  • Role of MMTV (mouse-mammary-tumor-virus) on glucocorticoid induced cleft palate in A/J mice

    A/J mice are infected with MMITV (mouse-mammary-tumor-virus) and the transcription of this virus is significantly stimulated by glucocorticoid. This study was aimed to find out the role of MMTV in glucocorticoid-induced cleft palate.
    Findings were as follows:
    C 3 H/HeN MTV-, BALB/c and C 57 BL/10 mice, which were inoculated with MMTV from AJ mice, were found to become susceptible to glucocorticoid-induced cleft palate.
    Therefore, MMTV was confirmed to be able to influence susceptibility to glucocorticoidinduced cleft palate in mice.

    Adenomatoid odontogenic tumor (16-year-old female) in the anterior mandibule was described, and immunohistochemicall detectable intermediate sized filament proteins were also examined in tumor tissue.
    Histologically, the tumor cells could be divided into two types; the type A cells showed spindle and columnar in shape and formed solid, ductal, tubular or whorled structures, and the type B cells showed small and compact cells located at the periphery of the A cell-containing forus.
    Immunohistochemically, the A tumor cells expressed slight or no staining to keratins, and the B tumor cells were characterized by co-expression with keratins and vimentin.

    Quantitative analysis of epithelial components, immunohistochemical staining of MoAb to keratin proteins, and 3H-thymidine labeling were tested to evaluate the carcinogenic process in experimentally hyperkeratinized epithelia induced by topical application of DMBA in hamster cheek pouch. The epithelial lesions were classified histologically into 3 types; A type (acanthotic type), B type (mixed type), and C type (basal hyperplastic type). Initial change of hamster pouch epithelium (0-3 W) was begun from the A type (acanthotic epithelium) and in more advancing stages (5-7 W), lesions showed highei frequency of the C type, and they fi nally occurred similar frequencies for 3 types.
    Keratin staining in the B and C types was characterized by irregular distribution and disappearance for PKK 1 keratin in basal cells and KL 1 keratin in the epithelial layer, although PKK 1 in the normal epithelium was confined to basal cells.
    3H-thymidine labeling was correlated strongly to decrease or tractable staining for PKK 1 keratins in basal cells of the precancerous epithelium.

    The laser doppler flowmeter was developed by Stern in 1975 and applied to practical use by Nilsson et al. in 1980. Since then it has been used in various fields of medicine. The laser doppler flowmeter's primary advantages are that it is noninvasive and can monitor the blood flow continuously and rapidly. The authors used a laser doppler flowmeter (a product of PERMED Co., Sweden; PERIFLUX PF 2 B; hereinafter abbreviated as PLDF) to continuously monitor the change in the facial peripheral blood flow (i.e., PERIFLUX blood flow; herein-after abbreviated as PBF) due to changes in the body position of patients in a dental chair, since such changes are unavoidable during the course of dental treatment.
    A change from the sitting position to the supine position caused an increase in PBF in 19 of 20 patients. In terms of the mean value per minute, PBF was about 25% larger in the supine position; the increase was 63% immediately after the change, 47% after 4 sec and 36% after 8 sec, all of which were statistically significant changes. After 60 sec, PBF returned to a level about 10% higher than the initial level in the sitting position.

    As Warthin's tumor is a comparatively rare salivary tumor, its diagnosis frequently worries us. ME (medical engineering) has made remarkable progress in recent years, therefore in the future we will have better diagnoses by using medical imagings.
    We recently experienced two cases of Warthin's tumor, one in a man aged 58 and the other in a woman aged 62. Both underwent operations for tumor enucleation in the right parotid gland after the diagnosis using venous kinds of medical imagings.
    Their progress are now good with no recurrence and no problems.

    Lipoma is a benign tumor derived from non-epithelial tissue origin and may arise from any portion where fat cells physiologically exist. The authors experienced a case of lipoma of the submandibular region.
    A 49 year-old woman was admitted to our department with a chief complaint of the swelling of the left submandibular region. The lesion was diagnosed preoperatively as a lipoma based on the clinical findings as well as the image findings of CT scan, echogram and MRI. The tumor was removed under general anesthesia, and diagnosed histologically as a lipoma.
    The authors will discuss the usefulness of MRI in the differential diagnosis of lipoma in this paper.

