  • A case-control study (I): Epidemiologic backgrounds of subjects and maternal episodes during early pregnancy

    To explore the factors related to the development of cleft lip and/or palate, a case-control study was attempted, based on 194 cases and 194 controls.
    Cases were incident cases born alive in five areas from April, 1978 to September, 1981. Controls were selected from normal infants in five major hospitals; matched for residence, sex, birth order and maternal age at birth. Epidemiologic information was collected by direct interview of mothers, using a questionnaire designed particulaly for this study.
    Major findings of interest are as follows:
    (1) Seasonal frequency of birth, birth place, gestational period, and presentation were not significantly different between cases and controls.
    (2) Mean birth weight and mean head circumference at birth were also not significantly different between cases and controls.
    (3) On an average, cases were significantly shorter at birth than controls in both sexes. Mean chest circumferences at birth in males were also much smaller in cases than in controls.
    (4) Among maternal episodes during early pregnancy, X-ray irradiation history and drinking habits of alcoholic beverages were similar between cases and controls.
    (5) Mothers had suffered from diseases and ingested drugs more often in cases than in controls, with statistical significance. Smoking mothers were also significantly more prevalent in cases.

  • A case-control study (II): Sock-cultural backgrounds and genetico-physical characteristics among parents

    Parents were compared between 194 cases and 194 controls to explore the sociocultural backgrounds and genetico-physical characteristics for the development of cleft lip and/or palate.
    Major findings which emerged from the present analysis arc as follows:
    (1) Parental birthplace, parental ages at marriage and at birth including age difference, frequencies of pregnancy, miscarriage or stillbirth, number of family members, type of living quarters, and pets breeding were not significantly different between cases and controls.
    (2) Parental education attainment was significantly different; fewer university graduates and more parents who completed only junior high school in cases than in controls.
    (3) Fathers significantly worked more often as professional and technical workers in controls, but more parents were unskilled laborers and service workers in cases.
    (4) On the average, parents were much shorter and fathers weighted much less in cases than in controls, with statistical significance.
    (5) Mothers experienced artificial abortions more often in cases than in controls.
    (6) Frequency of cleft lip and/or palate in relatives was significantly higher in cases, and parental consanguineous marriage was also more prevalent in cases.

  • I. Typing of component cells

    Two cases of mucoepidermoid carcinoma of salivary glands were studied ultrastructually; one of which was well-differentiated (case 1), and the other was undifferentiated (case 2). These tumors consisted mainly of mucous producing cells, epidermoid cells, and two types of intermediate cells which were identified as mitochondria-rich cells and undifferentiated cells. The clear cells under light microscopy in case 1 were observed as glycogenrich cells or ribosome-rich cells by electron-microscopy. There were large amounts of untypical form of component cells in the both tumors. In addition, a few dense granules, comparable with serous granules, were observed in some cells.

    Cefotetan distribution was examined in mice oral tissues by the agar well method.
    After abdominal administration of Cefotetan 200μg/g in mice, tissue concentration reached a peak in 5 minutes. The tissues in order of concentration were the serum, mouth floor, femur, mandibular bone, tongue, submandibular gland and masseter.

    Sagittal split osteotomy of ascending ramus is a well-founded method to correct mandibular protrusion. But it requires intramaxillary fixation for 6-8 weeks as in other operation methods. The use of compression screw or ceramic screw fixation shortens the period of intramaxillary immobilization and prevents relapse.
    On the other hand there are some problems in screw fixation techniques. The greatest problem is the displacement of the mandibular condyle, because it is difficult to reposition the external fragment of the ascending ramus after splitting.
    A new repositioning technique of the external fragment of the ascending ramus has been used in 9 cases of ceramic screw fixation in sagittal split osteotomy in our department. The characteristics of this technique are as follows;
    1. A cortical bone incision of internal surface and anterior border of the ascending ramus and buccal side of the mandibular corpus is made. In order to fix the position of the ascending ramus, Champy's mini-plate is attached between an acrylic resin plate set on the maxillary wire splint and the external anterior border of the ascending ramus.
    2. Removal of Champy's mini-plate and intramaxillary fixation.
    3. Sagittal splitting of the ascending ramus.
    4. Intramaxillary fixation and Champy's mini-plate is applied again. This procedure leads to correct condyle positioning.
    5. Ceramic screws are inserted percutaneously through a trocar to fix the fragments in the sagittal split osteotomy.
    6. Removal of Champy's mini-plate and suture of cutaneous and oral mucosal wounds.

