    In order to clarify the nutritional state of the elderly in Gunma prefecture, we analyzed the results of basic physical check-ups of residents in 29 districts (5 cities and 24 rural areas) conducted by the Gunma Health Foundation in 1993. The physical check-ups included blood pressure, blood hemoglobin, serum total cholesterol, HDL cholesterol, triglyceride, alubumin and uric acid. Compared with the national average of nutritional values compiled by the Ministry of Health and Welfare, the levels of nutrition in Gunma were somewhat inferior. Especially males over 50 years old showed lower levels of albumin, hemoglobin, and cholesterol, indicating that they are liable to become senile at an earlier age.
    Then, we classified the degree of obesity according to the chart made by the Ministry of Health and Welfare. There were few anemic, hypoalbuminemic or hypocholesterolemic cases among overweight people over 70 years old (which is regarded as an advantage from a nutritional viewpoint). Thus, obesity in the elderly and the younger generation should be considered from a different perspective.

  • COMPARISON WITH CONVENTIONAL EXERCISE STRESS METHOD

    The purpose of this study was to assess the effect of adenosine-triphosphate (ATP) on hemodynamics and left ventricular (LV) function, and to examine the value of ATP radionuclide-ventriculography (RNVG) for detecting coronary artery disease (CAD) in comparison with standard exercise RNVG. Thirty-eight consecutive patients with suspected CAD underwent ATP (0.18 mg/Kg/min) RNVG and symptom-limited exercise RNVG. Coronary angiography revealed 27 patients with CAD (Group S) and 11 without CAD (Group N). In group S, exercise induced an increase in LV end-diastolic volume (LVEDV) and LV end-systolic volume (LVESV), and a decrease in LV ejection fraction (LVEF). In contrast, ATP infusion caused LVESV to decrease in both groups, improving LVEF. Only 14 of 39 segments whose wall motion was deteriorated by exercise showed worsening regional wall motion during ATP infusion. The sensitivity and specificity of ATP RNVG for detecting CAD were 22.2% and 100%, while those of exercise RNVG were 77.8% and 81.8%, respectively. Although ATP induced a depression of peak filling rate in group S as compared with group N, the deterioration of systolic function was not apparent with this method. ATP RNVG was not adequate for detecting CAD because of its lower sensitivity compared with exercise RNVG.

    We measured human serum alkaline phosphatase activity derived from patients with malignancies and drug-induced hyperalkalinephosphatasemia using nine phosphate compounds including phosphoamino acids. Cluster analysis of activity against the nine substrates revealed that these substrates are classifiable into three groups : phosphoamino acids, pyrophosphate, and other artificial phosphatase. Pyrophosphatase activity was somewhat lower than that of phosphoamino acids and artificial substrates. These results suggest that alkaline phosphatase does not act as a pyrophosphatase, but rather that it works both within and without the cell as a protein phosphatase

    A 35-year-old man was referred to our hospital with complaints of cough and periodic episodes of fever in the past 10 months. Chest X-ray films showed an infiltrative shadow in the right upper lung field. Bronchoscopic examination demonstrated a light pinkish tumor with lobulated surface in the right B3 orifice, which was obstructed almost completely by the tumor. No exact diagnosis was obtained by biopsy specimen. Thus, a right upper lobectomy was performed. The histological diagnosis was bronchial lipoma with the findings of mature fat cells covered with normal bronchial epithelium. Bronchial lipoma is a rare benign tumor of the lung, and only 38 cases have been reported in Japan.

    A 64-year-old woman with complaints of cough and bloody sputum was diagnosed with pulmonary adenocarcinoma of the left upper lobe. Left pneumonectomy was indicated for a tumor invading the pulmonary artery, thoracic wall and S6. However, left pneumonectomy could not be performed because of her poor general condition. Thus, bronchial arterial infusion was attempted. After significant reduction of its size (reduction rate ; 75%), the tumor could be safely resected by left upper lobectomy. Preoperative bronchial arterial infusion is useful for invasive pulmonary adenocarcinoma.

    A 58-year-old man was admitted with dyspnea (Hugh-Jones' grade IV). A chest X-ray film showed left giant bullae. At admission respiratory function test results were VC 2020ml, %VC 58%, FEV1.0 820ml, FEV1.0% 46%. A respiratory nebulizer with steroid agent improved respiratory function just before surgery VC 2490ml, %VC 72%, FEV1.0 1030ml, FEV1.0% 55%. The giant bullae in the S1+2, S6 and S8 were safely excised. Postoperatively, the re-expansion of the residual lung was good. At 3 months after surgery, dyspnea was improved (Hugn-Jones' grade I). It was thought that perioperative management with steroid agents was useful to prevent complications from giant bullectomy with respiratory dysfunction.

    Almost all pulmonary hamartomas in Japan are intrapulmonary ; endobronchial hamartomas are rare. We report a patient who seemed to have an endobronchial hamartoma. An abnormal tumor shadow was observed on the chest X-ray film of a 51-year-old man with complaints of cough and bloody sputum. The tumor was diagnosed as a pulmonary hamartoma by transbronchial lung biopsy (TBLB), and he was followed carefully. One month after TBLB, the tumor shadow was disappeared in both chest X-ray films and CT scans. We speculated that the abnormal shadow probably due to atelectasis caused by an endobronchial hamartoma that disappeared with the improvement of atelectasis. Because many hamartomas are intrapulmonary, when the tumor is diagnosed as a hamartoma by TBLB, we are likely to consider the entire tumor shadow a hamartoma. But it is also necessary to consider that the tumor shadow may be an atelectasis caused by a rare endobronchial hamartoma.

    A 41-year-old man complained of an anterior neck mass and discomfort of the larynx. Thyroid cancer was suspected by palpation, ultrasonography, computed tomography and 131-iodine scintigraphy, followed by subtotal thyroidectomy. The histopathological diagnosis was subacute thyroiditis. Subacute thyroiditis shows various symptoms, and is frequently diagnosed as thyroid cancer. Thus careful consideration should be given to the accurate diagnosis.
