  • Inhibitors of cristal formation and other factors

    In the present study, inorganic pyrophosphate (PPi), Mg, alkaline-phosphate activity (Al-pase), Ca, Pi and total protein in submandibular and parotid saliva were measured in 19 sialolithiasis and 61 age-matched control subjects in order to understand the possilbe etiologic factor (s) or disposition of this disease. The results can be summarized as follows:
    1. Saliva constituents in control subjects
    1) The concentrations of PPi and Mg, which are known to be potent inhibitors of calcium phosphate crystal formation, were significantly low in the submandibular gland as compared to those in the parotid. Since available clinical and statistical data on this kind of investigation so far performed show that sialolithiasis occurs most commonly in the submandibular gland, the lower inhibitory activity in this gland might be responsible for the formation of calculus.
    2) The concentrations of PPi, Ca and Pi in the submandibular saliva increased with age, while no distinct relationship was demonstrated between the concentrations of PPi, Ca and Pi and age. This finding seems not coincident with the clinical observation that sialolitiasis occurs most frequently in the second, third and fourth decades of life.
    3) As for saliva constituents, no significant difference was observed between males and females. This finding well coincided with the clinical observation that no sex difference exists in the incidence of sialolithiasis.
    2. Saliva constituents in patients with sialolithiasis
    1) With regards to all parameters examined, there was no significant difference between saliva constituents from non-diseased submandibular glands of sialolithiatic patients and those from normal subjects. This suggests that the submandibular gland in the non-diseased side functions normally even in patients with sialolithiasis.
    2) The diseased submandibular glands had significantly lower concentrations of Mg and Ca as compared to normal glands, which may indicate that some local disturbancees occur in the diseased submandibular glands from patients with sialolithiasis.
    3) The Al-pase activity in the diseased submandibular saliva from patients with sialolithiasis was considerably higher than non-diseased submandibular saliva from sialolithiasis and control subjects, In addition, the concentration of PPi showed a negative correlation to the Al-pase activity (r=0.822, P<0.05). It is assumed tha PPi may rapidly decomposed by high Al-pase activity, ultimately leading to crystal formation together with low Mg levels in the affected submandibular glands.

  • Lyophilization procedure of cartilage

    Auricular and rib cartilages were taken from immature rabbits of 40 days post partum. Lyophilized cartilages were prepared by rapid freezing at -50°C with vacuum degree of 0.03 Torr.
    The results of this study on lyophilized cartilages are summarized as follows:
    1) Lyop ilized cartilages showed 10% reduction as compared to fresh cartilages. However, lyophilized cartilages recovered almost the same size and macroscopic findings as those fresh cartilages when immersed for 30 min in a phisiological saline solution.
    2) Histopathologically, lyophilized cartilages maintained their intrinsic cartilage structure.
    3) The residual moisture was 4.83% in a 5×5mm piece and 4.90% in a 5×10mm piece of the lyophilized auricular cartilage, while it was 4.32% and 4.50% in 5mm and 10mm pieces of the lyophilized rib cartilage respectively.
    4) Living cells were not observed in the tissue culture of lyophilized cartilages.

    We recently experienced two cases of relatively rare bilateral submandibular sialolithiasis, which are describe in ditail in this paper.
    Case No.1 was a 26-year-old male having a swollen submandibular region of the left only X-ray films showed a presence of salivary stone not only in the left but also in the submandibular. Both stones could be surgically removed from the oral cavity.
    Case No.2, a 20-year-old female, attended our department with a chief complant of diffuse swelling of the left submandibular. Symptoms appeared only in the left side but subsequent X-ray films showed a presence of salivary stone in both sides. However, neither of stones could be removed surgically.
    The structures of both right and left salivary stones were observed in the former case using a scanning electron microscopy.
    Both right and left stones were consisted of three layers (center core, laminated structure outer layer), showing a remarkable difference in the center core and outer layer between two stones.

