  • Involvement of the axis of upper premolars and molars

    The subjects were 39 female patients with juvenile TMJ arthrosis (T group) and 30 healthy females (N group) who visited the Dept. of Oral Surgery in the Dental Clinics of Nagoya University Hospital, Dept. of Oral Surgery in the Dental Clinics of Fujita-Gakuen Health University Hospital and the Clinics of the Lion Foundation for Dental Health during a period of 3 years and one month from April 1983 to April 1986. The following evaluations were made: 1) U 5 to occlusal plane angle, 2) U 6 to occlusal plane angle, 3) U 7 to occlusal plane angle, 4) U 5 to U 6 angele, 5) U 7 to U 6 angle, 6) U 5 to FH plane angle, 7) U 6 to FH plane angle, 8) U 7 to FH plane angle, and 9) FH plane to occlusal plane angle. The following results were obtained: 1. U 7 to occlusal plane angle in the T group (82.0±6.5) was significantly smaller than that in the N group (85.7±8.2). 2. U 7 to FH plane angle in the T group (113.0±6.8) was significantly larger than that in the N group (108. 4±8.5). 3. Significant differences between the T group (15. 2±4.2) and the N group (15.0±4.0) were not seen in terms of the FH plane to occlusal plane angle. 4. In conclusion, a majority of the T group patients showed a centrifugal inclination of the upper second molar.

  • Development and differentiation of orbicularis oris muscle by analyzing myosin Ca2+-ATPase activity

    In Japan, about 3 to 4 months after birth is widely accepted as the appropriate stage of lip plasty but some insist on operating a few days after birth or after 6 months. Natsume of our department investigated the orbicularis oris muscle which plays and important role in lip plasty morphometrically during development of rabbits. He suggested human lip plasty should not be performed before 3 months after birth.
    Subsequtently the author intended to examine the stage of lip plasty biochemically by analyzing the myosin changes during development, and then to clarify the growth process of orbicularis oris muscle in rabbits.
    Total activity of myosin Ca2+-ATPase in pure orbicularis oris muscle from the median area of lower lip was taken up as an index of the muscle development, and myosin Ca2+-ATPase activity per mg muscle protein was used as an index of its differentiation.
    The results were as follows:
    1. The variance of total activity of myosin Ca2+-ATPase during development was approximately the same as that of the weight of orbicularis oris muscle.
    2. The variance of myosin Ca2+-ATPase activity per mg muscle protein which was considered the important factor of muscle differentiation, presented the figure of 3 peaks. This suggested that there were differences in the stage and the rate of accumulation between myosin and other muscle proteins.
    3. Myosin of gastrocnemius muscle was chiefly accumulated in the growing stage. So it was supposed that orbicularis oris muscle was differentiated in the early stage.
    4. Supposing that lip plasty should be done after differentiation of orbicularis oris muscle, it seemed that the appropriate stage of the lip plasty was more than 40 days after birth in rabbits and more than one year after birth in human.

    Activation of the cellular oncogene ras may be an important event on tumorigenesis in many types of human malignancies. In the present study, expression of the ras oncogene product p21 (p21ras) in fresh tumor tissue from a squamous cell carcinoma of the maxillary sinus was examined by immunoperoxidase staining and immunoblotting. The tissue sections from biopsy material taken at pretreatment time were found to show intensively positive staining for rabbit antiserum or monoclonal antibody raised against p21ras However, the amount of p21ras present in the tumor tissues obtained from the primary site immediately after completion of cancer treatment including intra-arterial infusion of cisplatin (75 mg) into a superficial temporal artery and chemoimmunotherapy (UFT: 11.2 g, OK-432: 26.5 KE), was markedly decreased, when compared with the biopsy material. These findings led us to suggest that the expression of ras oncogene is intimately associated with occurrence of the maxillary cancer reported here and is modified by some cancer therapy.

