    A system to create a three-dimensional model from computer-aided three-dimensional images has been developed and clinically applied as an aid to planning specific surgical procedures. During the research and development stage, we prepared a maxillofacial bone model for a patient with a jaw deformity. In this report we present our clinical experience, introduce an outline for the system, and discuss issues related to the system's potential application.
    To develop the maxillofacial bone model we first took an X-ray CT for a three-dimensional image of the head and neck region before operation. We processed the CT using an AS4075 Toshiba processing system to obtain a three-dimensional image. From this image, we then prepared a three-dimensional model using the Solid Creation System from D-Mec Inc (The Solid Creation System makes three-dimensional models using resin which is hardened by laser irradiation). We then performed simulated surgery on the model. Next the actual operation was performed based upon the surgical procedures derived from simulated surgery. After the operation, another X-ray was taken and another three-dimensional model was made to observe the results of surgery.
    The problem at this stage is inability to produce an accurate reproduction of occlusion. Nonetheless, three-dimensional models offer great potential by enabling us to develop surgical procedures for specific jaw deformities based on three-dimensional structure.

    To evaluate the utility of subperiosteal tissue expansion (STE) for mandibular augmentation with hydroxylapatite (HA) particles, two experimental studies in adult mongrel dogs were performed. One was to observe histologic changes following STE and to compare the tissue response to STE with that to subcutaneous expansion. The other was to observe clinical and histologic changes following implantation of HA particles in the subperiosteal expanded bed. In the first experiment, small tissue expanders were placed and inflated subperiosteally in the mandible and subcutaneously in the breast. A fibrous connective tissue capsule was formed surrounding the implanted expanders. Periosteum was replaced by simple connective tissue during expansion. Capsule formation during STE was much more rapid than the subcutaneous expansion. In the subperiosteal group, a thick fibrous capsule consisting of coarse collagen fibers was observed one week after full inflation. Leaving the fully inflated subperiosteal expanders in place more than one month accelerated the resorption of the underlying bone. In the second experiment, one week after full expansion of the mandibular subperiosteal expander, the expander was removed and HA particles were implanted in the expanded bed. Despite marked augmentation with excessive amounts of HA particles, there was no deformity or infection of the grafts, which maintained their designed controur. The firm fibrous capsule formed by STE permitted consolida tion of HA particles easily and effectively, and prevented migration of the particles. Immobilization of the grafts was achieved within two months following implantation of the HA particles.

    Early invasive carcinomas (EIC) of the tongue (10 lesions), gingiva (7 lesions), floor of the mouth (5 lesions) and buccal mucosa (one lesion) were clinically and histopathologically investigated. Clinically, these EIC were subdivided into leukoplakic (13 lesions), erythroplakic (6 lesions), and erosive/ulcerative type (4 lesions). Coexistence of ulcer/erosion with leukoplakia and erythroplakia indicated a high risk of malignancy. In a few cases, tumor cell invasion into the underlying layer was observed in two or more parts of HE-stained specimens. Furthermore, special staining for silver coloid (Ag-NORs)-positive nucleolar protein granules and a ras-oncogene product (p53) revealed that the staining intensities of Ag-NORs and p53 protein in tumor cells of the EIC were intermediate between those of normal oral mucous membrane epithelial cells (negative control) and deeply invading carcinoma cells (positive control). Compared to leukoplakic type, tumor cells in the other two types expressed much more p53 protein. These results seem to indicate that some oral mucous membrane carcinomas are sequenced from precancerous lesions, that multicentric cancerization may occur, and that p53 expression is increased along with cancer development. Furthermore, erythroplakic and ulcerative early carcinomas seem to be more active than leukoplakic ones.

    The purpose of this study was to identify factors affecting postoperative healing in diabetic patients.
    During the 8 years from 1984 to 1991, 61 patients with diabetes mellitus were admitted to hospital and underwent oral surgical procedures.
    The oral lesions of these patients consisted of 24 odontogenic infections, 18 tumors, 13 cystic lesions, 3 traumas and 3 other lesions. Fifty-four of the patients suffered from not only diabetes mellitus but also from other systemic complications.
    All patients were under the care of physicians and well controlled preoperatively; their fasting blood sugar was maintained under 150 mg/dl and no ketonuria was detected. Poorer postoperative healing was observed in 11 patients. As associated factors, consistent blood sugar control and the duration of surgery were significantly correlated with the healing process.