    Adult rhabdomyoma is a rare benign tumor of muscle origin favourably occurring in the head and neck regions, and only 60 cases of this disease have so far reported in the literatures concerned.
    We recently experienced a case of adult rhabdomyoma appeared in the mouth floor. The patient was a 47 year old man who had first noticed a mass lesion on the right floor of the mouth approximately 1 year before. Because the tumor began to increase rapidly from about 3 months before, he visited our department September 1982. A reddish brown tumor of pigeon egg size was found on the right floor of the mouth. The tumor was surgically removed October 1982, which was subsequently revealed to be an elastic, encapsulated tumor of 58×52×28 mm in size.
    Histological findings: The tumor was of solid type and consisted of large, round or polygonal cells with nuclei on the margin of cytoplasm. The cytoplasm was cosinophilic and contained many vacuoles. In some of these cells, stripes were found by staining with P TAH or Masson's trichrome specification. No cellular atypism or mitotic activity was observed.
    Electron microscopic findings: There were many myofilaments and thickened Z-band in the cytoplasm of tumor cells.

    A 9 month-old male was found to have localized swelling on his left niaxilla second deciduous teeth by his mother. The swelling gradually increased to 7-8 mm in one month. X-ray showed nothing paticular. The lesion was removed. In pathological findings, the epithelium was not observed in the cyst wall, but by clinical and radiographical findings, it was diagnosed an eruption cyst.

    The oral manifestation of generalized multiple neurofibromatosis (von Recklinghausen's disease) are relatively few, and its intra bony findings are still more infrequent. A case in which a neurofibroma of the mandible associated with generalized multiple neurofibromatosis, and looking like an intra bone cyst is reported.
    The outline is as follows: A 25 year old female was referred to the Gunma University School of Medicine because of impacted teeth. The patient was not aware of her impacted teeth until told by d dentist she visitied for dental caries.
    The patient's general health was satisfactory except for scoliosis. No neurological or other systemic abnormalities were evident except for the following: café-au-lait skin blemishes and many elastic-like soft cutaneous tumors were noted on her face and body which were from 5mm. to 20mm. in diameter.
    Oral examination revealed no presence of the lower left first, second and third molars, and the tissue color of the region was light purple with an elastic hard consistency.
    The panorama roentgenogram r evealed impacted molars, and an area of round multilocularradiolucency of about 30 mm. in diameter which surrounded the crowns of the impacted molars. It had the appearance of a follicular dental cyst.
    Under general anesthesia the lower left second and third molars were extracted, and the intra bony neoplastic tissue was enucleated, but with difficulty. The cut surfaces of removed tumors found to be solid.
    The pathologist report was neurofibroma similar to the cutaneous tumors. Healing was uneventful, and to date during the past three and a half years, postoperatively prognosis was good.

    2 cases of Paget's disease of bone with obvious symptoms in oral and maxillo-facial region were seen in Osaka Dental University Hospital.
    Case 1, a 32-year-old male with projection of zygomatic bone, alveolar expansion of maxilla and mandible, roots resorption of teeth, and malposed teeth was examined. His mother and elder brother were diagnosed Paget's disease of bone. In 4 year after the first visit, he had pathological fracture of right femur bone.
    Case 2, a 70-year-old female with projection of frontal and zygomatic bone, alveolar expansion of maxilla and mandible, and subperiosteal abscess of 876 region was examined. She recieved mamma carcinoma operation. After removal of offending teeth, discharging pus from extracted sockets had not subsided up to present.

    This report describes a girl aged one year and nine months with an alveolar soft part sarcoma of the tongue. The patient visited our hospital because a lingual tumor had been found at a pediatric examination four months before first examination. No abnormality was observed in her general condition, but a non-movable, elastic soft, circumscribed tumor of the size of the tip of a small finger was noted in the left part of the dorsum of the tongue. The covering mucosa was normal. No abnormal findings were present in laboratory tests. Resection of the tumor was carried out with general anesthesia under a diagnosis of benign lingual tumor. The tumor could be resected as a mass because there was no adhesion between the tumor and the surrounding tissues. Histological findings revealed that the alveolar structure was clear in most parts of the parenchyma of the tumor, and needle-like diastaseresistant PAS-positive materials were observed within the tumor cells. Electron microscopic findings showed that the tumor cells were closely connected, interdigitated, and contained the basal lamina, desmosomes, and hemidesmosomes. Periodic fibrous granules with limited membranes were observed within the cell body.
    The postoperative course is uneventful, and as of now, five years after surgery, no sign of recurrence or metastasis has been observed.

