    In this study, the clinical usefulness of dental X-ray computed tomography (CT) for postoperative assessment of secondary alveolar bone grafting was investigated.
    Nineteen bone-grafted alveolar clefts in 15 patients with cleft lip and palate were studied. All bone bridges were examined by dental three-dimensional (3D)-CT (PSR 9000: Asahi Roentgen, Kyoto, Japan).
    The postoperative three-dimensional morphology of the bone bridges was easily recognized. Dental 3 DCT images were suggested to be useful for assessment before installation of dental implants in bone bridges. In addition, the status of bone surrounding the installed dental implants and the periodontal space of teeth adjacent to the cleft could be clearly evaluated.
    In conclusion, Dental 3 D-CT provides clinically valuable information for the postoperative assessment of secondary alveolar bone grafting.

    Metastatic tumors of the oral region are relatively rare and comprise approximately 1 % of malignant oral neoplasms. Their prognosis is generally very poor because of systemic metastases. We clinically studied 17 cases of metastatic tumors of the oral region.
    Fourteen of the 17 patients were male and 3 were female. The mean age was 56.6 years. Metastases to the mandible were twice as frequent as those to the maxilla. The most common location of the metastatic lesions was the molar region of the jaws. The most common symptom of metastatic lesions was swelling. Paresthesia of the mental nerve was present in 4 patients. The most common site of the primary tumor was the lung, followed by the colon. Histologically, adenocarcinomas had the far highest rate of metastasis to the oral region.
    If both the primary tumor and generalized metastasis are controlled, the oral tumor must be aggressively treated. However, when the primary tumor was controlled but generalized metastasis was uncontrolled, patients died within 6 months despite oral treatment. Patients with uncontrolled primary tumors died within 7 months despite oral control, except for one patient who survived 13 months.

    In the present study, records were reviewed to delineate gender differences in tumor characteristics in 256 patients with previously untreated squamous cell carcinoma of the oral cavity who had undergone surgical treatment at our Department between January 1985 and December 1977. There were 180 men and 76 women. As compared with tumors in men, tumors in women were characterized by, more frequent occurrence in the gum and buccal mucosa, a higher incidence of exophytic growth, earlier stages of cancer, well differentiated squamous cancer, and a lower grade of cancer invasion. The rate of disease-specific survival at 5 years in men and women were 68.8% and 92.1%, respectively. Proportional hazards models among 256 patients revealed that independent risk factors were gender and tumor stage.

    This study examined reconstructive surgery performed with the use of free flaps at our department in patients with malignant tumors (59 cases) of the oral cavity. The free flaps used for reconstruction mainly included abdominis musculocutaneous flaps (21 cases), fibula flaps (19 cases), forearm flaps (16 cases), anterolateral thigh flaps (10 cases) and the scapular osteocutaneous flaps (2 cases). The take rate of the transplanted flaps was 88.2%. Subjects with postoperative complications were examined for effects of combined preoperative chemotherapy and radiation therapy, and were analyzed according to recipient and donor sites, and location of reconstruction.
    Complications of the reconstruction flap occurred at the recipient site in 22 patients, and the donor site in 6 patients. Surgical procedures were necessary in 6 patients. When the results were analyzed according to the site of reconstruction, it was found that most complications occurred in patients who required reconstruction of the mandible (10 subjects, 14.7%). Complications were observed in 15 patients (22.1%) in the group that received preoperative chemotherapy and radiation therapy.

    A follicular-type acinic cell carcinoma in the minor salivary gland is an uncommon lesion. We report a case of follicular-type acinic cell carcinoma occurring in the buccal mucosa. A 50-year-old womanwho had a painless swelling in the right side of the buccal mucosa was referred to the Department of Dentistry and Oral Surgery, Haga Red Cross Hospital. The swelling in the buccal mucosa was a hard and painless, darkpurple lesion, measuring approximately 20mm in diameter. No swelling of the submandibular or cervical lymph nodes was found. After establishing a clinical diagnosis of benign salivary gland tumor, the lesion was excised under local anesthesia. Histopathological examinations revealed that the tumor nest, which had a follicle-like appearance, was composed of poor atypical epithelial cells resembling normal serous cells.
    To clarify the characteristics of the tumor, Periodic acid-Schiff (PAS), digestive PAS, and immunohistochemical staining using monoclonal antibodies against secretory component (SC), saliva amylase, p53, p27, and Ki-67 were performed.
    The histopathological diagnosis was follicular-type acinic cell carcinoma in the minor salivary glands of the right buccal mucosa. The postoperative course was uneventful, and there was no evidence of local recurrence 15 months after excision.

