    I examined the effects of volatile anesthetics and ketamine on the glycine receptor channel expressed in Xenopus oocytes injected with single RNA encoding the α1 subunit of the glycine receptor. The glycine-induced chloride currents in these oocytes were enhanced by 4 volatile anesthetics (halothane, enflurane, isoflurane, and sevoflurane) at clinically relevant concentrations, in a reversible manner. The findings about the effect of halothane on the dose-response curve of the glycine-induced currents suggest that the sensitivity of the recombinant glycine receptor channel was increased by halothane. In contrast, ketamine, an intravenous anesthetic, depressed the glycine-induced currents. These results indicate that both the volatile anesthetics and ketamine modulate the functions of the glycine receptor channel by affecting the αsubunit. Additionally, we examined the effects of these anesthetics on membrane currents in Xenopus oocytes injected with mRNA extracted from the rat brain and spinal cord. Glycine-induced chloride currents in these oocytes were also enhanced by halothane but depressed by ketamine. GABA-induced currents were enhanced both by halothane and by ketamine. Kainate-induced currents were depressed both by halothane and by ketamine. The relevance of the present findings to the mode of action of these anesthetics was discussed.

    Five patients with chronic non-bacterial prostatitis, which was resistant to conventional treatments, underwent transurethral balloon laser hyperthermia (TUBAL-H), and were followed for more than one year. In regard to the chief complaints, the symptoms were resolved in 1 patient and improved in 4 at three months after treatment, and improved in 2 and persisted in 1 at one year after treatment. According to I-PSS scores, the symptoms were improved in 4 patients and persisted in 1 at three months, and improved in 2 and persisted in 1 at one year. White blood cells in expressed prostatic secretions were absent in 4 patients and decreased in 1 at three months, and absent in all cases between six months and one year. TUBAL-H was thought to be useful for treating chronic non-bacterial prostatitis.

    We reviewed the operative indications and the results of the extraanatomical bypass. Axillofemoral bypass grafting was performed in 38 legs of 26 patients and femorofemoral bypass grafting in 8 patients. The mortality rate was 88% in the femorofemoral bypass group but there were no deaths in the axillofemoral bypass group. The graft patency rate for axillofemoral bypass was 85%, which was better than the 50% for femorofemoral bypass. Early occlusion of femorofemoral bypass grafts was probably due to the quality of the donor arteries and the run-off of recipient arteries. Extraanatomical bypass grafting for a risky case is useful. Careful preoperative evaluation of the patient's general and local status and postoperative prevention of complications are indispensable.

    For 7 cases of bladder stones, we carried out transurethral lithotripsy by using an ultrasonic lithotripter (UL), an electrohydraulic lithotripter (EHL) or a ballistic lithotripter (BL), and evaluated the safety, usefulness and therapeutic indication for each lithotripter.
    With the UL, though the power was weakest, there was little stone movement at irradiation and the view was good owing to continuous irrigation, and there was no mucosal injury due to misdirected irradiation. With the EHL, though the power was strongest, stone movement at irradiation was greatest, and mucosal injury occurred due to misdirected irradiation, and the probe was exhausted rapidly. BL was similar to EHL, but BL is more useful than EHL when safety and running cost are considered.
    From our consideration, small stones less than 2 cm can be treated by UL only. In case of larger stones, we recommend that the stone be divided into small pieces by BL at first and that the small fragments be disintegrated by UL.

    Retroperitoneal lymphnode dissection after chemotherapy is a definite strategy for treating progressive testicular cancer. To access the surgical approach to the retrocrual space, we performed a dissection. The left diaphragm crus was adjacent to the left adrenal glad, the right one to the inferior vena cava. The ventral site of the aortic hiatus was not fully visible because of superior mesenteric and celiac arteries and surrounding connective tissue. Left side approach is the only route to retrocrual space, when the routine procedure for the approach to retroperitoneal space is used. To resect a metastatic mass in retrocrual space completely, other approaches, such as the thoracic approach, procedures for total gastrectomy, are necessary.

    In accordance with the wide use of stapling devices, low anterior resection for rectal cancer has been the standard operation except for a cancer that is located in extremely low in the rectum or extend to the anal verge. Low anterior resection is an excellent procedures in that the anal function is preserved completely and the technique is simple. But at the same time, if the ampulla of the rectum is resected, stool frequency occurs and impossible voluntary evacuation by abdominal pressure.
    In order to overcome these demerit, we make a pouch with the residual sigmoid colon by using linear stapling devices. The colon pouch plays the role of an artificial ampulla, and the patients who received these operation are able to have a voluntary stool evacuation by abdominal compression. And at the same time, the artificial ampulla can retain the stool, which decreases stool frequency. From the standpoint of quality of life, colon pouch formation is a useful surgical procedure for lower rectal cancer.

