The purpose of this study was to examine whether the levator veli palatini muscle (LVP) in patients with velopharyngeal incompetence who have undergone surgery for cleft palate is more easily fatigued than that of normal subjects during speech. Five patients with postsurgical cleft palate who were identified to have borderline velopharyngeal incompetence on nasopharyngeal fiberscopy were studied. Five normal subjects served as control. Each subject was asked to pronounce/pu/more than 50 times at a rate of one time per second. Electromyograms (EMG) of the LVP were recorded at a sampling rate of 2000/sec. The mean power frequency (MPF) of one syllable was obtained by power spectral analysis with an analytic window 1 second in width. In all normal subjects, the slopes of the regression line relating the MPF of the LVP EMG to the syllable number were slightly, but not significantly negative. However, in all patients with borderline velopharyngeal incompetence, the slopes of the regression line were significantly negative. These findings revealed that the LVP of speakers with borderline velopharyngeal incompetence was more easily fatigued than that of normal speakers during repetition of syllables. This study suggests that the fatigability of the LVP might be one cause of borderline velopharyngeal incompetence after surgery in patients with cleft palate.

Postsurgical dysphagia in patients with head and neck cancer is often improved by postural techniques, one procedure for compensatory treatment.
This study was done to analyze videofluorographic (VF) images of postsurgical patients with head and neck cancer by comparing upright posture with head tilting posture on the stronger side. Twelve postsurgical patients with head and neck cancer whose aspiration was improved by head tilting posture on the stronger side were studied.
The tilting magnitude, vallecular angle, and epiglottic angle were measuredon VF prints. Retained amounts of swallowed barium were evaluated in the oral region, vallecular region, and pyriform sinus on VF images.
Pharyngeal delay time, pharyngeal passage time, and pharyngo-esophageal (PE) segment opening time were measured on VF images. The results were as follows:
1) There was no correlation between any two of the following three variables: tilting magnitude, vallecular angle, and epiglottic angle.
2) The retained amount of swallowed barium swallow in tilting posture on the stronger side was less than that in upright posture in 10 of 12 subjects.
3) As compared with upright position, pharyngeal delay time and pharyngeal passage time were shorter in tilting posture on the stronger side.

Telomerase activity is thought to be crucial for the sustained growth or progression of most cancer cells, and its activation is an important step in carcinogenesis. Recently, three major subunits of human telomerase, h TR, h TEP1 and h TERT, have been identified. In this study, we quantified the m RNA expression of these telomerase subunits in oral tissues by RT PCR and evaluated the correlation between expression and telomerase activity. Telomerase activity was detected in oralleukoplakia (75%) and oral squamous cell carcinoma (SCC)(66.7%). Expression of both h TEP1 and h TR was detected in all tissue samples tested, including normal gingiva. In contrast, h TERT expression was detected in 12 of 22 (54.5%) oral leukoplakia and SCC samples and was negative in normal gingiva samples. In addition, expression of h TERT well correlated with telomerase activity. Detection of h TERTexpression in oral lesions might be a diagnostic biomarker of leukoplakia or oral cancer.

A case of cystic foreign body granuloma in the mantal region caused by inclusion of metal fragments of a sword for 60 years is reported. A 76-year-old man was referred because of a painless swelling in the mental region. Radiological examination suggested a cystic lesion containing foreign bodies. Enucleation of the lesion was performed. The foreign bodies were metal fragments of a sword, found floating in the cystic fluid in the lumen. Histopathologically, the formation of a cyst without an epithelial lining was recognized. The cyst wall was composed of granulation or fibrous connective tissue with hyalinization, many foamy and multinucleated giant cells, and dark brown exogenous pigmentation. The histopathological diagnosis was a foreign body granuloma with cyst formation.

We describe a rare case of a Japanese copperhead snakebite of the tongue in a 70-year-old woman. The venom of the snake caused life-threatning obstruction of the upper airway, accompanied by massive edema of the tongue and peripherial tissues. Nasotracheal intubation, intravenous infusion, and diuresis resulted in complete recovery without sequelae. Japanese copperhead snakebites of the tongue with airway obstruction are very rare and unusual.

Desmoplastic ameloblastoma (DA) is classified as a variant of ameloblastoma. DA is a rare lesion and is histologically characterized by extensive stromal collagenization or desmoplasia. We describe our experience with a case of hybrid DA consisting of DA and plexiform ameloblastoma.
The patient was a 52-year-old woman. Swelling was recognized from the mandibular right molar to the right ramus. A multilocular radiolucent area including an impacted tooth was present in the posterior part of the swelling and a radiopaque area with a honeycomb appearance was seen in the anterior part. After the diagnosis of DA, resection of the mandible and reconstruction with an iliac bone graft were performed. Histologically, the anterior lesion showed microscopic features of DA, and the posterior lesion showed those of plexiform ameloblastoma.
We discussed DA and hybrid DA by comparing our case with previously reported cases. We conclude that the diagnosis and treatment of DA require careful consideration.

Warthin's tumor, also known as adenolymphoma or papillary cystadenoma lymphomatosum, is a relatively rare benign salivary gland tumor that occurs most frequently in the parotid gland. We clinicopathologically studied 12 cases of Warthin's tumor (from 1976 to 2002), with reference to other relevant reports, and found that Warthin's tumor occurred more frequently in middle-aged or older men than in women. Eleven of the 12 cases arose exclusive'y in the parotid gland, while the remaining case showed coexistent tumors in the parotid and submandibular glands. Warthin's tumor is characterized by multiple occurrence, which was seen unilaterally in 4 of our patients. In total, 18 lumps were extirpated, and 1 contained a calcified mass, apparently caused by calcification of lymph nodes including embedded salivary gland tissue.

