    A solitary trichoepithelioma occurred on the face of a 66-year-old Japanese woman. The so-called mixed tumor of the skin was discussed from the aspects of clinical findings and histological features. The tumor formed a relatively large node measured 10×12mm and the surface was shiny and light-reddish, in association with teleangiectasia. These findings suggested the so-called mixed tumor. As the histopathology revealed numerous horn cysts of various sizes as well as the tumor cell nests with narrow epithelial strands, a typical solitary trichoepithelioma was diagnosed.

    A tumor on the scrotal skin of a 77-year-old patient was studied light and electron-microscopically. Tumor cells with a marked atypicality proliferated from a part of normal sweat duct toward the epidermis and formed numerous luminal structures. Histochemically, succinic dehydrogenase positive tumor cells were present within the epidermis and sweat ducts, and did not penetrate the basal membrane. Narrow lumina lined by tumor cells with microvilli were seen electron microscopically. Thus, the tumor was diagnosed as eccrine dermal ductocarcinoma in situ.

    A 48-year-old woman with primary biliary cirrhosis was treated with D-penicillamine. Five months later she had pemphigus-like lesions on the trunk. Laboratory findings: Direct immunofluorescent study; negative. Indirect immunofluorescent study; intercellular depositions of IgG, the titer, 1:64. Antinuclear antibody; positive. Histopathological examination: A intraepidermal blister containing acantholytic cells, neutrophils and eosinophils. D-Penicillamine was stopped and was prescribed corticosteroid. The skin lesions cleared while on this drug. The anti-intercellular antibody titer decreased to 1:8, but increased again to 1:128, 4 months later.

    In a 68-year-old man with unilateral Darier’s disease, a carbuncle appeared on the involved area of his left back. After excision, multiple organ failure, including pulmonary suppuration and edema, renal failure, liver dysfunction, digestive tract bleeding and consciousness disturbance occurred. The Darier’s disease also worsened. The eruption improved somewhat following improvement of his general condition, with intensive care. Immunological abnormality related to infection and the therapy, were discussed.

    Necrotizing fasciitis was evident at autopsy on a 57-year-old man with diabetes mellitus. A review of 21 cases of necrotizing fasciitis reported in Japan was made. All of the three fatalities among 21 cases reported had diabetes mellitus as the underlying disease, and in all, the inguinofemoral region was involved. Our patient had skin lesions on the dorsum of the right foot, which extended upwards involving the right inguinofemoral region. The role of anaerobic bacteria in the pathogenesis of necrotizing fasciitis appears to be prominent.

    Two patients with bullous pemphigoid were treated with plasmapheresis to reduce the dose of corticosteroids administered for the treatment of the skin eruption, for the following reasons, aggravation of diabetes mellitus due to a high dose of given corticosteriods in the first patient and surgery for gastric cancer in the other patient. Plasmapheresis rapidly improved the skin eruptions in both patients, with a significant decrease of the antibody titer against the basement membrane zone.

    We treated a 49-year-old Japanese man with xeroderma pigmentosum variant. He has multiple black papules on his face, and a reddish brown nodulo-ulcerative tumor on the right side of the nose. The histology of the tumors revealed basal cell epithelioma (solid type and cystic type). The DNA repair capacity of fibroblasts derived from the patient (XP4CB) was compared with that of a group A XP cell line (XP24TO) and normal control fibroblasts (YO, AK). XP4CB showed normal levels of unscheduled DNA synthesis, but was slightly sensitive to UV, determined by colony forming ability and host cell reactivation. Pulse-chase study by alkaline sucrose gradient centrifugation showed that the newly synthesized DNA by XP4CB after UV irradiation was considerably fewer than by normal cells and this inhibition was much more enhanced by caffeine. These results suggest that XP4CB has defects in the process of excision repair, as well as other unknown DNA repair mechanisms.

    A family case of Gardner’s syndrome was reported. Five patients were subjected to physical examination, bone survey, panoramic radiography, upper gastrointestinal series, gastroduodenal fiberscopy, barium enema and endoscopy. The results and skin lesions, especially cystic lesions were discussed. We propose clinicopathological characteristics of cystic lesions, as a result of dermatological approaches in diagnosing Gardner’s syndrome.

    A 78-year-old man with a so-called mixed carcinoma on the right ear and with a perforating lesion was reported. He had been a given Dermopan-irradiation to the right ear, under a diagnosis of basal cell carcinoma (BCC). After 9 years, the area of skin on the right ear became ulcerative and perforating. Histopathologically, BCC and SCC were present in parallel in the same specimen, thus, so-called mixed carcinoma was diagnosed. The lesion was removed surgically and the repair with double transposition plasty was made. The literature on so-called mixed carcinoma caused by radiotherapy was reviewed.

