    In December, 1967, Barnard transplanted a human heart, thereby ushering in a new era in cardiac surgery. Since then, heart transplantation has generally been the treatment of choice for patients who are severely ill with cardiac disease. However, Japan is the advanced country in which heart transplantation has never been performed.
    Beginning in January, 1996, the author had two months of direct clinical experience at The Transplantation Center of Pittsburgh University Hospital, a world famous center for organ transplantation. The author actively joined members of the cardiac transplantation team in early morning conferences, going the rounds of patients, participating in outpatient clinics, and so on. The author also assisted in surgical procedures on several occasions and garnered a wealth of information as a result of these experiences. The author reports his valuable experiences during this period and also discusses the present status of heart transplantation in Japan.

  • TEMPERATURE DISTRIBUTION IN A PHANTOM

    Transurethral balloon laser-enhanced thermotherapy (TUBAL-ET) using ultrafine carbon particles was performed, and the temperature distribution in a phantom was determined by using a thermograph. Carbon particles 0.1ml were injected into a fishcake phantom 5mm from a shielded balloon surface, and TUBAL-ET was performed at a laser power of 30, 40 or 50 watts. Three minutes after treatment, the temperature was highest 5mm below the balloon surface and the distribution pattern was ellipsoidal. Injection of carbon particles to the phantom raised the temperature to between 14°C and 21°C higer than on the contra-lateral side with continuous irradiation at 30 watts, between 15°C and 26.2°C at 40 watts, and 21°C and 33.6°C at 50 watts. Intermittent irradiation raised the temperature 11°C higher at 30 watts, 13°C higher at 40 watts, and 16°C higher at 50 watts. These findimgs suggest that TUBAL-ET could be performed by using a agent which absorbs Nd : YAG laser energy.

  • CLINICAL ANALYSIS OF EXERCISE IN HOT-SPRING WATER

    Respiratory function test and arterial blood gas analysis were performed before and after a 2-month rehabilitation program in a pool filled with 38°C hot-spring water in sixteen patients (66.4±15.2years of age) with stable chronic obstructive pulmonary disease (7 cases of pulmonary emphysema and 9 cases of bronchial asthma) treated at our hospital between 1993 and 1995. The ratio of forced expired volume in one second to forced vital capacity (FEV1.0%) was significantly increased after the exercise program (p <0.05), while the ratio of forced vital capacity to predicted normal value (%FVC) did not change. Furthermore, a tendency toward an increase in peak flow without an increase in maximal expiratory flow at 25 and 50 percent (V25 and V50) was observed. Although PaO2 was not increased, PaCO2 was selectively decreased by the exercise program (p<0.05). The changes in the respiratory function and arterial blood gas were considered to be resulted from respiratory muscle training rather than small airway clearance. Therefore, physical therapy in a pool filled with hot-spring water may be useful in treating chronic obstructive pulmonary disease.

    Tracheobronchial reconstruction is a safe and effective procedure for patients with the neoplasms of low-grade malignancy and bronchogenic carcinoma. Thirteen patients underwent tracheobronchial reconstruction for primary lung cancer : cardinal reconstruction in 1, sleeve lobectomy in 11 and wedge lobectomy in 1. The histological type was squamous cell carcinoma in 8 patients, and each of adenocainoma, mucoepidermoid carcinoma, carcinoid tumor, adenoid cystic carcinoma and undifferentiated carcinoma in 1 including stage I in 6 patients, II in 1, III A in 4 and III B in 2. Absolutely curative operations were performed in 7 patients, relatively curative operations in 2 and absolutely non-curative operations in 4. Postoperative complications occurred in 2 patients, but both patients could be discharged. The incidence of postoperative complications in tracheobronchial reconstruction was 15.4%, and lower than among patients treated by pneumonectomy (28.9%). All of patients treated by this procedure have survived, with survival times range from 11 to 105 months. Five of them have survived for 5 years or more. Intensive follow-up is needed according to the histological type and the pattern of tumor invasion.

    Malignant tumors of the small intestine are relatively rare. A case of leiomyosarcoma operated on to diagnose an ovarian tumor is reported. A 51-year-old woman was suspected of having an ovarian tumor as a result of an internal examination and ultrasonography. Laparotomy was performed in October, 1995, but there were no abnormal findings in the ovary or uterus. The tumor was located in the jejunum approximately 50 cm distal to the ligament of Treitz, and had grown extraluminally opposite the mesentery. The tumor was circumscribed and solid, and measured 6.6 × 6.5 × 5.0cm in size. There was extensive peritoneal dissemination. As a result, a diagnosis of leiomyosarcoma of the jejunum with dissemination was made, and partial resection of the jejunum was performed. The histological diagnosis was leiomyosarcoma. Although the patient has no symptoms at present 4 months after the operation, additional removal of the rest of the peritoneal disseminations may be necessary.

    Some reports have indicated that bladder cancer is induced by radiation therapy for cervical cancer. We encountered 6 cases of urothelial cancer (5 cases of bladder cancer and 1 case of ureter cancer) following radiation therapy for cervical cancer. Age at the time of diagnosis of cervical cancer ranged from 38 to 66 years, and the average was 51.2 ± 11.0 (S.D.) years old. Age at the time of diagnosis of urothelial cancer ranged from 53 to 83 years, and the average was 67.5 ± 10.3 years old. The interval between the diagnosis of cervical cancer and urothelial cancer ranged from 3 to 25 years, averaging 16.3 years. It is impossible to evaluate the risk of development of urothelial cancer after radiation therapy based on our data. However, it is important to make an effort to diagnose urothelial cancer at an early stage by educating patients (e.g., advising regular urine tests) after the follow-up period to cervical cancer.

    A 52-year-old male with left renal pelvic cancer (transitional cell carcinoma) underwent total nephroureterectomy, partial resection of the urinary bladder, and lymphadenectomy. After the operation, he received three courses of M-VAC chemotherapy. Two years seven months postoperatively, a single metastatic lesion was detected in the left lung. The tumor was surgically resected, and the patient received two courses of M-VAC chemotherapy. Aa of three years after resection of the lung metastasis, no new metastatic lesions have been detected in the lungs or other organs. Surgical resection and chemotherapy of the solitary pulmonary metastasis appears to have been effective.

    A 26-year-old man with testicular cancer was treated with combination chemotherapy (cisplatin, etoposide and bleomycin). He underwent salvage surgery after four courses of treatment. Postoperatively, he suffered pneumonia caused by methicillin-resistant staphylococcus aureus (MRSA) leading to septic shock. In spite of an inflammatory response, his peripheral leukocyte blood cell count was rather low due to post chemotherapy myelosuppression. We administered granulocyte-colony stimulating factor concomitant with antibiotics, such as vancomycin. Immunocompromised status caused by myelosuppression after chemotherapy and the stress of major surgery were considered worsening factors for MRSA infection. In patients with metastatic testicular cancer, in which major surgery is performed after chemotherapy, preoperative MRSA surveillance and careful postoperative careful management are important.

    A 60-year-old women with pollakiuria and urge incontinence was successfully treated with pentoxifylline. Urodynamic study revealed uninhibited bladder contractions. The urge incontinence and the uninhibited contractions suggested the latent cerebrovascular dysfunction. Although an anticholinergic agent was prescribed first, it failed to improve her symptoms, and she was treated with pentoxifylline. After 2 weeks, the incontinence resolved and the pollakiuria decreased. After 4 weeks, uninhibited contractions were no longer found on urodynamic examination. The bladder capacity increased. Pentoxifylline may be useful in treating elderly patients who are suffering from urge incontinence and pollakiuria.
