A real time CT-linked treatment planning system, called a CT simulator, has been developed. The basic system consists of a CT scanner, a multi-image display component, a treatment planning device with real time visual optimization and a laser beam projecting component. All the components are connected on line and operate simultaneously. The system can be used for 3-D planning and simulation for radiation therapy within a short time. This new CT simulator is a useful 3-D treatment planning system in radiation therapy.

We have discussed the index of the bone mineral density (BMD) at the distal radius measured by single photon absorptiometry. Initially, the shape at the distal radius was evaluated using an X-ray photogram of the forearm and a calculation formula of the cross-sectional area at the distal radius was performed using an X-CT photogram of the forearm. A new index for the bone mineral density (modified BMD, mBMD), bone mineral content/cross-sectional area, at the distal radius was obtained for 154 young normal subjects (20〜44 yrs.). No significant differences in the mBMD values between young normal males and females, except for the group 20〜24 year-old group, were observed. Furthermore, a significantly decreased in the mBMD values with aging was observed in females between the ages of 20〜24 and 40〜44. However, no significant changes in the mBMD values were recognized in the men. Thus, it was shown that the new BMD index, mBMD, was useful for evaluating the changes of the bone mass.

As [<99m>^Tc]d, 1-HM-PAO passes the blood brain barrier and is retained in the brain cells for several hours, this radiopharmaceutical is expected to be an excellent regional cerebral blood flow (rCBF) tracer. A basic study was carried out on radiopharmaceutical purity of the lipophilic fraction of [<99m>^Tc] d, 1-HM-PAO and data acquisition with the brain SPECT. The radiopharmaceutical was formed to have a high yield (>90 %) by the stannous reduction of <99>^M0/<99m>^Tc generator elute (<35mCi) using a ligand kit. The reduction of lipophilic fraction of [<99m>^Tc] d, 1-HM-PAO was observed with excess of <99m>^TcO_4-, because of scarcity of SnCl_2, as the reductant. Using the high yield (>90%) of lipophilic fraction of [<99m>^Tc]d, 1-HM-PAO, more than 6% of administrated dose of [<99m>^Tc], d, 1-HM-PAO accumulated in the brain. The rapid clearance of [<99m>^Tc] d, 1-HM-PAO from the blood allowed the taking of a clear SPECT image with a low background at 30 min after the injection. Phantom study on data collection showed that the SPECT image had more than 300 counts/pixel (raw data 300k counts/slice) needed to take the brain SPECT haveing quality good enough to detect a cold lesion of 12mmφ. This data acquisition criterion could be applied to the clinical study. In 15 cases of the cerebral vascular disease, all of the stenotic lesions (≧90%) of major cerebral arteries could be detected as hypoperfusion areas with [<99m>^Tc] d, 1-HM-PAO brain SPECT ; however, only 53% of the lesions could be detected as low density areas by a brain X-ray CT. In conclusion, the [<99m>^Tc] d, 1-HM-PAO brain SPECT was useful for detecting abnormalities in the cerebral blood flow.

We analyzed fine positional relationships between the radiological technologists and the radiological mechanical system with link methods which are used in human engineering. In the positional relationship, we carried out an analysis with the radiological technologists and radiological machines. In panel design, we set the link ratio with the link classification of the operaters and X-ray control panel, and we discussed the time scale into operation-diagram from map of eye movement.

Following the brilliant success of Thomas in the 1970s, bone marrow transplantation appeared to be the sole curative treatment modality for high-risk leukemia. In addition, a supralethal dose of total body irradiation was widely accepted as radiation conditioning for bone marrow transplantation. In this paper I described the present status of bone marrow transplantation and total body irradiation for leukemia in Japan based on the national survey in 1987. The records of 489 patients who underwent bone marrow tansplantation between 1975 and 1986 were collected from 33 hospitals. Survival of patients with acute leukemia treated with allogeneic bone marrow transplantation improved year by year, and reached 47% in 3-year observation. While one-year probability of developing interstitial pneumomitis, which was one of the leading causes of death, fell to 32% in the past 3 years. Change in the selection of patients in remission without infection as well as treatment modality, e.g., fractionated total body irradiation and selection of platelet donor with negative cytomegalovirus, resulted in the significant improvement in the survival and decreased incidence of interstitial pneumonitis. Technical reports of the total body irradiation were obtained from 40 institutions based on the national survey. There were many varieties of radiobiological, physical and technical aspects of total body irradiation. Since these varieties of methods make it difficult to search the optimal mode of total body irradiation, standardization of total body irradiation is indicated in the future investigation in Japan.