    A case of acinic cell tumor in the palate is reported in this article. Acinic cell tumor is a relatively rare type of adenocarcinoma of slivary gland origin. The 83-year-old man complained of swelling in the oral palate. A red soft mass (approximately 8mm in diameter) was noticed in the 6 region. Histopathological diagnosis of the surgically removed specimen was acinic cell tumor. The tumor invaded into the maxilla. Surgical excision with a border of normal tissue seems to be effective treatment. The patient had d, good clinical course without recurrence or metastasis.

    A case of multiple miliary osteomas of the skin which was incidentally found during the histological examination of the excised right cheek affected by squamous cell carcinoma of the buccal mucosa is reported in this article.
    This disorder of the facial skin is characterized by the formation of milium-sized bone masses in the corium.The etiology and pathogenesis of this lesion are discussed.
    The incidence of this disease could be more frequent than generally and previously considered.

    Internal derangements of the temporomandibular joint (TMJ) are a major cause of jaw pain and dysfunction as well as other related clinical symptoms.
    TMJ diagnosis is the abnormal position and appearance of the disk.Most X-ray-based methods arc useful for evaluating bony abnormalities, but their reduced soft-tissue contrast often makes the diagnostic evaluation of TMJ disorders difficult.Magnetic resonance (MR) imaging is a very recent addition to the medical diagnostic of TMJ diseases.MR imaging can produce high-quality tomograhpic images of greater soft-tissue contrast without ionizing radiation or known biological hazards.
    MR system was circular type Simens Magnetom1.5tesla.Display matrix was 256×256.
    A Modulated Helmholtz type coil of17cm in diameter was developed in Kameda General Hospital in order to increase signal to noise ratio in the area of bilateral TMJs.The distance between two coils was16-20cm.The head was placed in supine position in the center of two surface coils.
    A Modulated Helmholtz type coil:
    1. Modulated Helmholtz type coil was used as an emitter and a receiver.
    2.Modulated Helmholtz type coil had a pair of17cm coils, which were movable according to head width of each patient.
    3. MR imaging of bilateral TMJs was taken at once because of no necessity to reset a surfacecoil.
    4. It was easy to set the positioing of the head.

    It is well known that postoperative maxillary cyst often occurs about ten years after radical sinusotomy.
    On the other hand, symptoms such as buccal swelling and tooth pain are found in the patient with postoperative maxillary cyst, but it seems relatively rare to find exophthalmos complicated in these patients.This is a report of a case of postoperative maxillary cyst associated with exophthalmos.
    Our patient was a 63 year old male with a chief complaint of right buccal swelling and pain. He had a bilateral radical sinusotomy about 45 years ago.Right rhinostenosis and buccal swelling started about four months ago, became worse with ophthalmologic symptom. According to clinical diagnosis of a postoperative maxillary cyst associated with exophthalmos, we underwent radical sinusotomy and counter-opening under general anesthesia. A favorable course without exophthalmos followed the operation.

    A case of 54-year-old man with tongue cancer who received two courses of cisplatin and 5-FU infusion chemotherapy in the postoperative care was presented. The patient died one week after the course due to severe myelosuppression such as sudden leucopenia and thrombocytopenia, being thought the risk with chemotherapy was relatively low from clinical evaluation. We discussed causative factors on the drug-related death and obtained the following considerations:
    1) In the active chemotherapy, degree of the adverse reaction tends to severity with increase of the courses, and as frequent as possible blood examinations will be required.
    2) Chemotherapeutic agents by oral administration also considerably take part in manifestation of the adverse reaction. Then on the active chemotherapy, one month at least for pause of the drug will be needed.
    3) Though it has been considered that patients have adequately been relieved from adverse reactions by the improved admnistration of chemotherapeutic agents, even now risk has remained. Particularly, those of Japanese will have to be reduced in some measure in comparison with those of Caucasians.

