    A case of papillary eccrine adenoma is reported. A 30-year-old man had a nodule on his left upper arm of several years duration. The nodule was 1.2×1.5cm, firm and brown. The clinical diagnosis was dermatofibroma. Three cases of papillary eccrine adenoma reported in Japan are reviewed.

    We report findings in a 68-year-old man with extramammary Paget’s disease involving genital and bilateral axillary lesions. All three lesions were histologically confirmed to be Paget’s disease, and electron microscopic data revealed secretory Paget’s cells in all the lesions. There was no evidence of metastatic or deep-seated carcinoma.

    Keratosis punctata palmaris et plantaris developed in a brother (72-year-old) and a sister (60-year-old). The lesions in both patients consisted of miliary to pin-head sized keratotic papules disseminated on their palms and soles. The papules showed a yellow-brownish colored surface with crateriform depression. Although a keratotic column penetrating to an eccrine sweat duct was seen on the biopsy specimens, particular pathological changes of the duct were nil. Heredodiathesis was apparent in these patients.

    Two patients with lupus miliaris disseminatus faciei (LMDF) are reported. A 7-year-old girl had reddish papules on the eyelids and in the perioral region. A 33-year-old man had soy-bean-sized nodules on the forehead, head and the neck. Clinical appearances of LMDF and its relation to sex or age of onset were discussed, from the standpoint of statistics.

    The outbreak of Tsutsugamushi disease, an acute infectious disease, caused by Richettsia tsutsugamushi has increased in various areas of Japan, particularly in Kagoshima Prefecture, and the southern parts of Kyushu including Kagoshima Prefecture have been given attention because of recent outbreaks. We report the first four cases of Tsutsugamushi disease found in Minamata district in Kumamoto Prefecture, facing Kagoshima Prefecture. All of the patients were engaged either in agriculture or forestry and one was a housewife. In this paper, the symptomatology, epidemiology, diagnosis and treatment of Tsutsugamushi disease were discussed. Patients with this disease have often been overlooked, as cases of difficult diagnosis. Since this disorder is responsive to chemotherapy, it is urged that physicians have a better knowledge of this disorder and inhabitants will be educated concerning this disease.

    Scalded skin-like contact dermatitis developed at the site of application of 1% indomethacin ointment, namely around the skin on the achilles tendon 6 months after the initiation of application for the treatment of peritendinitis. Allergic contact dermatitis was suggested based on the results of patch tests with various concentrations of indomethacin ointment.

    Dermatoses due to tonsillar focal infection have attracted great interest by dermatologists and otorhinolaryngologists. In 1978, we reported data on these disorders obtained from dermatological departments of medical schools in Japan. We again sent out a questionnaire to dermatologists and otorhinolaryngologists using an similar method, in order to acquire knowledge on treatment of related disorders. The present data were from replies obtained from 44 dermatological and 42 otorhinolaryngological departments in Japan: In the previous report, 40 diseases from 13 disease groups were indicated to be dermatoses, possibly due to focal infection. In this report, 57 diseases from 15 disease groups were evident, thus indicating the variety of these diseases.

    Numerous assays for detection of CIC have led to important advances in understanding of immune-complexes-mediated disorders. We determined circulating immune complexes (CIC) by polyethyleneglycol precipitations complement consumption (PEG-CC) test in various dermatological diseases. Increased levels of CIC were found in sera of 11 of 20 patients with SLE. These levels correlated well with the clinical activity in the group as a whole, and with the course of the disease in individual patients. CIC in 3 of 10 patients with PSS revealed significantly elevated values. Two patients (18%) with morphea with elevated levels of CIC were diagnosed as cases of generalized morphea. Two patients (29%) with bullous pemphigoid, one patient (5.6%) with psoriasis vulgaris, 5 patients (63%) with squamous cell carcinoma and 3 patients (13%) with malignant lymphoma had significantly elevated levels of CIC.

  • Combined H1 and H2 Antihistamine Therapy

    There are two known types of histamine receptors, H1 and H2. Recent evidence revealed that human skin blood vessels possess both types of receptors. Thirteen patients with chronic urticaria were prescribed H1 (hydroxyzine) and H2 (cimetidine) antihistamines alone and in combination, in a cross-over trial. Combined H1 and H2 antihistamine therapy resulted in improvement in duration and distribution of wheal and itching, but there was no significant difference in wheal size. Twelve of thirteen patients reported improvement in subjective symptoms while on the combination therapy. Thus, H1 and H2 combined antihistamine therapy seems to be effective in controlling chronic urticaria.

    As contact dermatitis often occurs following application of percutaneous analgesic ointment, patch tests were performed using heparin-ointment, heparin gel-ointment. indomethacin ointment, ethofenamate ointment and the respective placeboes. Among these agents, indomethacin ointment provoked skin reaction which was significantly more potent than that of other agents, as determined in 36 dermatitis-free persons.
