    We clinically examined 29 patients with squamous cell carcinoma of tongue, floor of the mouth, and maxillary gingiva who received unilateral functional neck dissection with preservation of the sternocleidmastoid muscle, internal jugular vein, and/or accessory nerve.
    The 5-year cumulative survival rate of all patients was 80.4%, that of 19 pN (+) patients was 68.6%, and that of 10 pN (-) patients was 100%. Pathologically, 4 metastatic lymph nodes (LNs) were found in 1 patient, 3 LNs in 3 patients, 2 LNs in 6 patients, and 1 LN in 9 patients. LNs were located in level 1 in 5 paitents, level 2 in 12, and level 4 in 2. Extranodular metastatic lesions were found in level 1 in 1 patient and in level 2 in 4. Among the patients with a poor prognosis, 2 LNs were found in 2 patients, 3 LNs in 1, and 4 LNs in 1, extranodular lesions were found in 2 patients, and LNs were located in level 2 in all patients.
    Our results suggest that functional neck dissection is indicated for patients who have single and movable nodular metastatic lymph node.

    The purpose of this retrospective study was to investigate the incidence of surgical complications in patients who underwent reconstruction with flaps after the resection of oral malignant tumors. In addition, factors (age, underlying disease, serum albmin level, preoperative treatment, method of postoperative nutrition, total number of operations, T-classification, and stage grouping) considered to influence the occurrence of complications were also analyzed.
    One hundred two surgical records from 85 patients (40 women and 45 men, mean age 63.5 years) were reviewed. Complications occurred in 74 patients (72.5%). There were 47 systemic complications and 44 regional complications. The most frequent systemic complication was respiratory disease (24 cases), followed by diseases of the cardiovascular (13), digestive (7), and hematological systems (7). The most frequent regional complications was local infection (22), followed by dehiscence of surgical wounds (21) and marginal or total necrosis of the flap (12). Among predictive factors, the patient's age, method of postoperative nutrition, and T-classification were significantly related to the occurrence of the complications (chisquare test, P<0.05).

    Two cases of oral early carcinoma that developed from precancerous lesions showed the usefulness of mucosal staining with iodine and toluidine-blue solutions during follow-up. The appearance of toluidine-blue stained areas and enlargement of iodine-unstained areas indicated transformation from epithelial dysplasia.

    We report a case of carcinoma of the floor of the mouth coexisting with an extravasation mucous cyst. A 60-year-old man was referred to our hospital because of a painful swelling in the middle of the oral floor. The lesion appeared to be a mucous cyst and measured 29×19mm. The surface was red and eroded. Cytologic examination resulted in a diagnosis of malignant tumor. Surgical extirpation was carried out with the patient under general anesthesia. Histopathological examination revealed moderately differentiated squamous cell carcinoma associated with an extravasation mucous cyst. There has been no evidence of recurrence or metastasis as of 1 year 8 months after surgery. However, long-term follow up is required.

    We report a rare case of osteosarcoma of upper alveolar bone in which early radiographic changes from could be retrospectively examined.
    A 37-year-old woman presented with an 18-month history of a persistant unusual sensation in the left upper central incisor despite endodontic treatment. The patient showed extrusion of the left upper central incisor with enlargement of the alveolar bone. Radiologic examination revealed irregular bone formation with bone and root resorption. Since histopathological diagnosis of a biopsy specimen revealed low-grade malignant osteosarcoma, surgical excision was performed. Three years after excision, no evidence of recurrence or metastasis was detected. In our patient, we could examine radiographic changes of the lesion from the early stage of tumor development.

    Previous studies have shown that the ratio of non-Hodgkin's disease (NHL) to Hodgkin's disease is high in Japan (almost 90%). T-cell lymphoma has a lower incidence and a worse prognosis than Bcell lymphoma.
    A 63-year old man was referred to our department because of pain in the left side of the maxilla. Clinical examination revealed swelling and an ulcer on the left molar gingiva. A computed tomograhic scan showed a lesion extending from the left side of the maxillary gingiva to the left paranasal sinuses. Histopathologically, diffuse large-cell malignant lymphoma was diagnosed. Immunophenotypic analysis of surface markers disclosed T-cell features. Laboratory examination indicated high LDH values and a reversed ratio of neutrophils and lymphocytes.
    The patient received combination chemotherapy with CHOP and radiotherapy.
    The tumor disappeared after therapy, and the patient has been under observation for 4 years without recurrence.