    Though Erythroplakia is said to be one of the precancerous lesions with a high malignant transformation rate, very few reports have so far concerned this disease in Japan. We recently experienced a case of Erythroplakia occurring on the lateral side of the tongu of a 36-year-old female. The lesion was surgically resected with a favourable result as described in this article.
    Histopathologically, the isolated lesion consisted of laminated squamous propria and muscular tissue, the corneum of the epithelial tissue was fragible and the intercellular space markedly enlarged. Dysplasia was observed in the granular layer, spinal layer and basal layer, however, the basal layer was distinct with no evidence of cancer cell or infiltration. In the lamina propria, dilated capillary vessels and severe infiltration of small round cells mainly including plasma cells were found. The basal membrane was maintained normally and no lesion was noticed around the resected site. No recurrence tendency is now observed 3 years and 5 months after operation.

    177 patients who were suspected as Sjogren's syndrome (SS) due to having dry mouth, dry eyes and/or another autoimmune disease were investigated, in order to clarify a relationship of histopathological changes in the salivary gland to clinical and sialographical findings in SS.
    98 patients were indicat, d as SS by hitopathological examinations of the parotid and/or labial glands (Patho-positive group). Tlta salivary glands of 79 patients were not demonstrated of obvious changes histopathologically (Patho-negative group). The amounts of salivary flow from the parotid gland, stimulated by 10% citric acid, in Patho-positive and negative groups were 3. 4 ml/5 min and 3. 5 ml/5 min, respectively, and were significantly lower than that of normal healthy individuals (4. 5 m//5 min). 59% and 46% of Patho-positive and negative groups were diagnosed as havin, ; keratoconjunctivitis sicca (KCS), respectively. 53% and 39% of Pathopositive and negative groups had other autoimmune diseases. 27%, 36% and 20% of the patients in Patho-positive group were classified as Class II, Cass 111 and Class lV, respectively, acccrding to Fukuda's sialographical classification for SS. 14% of Paiho-positive group were suspected as non-specific sialoadenitis and 3 % of the patients did not show any obvious changes sialographycally. 1%, 51%, 11% and 1 % of the patients in Patho-negative group were classified as Class I, Class II, Class 111 and Class W, and 35% of the patients were suspected as non-specific sialoadenitis sialographically.

    We have experienced 3 cases of von Willebrand's disease.
    Case 1: The patient was a 3-year-old girl. We enucleated epidermoid cyst under auxotherapy with DDAVP (deamino-8-D-arginine vasopressin) and Transamin.
    Case 2: The patient was a 31-year-old man. We enucleated follicular cyst under auxotherapy with Transamin, Adona and Kativ-N.
    Case 3: The patient was a 2-year-old girl. After the operation, resection ofpanniculitis, she bled abnormally, hemostasis was obtained by local hemostatic method (electrocoagulation) and infusion of Transamin, Kativ-N. All cases were not abnormal in preoperative inquiry. We found that there were von Willebrand's disease by preoperative screening teston case 1, 2 and postoperative hemorrhage on case 3.

    Clinico-statictical study was made using ten cases ofnon-epithelialmalignant tumors (sarcomas), excepting malignant lymphoma, of the oral region in the file of the First Department of Oral and Maxillofacial Surgery, Iwate Medical University, during the past 20 years from 1967 to 1986. They were approximately 3.2% of all patients with oral malignant tumors treated in our clinic during the same period. According to histological types and locations, 6 cases occurred in the mandible, i.e., 3 osteosarcomas, a chondrosarcoma, a myeloma and an Ewing's sarcoma, and 4 cases occured in the maxilla, i.e., 2 fibrosarcomas, an Ewing's sarcoma and a malignant fibrous histiocytoma (MFH). There were 4 males and 6 females ranging from 2 to 73 years old.
    Their initial symptoms were mostly swelling or formation of tumor mass of the oral cavity, and the durations from onset of the first symptom to a visits to our clinic were 1 week to 8 months. Clinically, the boundary of the oral tumors were well circumscribed in 4 of 6 mandibular cases. On the other hand, they were ill circumscribed in all maxillary cases. The tumors' surface appeared reddening in 8 cases, ulceration in 6 ewes and both in 5 cases.
    Radiographically, destruction of the jaws infiltrated by the tumors were widely and irregularly. The periosteal reactions were observed as spicula in 3 mandibulr cases. In 8 of 9 cases in which the tumor involved denturous areas, the periodontal membrane space was widened. The histological diagnosis for oral sarcomas were difficult, in which 7 cases could not be determined histological types at first biopsy.
    Surgery with chemotherapy and/or radiotherapy in 8 of 10 cases, and chemo-and radiotherapy in 2 cases wee performed. The prognoses were poor, and only 2 patients ofa mandibulr chendrosarcoma and a maxillary fibrosarcoma were alive without tumor for 10 and 14 years. The remaining 8 patients died, 4 were local recurrence, 4 had distant metastases.

