Endoscopic examination was performed on 306 pediatric cases suspected of having diseases of the upper digestive tract. As a result, abnormalities were found in a total of 138 cases (45. 1%), including gastric redness (36 cases), erosion (19 cases), hemorrhage (12 cases), ulcer (16 cases), duodenal erosion and/or hemorrhage (9 cases), ulcer (38 cases), and esophageal lesion (8 cases). Gastric erosion was seen in older children (over 9 years of age) with chronic abdominal symptoms such as abdominal pain and vomiting, hemorrhage in the gastric body was noted in cases with hematemesis, and hemorrhage in the gastric antrum was seen in older children with abdominal pain : Gastric ulcer was noted in cases over 8 years of age. In older children, a duodenal ulcer similar to that seen in adult cases was observed, and in younger children it characteristically developed with massive bleeding.
The present investigations have demonstrated that diagnostic values for symptoms and signs differ between older and younger children. But vomiting, hematemesis-bloody stool, anemia in all children, worsening of abdominal pain at night, family history of peptic ulcer and history of taking drugs that could cause lesions of the digestive tract in older children are important indicators suggestive of the presence of gastroduodenal lesions.