Cervical cystic hygroma usually occurs in infants and children, but rarely develops in adults. We report a case of cystic hygroma arising in the left submandibular region of a 46-year-old woman. The patient presented with a tumor in the submandibular region on December, 1997. Physical examination revealed a soft mass, measuring 11×4 cm, elastic on palpation. MRI revealed a lesion extending from the submandibular region to the oral floor. Surgical resection was performed on January, 1998, followed by continuous low-pressure suction for 7 days. Residu al tumor was treated by sclerotherapy with local injection of hypertonic glucose solution, performed 4 times. The postoperative course was uneventful, with no neurovascular injuries. There has been no evidence of recurrence as of 6 years after treatment.

An extremely rare case of urinary bladder cancer metastasizing to the mandible is presented. A 64-yearold man was referred because of paresthesia of the lower lip and a well-defined, hard mass of the mandibular angle region. Oral examination showed a lump extending from the mandibular molar to the buccal region, covered with normal-appearing mucosa. Panoramic radiograpy and computed tomography demonstrated a lesion with bone destruction, measuring of 6×6×5cm in the posterior part of the mandible. The lesion was histologically diagnosed as undifferentiated carcinoma and irradiated. During radiotherapy, bladder cancer was detected, and grade 3 transitional cell carcinoma was diagnosed. Because the histologic features of the mandibular lesion were similar to those of bladder cancer, the former was diagnosed as metastasis from the latter. After the completion of radiotherapy with 56 Gy, which reduced the size of the mandibular lesion, the patient received three courses of systemic chemotherapy to treat the bladder cancer. However, severe myelopathy and electrolyte imbalance developed, and the patient died of pulmonary embolism.

Adenocarcinoma, not otherwise specified (NOS), is a salivary gland carcinoma that shows glandular or ductal differentiation but lacks the prominent histomorphologic features that characterize other, more specific types of carcinoma. We report a rare case of adenocarcinoma, NOS, arising in the submandibular gland.
A 32-year-old woman was referred to the Department of Dentistry and Oral Surgery, Prefectural Central Hospital because of a tumor in the left submandibular gland. Fine needle aspiration biopsy was performed, and the histopathological diagnosis was Warthin's tumor. The tumor was resected with the mandibular gland. The resected tumor was diagnosed as adenocarcinoma, NOS, and a supra-omohyoid neck dissection was performed. There has been no recurrence or metastasis of the tumor as of 2 years after presentation to our department.

Hepatocellular carcinoma (HCC) with sarcomatoid changes is a rare neoplasm more frequently associated with extrahepatic metastasis than ordinary HCC. A case of gingival metastasis from primary HCC with sarcomatoid changes is described. A 67-year-old man presented with a gingival tumor. The patient had a history of HCC, treated by percutaneous ethanol injection (PEIT) and transcatheter arterial embolization (TAE). Three years after treatment, recurrence and the gingival tumor were recognized. The gingival tumor was dark red and bled easily. Biopsy was carried out, and the diagnosis was a metastatic tumor from the HCC with sarcomatoid changes. The tumor size was so large that the patient complained of difficulty in mastication. Radiotherapy was used to shrink the gingival tumor. It was very effective, and the tumor disappeared in response to only 40 Gy irradiation. Complications of irradiation were not serious, and radiostomatitis resolved promptly. The gingival tumor has not recurred. The patient was able to eat more than before treatment. Radiotherapy of the gingival tumor improved the patient's quality of life.

The incidence of double cancers has recently increased, but quadruple cancers are rare. We report a case of quadruple cancers arising synchronously in the upper gingiva, penis, and kidney 2 years after colon cancer treatment. A 75-year-old man was initially referred to our department because of a swelling on the upper molar gingiva. Resection of colon cancer had been performed about 2 years previously. Two years after the resection, the patient experienced discomfort of the upper molar gingiva. Intraoral examination revealed a white granulated soft mass, measuling 10×9 mm in the upper molar gingiva. The histological diagnosis was squamous cell carcinoma (involv: Lng spindle cells). Chemotherapy with 5-fluorouracil intra-arterial infusion and radiotherapy with
60Co X -ray were performed. After these therapies, hemiresection of the maxilla was done. On admission, penile erosion and tumor formation in the left kidney were detected. Dermatological and urological examinations were performed. Resection of penile cancer and renal cancer was done 2 months after the operation in our department. There has been no sign of recurrence as of 1 year after operation.

®). He suddenly lost consciousness, and convulsive seizures and ataxia developed during antineoplastic combined chemotherapy including 5-FU
®. Brain MRI showed a high-signal-intensity lesion involving the white matter bilaterally and symmetrically. Seven weeks after the cessation of 5-FU
®, the neurologic symptoms subsided. Although the mechanism of leukoencephalopathy remains to be elucidated, clinicians should be aware of the possibility of leukoencephalopathy occurring as an adverse neurologic effect of 5-FU
®.

We describe the use of bioabsorbable screws for the fixation of distraction devices. Distraction osteogenesis was applied to the mandible in a patient with severe mandibular deficiency. The devices were held in place with bioabsorbable screws, and distraction was done according to the presurgical plan. There were no complications intraoperatively or postoperatively. The distraction devices were removed by an intraoral approach, without using a screwdriver. This method decreases surgical time and eliminates the need for sedation or general anesthesia during device removal.