    A 38-year-old female patient was referred to our department on March, 31, 1983 because of hemispherical swelling of the right masseter region at contraction. On the clinical examination, it was found that this swelling appeared immediately following the contraction of masseter and disappeared soon after the masseter was relaxed. In addition it was gradually reduced within 4-5 seconds when the contraction of masseter was continued. This hemispherical swelling was elastically soft of 2.5×2.5 cm size without spontaneous pain or tenderness. No pulsation or fluctuation was felt on the swelling, and no bruits were heard. Skin surface had normal color. The intraoral examination revealed no abnormal findings. 2-3 cc of dark red blood was aspirated by puncture, producing a temporal disappearance of swelling. The external carotid arteriogram showed a pooling of contrast medium in the rami masseterica of A. traversa faciei.
    Clinical diagnosis: Erectile hemangioma on the masseter region. On April 19 1983, the tumor was surgically removed by extraoral approach under general anesthesia. No complication appeared after operation, the except for mild, trasient palsy of the facial nerve on the affected side. As of 8 months after operation, the patient is favorably progressing with no complications or signs of swelling at contraction of the masseter.
    Histopathological diagnosis: Cavernous hemangioma

    A 27-year old female patient visited our department because of painless swelling of the left maxillary premolar region June. 1982. This was first noticed about 6 years before and 45 teeth were extracted by a certain dentist with no success.
    This tumor-like swelling was a hemispherical, thumbtip sized phyma. Radiografic examination revealed. polycystic honeycomb bone resorption in the affected site which could be clearly discriminated from normal bones. In addition to 36 extraction, the tumor was surgically removed. The prognosis is evenful without any complication at present.
    The histopathological diagnosis was odontogenic myxoma.

    Four cases of marsupialization: Effective Guiding Eruption of the Permanent Tooth in Jaw Cyst.
    Jaw cysts often involve teeth with incomplete root formation in young patients. Although extirpation of cysts is indicated in these cases, postoperative malocclusion may be produced by the inclination of adjacent teeth and elongation of opposed teeth.
    In order to prevent such a complication, guiding eruption utilizing obturator was performed in 4 cases after marsupialization. Three among the four cases recovered almost normal occlusion, however, the tooth extraction had to be performed 27 months after the treatment in the remaining 1 case.
    It seems effective to perform guiding eruption of the permanent tooth with incomplete root formation in jaw cyst by means of obturator after marsupialization.

    A 24-year-old female with Melkersson-Rosenthal syndrome who was received surgical correction for swelling of the lips was reported.
    27 cases of Melkcrsson-Rosenthal syndrome which were reported in Japan were collected.
    In about half these cases there was no evidence of lip granuloma and the author insisted that surgical correction for lips with continuous swelling should be done.

    Clinical investigation was conducted on 10 patients in whom osteoradionecrosis developed after irradiation for intraoral cancer.
    (1) The patients were 4 males and 6 females whose ages ranged from 39 to 79 years (mean 55 years).
    (2) Onset of osteoradionecrosis was from 1 to 42 months after irradiation (mean about 10 months). This complication occurred more rapidly in patients undergoing combination radiotherapy and chemotherapy than in patients undergoing radiotherapy alone.
    (3) This complication occurred most frequently in the bone body of the lower jaw and was found in accordance with the tumorous region.
    (4)It is said that even an external irradiation of about 6, 000 rad sometimes impedes healing or regeneration of the tooth socret. As a matter of fact, osteoradionecrosis was found in 3 patients who had received extraction prior to irradiation.
    (5) Nine patients underwent surgical treatment. Hemimandibulectomy and immediate reconstruction were performed in 2 of these 9 patients.