    114 patients with maxillo-oral malignant tumor visited our clinic during the period from 1952 to 1972, and 18 cases among them (15.8%) survived for 10 years or more.
    This paper described the clinical and statistical data on these survived cases, including age, type and location of the tumor. TNM classification, treatment methods and prognosis.
    The results were as follows:
    1) This type of tumor were more common in males than in females and developed frequently in 50-59-years-old patients.
    2) The primary site of tumor was predominantly in the maxilla and mandibular, initially appearing as a type of ulcer in most cases.
    3) Histopathological examination showed that most of these tumors were of squamous carcinoma.
    4) According to TNM classification, T 4 cases were most frequently encountered.
    5) 7 cases were cured with radiotherapy and additional 6 cases were cured with surgical intervention plus radiotherapy.
    6) Radiotherapy was mainly performed in T 1-T 3 cases, while surgical intervention mainly given in T 4 cases, combining radiotherapy or chemotherapy lissome cases.

    Two cases of recurrent dislocation of the temporomandibular joint were treated by means of the Dautrey procedure, which was reported in 1975, and the advantages of this procedure could be specified as follows.
    1) The technique to increase the height of the articular eminence is very simple, because an oblique cut through the zygomatic arch is made just in front of the articular eminence without opening the capsule of the joint.
    2) There is no need to graft the bone chips or the alloplastic materials.
    3) It is not necessary to apply the intermaxillary fixation postoperatively.
    4) There are few complications and functional results are very good.

    In this paper, a new classification of acrylic resin splints to be used for the treatment of maxillofacial fracture was proposed. This classification was based mainly on the state of remaining teeth, which are important for the restoration of occlusion. The classification includes intermaxillary-, lingual-, and buccolingual-type splints which hold dental arch in addition to a splint covering dental arch.
    The indications of these types are as follows: Intermaxillary type is applied for multiple teeth defect cases, lingual type for cases with suspected intraversion of bone fragment and bucco- lingual type and covering type for cases with deciduous or mixed dentition.
    When these type splints were used in our 43 patients according to this classification, successful results could be obtained. In addition, the usefulness of these acrylic resin splints as intraoral apparatus was discussed.

    Although persistent parotid salivary fistulas and sialoceles are relatively rare surgical entities, patients with these diseases are considerably suffered from facial deformity or massive outpouring of saliva in general. Many therapeutic methods ranging from conservative treatment to more radical surgical intervention have been used for treating these diseases. In this paper, the therapeutic effect of pressure dressing, one of these measures, was examined in 7 cases of parotid salivary fistula or sialocele. Pressure dressing was performed using a sponge and elastic mesh or elastic bandage. This simple procedure resulted in closure of fistula or disappearance of sialocele in all cases. In the case of lacerated parotid duct, however, the parotid function was degenerated, suggesting that the gland underwent atrophy.
    It seemed that pressure dressing is suitable for such cases as having lacerated parotid gland or those in whom the parotid function is impossible to maintain. In addition, this more easy and safe pressure dressing is considered to be a measure of first choice for intending functional cessation of the parotid gland, although other treatments including removal of the auriculotemporal nerve or radiation are also applicable.

    A case of solitary xanthoma without hyperlipoproteinemia arising from the lower lip mucosa in a 14 year-old was described.
    Xanthoma is a tumor-like lesion made up chiefly of xanthoma cells, occasional Touton-type giant cells, and varying amounts of fibrous connective tissue.
    This lesion is multiple and occurs in association with hyperlipoproteinemia.
    Occasionally a solitary lesion occurs in the skin without hyperlipoproteinemia, but is very rare in the oral mucosa.