    The purpose of this study was to clarify the facial morphologic changes between before and after two kinds of lipplasties which were performed by the rotation advancement flap method and the triangular flap method by measuring the points of the facial impression models taken before and after the operation in 40 unilateral cleft lip and palates and 40 unilateral cleft lips.
    The results were as follows.
    1) Changes in measured points by lipplasty
    All measured points shifted horizontally and vertically after lipplasty. The points on the cleft side shifted upward more than those on the non cleft side. The point of cupid's bow on the cleft side shifted downward more than that on the non cleft side.
    The points around the nose on the cleft side moved foreward, but the points around the mid-line on the non cleft side shifted backward.
    2) Post-operative symmetry by the two operation methods
    Although post-operative symmetry of the face from the frontal view was achieved by both methods, there was a tendency that the cleft side was over corrected by the rotation advancement flap method and less corrected by the triangular flap method.
    Three-dimensionally the points on the cleft side shifted foreward more than those on the non cleft side after both operations. However, the changes of points by the rotation advancement flap method was more than those by the triangular flap method, resulting in loss of symmetry.
    3) Characteristics of each method Deformed nose was better corrected three-dimensionally by the rotation advancement flap method, whereas, post operative effects by the triangular flap method were remarkable on the lower part of the upper lip, one of which was that mid-line of the lower part of the lip deviated from that of the face.
    Maxillary deviation before surgery greatly affected facial morphologic changes after operation, suggesting that preoperative orthopedic treatment for correcting the deviated maxilla is necessary in cleft lip and plate therapy.

    Although skeletal mandibular prognathism can be corrected by various surgical procedures, the tongue is always pushed backward by the retruded mandibular after surgery, resulting in an increase of reactive tongue pressure to the mandibular dental arch. This change of tongue pressure seems one of causes of relapse after surgery. This is a study on the changes in tongue pressure to the mandibular dental arch when pronouncing various sounds during one year before and after sagittal splitting osteotomy in 21 cases (5 males and 16 females). After a cantilever type pressure transducer utilizing a strain gauge was attached to the lingual aspect of the anterior teeth and the molar teeth on both sides, the pressure of the tongue during the pronounciation of ten Japanese sounds was measured before and 3, 6, and 12 months after the operation.
    The results were follows.
    1. The pressure to the mandibular dental arch greatly varied according to the sounds pronounced.
    2. The pressure on the molar teeth was higher than that on the anterior teeth.
    3. For one year, the pressure on the anterior teeth after operation was continuously higher than before.
    4. For one year, the pressure on the right molar teeth after operation was continuously lower than before.
    5. The pressure on the left molar teeth was higher 3 months after the operation than before but decreased gradually resulting in a lower level than the preoperatie pressure 12 months after surgery.
    6. Considered as a whole, the increase in postoperative tongue pressure is not so high after sagittal splitting osteotomy, so that partial glossectomy at the same time does not seem to be clinically absolutely necessary.

    This paper examined bacteriological study of 69 contents, which were classified based upon property of asymptomatic and few-symptomatic radicular cysts. Bacteria were isolated from 100% of 14 purulent cases (Group I), but only from 14. 5% of 55 serous cases (Group II). From Group I, anaerobes, especially Gram-negative rods such as Fusobacterium necrophorum were predominantly isolated, as facultative viridans type streptococci from positive culture cases of Group I, MIC values of the anaerobic isolates against various antibiotics were higher than those of bacteria indigenous to oral cavity. Many isolates from contents of radicular cyst showed various enzymatic activities thought to be toxic to periodontal tissue. In Group I and positive culture cases of Group II, scanning electron microscopic study revealed bacterial mass clumping in both the upper part of root apex and the area of apical foramen, but not in the area of apical foramen in negative culture cases of Group II. These results suggested that anaerobic isolates from contents of radicular cyst had ability to remain after antibiotic administration and to assist growth of radicular cyst. Anaerobic bacteria persisting in the upper part of root apex might proliferate and extend to contents of radicular cyst through apical foramen when host defence mechanisms decreased.

