    Endoscopic examination was performed on 306 pediatric cases suspected of having diseases of the upper digestive tract. As a result, abnormalities were found in a total of 138 cases (45. 1%), including gastric redness (36 cases), erosion (19 cases), hemorrhage (12 cases), ulcer (16 cases), duodenal erosion and/or hemorrhage (9 cases), ulcer (38 cases), and esophageal lesion (8 cases). Gastric erosion was seen in older children (over 9 years of age) with chronic abdominal symptoms such as abdominal pain and vomiting, hemorrhage in the gastric body was noted in cases with hematemesis, and hemorrhage in the gastric antrum was seen in older children with abdominal pain : Gastric ulcer was noted in cases over 8 years of age. In older children, a duodenal ulcer similar to that seen in adult cases was observed, and in younger children it characteristically developed with massive bleeding.
    The present investigations have demonstrated that diagnostic values for symptoms and signs differ between older and younger children. But vomiting, hematemesis-bloody stool, anemia in all children, worsening of abdominal pain at night, family history of peptic ulcer and history of taking drugs that could cause lesions of the digestive tract in older children are important indicators suggestive of the presence of gastroduodenal lesions.

  • RUNNING ACTIVITY AND DRINKING BEHAVIOR IN MICE

    Acute and subacute effects of methamphetamine (MAP) on wheel-running and drinking activities in mice were investigated by means of long term observation. Each mouse was placed in a breeding cage equipped with a wheel and drinkometer, and then spontaneous and progressive increase in wheel-running activities (WRA) was observed without drug administration for 2-3 weeks. After a stable base-line of WRA had been established, the effects of MAP (1, 2 and 4 mg/kg) were investigated. A dose-dependent increase in WRA for 2-4 hours was observed when administered during a light period (11 : 00). The WRA-increasing effect was slightly sensitized by repeated administration, 5 times at an interval of 3-4 days. However, the WRA and drinking activities during the dark period were significantly suppressed more than 7-2 hours after MAP administration (rebound phenomenon). This phenomenon was affected by many factors ; i. e. the dose administered as well as the time and intervals of administration. The suppression could not have been caused by physical fatigue due to the acute WRA-increasing effect of MAP, because the same phenomenon was observed after a WRA-confined experiment. In addition to these findings, WRA and drinking activity during the light period increased even on days when MAP was not administered, which was considered to reflect a conditioning phenomenon. These effects of MAP became indistinct when the drug was administered during the dark period (18 : 00).

    One hundred and eight adult patients over 30 years of age who underwent atrial septal defect (ASD) closure were studied for the time of surgical intervention through atrial fibrillation (Af), cardio-thoracic rate (CTR) and clinical symptoms. After leaving the hospital, Af changed to sinus rhythm in four patients, and new Af appeared in four, in whom preoperative left atrial size (mean : 44mm) and CTR (mean : 59%) were significantly larger than those of patients without Af. Subjective symptoms such as numbness of the extremities, arrhythmias and the necessity of medication were more frequent in Af patients than in sinus rhythm patients. Eighty-four % of sinus rhythm patients were in class I according to the NYHA functional classification, while 48% of Af patients remained in class II, post-operatively. Surgical intervention in adult ASD patients should be considered before the appearance of left atrial dilatation.

    Cytomegalovirus (CMV) infection, especially CMV pneumonia, is a serious event in immunocompromised individuals such as recipients of bone marrow transplantation. Since there is no effective treatment, prevention is the most effective way to control CMV infection. Blood transfusion is one potentially dangerous source of the virus. Screening for CMV negative donors is mandatory in order to provide blood components for these patients. We have screened 1, 827 donors for CMV antibody by using the gelatin particle agglutination test and ELISA. As it has been shown that the positivity rate is closely related to donor age, we tested mainly younger individuals. The results showed that the CMV antibody negative rate was 18.3%.

    A leukemic cell line, IEZ-3, was successfully established from a ly8m old girl with a diagnosis of acute megakaryocytic leukemia. These cells were negative to peroxidase reaction, PAS stain, and esterase stains, just as the original leukemia cells were. Immunological study using monoclonal antibodies was positive to GP IIb/IIIa, suggestive of megakaryocytic origin. These findings show that IEZ-3 is a unique cell line established from a de novo leukemia, expressing megakaryocytic features.

    We performed cineventriculography to evaluate the regional left ventricular wall motion in 7 cases of endocardial fibroelastosis (EFE). For controls, we also evaluated that of 7 cases with other endomyocardial diseases and 10 cases of small shunt VSD.
    In EFE, left ventricular end diastolic volumes increased and global ejection fractions decreased significantly, but cardiac indexes were preserved. There were global reductions in left ventricular wall motion especially in posterobasal, inferior, septal and posterolateral regions, but the wall motion around the anterolateral region was relatively preserved in surviving cases. Thus, reduction of anterolateral wall motion is predictive of poor outcome.

    A 67-year-old female patient with idiopathic thrombocytopenic purpura (ITP) who had responded to neither prednisolone and azathioprine underwent a successful splenectomy. Since it has been reported that the effective rate for splenectomy is lower in elderly patients, means of predicting its usefulness should be established.

    A case of breast cancer with cardiac tamponade due to diffuse metastasis to the pericardium confirmed by pericardiography is reported. A 51-year-old woman who had undergone radical mastectomy 6 years previously presented complaining of easy fatigability, shortness-of-breath and palpitation. Chest X-p showed pleural effusion and enlargement of CTR, and a diagnosis of pleural and pericardial carcinomatosis was made. Pericardial drainage was performed immediately because of cardiac tamponade with a central venous pressure of 22cm H2O. Pericardiography with contrast medium showed diffuse metastatic nodules of the pericardium which were confirmed malignancy histologically.
    Pericardial carcinomatosis is a fatal disease, therefore pericardial fluid must be definitively identified by ultrasonography in breast cancer patients. The extent of pericardial metastasis can be confirmed by the pericardiography.