    Pleomorphic adenoma is the commonest neoplasm of the salivary gland. It occurred most frequently in the major salivary glands, especially in the parotid glands. In the minor salivary glands, it occurred most commcnly in the palate, and relatively uncommon in the upper lip. We described two cases of the pleomorphic adenoma occurred in the upper lip. The two females were referred to our ward because of tumorous lesions in their upper lips. The tumors were well-demarcated and movable, and could be easily resected under local anesthesia. No recurrence was noted. Histologically, they were pleomorphic adenomas showing epithelial predominant pattern, which is one of the four types subclassified previously.

    The authors recently performed a clinical test of Glass ceramic artificial tooth roots created and developed by Matsui, Shibuya et al., from March 1987 to June, 1989, for two years and 3 months, concerning the attachment rate in 11 male and 13 female patients, totally 63 dental roots, over 5 month after implantation.
    The bio-live attachment rate for up to 3 months after operation, before mounting of upper structure, was 90.4% and spontaneous fall-out was observed at 6 dental roots in 3 cases with continued unpleasant feeling and slight pain probably due to inflammation after implantation.
    On the other hand, the functioning rate after mounting of upper structure was 94.7%, with fallout probably due to trauma observed at 3 dental roots in one case.
    Principally centering on glass ceramic implanting operation mode and results, the outline has been given as above.
    Stable and satisfactory implanted condition of glass ceramic tooth root in the jaw bone was confirmed by this clinical test and the possibility of clinical application is considered to be quite high.

    The predilection places of adenoid cystic carcinoma are the submandibular gland of the major salivary glands and the palatine gland of the minor salivary glands.
    In this paper 2 cases of adenoid cystic carcinoma of the maxilla are presented. The sites of the tumors were the anterior alveolar region and the relatively anterior region of the hard palate, and these sites wee considered to be fairly rare.

    In floor of the mouth appears mucous cyst, ranula, dermoid-epidermoid cyst, thyroglossal duct cyst, branchial cyst. If it occurs in the sublingual region, we would have some difficulties in diagnosis.
    This paper reports a case of congenital sublingual and submental type dermoid cysts accompanied by a thyroglossal duct cyst.
    The patient, a 2 month old baby, was referred to our hospital with complaints of sublingual mass and submental fistula. The operation was performed when he was 6 months, so he didn't have sucking difficulty.The mouth sublingal mass was extirpated and we found and also extirpates a new mass beneath it. The submental fistula was extirpated from outside the mouth.These 3 masses were independent of each other with no evidence of adhesion together.Histopathologically, the sublingual mass and the submental fistula were dermoid cysts and the mass beneath the sublingual mass was a thyroglossal duct cyst.

    Case of aneurysmal bone cyst occuring in the left mandible of a 33-year-old female was reported. Although the lesion was incidentally found by a routine orthopantomography ten years ago, it gradually increased because it was not treated.
    The patient was referred to our clinic on July 13, 1988, complaining of swelling of the left mandible. On oral examination, bone expansion with partial bone resorption covered with normal mucosa was observed in the left lower canine to the second molar region. Roentogenographic examination revealed a multilocular radiolucent area showing scalloping without resorption or divergence of tooth roots. A puncture of the cystic lesion demonstrated the aspiration of arterial blood-like fluid. Angiographic finding showed a pooling of blood, coincident with radiolucent area, supplied from the inferior alveolar and facial arteries as main feeding vessels.
    After ligation of the left external carotid artery to avoid excessive bleeding, she was treated by segmental mandibulectomy reconstructed with iliac bone transplantation and titan reconstruction plate. Histologically, cavernous blood-filled cavities without endothelial lining and fibrous connective tissue containing young fibroblasts and many multinuclear giant cells were observed in the bone marrow spaces.
    At six months postoperation, no signs of recurrance and good fixation of the transplanted bone were seen.

