    Cases of senile keratosis, and xeroderma pigmentosum on the heads and faces which had been collected from 5 universities in the West Japan during the last decade were respectively 44, and 30 instances. In cases of senile keratosis, the sites of eruptions included the cheek, forehead, retroauricular region, and chin of 23 instances whereas the anterior, lateral and vertex of the head instances, the dorsum and apex of the nose of 5 instances orbital region of 2 cases, and one instance each of the eruptions on the occipital and auricle. In cases of Bowen’s disease, all instances encountered were the females including 2 instances on the cheek, and one each on the nucha, and the lip. Xeroderma pigmentosum were found more in the malesthan in the female with ratio of 3 to 2, while 81% of the total cases were reported younger than 4 year of age in the onset of the eruptions. Pathological and therapeutic observations were also presented with review of some literatures concerned.

    The squamous cell carcinoma on the skin of head including face is known to have following clinical features: (1) its incidence rate is conspicuously high, a contrast to that of the squamous cell carcinoma on the skin of the other anatomical sites; (2) its lesions are apt to invade neighbouring important organs such as eye, ear, nose and brain; (3) wide surgical excisions are often difficult or impossible; (4) cosmetic problems dangle after operative therapy, specially in the lesions of face. With special reference to the above-mentioned points, a clinical survey of the total number of cases of the squamous cell carcinoma developed on the skin of head was performed by the clinical study of cases as well as the enquête investigation, with collaboration of the dermatological clinics of Kyushu University, Kumamoto University, Kagoshima University, Tokushima University and Yamaguchi University. There were 179 cases of the squamous cell carcinoma on the head skin treated in five dermatological clinics during the last ten years (1963-1972). These 179 cases consisted of 107 cases of male, 70 cases of female and 2 unknown cases. Incidence rate increased remarkably in number after over sixty years of age. Of these cases, 95 developed on normally appearing skin, 12 on burn scars, 3 on chronic radiodermatitis, 11 on xeroderma pigmentosum, 3 on senile keratoma, 7 on leucoplakia and 35 on miscellaneous precancerosis. Incidence rates in lip, nose and scalp were higher than those in the other sites. The rate in lower lip was higher than that in upper lip, and the rate in lower orbita higher than that in upper orbita. Analysis by TNM classification revealed that the cases were devided into 73 cases of T1, 65 of T2, 33 of T3, 8 of T4 and into 148 of N1n 26 of N3, 33 of N2 3 of N4, and that allcases were M0. Histopathological study showed that the cases were devided into 86 cases of grade 1,38 of grade 2,25 of grade 3 and 0 of grade 4, according to Broders grading of malignancy.

    The present investigation regarding the effectiveness of treatment of squamous cell carcinoma on the head and neck was made on the basis of questionnaire survey at the five university hospitals, i. e., Tokushima, Yamaguchi, Kumamoto, Kagoshima, and Kyushu. The main results obtained were as follows:
    1) Major treatments were radical local excision, soft X-ray irradiation with Dermopan, and chemotherapy with bleomycin.
    2) Among patients treated for the first time in these hospitals, operation (Op.) alone, Chemotherapy (Chem.) + irradiation (Irr.), and Op. + Irr. were most widely used.
    3) Op. alone and Op. + Irr. were most frequently used in the treatment of T1 of TNM classification, for T2, Op. alone and Chem. + Irr..
    4) When treated forthe first time, Chem. was most frequently used for the lips, Op. for the head, and Irr. for the Nose.
    5) The patients treated for the first time had better prognosis than pre-treated patients.
    6) The patients treated with Op. + Irr. or Op. alone had better prognosis than those treated with other means.
    7) Op. appeared to give the best prognosis in the various methods of treatments in the first treated patients.
    8) TNM classification and Broders’ malignancy degree had a close association in regard to the prognosis of the patients.

    One hundred and eighty eight cases of basal cell carcinoma in the Department of Dermatology including Tokushima, Yamaguchi, Kyushu, Kagoshima and Kumamoto University were studied clinicopathologically. In these cases, it was noted that an equality of the male-female ratio, 62.1 % in over 60 year-old age, appearing of the tumor mostly on the face especially on the lower eyelids, nose, nasolabial regions and the adjacent sites. Histopathologically, a solid undifferentiated type was seen in 69.9 %, an adenoid type in 10.4 %, and pigmented lesions were noted in 44.5 %, No cases with distant or regional lymph node metastasis were observed. Histogenesis of basal cell carcinoma was discussed from the above clinical, light and electronmicroscopical findings.