    A case of teratoma arising from the hard palate in a new-born infant is presented. Histological findings revealed that the present tumor was consisted of two germ-layers, i. e., ecto-and meso-dermal tissues, with several tooth-germs probably in the late bell stage.
    We further reviewed 63 cases of teratoma in the oral and nasopharyngeal regions so far reported in Japan, and the following findings were then obtained:
    1. This type of tumor occurred predominantly in infants under one year old, and about 75% of the cases examined were female.
    2. These tumors occurred most frequently in the nasopharyngeal region and in the palatal region when limited in the oral cavity.
    3. Complicated deformity was often observed (21 cases), including cleft lip and palate in most cases

    A case of calculus in d 54-year-old female who complained of radiopacity in the right submandibular region was reported.
    Using X-ray diffraction and electron microprobe x-ray analyzer, its components were analyzed and the ultrastructure of the cracked surface of calculus was observed with scanning electronmicroscope.
    The results were as follows:
    1) Main component was hydroxyl apatite.
    2) Ca and P showed homogenous distribution.
    3) In a SEM findings, it was found that calculus was covered with a thick fibrous membrane and, had not a laminated structure such as a salivary stone.

    Frey's syndrome is characterized by sweating and flushing in the anterior auricular region during ingestion which may appear several weeks or months after trauma or operation in the parotid field. This syndrome is localized and often persistent when once appeared. We recently experienced a case of Frey's syndrome appeared after operation for condylar fracture and the clinical findings were evaluated including image diagnosis.
    A 20-year-old female received a corrective fixation for the fractures of mandibular body and bilateral condyles. The postoperative course was uneventful, however, abnormal sweating in the left anterior auricular region began to appear during ingestion from 22 months after operation. A starch-iodine test showed a sweating reaction in the left auriculo-temporal nerve region. No abnormal finding was observed by palpation, salivary scintigraphy and sialography except for slightly decreased salivation.
    The incidence of this syndrome varies from 35% to 62% by investigators and various treatment methods have been attempted, including general and local anticholinergicdrugs, blocking of ear nerve ganglion or tympanic plexus, radiotherapy and skin flap excision. However, these therapies mentioned above must be further improved because of possible recurrence and complication.

    The characteristic feature of the lip bilateral clefts is the short columella of the protruded premaxilla, resulting in the flat nose after lip plasty.
    We modified the method of Skoog in that two vertical flaps from the prolabium based superiorly on the both sides of the columella are transposed into an incision transecting the columella.

    A surgical opening procedure was successfully performed in a case of severe trismus appeared after the combination therapy of radiation, chemotherapy and salvage operation for treating maxillary cancer.
    By performing extensive removal of the ascending ramus area, excision of fibrous internal pterygoid and the coronoid process of temporal muscle, and partial resection of coronoid process of mandible and anterior margin of ascending ramus affected by osteomyelitis, the maximum opening area of front teeth increased from 5mm to 28mm. In consequence, maxillofacial prothodontic became possible to perform, resulted in a satisfactorily improved oral function.

    Sixty three patients with oral cavity squamous cell carcinoma were entered in a cooperative clinical trial of methotrexate with leucovorin (MTX+LV) rescue at 4 institutions. The dosages of MTX were 50mg/m2 iv bolus weekly (Arm 1), 50mg/m2 iv bolus twice weekly (Arm 2), and 350mg/m2 6-hour drip infusion weekly (Arm 3). LV rescue was started 24 hours after initiation of MTX administration, in Arms 1 and 2 with dose of 15mg iv, and in Arm 3 with 15mg iv, po or im every 6 hours for 5 doses. Response was evaluated by both the Direct Response Evaluation Criteria for Solid Tumor and the Response Evaluation Criteria for Head and Neck Cancer (proposed). The response rate evaluated by the former criteria was 33.3% with CR 1/51 and PR 16/51, and that by the latter was 51.9% with CR 3/52 and PR 24/52. The response rates for primary cases evaluated by the former and latter criteria were 41.4% and 66.7% respectively, and for secondary cases the rates were 22.7% and 31.8%, respectively. In the primary cases response rates for 3 arms were similar, but in the secondary cases high rates were noted for Arm 3. Plasma MTX concentrations were determined, 6, 24, 48 and 72 hours after initiation of MTX administration, the levels were all within a safe rauge. Clinically noteworthy toxicity was not observed. In view of the above, we concluded that MTX+LV rescue therapy was useful for squamous cell carcinoma of the oral cavity.

