In the angiocardiographic examination of a patient with cardiovasucular disorders, any technical mistake should never be allowed. So, the apparatus must have full safety and high trustworthiness. The instrumentation required for angiocardiographic exminations is a combined system that consists of many kinds of instruments (A.O.T.Injector, V.T.R., Cine Camera, Spot Camera, M.E.Equipments). For improving the safety and reliability of this instrumentation as a whole system, we investigated many technical problems which might occur during the angiocardiographic procedure, based on our 8 years experiences, and equipped several safety mechanisms, as follows. 1) All control equipments were centralized. 2) Several automatic safety guarded control systems, 3) a switch boad for the aid of confirmation of the procedures, 4) the safety device for injection, (against a meaningless and harmful injection of contrast media without any x-ray filming.) 5) and a remote control switch for the simultaneous recording of the x-ray filmings, injection signal and E.K.G., were all equipped.

The studies were performed to discuss the usefulness of x-ray synchronizer, Thermister and Mercury-Straingauge, which are used for taking x-ray chest film for infant and child. In clinical studies, the movement of diaphragm was studied by x-ray cine film. At the same time, synchronizing point to the diaphragmatic movement were also checked on the same cine film. Then, diaphragmatic movement were compared to synchronizing points which were taken by the x-ray synchronizer. On the other hand, the foundamental experiments were performed in order to examine the character of the Mercury-Sensor of the Straingauge. From our experiments, following results were observed. (1) In the cases of slow and regular breathing, both Thermister and Mercury-Straingauge are muched at the phase of maximum deep inspirtion. (2) In the cases of quick and irregular breathing, Thermister was synchronized at 220 msec. later at the inspiratory phase, and Mercury-Straingauge was delayed 400 msec. for works to synchronization in the phase of inspiration. From these data, it was conformed that the Mercury-Straingauge was mainly synchronized at the next phase of breathing which is close to the maximum inspiratory phase.

Using the chamber and film method, the depth dose of cobalt-60 γ-rays was measured and compared. In film method, Fuji nomal gravure film (TAC-135), which could be dealt with automatic film processor, was applied The results obtained are as follows : 1) In the stationary exposure of 10×10 cm field size, the film method has a density-exposure dependence on the depth from the surface of phantom. When film of maximum density of 1.5 and the characteristic curve (density vs exposure) in 20 cm deep of the phantom are used, the difference between isodose distribution by film method and depth dose rate by chamber method is almost negligible up to 50% depth dose. 2) In the moving field exposures of 10 cm field size using 20,25 and 30 cmφ phantoms, the difference of the isodose distributions between the film (about 1.5 maximum density) and chamber method is less than ±5% in the region up to the depth of 50% isodose distribution. 3) In both the moving (abdomen and breast, etc) and wedge field exposure, which are clinically applicable, the difference of the isodose distributions between film and chamber method is the same to the results in (2). It is concluded from these results that film method is suitable for the region up to 50% isodose distribution.

It is the technical or clinical purpose of the angiography of the lower extremity in vascular disease to obtain the exact and detail angiogram of whole arterial system at the region from the thigh to the tops of toes. Large-field angiography using US differential screen has generally been performed. An angiographic picture obtained by the technique of this method, however, gives a dilated or strained appearance at the portion of both thigh and foot, and occasionally shows a lack of radiographic density or sharpness at the portion of the thigh caused with doubleness of arteries and veins, according to prolonged infusion of relatively much of radiopaque. For the purpose of reduction of these disadvantages, angiographic scanography-called angioscanography was employed in present studies using mechanism of table sliding of modified KOORDINAT KOMBI apparatus. An angioscanographic picture was taken by the proportional motion of table sliding with the stream of infused radiopaque under the condition of fixation of narrowing slit which was attached just benath the x-ray tube. The experiments on the scanographic examination by the technique of present method indicated the followings. The reduction of Strobo effects was obtained by narrowing of slit width, slowing of scanning speed, and supplying of flat electric current to the x-ray tube. In present investigations, therefore, were employed the three phase 12 pulses of GIGANTOS's. Regarding the relationship between slit width, scanning speed, and radiographic sharpness, which was shown in CTF curve, radiographic sharpness was reduced when scanning speed was slowed and remarkably when slit wided. The results of fundamental examination indicated that radiographic sharpness was obtained by the maximum out put of x-ray apparatus and by narrowing slit width. Clinical angioscanography was performed on the base of the fundamental examination. Favorable angioscanogram in the long area such as the aorta or the extremity was obtained by the infusion of high concentrated radiopaque for a few seconds under the condition of proportional motion of table sliding and by adequate adjustment of electric current for radiographic density in each objective area.

Subjects involved in tomographic equipment and techniques required for taking axial transverse tomogram suitable for diagnosis were summarized into four items and were explained, that is adjustment of axial transverse tomograph, application of moving filter, device for reducing obstractive shadow, tomography while changing the tube voltage. Axial transverse tomography is considered indispensable to the planning and implementation of radiation therapy. The contents were explained for cases of two oblique field irradiation, moving field irradiation, hollow out technique irradiation and high energy electron beam irradiation.

This paper deals with an intra-operative irradiation technique for advanced stomach cancer. 1) An operating table for intra-operative irradiation was made in such a way as to be movable up and down, left and right, so that a treatment cone can be precisely positioned. 2) A treatment cone of pentagonel shape was especially made, which encompasses the lymph nood groups around the celiac axis. 3) The isodose distribution curves of electron beams through this treatment cone were obtained. i) When the entry surface is flat or smooth, the isodose lines run parallel to the surface. However, when it shows sharp irregularity, the lines are very complex. ii) When air space exists between the treatment cone and the tumor surface, the exposure on the tumor surface can be corrected by an inverse-square law. The absorbed dose at a given point in tissues was measured according to the measurement method of ICRU Report(21).

The most important megavoltage units are now the linear accelerator, the batatron, and the telecobalt units. The proper maintenance and control of these equipments are essential. In an attempt to know maintenance troubles and dosimetry problem etc, questionnaires were sent to 78 institutions or hospitals in which magavoltage equipments are installed, some 55 (70.5%) of the questionnaires were returned. The findings are indicated that the linearaccelerator and the betatron had machine troubles as twice as that of cobalt units and the dosimetry (out put check) were done only once a month in many institutions. According to our recent data in five years, the main time between failure (MTBF) was 104 hrs in the linear accelerator and stablity of out put dose was not enough. It may change around 4% in a week. The cost for 100 rad was culculated which shows about 1,100 yen for betatron and about 800 yen for linear accelerator and telecobalt unit.

Since there are many fashions of cobalt 60 teletherapy units, linear accelerators and betatrons available at present, we studied about the way of selection of high energy radiotherapy units for deep therapy, from a stand point of management and control based on our past measurments. Cobalt 60 teletherapy unit equiped with a source of 1500 Ci assigns us a little labor for management and control and it has a high commutability of physical data each other, but the number of patients being treated is not great (about 30 patients a day). Linear accelerator can treat more (about 75 patients a day), but assigns us a great labor for management and control. So, the way to select cobalt 60 teletherapy unit or linear accelerator should be decided from a balance between the number of patients and the labor for management and control. Output dose of betatron x-ray is less and stability of it is lower than other high energy teletherapy units for deep therapy. We should deem it is used as an exclusive unit for electron beam therapy only.