    Prognoses were surveyed in 36 patients who underwent insertion of osseointegrated implants (Brånemark system) from January 1988 to September 1992.
    One-hundred and forty-one fixtures were inserted in the mandibula, 66 in the maxilla, and 12 flange fixtures were inserted in the orbital or temporal region. The 1-year success rate of osseointegration was 98.2%, the 2-year rate was 97.5%, and after 3-years osseointegration was not lost in any case. In 3 cases, osseointegration was not obtained. Two of these 3 cases underwent radiation therapy before fixture insertion, and in the other an onlay bone graft was performed concurrently with fixture insertion in the maxilla. These results indicate that this implant system is very reliable in routine cases. However, irradiated patients and patients who have received bone transplants pose problems. Further studies are required to solve these problems.

    We encountered a case of tongue cancer with marked neutrophilia and hypercalcemia. The plasma granulocyte-colony stimulating factor (G-CSF) level underlying neutrophilia was elevated to 1, 630 pg/ml. The serum parathyroid hormone-related protein (PTHrP) level associated with hypercalcemia was increased to 410. 9 pmol/l. This case was a rare occurrence of a G-CSF producing tumor associated with hypercalcemia.

    A 42-year-old woman presented with a hemispherical bony-hard nodule (size about 35×30 mm) in the muco-gingival area from the lower right first premolar to the left central incisor. X-ray examination showed a relatively clearly demarcated tumor mass, consisting of a mixture of tissue, which seemed to infiltrate the surrounding tissue in some areas. Biopsy was performed under the clinical diagnosis of a mandibular tumor, and the lesion was diagnosed to be an ameloblastic fibroma. Tumor extirpation and apicoectomy from the lower right first premolar to the left central incisor were performed. There were no distinct signs of a capsule and many microcysts were observed on the bone cavity surface after tumor excision. There has been no evidence of recurrence.
    Light microscopic examination revelaed proliferation of both epithelial and mesenchymal tissue in the excised tumor. In some large epithelial nests, columnar cells lined the periphery and loosely arranged cells resembling the stellate reticulum were in the central portion.
    Immunohistochemical properties were studied using a rabbit anti-mouse amelogenin antibody by ABC method. Collumnar cells forming the epithelial nest expressed a positive pattern. Tooth bud-like cells and cells resembling the stellate reticulum were not stained distinctly, and areas of hyalinization between epithelial and mesenchymal tissue were negative.

    A rare case of pleomorphic adenoma of the upper lip in a 10-year-old boy is presented. He was referred to our clinic due to swelling of the left upper lip. This upper lip lesion was well-demarcated, movable, firm, the size of the head of the little finger, and removed under local anesthesia. Histopathological diagnosis was pleomorphic adenoma.

    A large simple bone cyst coexisting with a cystic odontoma in the mandible of a 12-year-old boy is reported. The presence of sound bone between these lesions indicated that they occurred separately and accidentally.
    It seems that a complex odontoma which was situated above an impacted 171 tooth, gave rise to the cystic odontoma. Based on the biochemical examination results of the liquid contained in the simple bone cyst and the characteristics of the cyst, the etiology of this disease was discussed.

    Cleft lip and palate patients at our department for the past 10 years since its establishment (April 1981) were analyzed statistically.
    1. The number of primary cases was 341. It corresponded to 4. 5% of all new patients in our clinic.
    2. Among the total, 280 patients (82.1%) were residents of Kagoshima prefecture, and 16 were living in outlying islands.
    3. Included were 122 cases (35.8%) of cleft lip and palate (CLP), 115 (33.7%) of cleft lip (CL) and 104 (30.5%) of cleft palate (CP).
    4. CLP and CL were more frequent in males, and CP in females.
    5. Of the CLP and CL cases, 183 were unilateral and 54 bilateral. The left-right ratio of the cleft side was 1.9: 1.
    6. Among all patients, 48. 4% visited our clinic within one month after birth.
    7. Among all patients, 61. 7% were referred by obstetricians.

    A ganglion is a benign cystic lesion, frequently arising on the extensor surface of the wrist from the tendon synovial sheath or articular capsule. It most commonly occurs in females aged 10 to 20 years. However, ganglia of the temporomandibular joint (TMJ) are very rare and only seven cases have been reported. This report describes a ganglion originating in the TMJ in association with deformity of the mandibular head.

    A 46-year-old male came to our hospital complaining of sudden drowsiness while driving during the daytime. He was obese with a body weight of 84kg and a height of 170cm. He was admitted and underwent polysomunography.
    During the analysis, he snored and apnea occurred (apnea index: 46). As apnea was of the obstructive type, he was diagnosed as having the obstructive sleep apnea syndrome (OSAS). To treat this disease, he attempted weight control and a nasal continuous positive airway pressure system, but both were in vain. We then attempted to maintain the mandible in a forward position by means of a splint. Previously, we constructed eights plints for test purposes. Each of them was used to test his sleep pattern with an APNOMONITOR, and the best mandibular position was chosen. Based on these results, the optimal splint was prepared. When he slept wearing this splint, apnea was decreased substantially (apnea index: 5). Although he wore the splint every night, there was no evidence of teeth movement or any TMJ pain.
    We concluded that splint therapy can be a very useful, cheap, and simple method for the treatment of the OSAS.