    Various treatment methods are now used for facial paralysis, however, no established method is yet available. In the present study, a combination therapy of stellate ganglion block and electric acupuncture was performed in 42 cases of facial paralysis with favorable results obtained as follows:
    The combination therapy was effective on not only fresh cases but also old cases. In addition, it was also useful for treating diabetes mellitus and hypertension.
    Some fresh cases of Bell's paralysis were, on the other hand, resistant to this therapy probably due to virus infection. In fact, an increased virus antibody titer in serum was occasionally observed in some of these intractable cases.
    Thus, in the case of facial paralysis, stellate ganglion block and electric acupuncture are recommended to perform after the onset of disease as soon as possible in addition to the determination of virus antibody titer.

    A case of elongated styloid process with psychogenic reaction is described in this paper. Elongated styloid process, which may be a result of ossification in the area of stylohyoid ligament, sometimes causes various clinically important symptoms. Many authors have reported the cases, incidence and etiologies, and have variously termed this condition according to their view points like Eagle syndrome, styloid-stylohyoid syndrome and so on.
    This case was a 38-year-old single woman who visited our outpatient clinic with main complaints of discomfortable feeling of throat and pain in the left inner mandibular angle when moved her jaw. The patient had been under neurological and psychosomatic care in a certain hospital because of suspected epilepsy and masked depression.
    Intraoral and radiological examinations revealed the presence of bilateral elongated styloid processes, while the psychic evaluation suggested a possible involvement of psychogenic reaction in the onset of this symptom.
    After her bilateral elongated styloid processes were surgically removed (organic factor), the psychogenic reaction was treated in our psychiatric department (mental factor). These treatments relieved all her symptoms.

    A case of multiple supernumerary teeth impacted in the jaw is presented. The patient, an 11-year-old girl, with a main complaint of anterior cross overbite was referred from the orthodontic department of our hospital for detailed examination of impacted teeth. An orthopanthomograph revealed eight supernumerary teeth impacted in the maxilla and mandible, two each were symmetrically located in the right and left premolar regions. These supernumerary teeth inhibited growth of permanent teeth. The patient was also suspected of congenital diseases such as cleido-cranial dysostosis which were subsequently denied by further detailed examination.
    Eight supernumerary teeth were extracted, followed by the morphological analysis, and review of literature on this syndrome was also made.

    A case of Congenital Salivary Fistula on the Cheek.
    A congenital salivary fistula successfully operated by Delore's method is reported. A twomonth-old boy had a congenital fistula on the left cheek near the angle of the mouth with some saliva-like discharge from it when sucking milk. The fistula was associated with an aural appendage. He had been observed regularly up to 5 years old when the secretion increased markedly and the surrounding area of the fistula always became damp and inflammed. According to the sialograms from the fistula and the left parotid papillary region, it was revealed that the fistula was originated from isolated lobule of the fistula anteriorly bordering the left parotid entity. The orifice of the fistula was moved onto the mucosa of the cheek by Delore's method. The postoperative course was favorable, resulted in an outflowing of saliva through thus moved fistula orifice.

