    Tissue levels of N1-acetylspermidine, polyamines and CEA were measured in colorectal polyps, which had been either endoscopically polypectomized or surgically removed. The polyps examined were either adenomas with mild, moderate or severe dysplasia, or adenocarcinomas. Control mucosae were taken from macroscopically and microscopically normal regions of surgicallyremoved colorectal tissues.
    The colorectal polyps contained approximately 13-fold higher levels of CEA than control mucosae. The CEA levels of adenocarcinomas were twice as high as those of adenomas, though the difference was not statistically significant. The N1-acetylspermidine levels were significantly higher in severe dysplasia and adenocarcinomas than in control mucosae, and in adenomas with mild or moderate dysplasia. There was a significant positive correlation between the levels of N1-acetylspermidine and the size of colorectal polyps, whereas no significant correlation was found between the levels of N1-acetylspermidine and CEA. The results of the present study suggest that N1-acetylspermidine could be used as a more specific marker of colorectal adenocarcinoma than CEA.

    Since the advent of CT scan, the investigation of intracranial lesions has been easy and accurate. With the improvement of diagnostic ability, subdural fluid collection (SFC) has also been detected frequently after head injuries and neurosurgical operations.
    The clinical symptoms caused by SFC are mild in most patients and most cases of SFC disappear spontaneously as shown by repeated CT scan. Therefore the indications for surgical treatment of SFC should be limited.
    CT scan has newly revealed that some cases of SFC develop to chronic subdural hematoma (CSH). The clinical significance of SFC is that it can be a precursor of CSH. The factors related to development of CSH from SFC are not sufficiently known. Further studies are thus needed to clarify these factors and moreover to prevent progression to CSH from SFC.

    Prostatic cancer is becoming one of the most common diseases in Japan as the population ages, diagnostic techniques are improved, and the prevalence of the disease increases. We analyzed 221 patients with prostatic cancer found between 1985 and 1986. The following results were obtained :
    Most cases were in the advanced stage, which related to the degree of the differentiation of prostatic cancer cells. Metastasis was frequently observed in bone and was strongly related to serum prostatic acid phosphatase and serum alkaline phosphatase values.

    A needle-type polydepsipeptide ([AlaAlaGlu (OEt) Lac]) formulation contaning cis-platinum was implanted into the nomal kidney of a male Wistar rat. The kidney tissue surrounding the formulation could be divided into three layers : necrotic, necrobiotic, and normal. The maximum thickness of necrotic tissue was observed on the seventh day after implantation and was found to be 2800μm. Cis-platinum released from the formulation was retained in the surrounding tissue and was visualized by soft X-ray. A polydepsipeptide polymer formulation without cis-platinum implanted into the kidney was degraded completely within 7 weeks. However, the biodegradability of the polymer formulation containing cisplatinum was markedly suppressed because of the necrotic tissue barrier. It was 37% after 7 weeks of implantation. Electron microscopically, small holes were found in the formulation after drug release. The number of such small holes increased with time. These findings indicated that the release of cisplatinum proceeds from the surface to the center of the needle-type formulation.

    Three hundred and eighty-five strains of Pseudomonas aeruginosa were isolated from patients in Gunma University Hospital from 1984 to 1987. Susceptibilities to eight kinds of antibiotics (PIPC, CFS, CAZ, IPM, AZT, GM, AMK, NFLX) were investigated and following results were obtained.
    1) Ninety of 385 strains (23%) were resistant to some kind of antibiotic.
    2) The isolation frequency of resistant strains increased year by year for most antibiotics, especially NFLX and PIPC. Strains with multiple resistance increased in 1986 and 1987.
    3) Minimum inhibitory concentration curves indicated that, CFS, CAZ, IPM, AZT, and AMK were still effective for Psuedomonas infections.
    4) The isolation frequency of resistant strains to each drug varied from source to source. For example, GM-resistant strains were isolated more frequently from urine (33%) than from the airway (3%).

    Seventeen cases of asplenia syndrome in Gunma children's medical center are reported. All of these patients had a cardiac anomaly such as common atrium, single ventricle, pulmonary stenosis, pulmonary atresia, total anomalous pulmonary venous return (TAPVR), common atrioventricular canal, and bilateral superior vena cava. Many patients had cyanosis and congestive heart failure.
    Prostaglandin E1 was administrated to patients with severe cyanosis due to pulmonary atresia or severe pulmonary stenosis. The hypoxic state of these patients improved, and no patient died in the neonatal period. Five patients were operated on by the Blalock-Taussig shunt procedure, and in all of them the hypoxia improved.
    Twelve patients died in infancy or childhood (3 mo-11 y 1 mo). all 10 patients who had TAPVR died of congestive heart failure and infection, and the other 2 died of infection.
    Only five of the patients are still alive (2 y 8 mo-5 y 9 mo), but if the operation for TAPVR is indicated and successful, the prognosis of asplenia syndrome seems to be improved.

    A 74-year-old man was admitted to our hospital for a medical examination in May 1986. There was no abnormality on physical examination. He was diagnosed as having primary macroglobulinemia because of lymphocytosis in the blood, a monoclonal increase in serum IgM-κ, and an increased number of lymphocytes and lymphocytoid cells in the bone marrow. In addition, early gastric cancer (II a, 4.5 mm in diameter) was found on endoscopy. Histologically, it was adenocarcinoma. While the immunoregulatory system is usually disturbed in macroglobulinemia, the results of blood lymphocyte subpopulations, their response to phytohemagglutinin, and the serum concentrations of immunoglobulins were all normal in this patient. However, it is undeniable that the presence of macroglobulinemia may have induced the development of gastric cancer.

    A 33-year-old farmer who noticed a hard mass in his scrotum was admitted to our hospital. The physical examination was essentially normal except for the Y-shaped scrotal mass. He denied injection, and had no previous injury of the scrotum. Since the rapid growth of the mass suggested a malignant disease, the patient underwent en bloc excision of the mass. The cut surface of the removed mass was grayish white. Histological examination revealed lipodegeneration with phagocytosis and a high degree of fibrosis. There were many giant cells and eosinocytes. Histopathological diagnosis was the latter stages of sclerosing lipogranuloma. In the Japanese literature, a characteristic feature of sclerosing lipogranuloma of the scrotum is the Y-shape mass (79%).