    It seems uncommon that nodular fascntis usually arising from the fascia and showing a rapid proliferation of fibroblasts appears in the oromaxillary region. We recently experienced a case of nodular fasciitis arising from the submandibular region in a 2-year-old boy. Only 10 cases including ours of nodular fasciitis in the oromaxillary region have been so far reported in our country, and our patient was the youngest.

    We recently experienced a case of malignant tumor for which an immediate mandibular reconstruction by A.O. osteosynthesis and deltopectoral flap was successfully performed following anterior mandibulectomy and dissection of tongue and mouth floor. A. O. osteosynthesis used in the primary operation aimed to attain a plastic reconstruction of the jaw by giving a curve in coincidence with the anterior mandibular portion. However, the incision line on the middle of the jaw was torn by tissue contraction due to too large tissue defect of the mouth floor, so that the secondary operation was performed by exchanging a curved plate to a straight one, resulted in the complete suture.
    In addition, for preventing an excess salination as a cause of oral-submental fistula formed in the mouth floor, a continuous tubing with two cut-down tubes was performed into both Stensen canals to keep dryness of the wound. Thus the purpose of reconstruction could be achieved after about 6 months after operation.

    Rapidly advanced reconstructive flap surgery and maxillo-facial prosthesis in reent years has palyed a great role in oral cancer treatments, among them, D-P flap, pectoralis major myocutaneous flap and forchead flap are widely used and very effective for improving disturbed oral function.
    On the other hand, the use of these flaps may be hesitated against relatively small defects within oral cavity, in othere words, the avlative procedure itself may be limited to some extent by the mode of reconstructive operation expecially in benign lesions.
    From these standpoints, an easy, intermediate sized flap such as palatal flaps is considered to be useful for surgery against rather small malignancies and/or relatively large benign lesions i oral cavity.
    In this paper, two cases of soft palate reconstruction with vascular pedicled mucoperiosteal flap are presented, with the following conclusions thus obtained.
    1) The vascular pedicled palatal mucoperiosteal flap is very useful for reconstructing defects of medium size (about 2.5×4 cm) in soft palate.
    2) This flap is of axial pattern with abundant blood flow and very easy to prepare.
    3) As compared to other flaps, this kind of flap produces no cosmetic or functional disturbances, resulting in a favorable seondary healing of donor site without any special treatment.

  • A comparative study with Kenalog®

    A comparative clinical trial of TL-102M (adhesive powder preparation containing beclomethasone dipropionate) and Kenalog in patients with erosive or ulcerating stomatitis was performed cooperatively in 7 institutions.
    The results could be summarized as follows:
    1. The degree of overall improvement more than ‘effective’ was 80.6% in TL-102M treated group and 51.9% in Kenalog treated group, providing a significant difference in favor of TL-102M (P<0.05, X2-test).
    2. No adverse reactions were noted in either the TL-102M or Kenalog groups.
    3. The rate of overall impression more than ‘good’ by patient was 93.3% in TL-102 group and 70.4% in Kenalog group, indicating a favorable patient's impression in TL-102M group (P<0.10, X2-test).
    4. The degree of usefulness more than ‘useful’ was 90.3% in TL-102M group and 63.0% in Kenalog group. A significantly superior result was obtaned in TL-102M group (P<0.05, X2-test).
    From these results mentioned above, TL-102M was concluded to be effective, highly safe, and useful drug for treating stomatitis accompanying erosion or ulcer.

  • A comparative study with Aftach

    The effect and safety of TL-102M (adhesive powder preparation containing beclomethasone dipropionate) was compared and examined in patients with multiple aphthae from 6 institutions using Aftach (adhesive topical preparation containing beclomethasone dipropionate) as a control. The results could be summarized as follows:
    1. The degree of overall improvement more than ‘effective’ was 82.8% in TL-102M group and 89.7% in Aftach group.
    2. No side effect was observed in either the TL-102M or Aftach groups.
    3. The rate of overall impression more than ‘good’ by patient was 96.3% in TL-102M group and 82.8% in Aftach group. Although no significant difference was noted between the two groups, significantly fewer patients in TL-102M group complained no pain at the time of application as compared to those in Aftach group.
    4. The degree of usefulness more than ‘useful’ was 96.6% in TL-102M group and 86.2% in Aftach group, both showing a higher usefulness in this type of disease.
