic diathesis. 37 of the patients with atopic diathesis, (37/54, 68%) had hand eczema following delivery. This leads to the conclusion that hand eczema in women is highly related to atopic diathesis and delivery.