  • Effect of analgesics on evoked potential

    In recent years, an attempt to quantitatively and objectively express pain in concrete terms, which by conventional methods can appear only as a subjective experience, has been made through the application of somatosensory evoked potential (SEP). On this occasion, we experimentally induced pain by applying electrical dental stimulation to a tooth, and then we analyzed the effect of the pain stimulation on SEP and changes in SEP by administration of analgesics as well as the features of SEP at the central level. The results we obtained are described below:
    We evaluated the effect of electrical dental stimulation on somatosensory evoked potential (SEP), which is closely related to physiological and psychological aspects of pain, under the application of analgesics in 15 healthy adult males (diflunisal-treated group in 9 and control group in 6), and the following results were obtained.
    1) Five-phase waves showing peaks at N 75, p100, N 155, p255 and N 335 were seen during the post-stimulation periods, from 75 to 335 msec. These findings were practically identical among all the subjects in both groups.
    2) The 2 groups showed almost the same latency, indicating no effects of analgesics on latency.
    3) There were no noticeable changes in amplitudes in N 75-P 100 and p100-N 155, which are thought to be related to the intensity of stimulation.
    4) Among N 155-P 255 and P255-N 335, which are considered to be closely related to pain sensation, amplitudes in N 155-P 255 were significantly reduced in the treated group.
    5) Although the pain scale was decreased over time in both treated and control groups, there were no significant differences between the 2 groups.
    6) The amplitudes of SEP were altered by the administration of analgesics, while no marked differences in verbal expression of the pain were seen between the 2 groups.
    7) These results suggests that the expression of pain, which before now could only be expressed by subjective scales, will now be measured with quantitative scales using SEP.

    Osteosarcoma is a highly malignant tumor with extensively destructive potential, although it is not usually found in the jaw. A 44 year-old male complained of a swelling which was diagnosed as an osteogenic sarcoma. Left partial maxillary resection, supplementary chemotherapy and radiation treatment have been done. There is no evidence of reccurence or metastasis after 3 years.
    In reports of English literature between 1976 and 1985, 56.2% of 1, 020 osteosarcomas were in males and 43.8% in females. Tumors unclear mainly in the jaw were 3.4%. It is noteworthy that the most frequent age of patients with osteosarcoma in the jaw region was from 3rd decade to 5th decade, though those in the long bone region were patients in the 2nd decade.

  • Part 2: Pregnancy epulis

    A pathological analysis of 18 cases with pregnancy epulis were performed. Most of the tumors appeared at latter half of pregnant period, and gradually increased in size before delivery. It was thought that pregnancy epulis had a tendency to reverse its growth after delivery, but a few cases did not show size regression. The tumor arose most frequently in the anterior region of the maxilla.
    Histologically, vascular component predominated in 11 cases, fibrous component predominated in 5 cases, and granulation tissue predominated in 2 cases. Of these, the cases which were excised at latter half of pregnant period were chiefly composed of vascular component. The cases which were excised after delivery showed an increase in the fibrous component.
    In the epulis of nonpregnant women and men in the same generation, the most frequent cases were histologically prominent fibrous components.

    A 11-year-old boy was described who had the characteristic clinical features of the Cornelia de Lange syndrome, which included such symptoms as physical and mental retardation, eyebrows met in the midline, long thin overhanging upper lip, adverted nostrils with depressed nasal bridge, cleft uvula and various anormalies of the extremities. Dental treatment and palatoplasty were carried out under general anesthesia. Abnormal data were not shown in the hematological and serological examinations, and chromosomal abnormalities were not recognized.

