  • ON THE NEW GUNMA-METHOD

    Fluoroscopic examination has been widely adopted as the primary screening in gastric-mass-survey. More subjects are to be examined and early discovery of cancer and its immediate medical treatments are necessary in order to decrease the death rate of gastric cancer. Under the present circumstances, however, fluoroscopic examination can hardly meet the need for all the cancer ages.
    Since 1962, we had adopted the Gunma-Method as the primary screening in which we picked up those who proved positve-over Type V by Shichijo's reaction (a non-specific, biological test of gastric caner), those who complained of their gastric troubles, and those who had hereditary character of the digestive tracts.
    This method was very efficient, but it was suspected that this method might fail to detect some gastric cancer. Therefore we have adopted fluoroscopic examination accompained with this method as the primary screening since 1964. Thus we have made a comparative study of the two methods.
    We examined 39, 049 cases during 2.8 years from 1964 to 1967
    Suppose we made the primary screening of those cases by the Gunma-Method, we would be able to omit 14, 459 cases (37.0%). The fact was that 22 cancer patients were found among those omitted cases. This figure occupied 23.4% of 94 cancer patients who were detected in the primary screening when fluoroscopic examination was used jointly with Shichijo's reaction. when we shifted the screening line from TypeV to Type III and also picked up even those who complained of their gastric troubles “once in a while” at the time of the primary screening, 12, 407 cases (32.2%) were to be omitted. In this case only 8 cancer patients (8.5%) were found among those omitted cases. In this way, the failing number was to be much decreased.
    We call this method “new Gunma method” and we believe this new method will be effectively and conveniently used as the primary screening of gastric-mass-survey

  • I. ON SOME TECHNICAL PROBLEMS

    As a functional test of blood circulation, either electrocardiography or phonocardiography are essential in daily clinic. On the other hand, a plethysmography is not carried out so frequenfly, because either techniques or interpretation of results in the plethysmography differ with the examiner. In a series of the present experiment, it was planned to standardize the plethysmography as a method for examining a function of peripheral blood circulation. In the first section, experiments were performed to standardize the technique in the plethysmography.
    1. In a series of the experiment, a reflexion photoelectric plethysmograph was used because it was easy to handle comparing with other methods of the plethysmograph.
    2. A wave form of a plethysmogram was modified its shape due to a low frequency cut of the plethysmograph. If a time constant of the apparatus is over 2sec, however, the change was insignificant.
    3. An amplitude as well as a wave form of the plethysmogram was markedly changed due to compression a skin surface beneath the plethysmograph. It was discussed, therefore, that the plethysmograph must be attached to the skin with the pressure by which the plethysmogram showed its maximum amplitude.

  • II. SELECTION OF EXAMINATION ITEMS AND RESULTS OF EACH ITEM IN NORMAL SUBJECTS

    In this section, experiments were performed in normal subjects in order to study whether several items of plethysmographic examination are useful or not for test a peripheral biood circulation, and gain control data in the normal subject.
    1. There were spontaneous variations in an amplitude as well as a base line of the plethysmogram at rest. In a grade of the spontaneous variations daily and individual differences were recognized.
    2. A crest time, a basic oscillation time, an amplitude of initial wave and its half decay time were measured on each wave of the plethysmogram. Since individual differences in the crest time and the half decay time were little, a comparison of these two element with a pathological wave form are regarded to be significant.
    3. A propagation time of pulse wave was represented by a time from the Q wave of electrocardiogram to a rising point of the plethysmogram. It was 0.244sec (0.219-0.272sec) for the third finger and 0.313sec (0.301-0.324sec) for the big toe.
    4. When an extremity under test was shifted passively from a heart level, there were specific patterns in variations of a base line and an amplitude of the plethysmogram for each position (elevated or lowered)
    5. Whcn a hand clap or a mental arithmetic were applied to the subject, a base line of the plethysmogram moved to a direction of vasoconstriction.
    From these results, following items were selected as standard items of the plethysmographic examination. (1) Spontaneous variations in an amplitude and a base line of the plethysmogram. (2) A wave form of the plethysmogram. (3) A propagation time of pulse wave. (4) Changes in a base line and an amplitude of the plethysmogram when an extremity under test was raised or lowered from a heart position. (5) Vasomotor reflex.

  • III.APPLICATION OF STANDARD PLETHYSMOGRAPHY TO PERIPHERAL VASCULAR DISEASES

    In this section, the plethysmography standardized in a series of the present experiment was applied to patients who suffered from peripheral vascular diseases.
    1. Spontaneous variations in an amplitude and a base line of the plethysmogram were very poor in a Buerger's disease differently from that in a normal subject.
    2. As to a wave form of the plethysmogram, a crest time lengthened, a half decay time shortened, and the wave form became monophasic due to disappearance of a dicrotic notch.
    3. A propagation time of pulse wave became shorter than the normal control value; 0 218sec (0.183-0.253sec) for a finger, 0.308sec (0.197-0.362sec) for a toe.
    4. There were marked changes in patterns of variations in a base line and an amplitude of the plethysmogram when an extremity under test was raised or lowered from a heart position.
    5. In a vasomotor reflex by sensory stimulation, a base line of the plethysmogram shifted to a direction of vasodilation in contrast to that in a normal subject.

  • REPORT 11. RELATION BETWEEN INFILTRATING CELLS AND CIRCULATING ANTIBODY TITER IN EXPERIMENTAL ALLERGIC CONJUNCTIVITIS

    Allergic coujunctivitis was experimentally induced in male albino guinea pigs by applying to the eye pollen of Pinus Thumbergii as sensitizing antigen, with the following results :
    1) At 50-70 days of observation, chief infiltrating cells were neutrophils, lymphocytes and plasmocytes. Cellular infiltration was most intense in a group which received first the sensitizing and later the provoking application, followed by a group which received the two applications simutaneously, and a group which received only one application.
    2) Examination of the times of two succeeding applications revealed that the appearance of neutrophils, lymphocytes and plasmocytes was independent from the time of the first and later application.
    3) The descending order of intensity of cellular infiltration and epithelial proliferation in various parts of the conjunctiva was limbus corneae, conjunctiva tarsi, conjunctiva fornici, and conjunctiva bulbi. This is the same as in allergic conjunctivitis such as phlyctaen and conjunctiviuis vernalis.

  • SOME ASPECTS OF THE RESULT IN COMPARISON WITH THE OTHER PSYCHOSOMATIC DISEASES

    I have already reported the study on Rorschach test in bronchial asthmatic patients in connection with other medical factors : causative factors, the degree of severity and the duration of illness
    The present paper described some comparative results between asthma and some other so-called psychosomatic diseases : hyperthyroidism and lung tuberculosis.
    The following results were obtained :
    1. In comparison the asthmatic patients with the hyperthyroid patients, the former showed more delayed Reaction Time and much Arbitrary Color responses that the latter. On the other hand, the latter showed the higher scores in Rejection, FC<CF+C and At% than the former These findings suggest that the asthmatic patient seeme to be more repressive and seclusive, while the hyperthyroid patient looks more ego-centric, impulsive and hypochondriac.
    2. In comparison the asthmatic patients with the tuberculosis patients, the former showed the lower Form Level and the higher scores in M>∑C and (H) + (A) than the latter. Namely, the asthmatic patients looked more introversy and seclusive, while the tuberculosis patient looked more adjustable.
    3. In all groups, the following tendencies were observed : R<20, W>D Rejection ≥1, Color Shock +, M<3, >∑C<3, A%≥50 and Δvalue=-2. 5 (Deviant Verbalization) in many. These results indicate that these all subjects are repressive, obsessive and neurotic.