    The rectus abdominis myocutaneous flap based on the inferior epigastric vessels was applied for reconstruction after oral cancer ablation. In all 5 cases, the flaps were successfully transplanted without any necrosis. This flap showed advantages compared to pectoralis major myocutaneous flap, deltopectoral flap and others. The advantages include: 1) The large skin paddle could be taken with good blood supply. 2) The inferior epigastric vessels were anatomically constant with adequate length and size for anastomoses as the nutrient vessel.3) The muscle volume could be adjusted. 4) All these procedures, such as tumor ablation, elevation of flap, reconstruction etc. could be simultaneously performed in a supine position.
    We briefly reported the technique of elevating rectus abdominis myocutaneous flap, clinical course in five cases of the rectus abdominis myocutaneous reconstruction and advantages and disadvantages of this flap.
    Our experience suggested that the rectus abdominis. myocutaneous flap was one of the most suitable flaps for recostruction after ablation of oral cancer.

    162 patients with TMJ arthrosis, who visited our clinic during two years from Dec. 1985 to Nov. 1987, were evaluated clinically according to a new classification proposed by Japanese Society of Temporomandibular Joint.
    The patients consisted of 44 males and 118 females (sex ratio: 1: 2.68), mean age was 35.0 years with 8 to 70year range.
    1) The type distribution in our series was as follows; Type III was most commonly encountered (45.1%) followed by Type II (29.6%), Type IV (12.3%), Type I (10.5%), and Type V (0.6%). The majority of Type I, IV were females.
    2) The mean age was higher in Type IV (55.0 years) and Type I (47.2 years) than Type III (28.7 years) and Type II (34.7 years).
    3) The mean period before visiting our clinic was significantly shorter in Type I (6.8 months) than Type II (19.6 months), Type III (25.1 months) and Type IV (27.8 months).
    4) The functional disturbance measured by dysfunction index was more severe in Type IV than other Type (I-III).
    5) Effectiveness of standardized conservative treatment was reduced in the following order; Type I to Type IV.
    6) In regard to complex type, it needs further study to determine its diagnostic criteria and application.

    The obwegeser II surgical procedure is usually indicated in the treatment of mandibular prognathism when the amount of mandibular set back is 15 mm or more. Two patients with severe mandibular protrusion healed with this procedure are shown here.
    Case 1: The mandible was set back 14 mm in the left side using the Obwegeser-Dal Pont method. The right side was set back 18 mm using the Obwegeser II method.
    In order to prevent post surgical relapse, Egyedi-Obwegeser glossectomy was performed using CO2 laser.
    Case 2: The mandible was set back 13 mm in the left side and 17 mm in the right side by a method similar to that of case 1.
    Both cases were treated successfully. The combined Obwegeser-Dal Pont and Obwegeser 11 method seems to be effective for achieving ideal aesthetic and functional results.

    In 2 female patients with tongue and mouth floor defects, a device was made on the incision line when the pectoralis major myocutaneous flap was prepared. In the first patient, a relatively favorable result was obtained in the follow-up findings 3 years later, as reported below.
    The radical neck dissection was used MacFee and the lower line of incision was utilized. Another line of incision was made from the axillary region along the lateral edge of the mamma to prepare an insular skin flap.The muscle pedicle portion was prepared beneath the skin tunnel.The donor defect was closed, leaving a deformity of the mamma. Absence of incision in the upper part of the anterior chest prevented scar formation. Relatively favorable postoperative state was achieved.

    Clinical study of Sultamicillin (SBTPC, Unasyn®) in infections in the field of oral surgery was carried out.Sultamicillin was orally given in a daily dose of 1, 125 mg (375 mg × 3 times) to 70 patients with two type of infections (41 ostitis of the jaws, 29 pericoronitis).
    The following results were obtained.The clinical effects obtained by subjective judgement were excellent in 34 cases, good in 29, fair in 2 and poor in 2.The clinical effects obtained by scoring judgement were excellent in 31 cases, good in 35, poor in 4.
    As for side effects, mild diarrhea in 5 cases was observed.
    In laboratory examinations, a rise of BUN was observed in 2 cases.
