    Fourteen patients (17 joints) with TMJ closed lock treated by stabilization splints, were evaluated with regard to condyle movement and changes in the disc and bone by means of MR imaging before and after treatment.
    In all 13 joints that responded to treatment, condyle mobility increased after treatment. Disc displacement and deformity progressed in 3 joints. Disc reduction was seen in 2 joints, and although 11 joints were still locked, disc mobility improved in association with structural changes. In 4 joints with no response to treatment, these changes were not seen. Pseudodisk formation and osseous changes were not observed in any patient.
    These results suggest that the use of stabilization splints in patients with closed lock may promote disc reduction and increase condylar and disc mobility even when the disc is still locked.

    Effects of growth hormone on dental development were studied by comparing dental development and skeletal development between patients with endocrine and non-endocrine diseases. We examined 25 patients with growth hormone deficiency, 43 patients with constitutional growth delay, and 8 patients with Turner's syndrome during the 6-year period between October 1985 and September 1991. As control, we examined Japanese subjects with normal height between November 1990 and June 1991. In growth hormone deficiency, the degree of dental retardation was less than that of skeletal retardation. It seems that growth hormone may be essential to dental development in growth hormone deficiency. In patients with non-endocrine developmental delays of constitutional growth, the development of the teeth was delayed, but to a lesser extent than that of the skeleton. This dental retardation may be caused by non-endocrine factors. Patients with Turner's syndrome, tended to have delayed dental development, considered to arise primarily from genetic factors. These results indicate that the effect of growth hormone on dental development was less marked than that on skeletal development.

    A case of trismus caused by glass fragments buried in the temporal region is reported. A 45-year-old man was referred to our clinic because of a limited range of opening, persisting for 2 months after sustaining a facial injury caused by fighting. A scar was found in the left temporal region, and mouth opening was limited to 15 mm. Radiographic examination showed a radiopaque mass in the temporal muscle medial to the zygomatic arch. On operation, three buried pieces of glass located medially to the zygomatic arch were removed. One piece measured 30×25×5 mm, and the other two measured 5×4×4 mm and 3×2×2 mm. The postoperative course has been uneventful for 3 years.

    In this study, we clinically observed 146 patients with sports-related oral and maxillofacial fractures treated at the First Department of Oral and Maxillofacial Surgery, Osaka University Faculty of Dentistry during the past 15 years. The results were as follows.
    Among 146 patients, 134 were male and 12 were female. Although 62.5% of the patients were 15 to 24 years of age, there was an annual trend toward a decrease in patients aged 15 to 19 years. The most common causative sport event was baseball (54 patients), followed by rugby football (26 patients) and soccer (15 patients). There was also a decrease in baseballrelated cases and an increase in soccer-related cases in recent years. Isolated mandibular fractures occurred in 92 patients, and the most common site was the mandibular angle, followed by the condyle and canine. Among 37 isolated midface fractures, the most common site was the zygomatic arch (14 patients).
    Baseball-related mandibular fractures had a high incidence of two-line fractures and fractures of the anterior portion of the mandibular body as compared with other events.

    Adenoid cystic carcinoma rarely develops in the minor salivary gland. We report a case of adenoid cystic carcinoma that developed in the upper lip. A 46-year-old woman had had a painless swelling on the right side of the upper lip for 12 years. There was a welldelineated swelling with an irregular surface (10×8mm). It was elastic, hard, and movable on palpation. Computed tomography (CT) and ultrasonography showed a lesion with a welldefined, high density rim. Histopathologically, an aspiration biopsy specimen suggested adenoid cystic carcinoma. The tumor was excised with a 10-mm safety margin under general anesthesia.
    Histopathological examination indicated adenoid cystic carcinoma. There has been no evidence of recurrence as of 24 months after the operation.

    Hyalinizing clear cell carcinoma is a minor salivary gland tumor, proposed as a new entity by Milchgrub et al. in 1994. The tumor usually occuss in women and is thought to be a low-grade malignancy. A review of the literature indicated that this tumor can be associated with regional lymph node metastasis and local tumor recurrence. However, no distant metastasis or tumor death has been reported. This report describes a case of hyalinizing clear cell carcinoma arising from a palatal salivary gland in a 66-year-old woman.

    A rare case of amelanotic malignant melanoma of the maxillary gingiva is reported. A 52-year-old man was referred to our hospital because of swelling of the maxillary gingiva. A clinical examination revealed a 14×10 mm amelanotic nodule of the 5 6 region with swelling of the cervical lymph nodes. The histologic diagnosis of a biopsy specimen was spindle cell carcinoma or suspicion of sarcoma. The patient underwent partial maxillectomy and radical neck dissection. Histologic examination of the resected specimen revealed spindle and polygonal tumor cells with no apparent melanin. There was metastasis to three cervical nodes. These tumor cells showed a positive reaction with anti-S100 protein and anti-HMB45 monoclonal antibodies, and a final diagnosis of amelanotic malignant melanoma was made. The patient is free of tumor as of 13 months after the operation.

    A rare case of angiolipoma of the tongue is described. A 1-year-old girl was referred to our department by her family doctor. One month previously, when the same doctor had examined the patient's pharyngeal region, she found no lesions on the tongue. Clinical examination revealed a firm, fixed mass, measuring 5×5 mm, on the left side of the dorsum. The lesion was treated by surgical excision under general anesthesia, and there have been no signs of recurrence in the approximately 2 years since the operation.

    Intravascular papillary endothelial hyperplasia (IPEH) is a benign reactive vascular lesion characterized by papillary proliferation of endothelial cells.
    We describe a case of IPEH arising in a 28-year-old man. On admission, a bluish red nodule was noted on the right side of upper lip. The pathological diagnosis of the resected mass was IPEH (pure form) with a thrombus.
    We briefly also review cases of IPEH in Japan.

    Substance P has various neurotransmitter functions and is thought to play an important role in arthritis.
    In this study, we immunohistochemically analyzed substance P localization in the temporomandibular joint disk of a patient with painful closed lock and a patient with osteoarthrosis.
    Substance P showed two patterns of localization in these patients. Substance P was distributed in hypertrophic synovial membranes and newly developed capillary vessels of synovial membranes.

    We report a rare case of a congenital midline fistula of the upper lip in a 1-year-old girl.
    The fistula was 1mm diameter and 3mm in depth and was located on the lower third of the philtrum.
    There was no mucous discharge from the fistula.
    A fibroma was present in the palatal gingiva in the upper incisor region.
    Both lesions were excised, but a connection between them was not found.

    We report a case of pustulosis palmaris et plantaris (PPP) caused by an allergic reaction to dental alloys in a 27 year-old man. PPP developed 1 month after placement of a full cast crown on the right lower second molar, and the patient visited the department of dermatology at our hospital. PPP did not respond to dermatological treatment. Patch tests revealed a positive reaction only for Zn 2+. Removal of the full cast crown containing Zn2+ resulted in complete remission of PPP 3 months later.