    Skull base metastasis of hepatocellular carcinoma is relatively rare, and especially rare when the chief complaint is involuntary movement of the eyeball. We reported a 60-year-old man whose first symptom was involuntary movement of the left eyeball. A tumor, 4 × 3 × 2 cm3, in the skull base was detected by magnetic resonance imaging and a partial resection was performed. The histological diagnosis was consistent with hepatocellular carcinoma. Tumors were detected in the liver by computed tomography and gallium scintigraphy. Therefore it was diagnosed as bone metastasis of hepatocellular carcinoma. Radiotherapy was directed to the skull base, liver and the other sites of bone metastasis. The symptoms were improved.

    A 62-year-old man was referred to a hospital because of back pain, lower limb palsy, sensory disturbance and recto-urinary dysfunction. Magnetic resonance imaging (MRI) revealed a bone tumor compressing the spinal cord posteriorly. He was diagnosed as metastatic bone tumor and treated with radiation therapy on the day he came to our department. All symptoms subsided with radiation therapy and he returned to the life style he had before the treatment. Radiation therapy is effective for pain control and recovery from nerve palsy.
    Symptoms due to spinal cord compression can be completely recovered by early radiation therapy.

    A 64-year-old man was admitted with complaints of weight loss and back discomfort. Multiple infarctions of the kidney, spleen and heart were found. MRI and echocardiograms revealed a pedunculated tumor in the left ventricle. Immediately following a median sternotomy, severe cardiac failure occurred probably as a result of removal of thrombi in the left ventricle. Intraballoon pumping and left ventricular assist device were necessary to weaning him from the cardiopulmonary bypass. His cardiac function did not recover and, association with serious brain injury due to embolism of thrombi, resulted in death on postoperative day 8. The pathological diagnosis of the tumor was thrombi secondary to myocardial infarction.

    A 74-year-old woman complained of bloody stools and dizziness for 5 years. She had undergone Miles' operation 28 years earlier because of fulminant melena, which was diagnosed as ulcerative colitis (UC) after the operation.
    Barium examination revealed an extremely narrowing transverse colon, and colonoscopy could not be performed to observe the more orad portion of the colon.
    Surgery revealed an advanced carcinoma of the caecum. The pathological examination showed that the tumor was a mucinous carcinoma, and partly signet ring cell carcinoma, but without dysplasia. Long-term UC is apt to complicate malignancy, so that patients with a long history of UC should undergo surgery when the malignancy cannot be excluded by colonoscopic examination.

    We present a rare case of pigmented villonodular synovitis of the hip joint. Laboratory findings were positive CRP and elevated ESR. Roentgenographic findings were cystic changes in the acetabulum and the femoral head and narrowed joint space. Low increased signal intensity areas were demonstrated on T1-weighted and T2-weighted MR images. Histo pathogenic examination revealed a nodular area of a diffuse pigmented synovial lesion in which the tissue contained lymphocytes, plasma cells and multinuclear giant cells. These findings suggested an inflammatory character.

    A 49-year-old woman was admitted to the hospital because of genital and anal bleeding. The patient was diagnosed as cervical carcinoma (FIGO stage IVa) and treated with radiation therapy. The tumor in the primary site disappeared, but multiple subcutaneous metastases were seen three weeks after the end of the therapy. A sample of a subcutaneous tumor revealed squamous cell carcinoma with high mitotic ratio. Only 31 cases of subcutaneous metastasis from the cervical carcinoma have been reported in the literature. This report describes an additional case of subcutaneous metastasis from cervical cancer.

    A 24-year-old man visited our clinic with the chief complaint of weakness of the extremities. On the basis of clinical observations and tests, a hypokalemic periodic paralysis attack followed by Graves' disease was diagnosed. Potassium administration rapidly restored his muscle strength. On the next day, his serum potassium level was within normal limits with the restoration of his muscle strength, and the paralysis disappeared.
    Methimazole, propylthiouracil and iodine agents were not tolerable because of their side effects. The hyperthyroid state was satisfactorily controlled with β-blockers and adrenocortical hormones in a relatively short period before surgery. Therefore a subtotal thyroidectomy was successfully performed under general anesthesia. Although periodic paralysis is not common as an initial symptom of Graves' disease, this disease should be taken into consideration in patients with periodic paralysis.

    A 55 year-old female patient was admitted to the hospital because of obstructive uropathy and right hip joint pain. After admission, examinations revealed urethral carcinoma showing bone metastases with a histological diagnosis of adenocarcinoma. External beam radiation therapy combined with intracavitary brachytherapy using 137Cs sources and hyperthermia was delivered. Brachytherapy was performed 3 times at the dose of 16.5Gy in the course of external radiation therapy, and 2 times of hyperthermia treatments via the vaginal cavity was also combined with brachytherapy simultaneously. Tumor regression and pain improvement was obtained, and urination was satisfactory after the urethral catheter for obstructive uropathy was removed 48 days after completion of the treatment.
    Radiation therapy combined with brachytherapy and hyperthermia is considered a treatment of choice for urethral carcinoma. However a treatment modality is not established because of the low incidence of the disease.