    A case of hyponatremia associated with incurable mandibular osteomyelytis was reported in this paper. A 77-year-old male admitted to our hospital with a complaint of pus discharge from extraoral fistel of mental region on March, 1980. Resection and reconstruction of major portion of mandible using the rib bone graft, was performed under a diagnosis of chronic mandibular osteomyelitis. However, this operation did not cure him. He had been given heavy antibiotics chemotherapy and operated frequently during his 3 years stay in our hospital.
    Since September, 1983 his plasma Na+ concentration reached 130 mEq/l with no symptoms of hyponatremia. After his final operation on December, 1983 nausea, vomiting and drowsiness appeared with low concentration of plasma Na+ (120 mEq/l) and low plasma osmorality (260 mOsm/Kg. H2O). Clinical examination revealed that there were no abnormality in functions of liver, kidney, heart and adrenal glands. Plasma ADH was within normal limits. These symptoms were in accordance with hyponatremia, the so-called “sick cell syndrome”
    We had treated this patient with infusion of 3% NaC1 1, 000 ml/day, and medication of predonisolone 30 mg/day. G-I-K terapy was tried for 1 week, but it was not effective in this case. These therapies had continued a year and a half untill his death. The cause of death was airway obstraction by his own tongue while sleeping.
    Hyponatremia is common in severely ill patients. It has been reported that decrease in plasma Na+ concentration results from disturbances of cell function by illness. It seems to be important to think of sick cell syndrome, when hyponatremia appears in wasting disease patients such as incurable infection and terminal phase of cancer.

    Statistical analysis was made on 1, 341 biopsy specimens from patients treated in our department during the last five years. Seven hundred and eleven patients were males and 630 were females. Mean age of the series was 42.1 years. Cystic lesions were most frequently seen in this series and 388 out of 526 cystic lesions were odontogenic origin. Inflammatory lesions were seen in 281 patients. Of the 285 cases of tumors, 142 were malignant, 61 were odontogenic and 82 were benign tumors of non-odontogenic origin. Other lesions found in this series were leukoplakia (58 cases), epulis (60 cases), polyp (86 cases), ulcer (10 cases), etc.

    The patient was a 15-year-old female with a radiolucent lesion involving the inferior border in the angle of the mandible. This lesion was curetted and histologically diagnosed as giant cell reparative granuloma. Thirteen months after surgery, x-ray film showed a multiocular radiolucent lesion in the curetted area. The recurrent lesion, which was very enlarged in comparison to the initial lesion, was totally removed. Recurrent signs of the lesion were not observed during the follow-up period of fourteen months.

    Classification and treatment of oral melanoma have not been established. We reported the heterogeneity of oral melanoma consisting of acral lentiginous melanoma (ALM) and nodular melanoma (NM) in Part I. In this Part, the clinical courses of our nine and 47 reviewed cases of oral melanoma were analyzed retrospectively.
    Most of the reviewed cases had poor prognosis because of distant metastasis as well as local recurrence. We have treated oral melanoma by 1) surgical excision of the primary tumor, 2) neck dissection in Stage II case, 3) adjuvant immunochemotherapy with DTIC, ACNU, VCR, and OK-432. The prognosis of our cases are fairly good in contrast to those of the reviewed cases.

    A 19 year old female came to our clinic complaing of right cheek swelling. As a result of X-ray examination, a polycystic bone resorption image with clear cut boundary extending over nearly the entire region of mandibular ramus from the diaphysis in the area corresponding to the 5 root apex to the articular process and the coronoid process inclusion, was seen. The 6 root resorption was also found. Histopathological examination, revealed a diagnosis of ameloblastoma. In the selection of the type of operation and rebuilding material, in consideration of the fact that she was a young unmarried female, an intraoral method was selected and immediate rebuilding operation with an artificial bone of porous hydroxylapatite ceramic block was chosen.
    The porous hydroxylapatite ceramic block with 50% porosity was made with the cooperation of the Asahi Optical Co., Ltd.
    The operation was performed, under general anesthesia, GOF, by the intraoral method, dissecting the right mandibular bone from the 3 region and extirpating the coronoid process and the articular process inclusive, with the periosteum preserved intact since no tumor infiltration was observed. The artificial jaw bone of porous hydroxylapatite ceramic block was nearly equivalent to the extirpated right mandible inclusive of the articular process. This was made by referring beforehand to the cephalogram and pantomogram and was fixed to the cut end of the left mandible, the periosteum and mucosa were sutured and continuous suction was performed for 6 days after operation to avoid dead space. The intermaxillary fixation was performed for approximately 2 months following the operation. Up to the present, about 2 years and 4 months have elapsed and the course has been uneventful. In the right mandibuar vacant tooth portion, the prothesis using a swing lock attachment was mounted, to the satisfaction of the patient from a functional and also from cosmetic point of view.