    A rare case of myoepithelioma originating from the submandibular gland is reported. A 28-yearold woman was referred for treatment of an asymptomatic mass in the left submandibular region. On clinical examination, a 30×15mm hard, mobile, non-tender mass was noted. Ultrasonography and computed tomography demonstrated a large, well-demarcated mass that was not distinct from the submandibular gland. The tumor was removed surgically in March 1998. Histopathologically, the tumor consisted of both spindle-shaped cells and plasmacytoid cells with proliferation of many vessels in the stroma. The neoplastic cells expressed S-100 protein, DF-3, and EMA immunohistochemically. The histopathological diagnosis was myoepithelioma. There was no evidence of recurrence 2 years postoperatively.

    Infantile fibromatosis, which is a fibromatous lesion characterized by tumor invasion of soft and hard tissue, occurs rarely in the head and neck. We report on a 3-year-8-month-old girl with infantile fibromatosis in the submandibular region.
    An orthopantomogram and computed tomographic and magnetic resonance imaging scans showed a large submandibular mass with resorption of the mandibular bone.
    Microscopic examination revealed proliferation of elongated, spindle-shaped fibroblasts with interlacing bundles of collagen fibers. Cellular atypia and mitotic figures were sparse. There has been no recurrence as of 4 years 10 months after treatment.

    Liver transplants from living related donors has been established in Japan as a treatment for patients with end-stage liver disease. Liver transplants from brain-dead donors is already a common treatment option in most developed countries. It is therefore important that most oral surgeons should be aware of clinical problems when treating such patients. We have successfully performed tooth extraction in 3 patients who had undergone liver transplantation. Careful attention was paid to the risk of infection because of life-long immunosuppression and the risk of bleeding because of liver failure.
    In this paper we focused on the management of patients with end-stage liver disease who underwent liver transplantation and on the role of oral surgeons as an essential part of a liver transplantation team.

    We report our clinical experience with a patient who intially had marginal periodontitis, which progressed to peritonsillitis and subdural empyema.
    The patient was a 57-year-old woman. On June 16, she visited our department. The diagnosis of peritonsillitis originating in 6 was made at our department. Chemotherapy was given and the pus was discharged by incision. The deeply spreading abscess and the presence of headache before surgery suggested that the inflammation extended into the head. A computed tomographic scan revealed that the left side parieto-occipital lobe had an abnormal appearance. After consultation with the department of neurosurgery, Nihon University, School of Medicine, a diagnosis of subdural empyema was established. The patient was transferred to the department of neurosurgery, and drainage by craniotomy was carried out on June 20. The pathogens were shown to be Streptococcus constellatus and Bacteroides mellaninogenicus group. On July 4, surgery was agein performed to create a path for drainage. A magnetic resonance imaging scan showed no evidence of the abscess on July 18, and the drain was removed. On August 19, the patient was discharged because symptoms had improved.

    We describe a case of contact stomatitis due to a surgical pack.
    A 61-year-old woman had the upper right first premolar extracted by a local dentist on January 22, 1999. Thereafter, she had pain associated with gingival recession at the upper right canine. For treatment, surgical packs were applied to the upper right canine region on February 17. From February 20, swelling developed in the right upper lip and cheek and reddening and pain developed at the right margin of the tongue. The patient was referred to our department on February 23. The swelling was edematous, and blood examination found no signs of inflammation. Interview of the patient suggested an allergic reaction to the surgical pack. We therefore referred the patient to the department of dermatology to undergo patch testing. There were no abnormalities with the powder and control, but reddening was noted 48 hours after the liquid had been applied to the skin of the back (evaluated as +), and blisters were seen after 72 hours (evaluated as+3). These findings resulted in a diagnosis of contat stomatitis caused by the surgical pack liquid.

    A case of bilateral sialolithiasis of the superior labial glands with multiple salivary stones is reported. The patient, a 63-year-old man, complained of swelling of the upper lip. Clinical examination revealed diffuse swelling with redness at the right side of the upper lip. On palpation, the swelling was thumb-sized, tender, and elastic hard. An azuki-bean-sized, elastic hard mass containing bony hard nodules was noted at the left side of the upper lip. Computed tomography showed calcified nodules at both sides of the upper lip. The clinical diagnosis was sialolithiasis of the superior labial glands, and the masses were removed. Histopathologically, multiple salivary stones were seen in dilated ducts of the superior labial glands. In the buccal and inferior labial glands, salivary gland ducts were dilated by a mucous-like substance and were surrounded by fibrous tissues and inflammatory cells. Some of the acini had disappeared.

    We report a case of unusual dental injuries associated with facial fractures. The patient was 17-year-old boy, who had injured his jaw by leaping from the fourth floor of a building. He had sustained full bilateral intrusion of the molars (8 7 6 5 4 5 6 7) into the maxillary sinus associated with maxillary and mandibular fractures, resulting in severe hemorrhage. The intruded teeth were removed through the canine fossa, and bilateral sinusotomy was performed.