    Primary malignant melanoma of the oral region is relatively rare. The prognosis of malignant melanoma in the oral region is poor. We report a case of malignant melanoma treated successfully with chemotherapy, local administration of high-dose interferon-β, and surgery. A 40-year-old man was referred to our department because of swelling of the upper alveolar gum. On67Ga-citrate scintigraphic examination, abnormal accumulation of RI was observed in both the maxillary region and the left upper neck region. Signs of metastasis to the regional lymph nodes were evident on computed tomograhic examination.The patient was treated by combination therapy consisting of surgery, chemotherapy, and immunotherapy. Before operation, one cycle of chemotherapy with CDDP, DTIC, and local administration of high-dose interferon-βwas carried out. Postoperatively, three cycles of DAV chemotherapy were administered. In the 7 years after operation, neither local recurrence nor distant metastasis has been clinically detected. However, it is very important to continue follow-up because malignant melanoma has a high recurrence rate.

    Venous aneurysm can develop in an irreversible and meandering manner or as diffuse or localized lesions. This aneurysm occurs often in the lower extremities and upper alimentary organs, but very rarely develops in the head and neck. This paper describes a rare venous aneurysm of the retromandibular vein in a 74-year-old man.
    The patient was introduced to our department because of sudden expansion of a slightly elastic hard tumorous lesion in the paotid region. The lesion was a movable mass on palpation. Computed tomography revealed a cystic lesion with a relatively sharp margin, clinically suspected to be Warthin's tumor. During surgery, a venous aneurysm was strongly suspected, and the diagnosis was confirmed histopathologically.

    Two cases of nasoalveolar cysts presenting with characteristic clinical features are reported.
    A 42-year-old woman was referred to our hospital because of upper lip swelling by her dentist. A computed tomographic (CT) scan revealed bilateral nasoalveolar cysts.
    A 50-year-old woman was referred to our hospital because of a swelling at the right nasal ala with epiphora by her dentist. A CT scan revealed a right-side nasoalveolar cyst, possibly developing from the nasolacrimal duct. Both cysts were removed surgically under general anesthesia, and they adhered to the nasal mucous membrane.
    Histopathological examination revealed that the cyst walls consisted of columnar epithelium with goblet cells and fibrous connective tissue with minor glands.

    Aicardi syndrome is a rare congenital anomaly characterized by infantile spasms, agenesis of the corpus callosum, and chorioretinal lacunae. This syndrome has been observed only in females. A case of Aicardi syndrome with cleft palate is reported.
    A six-month-old girl was given a diagnosis of Aicardi syndrome with cleft palate.
    At 3 years 4 months of age, when the patient weighed 11.3kg, we performed a push-back operation without complications. She is now 8 years old and is not able to use intelligent words. Nearly 200 patients with Aicardi syndrome have been described in the literature. This report is the seventh case associated with cleft lip and palate, and the second case to undergo palatoplasty.

    To improve the swallowing function of glossectomized patients with dysphagia after glossectomy, we newly fabricated a bolus-transportable palatal augmentation prosthesis (Trans-PAP), which is mounted with a bolus-transporting device to a conventional palatal augmentation prosthesis (PAP). In this paper, the structure of Trans-PAP is described and its clinical effectiveness for dysphagia in a patient undergoing subtotal glossectomy is reported.
    The patient was a 57-year-old man who underwent surgical reconstruction using a rectus abdominis myocutaneous flap immediately after subtotal glossectomy. Postoperative dysphagia was mainly caused by dysfunction of the pumping movement of the tongue. A Trans-PAP was applied. The bolus-transportation plate of Trans-PAP can press on the reconstructed tongue by spring action. Consequently, it is possible to compensate for tongue pressure and the pumping movement of the tongue. The results of functional examinations showed that swallowing function was restored by Trans-PAP.