    A 78-year-old woman diagnosed as IgG·ktype multiple myeloma by the secondary division, department of Internal Medicine in our hospital was referred to our department with a view to the biopsy of intraoral tumor of 56 giniva.
    The biopsy confirmed the secondary product of multiple myeloma.
    According to electron microscope observation, there were multivesicular bodies in some tumor cells and we thought that it would be a specific feature of this case.

    Two patients recieved subtotal resection of floor and mandible, including radical neck dissection, because of advanced cancer of mandibular gum. Hemiglossectomy was done in one patients, and one-third of tongue was resected in the other.
    Both glossectomies were performed in the frontal direction. Immediate reconstruction proceeded using A-O plate and free radial forearm flap.
    The kinetics of the repaired tongues were analysed longitudinally with lateral cephalograms enhancing the tongue, ultrasonic scanner and speech intelligibility test of Japanese syllables (dividing the tongue into anterior, middle and posterior portions).
    As a results, the area of difficulty in postoperative movement of the tongue was wider than the resected portion in both patients. Longitudinally the kinetics remained stable more than 6 months after surgery.
    The mobility of the reconstructed tongue was thought to improve by six months after operation. But compensatory function was considered effective to compensate the deficiency of the tongue function.

    A rare case of malignant acanthosis niglicans associated with lung cancer in a 70 year-01d male who showed various immunolegical disorders including antinuclear antibodies and CEA is presented.
    The main symptoms and signs werc a rough feeling of the lips and tongue, formation of many papillary tumors 1-2mm in diameter, papillary hyperplasia of the cheeks and the resulting formation of grooves, pigmentation of the Iips and the mucosa of the cheeks, furrodwed tongue and loss of taste sensation.
    The administration of cisplatin, doxorubicin and mitomycin C resulted in marked regression of the lung cancer, and the oral lesions showed marked improvement in association with the improvement in the immunological disorders. The improvement in the oral Iesions was also shown in the histopathological findirags.

    This report includes two cases of lateral cervical cysts.
    Case I. A 68 year-old female complained of diffuse swelling of the left side of her neck. Antibiotics medication made the swelling soft and localized. The swelling was incised and then disappeared. After 6 months she compalined of gain of soft and localized swelling her neck. Puncture and X-ray image with contrast medium suggested the presence of lateral cervical cyst. Although the cyst adhered to the cervical vein, it was separated and extirpated. The cyst wall was covered with stratified squamous epithelium, and lymphocyte infilitration was observed.
    Case 2. A 26 year-old male complained of swelling of the right side of his neck. The localized swelling with fluctuation was examined by X-ray with contrast medium. It showed the image of lateral cervical cyst. Extirpation was carried out easily. Microscopic examination revealed stratified squamous epithelium and lymphofollicles.

    A case of large fibroma occurring hard palate of 34 years old man was presented.
    He found a small tumor of the hard palate from six or seven years ago, but he left it.
    He visited our Department of Oral Surgery of Kurume University hospital for the first time because it is getting bigger and the speech disturbance appeared.
    He underwent an excision operation and prognosis come to good.

    Two cases of pleomorphic adenoma in the buccal region are reported. They were females, who complained of mass of this part. The masses that were well-demarcated and movable could be resected easily under local anesthesia.
    The prognosis were excellent.
    In minor salivary glands this tumor is most affected at the palate, incidence of buccal region are comparatively low.
    20 patients with pleomorphic adenoma in minor salivary glands were treated at the Department of Dentistry and Oral Surgery, Shinshu University Hospital during the 21-years period from 1966 to 1986.Incidence of this tumor in the buccal region is 10% in our department.