    An 8-year-old boy with a history of bilateral cleft lip, alveolus and palate presented with multiple jaw cysts, and was diagnosed as basal cell nevus syndrome. Detailed examination revealed bifurcation of the ribs, calcification of the falx cerebri, pituitary fossa bridging, retained testis, ocular hypertelorism, and a large head. His parents also have malformation of the ribs. This syndrome is very complex and includes a wide variety of possible abnormalities. In addition, clinical symptoms do not necessarily appear and there is considerable variation in the age of onset.
    Therefore, this syndrome is very difficult to diagnose. We discussed some criteria for diagnosis.

    A case of bilateral hypertrophy of the masseter muscles, treated surgically, is reported. A 24-year-old woman visited our hospital with a chief complaint of painless swelling at the bilateral angles of the mandible.
    The lower and inner portion of the masseter muscle was bilaterally resected by an intraoral approach and the expanded angle of the mandible was trimmed off by an extraoral approach. No problems occurred postoperatively and the patient is aesthetically satisfied with the result.

    An osseous choristoma occurring on the tongue of a Japanese 11-year-old girl is reported. The lesion was a polypous mass, measuring about 8×8×5 mm, in the centroposterior region of the tongue. The lesion was removed under general anesthesia.
    Histologically, the lesion consisted of dense lamellar bone with Haversian canals and bone marrow. It was surrounded by a thin layer of fibrous conective tissue and covered by stratified squamous epithelium. The origin of this lesion was discussed briefly, referring to the literature.

    Pemphigus vulgaris is accompanied by oral erosions in the overwhelming majority of cases. The authors studied oral findings at the first visit of 12 patients with pemphigus vulgaris.
    1. In 9 cases, the chief complaint was pain of the gingiva and/or oral mucosa. In 2 cases, it was erosion of the oral mucosa, and in 1 case it was oral bleeding.
    2. In 11 cases, erosion was observed in several regions of the oral cavity.
    3. Erosion was frequently observed on the buccal mucosa, gingiva and hard palate. However, erosion of the alveolar mucosa and tongue was rare.
    These results suggest that the characteristics of erosion differ depending on the region of the oral cavity which is affected.

    Tetanus is a fatal disease if early treatment is not appropriately performed. Numerous patients complaining of trismus consult dentists or oral surgeons. Trismus frequently occurs as an initial sign of tetanus. Tetanus should therefore always be taken into consideration when examining patients with trismus.
    A 62-year-old female was referred to our clinic by a physician because of trismus and shoulder stiffness. However, trismus and spasms of the jaw, neck and shoulder musclature were rapidly aggravated. Although Clostridium tetani was not detected, clinical symptoms such as severe trismus and facial grimacing were strongly suggestive of tetanus.
    The patient was treated by a prophylactic tracheotomy, tetanus antitoxin and high dose antibiotics therapy. The clinical course of the patient improved without convulsions.

    We studied the proliferating activity of odontogenic keratocysts. Seventeen odontogenic keratocysts and 20 radicular cysts were examined immunohistochemically using anti-PCNA monoclonal antibody. The average PCNA (proliferating cell nuclear antigen) positive score of odontogenic keratocysts was 19. 7% and that of radicular cysts was 9. 1%. Two recurrent odontogenic keratocysts showed higher scores of 38. 1% and 28. 1% respectively.

    A clinicopathological study was carried out by immunohistochemically analyzing proliferating activity using a monoclonal PCNA antibody in 56 cases of oral squamous cell carcinoma. Specimens were obtained before treatment, fixed in 10% formalin and embedded in paraffin. Immunostaining was performed by the streptoavidin-biotin-peroxidase complex method with monoclonal mouse anti-human PCNA antibody. PCNA positive cell index was calculated as the percentage of PCNA positive cells/1, 000 tumor cells. These indices were compared with clinicopathological features. The following rasults were obtained:
    1. The mean±S.E. of PCNA positive cell index in the 56 cases was 32. 4 ± 9.8%,
    2. PCNA positive cell index was correlated with the macroscopic growth pattern, stage and degree of cellular atypism.
    3. PCNA positive cell index was not correlated with the degree of histological differentiation and pattern of invasion.