    Schwannoma is an endodermal tumor originating in Schwann's cells of myelinated nerves and occurs in the gnathic oral cavity or relatively rate occasions. Schwannoma in the tongue has often been reported but occurrence in the buccal mucous membrane, palate or gingiva in the floor of the mouth is rare.
    We recently experienced a case of schwannoma in the floor of the mouth with the result described below.
    The patient, a 63-year-old woman, visited our hospital with a chief complaint of mass in the hypoglottis. She first felt a swelling in the hypoglottis about three months previously without any treatment thereafter because of the absence of pain. However, the patient visited a domestic hospital because she noticed that the mass was growing two weeks before coming to our hospital. She was diagnosed as having a dermotoid cyst and referred to us on December, 6, 1983. Intraoral examination revealed a diffuse tumoid rising in the right floor of the mouth. A walnut-sized mass with relatively clear bordering was palpable. It was elastic in consistency without tenderness. In addition, the mass was movable within the surrounding tissue.
    Under the clinical diagnosis of dermoid cyst, the tumor was removed under local anesthesia on December 15, 1983. The mass was covered with a thin capsule without adhesion to the surrounding tissue, so that it was easily resected en block. Thus removed mass was spherical, 15×17×12 mm in size, and covered with a capsule with smooth, grey-yellow surface and slightly low elasticity.
    Pathohistological examination revealed that the mass was a solid tumor covered with a thin capsule consisted of connective tissue, and bordered with interstitial tissue containing fi broblasts and collagenous fibers. In addition, tumor cells arranged and proliferated in a close bundle or palisade fashion, i.e. so called line of soldiers, in some parts, while they arranged in a network fashion in others. From these findings, the tumor was pathologically diagnosed as schwannoma (Antoni type A and B). This kind of tumor occurring in the mouth floor seemed very rare since it has less reported in the literature concerned.

    Limitation of mandibular movements may be secondary to a variety of causes including trauma, partial ankylosis, developmental malformation of associated with neoplastic change. Distinction must be made between involvement of the temporomandibular joint and pathologic conditions outside the joint that limit mobility of the mandible, such as enlargement of the coronoid process and masses developing from zygomatic arch. However, elongated masses of the maxilla extending posteriorly toward the coronoid process can also affect temporomandibular joint function and should also be considered.
    The present report concerns. rare case of unilateral pseudoankylosis of the temporomandibular joint due to formation of joint like structure between the coronoid process and maxilla.

    15 minor oral operations were performed in 12 patients under hemodialysis in Hemodialysis Center of Matsuc Seikyo Hospital, and obtained the following findings:
    1) The rate of postoperative hemorrhage was lower in patients treated on the day after hemodialysis than those treated on the day of or before operation.
    2) The rate of postoperative hemorrhage was remarkably low in patients treated on the day after hemodialysis with additional local hemostatic procedure as compared to other cases.
    3) Cephalexin (1/2 of usual dose as base) and erythromycin (usual dose as base) were used as antibiotics for preventing or treating postoperative infections (e.g. abscess) without any serious adverse reactions.
    4) Both rabbit and sheep hemagglutinin-antibody titers were lower in hemodialysis patients than in normal subjects.
    5) No consistent relationship was demonstrated between hemodialysis duration and heterophile antibody titer.

    Watching platelet coagulability of collagen, a microfibrillar collagen hemostat, Avitene®, was applied to 22 affected sites in 18 cases operated in our department of oral surgery with the results obtained as follows:
    The drug was applied to various breeding episodes including hemorrhage from soft tissue, bone and wounds after tooth extraction. The effective rate of hemostasis was 91%(more than “effective”), which was higher than other conventional hernostatics.
    Some difficulty may exist in the use of this drug because of its high adhesivity requiring great skill for applying, which may be however sloved by practice. Moreover, the drug has no possibility of becoming a foreign substance because of its good adhesive property and easy removal after hemostasis as compared to other conventionally used oxidized cellulose, gelatin preparations and bone wax. As a result, Avitene® is considered to be a safe and effective drug applicable to both soft and hard tissues.

    Alimix tablet, a combination of ampicillin (AB-PC 250mg) and dicloxacillion sodium (MDI-PC, 125mg), was administered to 130 patients with oral and maxillofacial infections and additional 89 patients after minor dental surgery. Ampicillin capsule (250mg) was used as control drug.
    1) In patients with oral and maxillofacial infections, Alimix tablet was useful equally to ampicillin capsule.
    2) In patients after dental surgery, Alimix tablet was significantly superior to ampicillin capsule in preventing infections.
    3) The sensitivity of Alimix tablet against isolated bacteria from oral and maxillofacial infections was superior to AB-PC, MDI-PC, AM-PC and CEX, suggesting its high antibacterial activity.