    External factors influencing the magnitude of contact sensitivity in mice were examined. (1) Magnitude of contact sensitivity was affected by doses of the antigens applied. Immunization with 300μg to 3 mg of oxazolone (OX) induced the greatest magnitude of contact sensitivity in A/J mice. Painting with 500μg of picryl chloride (PCl) induced the greatest magnitude of contact sensitivity in C57BL/6 mice and the contact sensitivity was reduced when miced painted with 5mg of PC1. (2) Effects of immunization by different routes were examined. Contact sensitivity with PCl did not sensitize in case of painting the clipped abdomen or tail and oral administration of PCl failed to induce contact sensitivity in mice. Painting of dinitrofluorobenzene on the clipped abdomen and footpad led to sensitization of the contact sensitivity but tail painting with the same antigen did not. (3) Effect of immunization by solvent dissolving antigens was examined. Immunization with PCl or OX dissolved with acetone induced a lesser magnitude of contact sensitivity than did immunization with other solvents such as ethanol or olive oil. It was suggested that these phenomenon were probably associated with activation of the suppressor pathway. The magnitude of contact sensitivity with PCl was reduced in the 13-month-old C57BL/6 mice.

    An ultrastructural study of secondary hemochromatosis was performed. Small electon-dense particles were observed in eccrine sweat gland cells and dermal macrophages but not in epidermal keratinocytes or in vascular endothelial cells. In the eccrine gland, these particles located at the luminal side of each cell, but not in the thick lamina densa. They contained iron as determined by energy dispersive X-ray microanalysis.

    Contact sensitization to 2,4-dinitrochlorobenzene (DNCB) was achieved in inbred JY1 guinea pigs by intradermal injection of epidermal cells prepared from DNCB painted skin (DNP-EC). Pretreatment by tape stripping on the induction site of contact sensitization with DNP-EC diminished the rate and intensity of challenge reactions to DNCB. The ability to induce contact sensitivity (CS) with DNP-EC returned to a normal state when normal peritoneal macrophages were given together with DNP-EC into the stripped skin. This indicates that the immunologic function of mononuclear phagocyte system in skin may be impaired by tape stripping treatment. Normal epidermal cells (EC) had a similar effect. Using either anti-Ia antisera and complement or allogeneic EC (strains 2 and 13), Ia-positive cells in EC, presumably Langerhans cells (LC), were found to be essential for the recovery of CS. Tape stripping treatment also resulted in development of immunological tolerance, as assessed by subsequent painting with a sensitizing dose of DNCB. The tolerance was abrogated when a certain number of EC was added to DNP-EC and injected through the stripped skin. These results suggest that Ia-positive EC, presumably LC, have immunological functions similar to those of macrophages and may process hapten-coupled EC, resulting in induction of CS. The absence of the cell function, possibly leads to induction of tolerance.

  • A Comparative Study of Young Women with and without Acne

    The purpose of this study was to assess the roles of skin surface lipids and serum lipids in the pathogenesis of acne vulgaris. Sixty-seven young Japanese women with age and living conditions well matched, volunteered for the study, of thirty-six had acne, of thirty-one had not acne. In the skin surface lipids of the cheeks, those with showed higher levels of total lipid values, the percentage of free fatty acids, and the proportional contents of C12 and C14 in the total fatty acids, and lower level of C18, with statistically significant differences compared with the non-acne group. In the skin surface lipids of the scalp, the acne group showed significantly higher levels of the percentage of free fatty acids and the proportional consitituents of C14 in the total fatty acids and significantly lower levels in the percentages of triglycerides. Serum lipids values showed no significant differences between the two groups. In the skin surface lipids of the scalp as well as the cheeks, the percentage of free fatty acids, and the sum of free fatty acids and triglycerides increased with the total lipid values. The roles of P. acnes and hormonal factors in the pathogenesis of acne, in correlation to skin surface lipids were given attention. While skin surface lipids are vital factors, they are probably not the initial and major factors involved in the occurrence of acne.

    Intradermal test reactivity and lymphocyte proliferative responsiveness to gold sodium thiomalate (GST) and aurothioglucose (ATG) were evaluated in guinea pigs immunized with the two gold compounds.
    1) Intradermal test reactions to the gold compounds were all positive one week after the second immunization (3 weeks after the first immunization).
    2) Positive responses to the two gold compounds were observed in lymphocyte transformation tests one week after the second immunization, and also in intradermal test reactions.
    3) Although apparent positive responses to the two gold compounds in the intradermal tests and lymphocyte transformation tests were observed as above mentioned, cross-reactivity between the two compounds was not so potent as expected. Concerning the weak cross-reactivity, an interaction between gold compound and self body protein was discussed.
