It has been considered to be difficult to calculate the energy quantity, which is transfered to photons composing materials from incident photons, by means of present absorption coefficients. The ratio of transfered energy to photons composing water layer from incident photons (effective energy absorption ratio) was calculated theoretically. Consequently, the ratio of total exit energy to total incident energy as a function of thickness of layer was represented a sigmoid curve on semilogarithmic scale, and was saturated to the value of energy emerged from the layer by back scattered photons. It can be able to calculate the effective energy absorption ratio for given photon energy and for given thickness of layer by following equation, which is a function of photon energy and "effective energy absorption ratio coefficients" (k, n), obtained from the graphs. (effective energy absorption ratio) =1-k×E^n (E : primary photon energy [keV])

The authors described a new method of frontal section cerebral angioautotomography using a specially devised autotomographic head rest and reported the clinical results of this method in the analysis of intracranial aneurysms. The frontal section cerebral angiography was very useful for anterior communicating artery, middle cerebral artery and basilar artery aneurysms in the clear demonstration of not only the whole shapes but also the stalks of aneurysms with their relationship to the parent arteries.

In the diagnostic x-ray examination, it is very important to reduce exposure dose "as low as posible" by meanse using high sensitivity photographic system, for example. However, even employing a photographic system "ideal" in geometry and sensitivity, very low dose makes the image quality inferior because of fluctuation of x-ray quanta. The lower limit of exposure dose in x-ray examination is determined by this reason. Under the condition wherein a simple figure is superimposed on uniform noises, a formula was led mathematically showing a limit to recognize this figure. Then, the validity of the above formula was endorsed experimentally by a simulation system using a gamma-camera.

In October 1975,our hospital was installed with the first home made head unit (HITACHI CT-250 H). From the initial period, we have inspected the number of CT scanner as a part of maintenance twice every week. In this study, we have investigated a drift in the CT number of water and plaxiglas during the period between Sept.1976 and Oct.1977. The results showed that the average CT value of water was 0.55+1.47,and the average CT value of plaxiglas was 57.2+2.95. The fluctuations of the CT values in the present study were less than that of the standard CT value and, therefore, the periodical checking of CT number was useful in maintaining the CT scanner in good condition.

External counting was performed using a scinti camera on postmortem specimens (liver and spleen) of a case underwent an angiography with "Thorotrast" in 1939 and another living case. Spectrum analysis was also carried out on the two cases with a pulse hight analyzer of renogram apparatus. Results obtained were as follows : 1. Radioactivity in the liver and spleen specimens could be detected throgh a Mix-Dp phantom of 10 cm in depth by a scinti camera. 2. Radioactivity on the splenic area of a living Thorotrast case could be detected with external counting. 3. The gamma-ray spectrum from the specimens and living case was analyzed with a pulse hight analyzer of renogram apparatus. In conclusion, a external counting and spectrum analysis with a pulse height analyzer would be valuable in detecting thorium deposition in the body.

So called first generation CT scanner (EMI scanner CT-1000 MK-1) was instituted at this hospital in 1975. We considered about following items with our experiences (for 2 years and 8 months, 6500 cases), at present. 1 Positioning Limitation by construction and efficiency of the machine. Setting up the base line. Setting up the slice levels. Fixation of patients. 2 Relation between the patients irradiation dose and image Measurment of scattered X-ray dose. Irradiation dose at slice levels. Trial to decrease X-ray dose. 3 Maintenance 4 CT examination in infants and children Device in attachments 5 Operation of CT system Characteristics of the examination and necessity of doctor's attendance. Others.

Since computed tomography (CT) was first reported by J.Ambrose and G.Hounsfield in 1972,CT scanner has been popularizing in Japan and has found a wide spread use in clinical practice. The technical problems of CT scanner has been studied by many reseachers. The most important point, however, is the patient positioning and evaluation for CT scanners. So, we will choose section of the problems to which the attention should be drawn and publish them in this journal.

In evaluating the performance of the CT scanner it is necessary to take into account the fact that this apparatus is far more complicated in structure than the traditional x-ray apparatuses, consisting of (1) an x-ray system, (2) a detector system, (3) a reconstruction system, (4) a biological system to be scanned and (5) a recording system. Based on more than 2 years' experience with whole body scanners of the 1 st and 2 nd generation, we referred to some problems regarding the characteristics of this device, notably those which have clinical implications, e.g. spacial resolving power, linearity, slice thickness, artifact and radiation dose. Mention also was made of problems concerning the system of CT-image presentation and the usefulness of the multiformat system was stressed. The present-day whole body CT scanner, though leaving much to be desired with respect to accuracy, can play a great role in the diagnosis of lesions in the thoracic, hepatic, pancreatic and renal regions when used by those well aware of the limitations of its performance. The technological progress of this apparatus is now developing and the day will not be far distant when it, with a speed-up of procedure and high resolving power, will permit visualization of the interior of the myocardium and heart, thus helping to improve the qualitative diagnosis of lesions. We must endeavor to grasp and overcome every difficulty arising from technical innovation thereby progressing day by day.

The general problems of a Computed Tomography (CT) could be classified : one on systemic capability and the other on imaging technique. The date collection pattern of CT is fundamentally comparable to that of the ordinary X-ray radiography. The difference only lies in the imaging method. A study was made about the items as described from this point of view. A.Evaluation of Capability of CT scanner. 1. Linearity (the EMI values of a series of ionine contrast media solution). 2. Uniformity of CT image (not connection object size). 3. Slice thickness by original phantom. 4. Low density resolution by original phantom and its deterioration through aging of X-ray source. 5. High density resolution due to point spread function (1mmφ) 6. Exposure dose. B.User's technique of imaging task. 1. Effectiveness in using original sugar bag. 2. Countermove of motion artifact (development of display timer, etc.) 3. Necessity of warming-up photo-multiplier. 4. Viewing.

X-ray out put discrepancy is a singificant matter in diagnostic X-ray technology. The out put measurement, however, has very difficult factors in radiographic procedures, because of relatively low tube voltage, high tube current and short exposure time less than 0.1 seconds. The problems are X-ray tube voltage discrepancies, tube current errors, exposure timer errors and dosimeter response in diagnostic X-ray study. The X-ray out put study group of the Radiographic Technology Committee of the JSRT selected articles which X-ray out puts were studied, and compared the out put data with each other. The agreement range at 80 kV, and more than 2 mm of total Al filtrations is between +6 and -22%. This may be considered that the range is narrow unexpectedly. Further investigations on this problem are necessary to the technology.