    Author examined what kind of anti-cancer therapy was likely to have the most beneficial effect on basal cell epithelioma on face and head, through some literatures concerning treament for it, and some analytical studies of data gathered from the five university hospitals, and through some results of our performances carried out in our hospital. Attained results were reported.

    The operative therapies we have actually performed and the prognoses of the skin cancers and the precancerous conditions we have experienced in the past ten and some years (mostly squamous cell carcinoma and basalioma; several of meibomian gland carcinoma, fibrosarcoma and dermatofibrosarcoma protuberans, respectively) have been stated. It is necessary to consider postoperative cosmetic aspects carefully as well as eradicative effects, since head and neck most easily attract attention. For this purpose one is requrired to have full knowledge of operative procedures appropriate for the characteristics of each site— head, forehead, periorbital region, nose, perioral region, ear and so on. As it is for the skin cancer on the other parts of the body, it is desirable to perform operative therapy in the stage where metastasis to lymph nodes is not yet observed (stage ITN0M0). In cases that already have metastasis to the regional lymph nodes (NI-3), it is advisable to carry out radical neck dissection vigorously. For there has been a considerable difference in prognosis between the cases with dissection and those without. As to the cases that have the primary lesions of T3-4 and have somewhat questionable radicality on operative therapy, and as to those that are NI-3 and their radical dissection may not be done perfectly, it is prefarable, in order to improve prognosis, to positviely combine postoperative radiation therapy such as betatron for the former and 60Co for the latter. For multiple senile keratoma on the face, multiple Bowen’s disease on the body and the extremities, and xeroderma pigmentosum in the second and the early third stage, skin abrasion technique is recommended, which at the same time can renew the skin.

    The therapeutic value of the soft X-ray on the skin cancer in the head and neck region was discussed, especially in consideration of irradiation technics, protection of the underlying tissues, curative effects and cosmetic effects. The shielding technics of eyeball, teeth, cartilage and bone is the most important maters in this treatment, of which detail is illustrated. The curative effect is very excellent and the X-ray irradiation could be recommended as the first choice of the treatment of the skin cancers in this region, if the dermatologists have the sufficient knowledge about the dermatoradiology.

    Five per cent 5-fluorouracil cream under occlusion induced a selective destruction and cure of many of the superficial basal cell epithelioma and squamous cell carcinoma including Bowen’s disease and arsenical keratosis. This therapy was, however, less effective in deeper tumors and had better be combined with surgery or radiation therapy in solitary and deep skin cancers.

    At birth the infant was completely enveloped in the membrane-like collodion or baked apple. Under the membrane severe erythema was observed. Within the first 48 hours, the membrane dried, transverse fissures appeared at the lower abdomen, and lamellar desquamation of the membrane began. Erythema disappeared gradually. After 3 weeks, the envelope desquamated completely, and the skin remained normal there-after. The clinical course such as our case is comparatively rare.

    Storiform neurofibroma which developed on the back of a 23 year-old male has been reported. The growth of the tumor was slow but continuous and no malignancy was observed. The tumor was excised completely without showing any sign of recurrence afterwards. The characteristic matting-like structure and the presence of melanin in the cytoplasm of the tumor cells suggest that the tumor belongs to melanotic type.

    Thirty biopsies were taken from eighteen patients in pustulosis palmaris et plantaris. The specimens were examined electronmicroscopic and immtmohistochemically. The pustule wall consisted of epithelial cells, which contained various stages of keratinization. Desmosomes of pustule wall epithelial cells were partially disappeared, spongiosis and acantholysis were recongnized. In the altered epithelial cells intracellular organelles were also degenerated. In the nucleus of the epithelial cells inclusions bodies were existed, but the nature of their bodies was unknown. Within pustules, polymorphous leukocytes degenerated and showed abnormal appearance. The polymorphous leukocytes incorporated I131 γ-globulins and contained γ-globulin diffusely in the cytoplasm.
